The Diabetic Retinopathy Clinical Research Network (DRCR.net) and Its Contributions to the Treatment of Diabetic Retinopathy.

Over the past two decades, the Diabetic Retinopathy Clinical Research Network (now known as the DRCR Retina Network) has contributed to multiple and substantial advances in the clinical care of diabetic eye disease. Network studies helped establish anti-vascular endothelial growth factor (VEGF) agents as an effective alternative to panretinal photocoagulation for eyes with proliferative diabetic retinopathy (PDR) and as first-line therapy for eyes with visual impairment for diabetic macular edema (DME), defined treatment algorithms for the use of intravitreal medications in these conditions, a

Glucagon-like peptide-1 receptor agonists and diabetic retinopathy: a systematic review of diabetic retinopathy outcomes.

PURPOSE: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have transformed the management of type 2 diabetes mellitus, offering glycemic, cardiovascular, and renal benefits; however, their impact on diabetic retinopathy (DR) remains uncertain. METHODS: We conducted a PRISMA 2020-guided systematic review prospectively registered in PROSPERO (CRD420251237987). We searched bibliographic databases and ClinicalTrials.gov through 31 October 2025 for studies reporting DR outcomes in adults with type 2 diabetes treated with GLP-1 RAs. RESULTS: Thirty-one studies were included: ten randomized ev

SMART (artificial intelligence enabled) DROP (diabetic retinopathy outcomes and pathways): Study protocol for diabetic retinopathy management.

INTRODUCTION: Delayed diagnosis of diabetic retinopathy (DR) remains a significant challenge, often leading to preventable blindness and visual impairment. Given that physicians are frequently the first point of contact for people with diabetes, there is a critical need for integrated screening programs within diabetes clinics to enhance DR management and reduce the risk of severe vision loss. METHODS AND ANALYSIS: We will conduct a prospective cohort study comparing (i) the intervention cohort, screened at diabetes clinics and referred to eye clinics per the proposed pathway, and (ii) the st

Prognostic factors for the development and progression of proliferative diabetic retinopathy in people with diabetic retinopathy.

BACKGROUND: Diabetic retinopathy (DR) is characterised by neurovascular degeneration as a result of chronic hyperglycaemia. Proliferative diabetic retinopathy (PDR) is the most serious complication of DR and can lead to total (central and peripheral) visual loss. PDR is characterised by the presence of abnormal new blood vessels, so-called "new vessels," at the optic disc (NVD) or elsewhere in the retina (NVE). PDR can progress to high-risk characteristics (HRC) PDR (HRC-PDR), which is defined by the presence of NVD more than one-fourth to one-third disc area in size plus vitreous haemorrhage

Linking diabetic retinopathy with lipid abnormalities.

Diabetic retinopathy (DR) is a leading cause of preventable blindness in diabetes mellitus. Therefore, it is of interest to investigate the association between dyslipidemia and DR in 100 patients, divided into 50 with DR and 50 without DR. Lipid Abnormalities was found in 57% of participants, with a higher prevalence of DR in those with Lipid Abnormalities (35/57). The study also identified that the duration of diabetes significantly influenced DR development. Thus, a strong association between Lipid Abnormalities and DR, warranting further research on lipid-lowering interventions to prevent D

Diabetic retinopathy: Looking forward to 2030.

Diabetic retinopathy (DR) is the major ocular complication of diabetes mellitus, and is a problem with significant global health impact. Major advances in diagnostics, technology and treatment have already revolutionized how we manage DR in the early part of the 21st century. For example, the accessibility of imaging with optical coherence tomography, and the development of anti-vascular endothelial growth factor (VEGF) treatment are just some of the landmark developments that have shaped the DR landscape over the last few decades. Yet, there are still more exciting advances being made. Lookin

Blood pressure control for diabetic retinopathy.

BACKGROUND: Diabetic retinopathy is a common complication of diabetes and a leading cause of visual impairment and blindness. Research has established the importance of blood glucose control to prevent development and progression of the ocular complications of diabetes. Concurrent blood pressure control has been advocated for this purpose, but individual studies have reported varying conclusions regarding the effects of this intervention. OBJECTIVES: To summarize the existing evidence regarding the effect of interventions to control blood pressure levels among diabetics on incidence and progr

Comprehensive metabolic profiling of diabetic retinopathy.

Diabetic retinopathy (DR) is an important complication of diabetes mellitus and a prevalent blind-causing ophthalmic disease. Despite years of efforts, rapid and accurate diagnosis of DR remains a challenging task. Metabolomics has been used as a diagnostic tool for disease progression and therapy monitoring. In this study, retinal tissues were collected from diabetic mice and age-matched non-diabetic mice. An unbiased metabolic profiling was performed to identify the altered metabolites and metabolic pathways in DR. 311 differential metabolites were identified between diabetic retinas and non

Laser Treatment Modalities for Diabetic Retinopathy.

Diabetes is a chronic progressive metabolic disorder that is caused by the body's inability to regulate blood glucose levels. If uncontrolled, it can lead to various complications. Among its various complications, long-term diabetes leads to diabetic retinopathy (DR). It is a disease involving blood vessels and the destruction of retinal nerves. It is usually classified into two types: proliferative diabetic retinopathy (PDR) and nonproliferative diabetic retinopathy (NPDR). It progresses and causes loss of vision. The leading cause of loss of vision is diabetic macular edema (DME). The argon

Diabetic Retinopathy: Vascular and Inflammatory Disease.

Diabetic retinopathy (DR) is the leading cause of visual impairment in the working-age population of the Western world. The pathogenesis of DR is complex and several vascular, inflammatory, and neuronal mechanisms are involved. Inflammation mediates structural and molecular alterations associated with DR. However, the molecular mechanisms underlying the inflammatory pathways associated with DR are not completely characterized. Previous studies indicate that tissue hypoxia and dysregulation of immune responses associated with diabetes mellitus can induce increased expression of numerous vitreou

The worldwide epidemic of diabetic retinopathy.

Diabetic retinopathy (DR), a major microvascular complication of diabetes, has a significant impact on the world's health systems. Globally, the number of people with DR will grow from 126.6 million in 2010 to 191.0 million by 2030, and we estimate that the number with vision-threatening diabetic retinopathy (VTDR) will increase from 37.3 million to 56.3 million, if prompt action is not taken. Despite growing evidence documenting the effectiveness of routine DR screening and early treatment, DR frequently leads to poor visual functioning and represents the leading cause of blindness in working

Endoplasmic reticulum stress and diabetic retinopathy.

Endoplasmic reticulum (ER) stress is involved in the pathogenesis of several diseases including Alzheimer disease and Parkinson disease. Many recent studies have shown that ER stress is related to the pathogenesis of diabetes mellitus, and with the death of pancreatic beta-cells, insulin resistance, and the death of the vascular cells in the retina. Diabetic retinopathy is a major complication of diabetes and results in death of both neural and vascular cells. Because the death of the neurons directly affects visual function, the precise mechanism causing the death of neurons in early diabetic
