The existing recommendations for treatment of diphtheria involve the use of a hyperimmune serum neutralizing diphtheria toxin and subsequent penicillin therapy killing the pathogen. Unfortunately, diphtheria antitoxin is no longer available in the Czech Republic and this condition seems to remain permanent. Thus, the whole strategy for diphtheria treatment must be changed. Instead of penicillin, antibiotics inhibiting bacterial protein synthesis such as clindamycin or linezolid should be administered. Macrolides could be appropriate in mild to moderate disease.

The primary antibody response of iron-deficient children to diphtheria and typhoid immunization was measured. In 5 children immunized with diphtheria toxoid, there was no increase in antitoxin titre. Of 8 children who were given typhoid vaccine, 6 showed an increase in Salmonella typhi O agglutinin titre. All the iron-deficient children had normal isohaemagglutinin titres.

The re-emergence of diphtheria in South Africa in recent years warns of incomplete vaccination coverage. Recent outbreaks have been associated with a high mortality rate, due to late presentation, limited access to antitoxin and the occurrence of serious systemic complications. Death due to diphtheria is most commonly associated with diphtheritic myocarditis, which presents with heart failure, cardiogenic shock and conduction abnormalities. This case highlights the key clinical features and systemic complications, and examines the reasons for the return of diphtheria in our community.

We report the case of a 32-year-old woman who presented upon returning from India with cutaneous ulcers on the feet and pharyngitis. Microbiological testing showed the causative organism to be a toxigenic strain of Corynebacterium diphtheriae. She was treated successfully with penicillin and diphtheria antitoxin. This case emphasises the importance of maintaining a high index of suspicion for such rare but significant infectious diseases.

Tetanus antitoxin titres were determined in sera of 457 children, 6-, 10- and 16- year-old. Primary vaccination against tetanus had been given as 3 doses of DPT or DT vaccine at intervals of 4-6 weeks beginning in the 2nd or 3rd month of life. Beginning with a discussion of the main types of toxin-antitoxin mixtures of diphtheria vaccine, the author of this article goes on to review briefly the early work done on the conversion of toxin to toxoid and the introduction of adjuvants. Among these, special attention is paid to the aluminium compounds.
