Postoperative multisegmental lumbar discitis treated by staged ventrodorsoventral intervention.

BACKGROUND: Postoperative spinal infections are relatively rare. They can become life-threatening. CASE REPORT: A 56-year-old man developed multisegmental spinal infection with methicillin-resistant Staphylococcus aureus after discectomy at L3/4. A staged ventrodorsoventral intervention was needed for radical debridement and stabilization. After femoral head necrosis developed as a result of the infection, a Girdlestone hip was maintained until the joint was aseptic and a hip prosthesis could be implanted. Two years postoperatively, the patient remained free of infection recurrence. CONCLUSI

A Rare Case of Candida parapsilosis Lumbar Discitis With Osteomyelitis.

Fungal osteomyelitis is rare and usually seen in immunocompromised patients. We report a case of Candida parapsilosis osteomyelitis in an immunocompetent patient with no prior surgical history. He went for spinal laminectomy with debridement and drainage. Intraoperative culture grew C. parapsilosis, and the patient was treated with fluconazole.

Pathologic basis of pyogenic, nonpyogenic, and other spondylitis and discitis.

Pyogenic spondylitis and discitis are usually seen following a recent infection or surgery. A septic embolus causes an infarcted area within the bone. Pyogenic spondylitis is characterized by edema, vascular leakage, and supportive inflammatory reaction characterized with polymorphonuclear leukocytes. In tuberculosis of the spine, active lesions are characterized by formation of epithelioid granulomas with central caseating necrosis. Mycobacterium tuberculosis can be shown by histochemical stains for acid-fast bacteria or by immunochemistry. In brucella spondylitis, microgranulomatous prolifer

Discitis in an adult following acupuncture treatment: a case report.

Recent papers, including a review conducted by van Tulder et al., have suggested that there is paucity of information as to the efficacy of acupuncture treatment. However, there has been a significant increase in the use of acupuncture therapy for treatment of various ailments, including lower back pain. Chiropractors, along with other health care professionals, are using acupuncture as an adjunct to their main therapeutic intervention as demonstrated by a recent survey by the Canadian Chiropractic Protective Association (CCPA). However, like many other interventions, including NSAIDs and spin

Isolated Discitis in Melioidosis: An Unknown Presentation and a Microbiological Challenge.

Burkholderia pseudomallei causes Whitmore's disease or melioidosis which is endemic in many South Asian countries including India. This gram-negative bacterium is frequently found in the moist soil and agricultural workers get infected most commonly. Most of the infections are asymptomatic and have a wide spectrum of manifestations as in tuberculosis. Melioidosis of the spine manifests as spondylodiscitis with paravertebral and prevertebral abscess and presentation as discitis alone is not reported. We report the first case of melioidosis causing isolated discitis without any obvious bony invo

Acute pyogenic discitis in a degenerative intervertebral disc in an adult.

A 35-year-old male who had been receiving conservative treatment for L4 isthmic spondylolisthesis suffered from pyogenic spondylodiscitis in the degenerative L4/L5 intervertebral disc space, which could be identified by comparison with previous images. Symptoms improved with conservative antibiotic treatment. Neovascularization may occur in the annulus fibrosus of a degenerative intervertebral disc, which may increase the risk of hematogenous infection, leading to "discitis" even in adults.

Histopathologic effects of discitis on neural tissues: an experimental study.

To determine the cause of neurologic symptoms and signs seen in discitis, the neural histopathologic effects of discitis were investigated in an experimental study carried out on rats. Groups of seven rats each had their intervertebral discs inoculated with either Staphylococcus aureus, Klebsiella pneumoniae, Pseudomonas aeruginosa, or a control solution. Histopathologic examinations of the spinal cord and nerve roots were performed after 3 weeks. On histopathologic examination, vacuolar myelopathy in the spinal cord and vacuolar neuropathy within the nerve roots near the junction with the spi

Infective lumbar discitis in a sickler--an occult 'typhoid' spine?

Pyogenic infection of the intervertebral disc (discitis) is a rare infection and the diagnosis often depends on a high index of suspicion. The cases of infective discitis described in the modern literature are similar to, if not identical with what was described as 'typhoid spine'. Salmonella infection of the musculoskeletal system on the other hand is more common in patients with sickle cell anaemia. This case report highlights the bizarre presentation of infective lumbar discitis in a sickler (HbSS) and calls attention to the need for a thorough evaluation of low back pain in these patients.

[Discitis after spinal anesthesia for transurethral resection of the prostate].

