Eating Behavior, Low-Frequency Functional Mutations in the Melanocortin-4 Receptor (MC4R) Gene, and Outcomes of Bariatric Operations: A 6-Year Prospective Study.

OBJECTIVE: Data on the effects of eating behavior and genetics on outcomes of gastrointestinal surgery for diabesity have been sparse, often flawed, and controversial. We aimed to assess long-term outcomes of bariatric operations in patients characterized for eating behavior and rare mutations in the melanocortin-4 receptor (MC4R) gene, which is strongly implicated in energy balance. RESEARCH DESIGN AND METHODS: Between 1996 and 2005, 1,264 severely obese Swiss patients underwent current laparoscopic adjustable gastric banding, gastroduodenal bypass, or a hybrid operation. Of these, 872 patie

Beyond Glucose Thresholds: Nutrition, Glycaemic Dynamics and Vascular Dysfunction in Gestational Diabetes Mellitus.

Gestational diabetes mellitus (GDM) is increasingly diagnosed in women strongly driven by the increase in overweight and obesity in women of childbearing age, yet the literature still addresses this condition more often through the lens of diagnostic glucose thresholds and subsequent nutritional management of glycaemia than through broader concepts of glycaemic dynamics, vascular dysfunction and phenotypic heterogeneity. Both GDM and maternal obesity are associated with altered glycaemia, increased glycaemic variability and endothelial dysfunction within the foetoplacental circulation, with po

Obesity in Adults with Type 1 Diabetes Mellitus.

The prevalence of overweight and obesity in individuals with type 1 diabetes mellitus (T1DM) has increased significantly due to multifactorial causes, paralleling global trends. Beyond known causes of obesity, pathophysiological and biopsychosocial causes specific to T1DM also have a role in the development of obesity. Obesity in T1DM may arise from treatment-related factors (intensive insulin therapy and defensive carbohydrate intake), behavioral aspects (reduced physical activity due to fear of hypoglycemia), and pathophysiological mechanisms such as adipose tissue dysfunction. Clinically, o

Cellular mechanism of nutritionally induced insulin resistance: the desert rodent Psammomys obesus and other animals in which insulin resistance leads to detrimental outcome.

Animal species with genetic or nutritionally induced insulin resistance, diabetes and obesity (diabesity) may be divided into two broad groups: those with resilient pancreatic beta-cells, e.g. ob/ob mice and fa/fa rats, capable of long-lasting compensatory insulin over-secretion, and those with labile beta-cells in which the secretion pressure leads to irreversible beta-cell degranulation, e.g. db/db mice, Macaca mulatta primates, ZDF diabetic rats. Prominent in this group is the Israeli desert gerbil Psammomys obesus (sand rat), which features low insulin receptor density in liver and muscle.

Combined minimally invasive vagal cranial nerve and trigeminocervical complex peripheral nerve stimulation produces prolonged improvement of severe painful peripheral neuropathy and hyperglycemia in type 2 diabetes.

INTRODUCTION: Diabetic Peripheral Neuropathy (DPN), a debilitating complication of type 2 diabetes mellitus (T2DM), stems from bioenergetic failure and reduced vascular endothelial growth factor-A expression (VEGF-A), persisting despite optimal glycemic control. The meteoric rise of "diabesity"-the coexistence of obesity and T2DM-underscores the ongoing failure of symptom control strategies and the critical need to immediately address the root cause of metabolic dysfunction and neuropathic pain. METHODS: An analysis was performed on patients who received combined minimally invasive auricular

Placental mitochondrial impairment and its association with maternal metabolic dysfunction.

The placenta plays an essential role in pregnancy, leading to proper fetal development and growth. As an organ with multiple physiological functions for both mother and fetus, it is a highly energetic and metabolically demanding tissue. Mitochondrial physiology plays a crucial role in the metabolism of this organ and thus any alteration leading to mitochondrial dysfunction has a severe outcome in the development of the fetus. Pregnancy-related pathological states with a mitochondrial dysfunction outcome include preeclampsia and gestational diabetes mellitus. In this review, we address the role

Obesity in Type 1 Diabetes: Moving Beyond the "Lean" Disease Paradigm to Understand Risk, Complications, and Treatment.

