Coeliac disease (CD) is a complex condition resulting from an interplay between genetic and environmental factors. When diagnosing the condition, serological testing and genotyping are useful in excluding CD, although the gold standard of testing is currently histopathological examination of the small intestine. There are drawbacks associated with this form of testing however and because of this, novel forms of testing are currently under investigation. Before we develop completely novel tests though, it is important to ask whether or not we can simply use the data we gather from coeliac patie

PURPOSE OF REVIEW: In this review, we discuss current diagnostic testing modalities for both gastroesophageal reflux disease (GERD) and eosinophilic esophagitis (EoE) and then introduce mucosal impedance (MI), a novel technology that measures epithelial integrity in real time during endoscopy. We describe the advantages and disadvantages of MI as compared with traditional diagnostic testing.
RECENT FINDINGS: We review studies that demonstrate that GERD and EoE have distinct MI patterns, and that physicians can accurately diagnose and distinguish the two during endoscopy with minimal time adde

It is estimated that 5 million people in Germany currently suffer from swallowing disorders. Due to increasing life expectancy and the good outcome of very premature infants, this number is expected to increase. Swallowing normally occurs in four phases, and for each phase there are diagnostic tools to assess whether the problem is anatomic-organic or functional. A detailed case history is followed by clinical examination of the cranial nerves, the orofacial region, the swallowing reflex, and pharyngeal and esophageal transit. Attention is also paid to age-related changes in the swallowing pro

The role of the oesophago-gastric junction (OGJ) in gastro-oesophageal reflux disease is still not completely understood, and there is no clinically used method to assess the OGJ function in patients. Only indirect methods such as pH studies are carried out. The OGJ acts a valve controlling the flow of solids, liquids and gases between the oesophagus and the stomach. Manometry can determine if a sphincter is toned or relaxed; but, it cannot confirm that the sphincter region is actually open. Distension is a new technique for measuring function in the OGJ. By measuring the cross-sectional area

Wireless capsule endoscopy (WCE) can be viewed as an innovative technology introduced in the medical domain to directly visualize the digestive system using a battery-powered electronic capsule. It is considered a desirable substitute for conventional digestive tract diagnostic methods for a comfortable and painless inspection. Despite many benefits, WCE results in poor video quality due to low frame resolution and diagnostic accuracy. Many research groups have presented diversified, low-complexity compression techniques to economize battery power consumed in the radio-frequency transmission o

The appendiceal diseases, particularly appendicitis, are the most common disorders in the digestive system localized at the right lower quadrant area. However, appendiceal carcinoma with vesico-appendiceal fistula is a rare clinical phenomenon. Lacking specific symptoms, appendiceal carcinomas with fistula formations are often misdiagnosed as acute appendicitis cases. The purpose of this study is to increase awareness of appendiceal neoplasms and appendicovesical fistulas. We reported our experiences in three complex cases related to digestive and urological systems, and reviewed the literatur

Digestive endoscopy in children is increasingly used for the diagnosis and treatment of a broad range of diseases affecting the stomach, intestines, biliary tract, and pancreas, with the advantages of being minimally invasive and efficient. Endoscopic procedures in children differ from those in adults in terms of both indications and primary objectives. Furthermore, ensuring the safety and comfort of children during the examination necessitates additional considerations, such as the use of appropriately sized endoscopes, carefully tailored sedation protocols, and bowel preparation regimens. BACKGROUND: Due to its unspecific presentation, intussusception is often diagnosed with delay in adults.
METHODS: From 1986 to 2002, ten patients (men/women: 8/2, median age: 53.6 years) were managed for intussusception. Clinical, radiological and surgical management data were retrospectively analyzed.
RESULTS: All patients presented with abdominal symptoms (pain: 10/10, nausea and vomiting: 3/10, diarrhoea: 2/10, "red-currant jelly stool": 2/10) during a median time of 8.3 months (2 days - 6 years) and with a trend for longer duration of symptoms for benign compared to malignant underlying

AIMS: Paediatric tumour pathology poses diagnostic challenges due to morphological variation, overlap and the rarity and diversity of neoplasms, for which the WHO provides a comprehensive diagnostic framework. In clinical practice, however, diagnostic reasoning starts with an unknown tumour and a broad differential diagnosis which may conflict with theoretical requirements. This study evaluates the contribution of ancillary techniques to the diagnostic process of non-central nervous system (CNS) solid paediatric tumours.
METHODS: Clinicopathological data were retrospectively obtained from all