We described a case of discitis and meningitis following spinal anaesthesia for transurethral resection of the prostate. The patient received antibiotics for a month before surgery, because of Klebsiella prostatitis. Spinal anaesthesia was performed in L3-L4 interspace by using 22G Quincke needle. Bacteriaemia occurred during the first postoperative hours. Ten days after spinal anaesthesia, patient suffered from lumbar pain, exacerbated by vertebral percussion, and motor weakness within lower limb, which was marked on right side. MRI examination showed L3-L4 discitis with psoas abcess in regar

Microbial etiology of vertebral osteomyelitis/discitis amid the opioid epidemic.

OBJECTIVE: The primary goal of this study was to establish the current microbial trends in vertebral osteomyelitis/discitis (VOD) amid the opioid epidemic and to determine if intravenous drug use (IVDU) predisposes one to a unique microbial profile of infection. METHODS: The authors performed a retrospective cohort study consisting of 1175 adult patients diagnosed with VOD between 2011 and 2022 at a single quaternary center. Data were acquired through retrospective chart review, with pertinent demographic and clinical information collected. RESULTS: Staphylococcus aureus was the most culture

Incidence of Sacral Osteomyelitis and Discitis After Minimally Invasive Sacrocolpopexy.

OBJECTIVE: The incidence and associated risk factors for sacral osteomyelitis and sacral discitis after sacrocolpopexy remain unknown. The aim of this study was to determine the incidence of sacral osteomyelitis and discitis after minimally invasive sacrocolpopexy and their association with the method of sacral mesh fixation. METHODS: This is a retrospective cohort study of consecutive minimally invasive sacrocolpopexies performed by 11 female pelvic medicine and reconstructive surgery board-certified surgeons from January 2009 to August 2019 within a single health system. CT-guided discitis-osteomyelitis biopsies: needle gauge and microbiology results.

PURPOSE: To compare the microbiology results and needle gauge for CT-guided biopsies of suspected discitis-osteomyelitis. METHODS: All CT-guided biopsies performed for suspected discitis-osteomyelitis at our institution between 2002 and 2019 were reviewed. Biopsy location, needle type and gauge, microbiology, pathology, and clinical and imaging follow-up were obtained through chart review. Yield, sensitivity, specificity, and accuracy were calculated. A pairwise analysis of different needle gauges was also performed with calculations of odds ratios. Naïve Bayes predictive modeling was perform

Vertebral discitis after laparoscopic resection rectopexy: a rare differential diagnosis.

Vertebral discitis usually arises from haematogenous spread of pathogens to the discs and bones. Vertebral discitis can rarely occur as a complication after laparoscopic operations with fixating sutures on the promontory. We report the case of an 81-year-old woman who underwent a laparoscopic resection rectopexy because of rectal prolapse. Weeks after the operation, the patient developed lower back pain with radiation to both legs not responding to symptomatic therapy. Two months later, a magnetic resonance imaging of the lumbar spine showed vertebral osteomyelitis and discitis. A fixation on

Spontaneous fusion of isthmic spondylolisthesis after discitis: a case report.

This is the first case report of a child with isthmic spondylolisthesis and discitis who had spontaneous fusion develop at an unstable level with relief of symptoms after nonoperative treatment. Although the blood culture was negative, the 14-year-old boy with Grade III isthmic spondylolisthesis of L5 was diagnosed with discitis at the L5-S1 level, based on clinical findings, elevated C-reactive protein, plain radiographs, and magnetic resonance imaging scans. The patient was treated with antibiotics for 19 weeks and bed rest for 4 weeks followed by immobilization in a hip spica cast for 8 wee

Bacterial discitis caused by limb gangrene requiring below-knee amputation.

Two patients presented with disabling back pain and were unable to participate in physical therapy activities after being admitted to an acute rehabilitation center. Both patients had bacterial discitis of the lumbar spine that was apparently caused by infected ischemic limb tissue, ultimately removed at below-knee amputation. The literature describes many cases of bacterial discitis infected from many sources, but not from ischemic limb tissue requiring subsequent amputation. Many such cases may exist, however, and earlier recognition of this condition will enable appropriate treatment before

Discitis Mimicking Septic Arthritis in a 17-Month-Old Boy.

Discitis in children is uncommon, typically occurring between the ages of two and eight years. The etiology is not established, but it is generally considered a bacterial infection. Symptoms vary with age but commonly include irritability, refusal to walk, and back pain. These various clinical presentations cause delays in diagnosis. We present this case to draw attention and familiarise clinicians with its presentations. A 17-month-old boy presented with a one-week history of refusal to walk. Examination showed an unusual finding of the child keeping his right leg in a flexed position with hi