PURPOSE OF REVIEW: Overweight and obesity are increasingly common in people with type 1 diabetes mellitus (T1DM). This narrative review synthesizes current evidence on the epidemiology and mechanisms linking excess adiposity with T1DM, the obesity-associated burden of complications, and approaches to obesity management in this population. RECENT FINDINGS: Excess adiposity is now frequent in people with T1DM, approaching general-population prevalence, and obesity may also increase the risk of incident T1DM. In T1DM, weight gain reflects intersecting drivers, including intensive insulin therapy,

Autophagy as a therapeutic linchpin in metabolic diseases and obesity-associated diabetes.

Autophagy, a conserved lysosomal degradation pathway, is increasingly recognized as a central regulator of metabolic health. Its impairment contributes directly to obesity and type 2 diabetes by disrupting nutrient sensing, stress adaptation, and organelle quality control. Hyperactivation of MTORC1 with insufficient AMPK and SIRT1 signaling suppresses autophagic flux, driving lipid accumulation, insulin resistance, and mitochondrial dysfunction. Clinically relevant consequences include adipose inflammation and hypertrophy, hepatic steatosis with impaired β-oxidation, pancreatic β-cell failure

Ketogenic diet in endocrine disorders: Current perspectives.

Ketogenic diet (KD) is a high-fat, adequate-protein, and low-carbohydrate diet that leads to nutritional ketosis, long known for antiepileptic effects and has been used therapeutically to treat refractory epilepsy. This review attempts to summarize the evidence and clinical application of KD in diabetes, obesity, and other endocrine disorders. KD is usually animal protein based. An empiric vegetarian Indian variant of KD has been provided keeping in mind the Indian food habits. KD has beneficial effects on cardiac ischemic preconditioning, improves oxygenation in patients with respiratory fail

Diabetes and obesity: the role of stress in the development of cancer.

Diabesity is a condition where an individual has both diabetes and obesity, which can lead to severe complications including cardiovascular disease, a leading cause of mortality. Recently, cancer has become a leading cause of excess hospitalizations, and both diabetes and obesity are associated with a higher risk of developing several types of cancer. In this review, we propose that chronic stress significantly increases this association. Managing diabetes and obesity is challenging as they both cause significant distress. The relationship between stress and cancer is interconnected, with anxi

Identification of Four Mouse Diabetes Candidate Genes Altering β-Cell Proliferation.

Beta-cell apoptosis and failure to induce beta-cell regeneration are hallmarks of type 2-like diabetes in mouse models. Here we show that islets from obese, diabetes-susceptible New Zealand Obese (NZO) mice, in contrast to diabetes-resistant C57BL/6J (B6)-ob/ob mice, do not proliferate in response to an in-vivo glucose challenge but lose their beta-cells. Genome-wide RNAseq based transcriptomics indicated an induction of 22 cell cycle-associated genes in B6-ob/ob islets that did not respond in NZO islets. Of all genes differentially expressed in islets of the two strains, seven mapped to the d

Future is Brighter: New Potential Paradigm-Shifting Medications and Regimens for Diabetes and Obesity.

Diabetes is a chronic illness that can become debilitating owing to its microvascular and macrovascular complications. Its prevalence is increasing and so is its cost. Diabetes, particularly type 2, appears to have a very close relationship with obesity. While lifestyle modifications, exercises, and current therapeutics have substantially improved clinical outcomes, the need for new therapeutics and regimens continue to exist. Several new medications and regimens for diabetes, obesity, and diabesity are showing promising results in advanced clinical trials. For type 1 diabetes mellitus (T1DM),

Diabetes management in people undergoing metabolic-bariatric surgery: A guideline from the Joint British Diabetes Societies for Inpatient Care (JBDS-IP) Group.

