The combination of percutaneous manual and endoscopic Ho:YAG laser discectomy (PELD) is a new minimal intervention technique in treating patients with herniated lumbar discs that do not penetrate the posterior longitudinal ligament. The results in 100 patients treated with PELD were compared randomly with those in 100 patients treated by chemonucleolysis with chymopapain (CN) and 100 patients treated by automated percutaneous lumbar discectomy (APLD) at the same hospital. We followed the 300 patients postoperatively for 1 year, with physical examination, postoperative plain lumbosacral radiogr

Diskectomy for treatment of temporomandibular joint disk derangements was evaluated clinically. The study comprised 40 consecutive patients (50 joints) with pain in the joint area and functional impairment not alleviated with nonsurgical therapy for at least 6 months. Diskectomy was performed in 32 joints. In 18 joints both diskectomy and minor arthroplasty were performed. No disk substituting implants were used. At 1 year follow-up 85% of the patients were free of symptoms from the temporomandibular joint area and showed normalized mandibular movements. Twenty patients were followed for 3 yea

BACKGROUND: Posterior percutaneous endoscopic cervical foraminotomy and diskectomy has remarkably evolved with successful results. Although percutaneous endoscopic cervical diskectomy (PECD) has gained popularity, the risk of surgical failure may be a major obstacle to performing PECD. We analyzed unsuccessful cases requiring reoperation. The objective of this article was to find common causes of surgical failure and elucidate the limitations of the conventional PECD technique.
METHODS: Surgery-related complications were reviewed from the initial 252 cases of a single surgeon. The patients ha

BACKGROUND: Increasingly, pain management specialists (P-S) (e.g., anesthesiologists, radiologists, or physiatrists), who are not spinal surgeons, are performing percutaneous endoscopic laser diskectomy (PELD), percutaneous lumbar disc decompression (PLDD), and target percutaneous laser disc decompression (T-PLDD) in patients with minimal/mild disc herniations. Here, theoretically, the laser vaporizes/shrinks a small portion of disc tissue that lowers intradiscal pressure/volume, and thereby provides "symptomatic relief" (e.g., low back pain/radiculopathy). Nevertheless, the vast majority of t

PURPOSE: To retrospectively evaluate if the degree of disk degeneration and disk herniation volume at magnetic resonance (MR) imaging are risk factors for recurrent disk herniation.
MATERIALS AND METHODS: The institutional review board permits such retrospective studies, and individual informed consent was not required. MR imaging findings obtained before initial diskectomy in 30 patients (mean age, 42.8 years; 10 women, 20 men) with recurrent disk herniation (study group) and 30 patients (mean age, 42.2 years; nine women, 21 men) without recurrence for at least 2 years after surgery (control

INTRODUCTION: Durability of surgical treatment is important to patients, providers, and payers. Lumbar diskectomy is one of the most commonly performed spinal surgeries. Understanding the specific indications for revision surgery can help guide future research and quality improvement initiatives.
METHODS: A multisurgeon, single-institution database was queried for revision surgery following a primary lumbar diskectomy (CPT = 63,030) from 2014 to 2018 with a minimum follow-up of 4 years (N = 1,133). The mean patient age was 44.93 years, levels decompressed was 1.08, mean body mass index was 30

BACKGROUND: Multiple anterior cervical diskectomy/fusion (ACDF) techniques now use a variety of Polyehteretherketone (PEEK) cages; stand-alone (SA) and zero-profile (ZP) with/without screws, cages filled with demineralized bone matrix/autograft, and cages coated with hydroxyapatite or titanium. We compared the safety/ efficacy between different PEEK ACDF cage constructs in 17 studies, and in some cases, additionally contrasted results with "routine" ACDF (i.e. series/historical data performed with combinations of iliac autograft/allograft and plates).
METHODS: We focused on the clinical outco

BACKGROUND: Surgical simulators are useful in many surgical disciplines to augment residency training. Duty hour restrictions and increasing emphasis on patient safety and attending oversight have changed neurosurgical education from the traditional apprenticeship model. The Congress of Neurological Surgeons Simulation Committee has been developing neurosurgical simulators for the purpose of enhancing resident education and assessing proficiency.
OBJECTIVE: To review the initial experience with an anterior cervical diskectomy and fusion (ACDF) simulator.
METHODS: The first ACDF training modu

An internist performed percutaneous laser-assisted diskectomies (PLADs) on a patient with a sequestrated disc and stenosis. Subspecialists who perform PLADs should be trained in patient selection and lumbar diskectomy techniques. Chymopapain, percutaneous nucleotome-assisted diskectomy, and PLADs are alternatives to microdiskectomy for the management of lumbar disc herniations. PLADs were performed at the L4-5/L5-S1 levels in a 38-year-old woman with magnetic resonance (MR)-documented L4-5 stenosis and disc disease. After PLADs, she developed a subacute cauda equina syndrome. Two months later,