High-resolution manometry evaluates esophageal motor function using 10 supine water swallows. Superimposing impedance over high-resolution manometry pressure topography assesses the relationship between contraction and bolus propulsion and identifies inadequate clearance. Ancillary techniques and maneuvers augment the standard supine high-resolution manometry evaluation by challenging peristaltic function. Increasing bolus volume (rapid drink challenge) and altering bolus consistency (standardized test meal, solid swallows) enhance identification of esophageal outflow obstruction syndromes. Ph

Tests of gastric neuromuscular function are used to evaluate patients with symptoms referable to the upper digestive tract. These symptoms can be associated with alterations in the rates of gastric emptying, impaired accommodation, heightened gastric sensation, or alterations in gastric myoelectrical function and contractility. Management of gastric neuromuscular disorders requires an understanding of pathophysiology and treatment options as well as the appropriate use and interpretation of diagnostic tests. These tests include measures of gastric emptying; contractility; electrical activity;

Recent data on etiology, pathogenesis, diagnosis, and treatment of gastric erosion are reviewed. Classification of acute and chronic erosions is proposed. The authors point out to the lack of documented scientific information related to the role and significance of erosions in gastroduodenal pathology. Special emphasis is laid on risk factors, diagnosis, prevention, and treatment of gastropathies with reference to their clinical features and diagnostic criteria. The main provisions of the Maastricht Treaty 3 (2005) concerning NSAID gastropathies are outlined.

Acute digestive system haemorrhage is a recurrent cause of hospitalization. As to the upper segment of the digestive system, ulcerous peptic disease is often the cause of this pathology, above all in Western Countries. As to the lower segment, colic diverticula and angiodysplasy represent the most common cause. Bleeding usually clears up spontaneously or with hemodynamic pharmacological help. In some cases, the situation does not improve because of bleeding persistence, so more complex diagnostic and therapeutic techniques are necessary. Instrumental diagnostics is based on endoscopy (once wit

Airway isolation by endotracheal intubation or tracheostomy impedes or even interrupts speech and swallowing. Pharyngeal and laryngeal impairment frequently occurs after extubation or de-cannulation, common consequences being dysphonia, dysphagia and the aspiration of oral secretions, food, or fluids. Aspiration often leads to pneumonia and eventually death. Although the literature reports a high frequency of dysphagia following intubation and tracheostomy, the data vary considerably, and the true incidence of oropharyngeal dysphagia following artificial airway isolation remains to be establis

The purpose of this study is to review our initial experience with a minimally invasive approach to manage symptomatic epiphrenic esophageal diverticula. Five patients with symptomatic epiphrenic esophageal diverticula underwent surgical management between August 1997 and December 2002. All patients complained of dysphagia; had experienced symptoms for at least 12 months; and were evaluated preoperatively by a barium esophagram, esophagogastroduodenoscopy, and esophageal manometry. The epiphrenic esophageal diverticula measured 5 cm or less in all patients. Manometry demonstrated esophageal dy

BACKGROUND: Achalasia subtypes on high-resolution manometry (HRM) prognosticate treatment response and help direct management plan. We aimed to utilize parameters of distension-induced contractility and pressurization on functional luminal imaging probe (FLIP) panometry and machine learning to predict HRM achalasia subtypes.
METHODS: One hundred eighty adult patients with treatment-naïve achalasia defined by HRM per Chicago Classification (40 type I, 99 type II, 41 type III achalasia) who underwent FLIP panometry were included: 140 patients were used as the training cohort and 40 patients as

The present article attempts to model the reasoning underlying the process of diagnostic workup in a patient with GI symptoms. Diagnostic reasoning consists of two consecutive and repetitive steps. Test procedures help to contract a list of multiple competing diagnoses to one focal diagnosis. In a subsequent step, the focal diagnosis again becomes expanded to a second list of new diagnoses that are more precise than those on the first list. In the process of expansion, the focal diagnosis itself serves as a test with its own sensitivity values to generate the second list of associated diagnose