The global prevalence of obesity and diabetes continues to rise, with metabolic-bariatric surgery recognised as an effective intervention for obesity and type 2 diabetes, offering potential for type 2 diabetes remission and improved glycaemic control. This guideline, developed by the Joint British Diabetes Societies for Inpatient Care (JBDS-IP), provides recommendations for the management of diabetes in individuals undergoing metabolic-bariatric surgery. It emphasises the importance of multidisciplinary care and individualised treatment plans to optimise outcomes. Key recommendations include p

Reversal of the deleterious effects of chronic dietary HFCS-55 intake by PPAR-δ agonism correlates with impaired NLRP3 inflammasome activation.

Although high-fructose corn syrup (HFCS-55) is the major sweetener in foods and soft-drinks, its potential role in the pathophysiology of diabetes and obesity ("diabesity") remains unclear. Peroxisome-proliferator activated receptor (PPAR)-δ agonists have never been tested in models of sugar-induced metabolic abnormalities. This study was designed to evaluate (i) the metabolic and renal consequences of HFCS-55 administration (15% wt/vol in drinking water) for 30 weeks on male C57Bl6/J mice and (ii) the effects of the selective PPAR-δ agonist GW0742 (1 mg/kg/day for 16 weeks) in this condition.

Epigenetic malprogramming of the insulin receptor promoter due to developmental overfeeding.

AIM: Prenatal and neonatal overfeeding programs a permanent obesity and diabetes disposition, e.g., due to induction of hypothalamic insulin resistance. We investigated acquired alterations of the DNA methylation pattern of the hypothalamic insulin receptor promoter (IRP) which might be an underlying molecular mechanism. METHODS: Neonatal overfeeding was induced by rearing Wistar rats in small litters (SL). Methylation of CpG-dinucleotides of the hypothalamic IRP was mapped using bisulfite sequencing. RESULTS: Neonatal overfeeding led to rapid early weight gain, resulting in a metabolic synd

Microbiota-gut-brain axis: relationships among the vagus nerve, gut microbiota, obesity, and diabetes.

AIMS: The purpose of this review is to explore the interconnected pathways of the microbiota-gut-brain axis (MGBA), focusing on the roles of the vagus nerve and glucagon like peptide-1 in appetite control, and in the development of obesity and diabetes. METHODS: Type 2 diabetes mellitus (T2DM) and obesity are metabolic disorders whose prevalence has significantly increased in recent decades and is expected to increase every year, to pandemic proportions. These two pathologies often coexist and have substantial public health implications. The term "diabesity" defines the pathophysiological con

Diabetes and Nutritional Screening in Post-Bariatric Patients.

With the rise of obesity there has been a concomitant increase in the incidence of type 2 diabetes (T2DM). As a result, the term "diabesity" has become a popular disease entity in the past several years. When lifestyle modification and pharmacotherapy fail to achieve successful weight loss and diabetes control, bariatric surgery is a recommended treatment option. Bariatric surgery has been shown to promote sustained T2DM remission in 30-63% of patients and improve other components of metabolic syndrome; thus a preferred term has been "metabolic surgery". As the practice of bariatric surgery fo

The striking similarities in the metabolic associations of female androgen excess and male androgen deficiency.

Androgen excess in women and androgen deficiency in men facilitate abdominal adiposity and related metabolic disorders. Moreover, obesity-associated gonadal dysfunction consists of hyperandrogenism in women but hypogonadism in men. We have reviewed the existing evidence on the interplay between sex steroids, adipose tissue and lean mass distribution, and developed a novel hypothesis to explain these apparent paradoxes. We hypothesize that the most beneficial adipose tissue distribution and function is that of normal women, who have low androgen and high estrogen concentrations. Any imbalance f

The policy challenge of coexisting undernutrition and nutrition-related chronic diseases.

The original focus on energy and protein needs for combating malnutrition gave way to UNICEF promoted concerns for maternal care and complementary feeding in association with longer-term breast feeding. Nevertheless the World Food Summit's drive to halve malnutrition rates by 2015 was not accelerating the fall in malnutrition prevalences. The UN's Standing Committee on Nutrition's commission highlighted the crucial role of maternal nutrition and low birthweights, the need for a life cycle approach to prevention and the current global effects of maternal/fetal and childhood malnutrition in ampl