The results of diskectomy without fusion at L4-5 were compared with those at L5-S1. Strict indications for surgery had been applied to each group. Evaluation was carried out using the Oswestry Disability scale, as well as a modified Smiley-Webster scale, and a subjective improvement rating scale, as well as documenting work status, whether any future surgery was required, and other factors. Results were placed into categories based on these evaluation methods and were analyzed statistically. Average follow-up was 51 months with a minimum of 24 months. Overall excellent and good results occurre

BACKGROUND: Thoracic disk herniation is a rare event, and acute presentation of this lesion is even less common.
METHODS: Case reports.
FINDINGS: Both patients presented with rapidly progressing lower extremity weakness and sphincter dysfunction. Radiographic work-up revealed large herniated thoracic disks with spinal cord compression. Emergency thoracic diskectomy was performed.
RESULTS: Both patients had partial reversal of neurologic and sphincter abnormalities after emergency surgery.
CONCLUSIONS: Thoracic disk herniation, although rare, should be considered when patients present with

INTRODUCTION: Lumbar diskectomy is a commonly performed surgery for which patients may present with conditions predisposing to adverse outcomes, such as the presence of a colostomy. This study aimed to characterize patients undergoing lumbar diskectomy with versus without colostomy and determine whether matched cohorts had differential rates of postoperative complications.
METHODS: Adult patients undergoing primary lumbar laminotomy/diskectomy were identified from the administrative 2010 to 2022 Q4 PearlDiver M165 database. Two subcohorts were created: (1) patients with a diagnosis code for c

Surgical treatment of extremely migrated caudal and rostral lumbar disk herniations is technically challenging. Traditional open technique often requires more bone resection, which can lead to rare but significant complications such as pars fracture or instability requiring fusion surgery. Endoscopic diskectomy is a safe alternative to traditional open surgery with acceptable complication rates.1-4 Endoscopic diskectomy provides the advantage of excellent visualization and maneuverability while minimizing soft tissue trauma and bony resection, which can result in less postoperative pain, less

Study Design Case series. Objective Posterior percutaneous endoscopic cervical diskectomy (PECD) can preserve the disk in patients with a foraminal disk herniation. However, progressive angulation at the operated segment is a concern, especially for patients with cervical lordosis < 10 degrees. The change in cervical lordosis after posterior PECD was analyzed. Methods Medical records were reviewed of 32 consecutive patients (22 men, 10 women; mean age, 49 ± 12 years) who had single-level foraminal soft disk herniation. The operation levels were as follows: C4-5 in 1 patient, C5-6 in 12, C6-7 i

OBJECTIVE: We describe four cases of subchondral osteonecrosis of the vertebral body that occurred after percutaneous laser diskectomy. Follow-up MR imaging after laser intervention showed abnormal findings in the vertebral body immediately adjacent to the site of diskectomy that are consistent with subchondral osteonecrosis.
CONCLUSION: MR imaging features of this complication include a wedge-shaped low signal intensity on T1-weighted images, high and low signal intensities on T2-weighted images, and a contrast-enhanced area corresponding to high signal intensity on T2-weighted images. Possi

The indication for spine fusion in combination with diskectomy it neither clearly defined nor widely accepted. It largely depends on the specialist to whom the patient is referred. While orthopedists often perform primary fusions, neurosurgeons hardly ever do so, no matter what the nature of the complaint is. Therefore, the selection of the procedure seems to be a rather random choice. The question is not whether the combined operation or the simple disc excision is superior in itself, but which of the two should be chosen in a given case. For patients with disc displacement and radicular pain

BACKGROUND: Thoracic disk herniation is rare and difficult to treat. The minimally invasive lateral retropleural approach to the thoracic spine enables the surgeon to decompress the neural elements and minimize thecal sac manipulation through direct visualization with less exposure-related morbidity.
OBJECTIVE: To provide a detailed step-by-step overview of the minimally invasive retropleural approach for thoracic diskectomies, including preoperative planning through postoperative care as practiced at our institution.
METHODS: Lateral retropleural thoracic diskectomies performed at a single

BACKGROUND: A 26-year-old man presented with acute quadriparesis owing to a traffic accident. A computed tomography scan revealed a hangman fracture and locking of the left facet joint at C2-3. Magnetic resonance imaging showed a high signal change of the spinal cord on T2-weighted image and hemorrhage in the C2-3 level.
CASE DESCRIPTION: An emergency closed reduction after anterior cervical diskectomy and fusion were performed for spinal stability and decompression of the dural sac. Five months postoperatively, C1-2-3 posterior wiring using an iliac bone graft with the Brook method was perfo
