Risk factors for 1-year mortality among patients with cardiac implantable electronic device infection undergoing transvenous lead extraction: the impact of the infection type and the presence of vegetation on survival.

AIMS: Infections of cardiac implantable electronic devices (CIEDs) are infrequent but carry significant morbidity and mortality. We aimed to assess risk factors for 1-year mortality among patients with CIED infection and to evaluate if the type of infection and the presence of vegetation affect survival. METHODS AND RESULTS: We reviewed records of all patients with infected CIEDs who underwent transvenous lead extraction at our tertiary care centre between 2002 and 2008. Patients who presented with infection involving the device pocket were classified as 'pocket infection', and those who pres

Transvenous leadless pacemaker extraction: When, who, where, and how?

The widespread adoption of leadless pacemakers has introduced new clinical and technical challenges related to device management at end-of-service and in the setting of device-related complications. While transvenous lead extraction is an established treatment strategy for conventional pacing systems, there is currently no standardized framework for transvenous leadless pacemaker extraction (TLLE). With increasing numbers of leadless pacemakers approaching end-of-service and expanding indications for leadless pacing, a structured approach to TLLE has become clinically relevant. This review pro

Regional Antibiotic Delivery for Implanted Cardiovascular Electronic Device Infections.

BACKGROUND: Present guidelines endorse complete removal of cardiovascular implantable electronic devices (pacemakers/defibrillators), including extraction of all intracardiac electrodes, not only for systemic infections, but also for localized pocket infections. OBJECTIVES: The authors evaluated the efficacy of delivering continuous, in situ-targeted, ultrahigh concentration of antibiotics (CITA) into the infected subcutaneous device pocket, obviating the need for device/lead extraction. METHODS: The CITA group consisted of 80 patients with pocket infection who were treated with CITA during

Outcomes Associated With Extraction Versus Capping and Abandoning Pacing and Defibrillator Leads.

BACKGROUND: Lead management is an increasingly important aspect of care in patients with cardiac implantable electronic devices; however, relatively little is known about long-term outcomes after capping and abandoning leads. METHODS: Using the 5% Medicare sample, we identified patients with de novo cardiac implantable electronic device implantations between January 1, 2000, and December 31, 2013, and with a subsequent lead addition or extraction ≥12 months after the de novo implantation. Patients who underwent extraction for infection were excluded. Using multivariable Cox proportional hazar

Central venous occlusion is not an obstacle to device upgrade with the assistance of laser extraction.

OBJECTIVE: To assess the efficacy and safety of laser-assisted lead extraction for upgrade of existing pacemakers and defibrillators in patients with central venous obstruction. BACKGROUND: Implantable cardiac defibrillators and biventricular pacing have become the accepted therapeutic measures for patients with congestive heart failure. Many patients who are candidates for device therapy, however, already have existing right ventricular leads and the presence of central venous obstruction. Upgrade of existing devices in these patients is a dilemma, which is increasingly encountered by device

CIED-Related Tricuspid Regurgitation Managed With Lead Extraction, Leadless Pacing, and Annuloplasty.

BACKGROUND: Management of cardiac implantable electronic device-related tricuspid regurgitation remains challenging, particularly when lead interference coexists with annular dilation. CASE SUMMARY: A 69-year-old woman with a single-chamber pacemaker presented with symptomatic severe lead-related tricuspid regurgitation. Echocardiography demonstrated septal leaflet impingement by the pacing lead and marked annular dilation. Because isolated lead extraction was unlikely to restore valve competence, transvenous lead extraction, leadless pacemaker implantation, and transcatheter tricuspid annulo

Impact of generator replacement and other device procedures on timing of implantable cardioverter-defibrillator lead removal.

There is a suggestion that implantable cardioverter-defibrillator (ICD) lead failure is related to generator replacement. The aim of the study was to investigate the impact of ICD lead implantation and subsequent procedures on the risk of infection and lead performance. Data from 1068 transvenous lead extraction (TLE) procedures with ICD lead removal were analyzed. It was shown that the most common cause of ICD lead removal was infection (36%), mechanical lead damage and dysfunction of undamaged leads (34% and 21%). The most common procedures preceding TLE were primary implantation (56%) and g

Tricuspid valve-in-valve jailing right ventricular lead is not free of risk.

Transcatheter tricuspid valve in valve (TViV) is increasingly performed on patients with degenerated bioprosthetic valves and elevated surgical risk. Jailing a right ventricular (RV) pacer lead at the time of TViV implantation has been achieved without causing lead dysfunction; however, device-related complications that require lead extraction raise the need for better periprocedural strategies for TViV, in pacer-dependent patients. We describe a case of device pocket infection with incomplete lead extraction, due to a jailed RV lead at the time of TViV.

Use of a Sheath and Stylet for a Difficult Dorsal Root Ganglion Stimulation Lead Extraction: A Case Report.

Dorsal root ganglion stimulation (DRG-S) is a relatively new neuromodulation technique that has shown promising results in the treatment of chronic pain conditions. We present a case of a difficult lead extraction during the explantation of a DRG-S device. The lead was unable to be removed despite multiple attempts until a sheath and stylet were used to facilitate extraction. As DRG-S utilization becomes more widespread, DRG-S device explantation will inevitably become more common. The technique described in this report may be beneficial in certain cases of difficult DRG-S lead extraction.

Transvenous Lead Extraction in a European Low-Volume Center without On-Site Surgical Support.

Indications for cardiac implantable electronic devices (CIEDs) are increasing. Almost one-third of device-related infections are endocarditis. Transvenous lead extraction (TLE) has emerged as an effective and safe approach for treating device-related infections and complications. Multiple types of extraction tools are being used worldwide. Our goal is to evaluate the safety and effectiveness of TLE using non-powered extraction tools. The study included patients between October 2018 and July 2022 requiring TLE according to EHRA expert consensus recommendations on lead extraction. A total of 88

Risk Factors for Repeat Infection and Mortality After Extraction of Infected Cardiovascular Implantable Electronic Devices.

OBJECTIVES: This study sought to investigate the factors associated with repeat infection following lead extraction procedures. BACKGROUND: Although lead extraction is an essential therapy for patients with cardiovascular implantable electronic device (CIED) infection, repeat infection still occurs in some patients. METHODS: The authors reviewed data for consecutive patients who underwent extraction of infected CIEDs from August 2003 to May 2019. Repeat infection was defined as infective endocarditis, sepsis with no alternative focus, or pocket infection after extraction of infected CIEDs.

Left common carotid artery to left innominate vein arteriovenous fistula after transvenous laser lead extraction.

Transvenous lead extraction (TLE) is used for lead infection, lead debulking, venous recanalization and device upgrades. Lead extraction is performed using specialized tools including locking stylets, mechanical or rotating sheaths, femoral snares or laser sheaths. The most feared complications associated with lead extraction are bleeding, vascular tear, cardiac avulsion and tamponade. Despite technological progress, the incidence of major procedural complications including death remains slightly above 1%. This case depicts an asymptomatic left common carotid artery (LCCA) to left innominate v

Is Venoplasty an Alternative to Lead Extraction for Patients With Venous Obstruction?

BACKGROUND: Indications for cardiovascular implantable electronic devices (CIEDs) have increased over time. Patients may require lead replacement or implantation due to lead failure or need for device upgrade. Venous obstruction is a common finding in these patients and often poses difficulties for operators. Venoplasty as an alternative to lead extraction emerges as an important intervention in the treatment of selected patients, particularly with a view to minimising procedural risk. METHODS: Nine (9) patients with CIED related venous occlusion were treated with venoplasty at our institutio
