Diabetes mellitus as etiological factor of hearing loss.

Patients with diabetes mellitus often show symptoms such as dizziness, tinnitus, and hearing impairment. In general, hearing loss is sensorineural, which is sometimes confused with presbycusis, mainly because it develops in patients older than 40 years of age. Angiopathy and neuropathy caused by diabetes mellitus have been considered important factors for the vestibular-cochlear disorders found in these patients. However, there is controversy regarding the etiopathogenesis of hearing loss, as some researchers support that it develops due to neuropathy, others say it is due to angiopathy, or ev

Cutaneous Manifestations of Diabetes Mellitus: A Review.

Diabetes mellitus is a widespread endocrine disease with severe impact on health systems worldwide. Increased serum glucose causes damage to a wide range of cell types, including endothelial cells, neurons, and renal cells, but also keratinocytes and fibroblasts. Skin disorders can be found in about one third of all people with diabetes and frequently occur before the diagnosis, thus playing an important role in the initial recognition of underlying disease. Noninfectious as well as infectious diseases have been described as dermatologic manifestations of diabetes mellitus. Moreover, diabetic

The Metabolic Characteristics of Patients at the Risk for Diabetic Foot Ulcer: A Comparative Study of Diabetic Patients with and without Diabetic Foot.

BACKGROUNDS AND OBJECTIVE: Diabetic foot is a relatively severe complication in patients with type 2 diabetes (T2D), with peripheral neuropathy and angiopathy frequently serving as risk factors. However, it is unknown how the other major systemic metabolic factors impacted the profile of these patients, besides glucose management. Thus, we investigated the distinct characteristics of patients with diabetic foot ulcers and their relationships with angiopathy. MATERIALS AND METHODS: We obtained the laboratory data of 334 diabetic patients at Shanghai Pudong Hospital from 2020 to 2023. The compa

Streptozotocin-induced diabetes in rat. I. Influence of hypertension and myocardial infarction on the development of vascular complications.

Streptozotocin diabetes in rats was complicated by spontaneous hypertension (SHR) and myocardial infarction (MIC), considered as "risk groups". Renal function was assessed on the basis of blood urea nitrogen (BUN) and albuminuria. BUN increased by 36% in Wistar diabetic group, by 100% in SHR + diabetes, and by 51% in MIR + diabetes. Morphologic changes were assessed by estimation of PAS-positive glycosaminoglycans and measurement of vascular wall thickness of glomerular arterioles. The risk groups showed exaggerated tendency for development of diabetic angiopathy. A significant imbalance betwe

[Characteristics of foot ulcers in diabetic patients].

INTRODUCTION: Diabetic foot syndrome is often presented as a skin lesion in diabetics. The aim of our study was to analyse foot ulcerations in diabetics, together with ethiopatogenesis, location and grade of impairment. METHODS: We analysed foot ulcerations in 124 diabetics who attended outpatient foot clinic, or were hospitalized in the period from 1996 to 2006. Basic neuropathy screening examination was made with cotton wisp, pin-prick, tuning fork, and monofilament. Beside the evaluation of the presence of pedal pulses, the ankle-brachial pressure index was measured. If the infection of fo

[Blood serum proteins in diabetes mellitus. Euglobulin level].

Some physicochemical and immunochemical properties of blood serum euglobulins were studied in patients with diabetes mellitus with different degree of vascular lesions. Euglobulins were isolated from blood serum of normal subjects and diabetes mellitus patients by precipitation in low ionic strength dialyzing solution. As a result two euglobulin subfractions were obtained: soluble and insoluble in an isotonic saline. To study euglobulins, use was made of radioimmunoassay, gel filtration, and radial immunodiffusion. The data obtained indicate that patients with diabetes mellitus showed a signif

Carotid intima-media thickness and cerebrovascular reactivity in long-term type 1 diabetes mellitus.

PURPOSE: Diabetes mellitus (DM) is one of the leading risk factors for cerebrovascular diseases. Both macro- and microvascular damage may occur. The aim of the present work was to assess intima-media thickness as well as cerebral vasoreactivity in patients suffering from type 1 diabetes mellitus (IDDM). METHODS: Intima-media thickness and resting middle cerebral artery blood flow velocity was measured in 33 IDDM patients and 31 healthy controls. After baseline investigations, 30 seconds of breath holding (BH) and 60 seconds of voluntary hyperventilation (HV) were performed by the subjects. Tr

Effects of vitamin E supplementation on intracellular antioxidant enzyme production in adolescents with type 1 diabetes and early microangiopathy.

Defective intracellular antioxidant enzyme production (IAP) has been demonstrated in adults with diabetic nephropathy. To evaluate the effects on IAP of vitamin E administration in adolescents with type 1 diabetes and early signs of microangiopathy, 12 adolescents (aged 11-21 y; diabetes duration 10-18) were studied. Eight had retinopathy [background (four), preproliferative (three), or proliferative (one)], four had persistent microalbuminuria, and seven had both. Skin fibroblasts were obtained by biopsies and cultured in Dulbecco's modified Eagle's medium. CuZn superoxide dismutase (SOD), Mn

Retinal vascular changes induced by the oxidative stress of alpha-tocopherol deficiency contrasted with diabetic microangiopathy.

It has been proposed that oxidative tissue damage is involved in the development of diabetic angiopathies. To evaluate this hypothesis, experiments were conducted to identify the retinal vessel changes induced by the oxidative stress related to alpha-tocopherol deficiency and examine possible similarities with the lesions characteristic of diabetic retinopathy. Twenty-one-day-old male Fisher 344 albino rats were divided randomly to receive a basal, chemically defined diet either with (adequate group) or without (deficient group) alpha-tocopherol. After 6 and 8 months, some rats (n = 3 per grou

[Physiopathology of atherosclerosis in diabetics].

Vascular diseases, especially atherosclerosis, are the main cause of morbidity and mortality in diabetics. Diabetes greatly increases the risk of developing coronary heart disease, cerebral vascular accident and lower limb arteritis. The physiopathology of vascular disease in the diabetic patient involves endothelial and smooth muscle cell abnormalities. Metabolic disturbances which are characteristic of diabetes, such as hyperglycaemia or AGE accumulation, contribute to endothelial dysfunction and augment the inflammatory response at the vascular level. [Diabetes mellitus cerebrovascular disease].

Cerebral vascular accident (CVA) is the second leading cause of deaths in developed countries. The mortality rate from CVA in the Czech Republic in people under 65 years is twice higher compared to countries in Western and Northern Europe. Every year it affects about 500/100,000 citizens with mortality within one year approximately in 45%. Incidence of this serious disease is three times more frequent in diabetics than in non-diabetics. After ictus, the prognosis is worse and repeated cerebral strokes more frequent. From the risk factors, diabetics usually suffer from hypertension which is par

Both hyperglycemia and hyperuricemia aggravate acute kidney injury during cholesterol embolism syndrome despite opposite effects on kidney infarct size.

Kidney cholesterol crystal embolism (CCE) occurs in advanced atherosclerosis and induces a thrombotic (micro)angiopathy, a drop in the glomerular filtration rate (GFR), and an ischemic kidney infarction with necroinflammation. We speculated that common metabolic comorbidities such as diabetes or hyperuricemia would independently modulate each of these distinct pathophysiological processes. To test this, experimental CCE was induced by injecting cholesterol crystals into the left kidney artery of mice and thrombotic angiopathy, GFR drop, and infarct size were analyzed after 24 hours in the pres

Sheffield One-Stop Service: A potential model to improve the screening uptake of diabetic peripheral neuropathy and other microvascular complications of diabetes.

The world is experiencing an enormous rise in the prevalence of diabetes, which is associated with massive healthcare costs that threaten to overwhelm many healthcare systems. Most of the diabetes expenditure is attributed to the management of chronic diabetes complications, including diabetic peripheral neuropathy (DPN)/diabetic foot complications, chronic kidney disease, sight-threatening retinopathy and cardiovascular diseases. Of these complications, the most overlooked is DPN. Most consultations around the world do not even involve taking off shoes and socks to carry out a foot examinatio

[Clinical laboratory examination of diabetic patients in conjunction with metabolic syndrome].

Diabetic patients tend to show a reduced QOL because of macrovascular complications such as cerebral and myocardial infarction, as well as marked microvascular complications. It is important for the prevention and amelioration of these complications to diagnose diabetes mellitus (DM) early and effectively control glycemia, the blood pressure, lipids, and body weight. We examine fasting plasma glucose (FPG) and HbA1c for a diagnosis of diabetes at any time, but examine 75gOGTT for impaired glucose tolerance or DM. Examination to be necessary for a pathologic classification of DM is islet-associ

Diabetes-related histopathologies of the rat retina prevented with an aldose reductase inhibitor.

Recent evidence obtained from both galactosemic dogs and rats indicated that aldose reductase inhibitors could prevent several capillary lesions which were similar to those typical of diabetic retinopathy in humans. The present study demonstrates that diabetes-like histopathological changes in the intact retina, which were not visible in the vessel whole mounts used previously, can also be prevented. Sprague-Dawley rats were fed diets containing 50% galactose with or without an aldose reductase inhibitor (tolrestat). After 28 months of galactose feeding, the findings from retinal transections

[Diabetic skin changes from the dermatological point of view].

Even a half of diabetic patients are suffering from skin troubles. Hyperglykemia causes skin changes leading to higher incidence of bacterial and mycotic infections, provokes skin degenerative processes, macro- and microangiopathy and neuropathy. Diabetic dermopathy, rubeosis, bullousis and scleredema are based on these changes. Other skin diseases including necrobiosis lipoidica, granuloma anulare, vitiligo, perforating folliculitis accompany diabetes frequently but their etiopathogenesis is not clear.

[Prophylactic and therapeutic use of vitamins in diabetes (author's transl)].

The hypothesis that diabetes mellitus is a syndrome has been confirmed by recent physiopathological studies. In idiopathic diabetes differences in the genetic status and in the alternations of the beta cells allow to identify two completely different diseases: type 1 (or juvenile) diabetes and type II (or adult) diabetes. On the basis of the differences in the physiopathology of the different types of diabetes, the usefulness of vitamin supplementation for prophylaxis and therapy is discussed. The nutritional intake of thiamine is, for instance, recommended for patients suffering from the type

Microangiopathy: Is it relevant to wound healing in diabetic foot disease?

Chronic diabetic complications - both microvascular and macrovascular - have become serious health issues with their increasing prevalence paralleling the dramatic rise of the diabetic population worldwide. Of these complications, foot disease is a major cause of morbidity and mortality, consuming more health care resource than all other complications combined. Diabetic polyneuropathy and peripheral vascular disease constitute the two main risk factors, with trauma and foot infection being the most important initiating factors and contributors to delayed healing. Intracellular oxidative stress
