Dissection and rupture of the ascending aorta. Unusual complications of aortocoronary bypass surgery.

This is a report concerning two cases of unusual complications, dissection and rupture of the ascending aorta, secondary to saphenous vein bypass surgery. We recommend that evaluation of patients undergoing saphenous vein bypass surgery include careful image-intensifier fluoroscopic examination during coronary arteriography. If aneurysmal dilatation of the aorta or calcification is observed, ascending aortography should be performed to better evaluate the aorta, and surgeons should be alerted to use appropriate maneuvers and techniques to minimize the risk of complications in the aortic site o

Endovascular treatment of non-dissected ascending aorta disease: a systematic review.

Severe ascending aorta disease includes aneurysms, pseudoaneurysms (ascending aorta pseudoaneurysms), penetrating aortic ulcers and Type A aortic dissections. Surgical replacement of the aortic root, ascending aorta or aortic arch is the common treatment for severe ascending aortic disease involving the root, the ascending aorta and/or the arch. Despite good surgical results, there is still a risk for morbidity and mortality following surgery for ascending aorta replacement when elderly patients or patients at high risk for surgery are concerned. Less invasive endovascular treatments for ascen

Dissection of Ascending Aorta: A Complication of Transradial Artery Access of Coronary Procedure.

Iatrogenic acute dissection of ascending aorta following coronary angiography and percutaneous intervention is a rare complication. Most reports involve localized aortic dissections as a complication of cannulation of a coronary artery with propagation into the ascending aorta. It is usually treated by sealing the intima with a stent in the ostium of the coronary artery or conservative management, while extensive dissections may require a surgical intervention. We describe a case of the subclavian dissection extending into the ascending aorta that occurred during diagnostic catheterization usi

Dissection of the ascending aorta: a late complication of coronary artery bypass grafting.

Acute dissection of the ascending aorta as a late complication of coronary artery bypass grafting has been rarely reported. We report a case of a 61 year-old man in whom acute dissection of the ascending aorta developed 2 years after coronary artery bypass grafting. The ascending aorta was replaced with a Dacron graft, and an island of the aortic wall, on which previous proximal anastomoses had been placed, was implanted into the Dacron graft successfully.

Coronary bypass grafting with totally calcified or acutely dissected ascending aorta.

From August 1984 through November 1988, 10 of 2,658 patients undergoing coronary artery bypass grafting had ascending aortic disease that was not amenable to proximal anastomoses for coronary bypass grafting. This was due to a calcified aorta in 6 and acute aortic dissection in 4. There were 5 male and 5 female patients with a mean age of 71 years. Cannulation site was the femoral artery in 5, ascending aorta in 3, and aortic arch in 2. Profound hypothermia and ventricular fibrillation, with no cross-clamp or cardioplegia, was used in 9 patients, and circulatory arrest in 1. In 8 patients a si

Surgical treatment of dissecting aneurysm of the ascending aorta: immediate and late results.

The authors describe experience gained with surgical treatment of dissecting aneurysm of the ascending aorta in the period from 1978 to 1987. 30 patients were operated on using the techniques proposed by Bentall, De Bono and Carbrol. All patients exhibited the presence of annulo-aortic ectasia. Hospital mortality in the past four years was 20%. On the basis of an analysis of immediate and late results the authors come to the conclusion that Bentall's, De Bono's and Cabrol's technique is the method of choice for surgical corrections of annulo-aortic ectasia and Marfan's syndrome.

Surgical treatment of acute dissection of the aorta superimposed on pre-existing aneurysm of the ascending aorta. Total replacement of the ascending aorta with reimplantation of the coronary arteries. A report on five cases.

Five cases of acute dissection of the ascending aorta superimposed on a pre-existing aneurysm, including two with propagation of the dissection into the coronary arteries, were treated by total replacement of the ascending aorta utilizing a valve containing tube graft and reimplantation of the coronary arteries. Diverse techniques such as the use of an intermediate tube for reconnecting the coronary arteries, correction of the coronary dissection by reapproximation of the two layers or with bypass grafting, support of the distal aortic suture line with Teflon felt and fistulization of the peri

Echo-guided seldinger technique facilitates ascending aorta cannulation in type A aortic dissection.

BACKGROUND: The ascending aorta is the most simple and rapid arterial access for the establishment of antegrade systemic perfusion. In acute type A aortic dissection, prompt establishment of antegrade central perfusion, especially in unstable hemodynamic emergency operations, help to diminish organ malperfusion and to prevent retrograde embolism. The effectiveness as well as the safety of antegrade perfusion under ultrasonographic guidance through the dissected ascending aorta was evaluated for the repair of type A aortic dissection utilizing a new echo stabilizer. RESULTS: Ascending aortic c

Ascending aorta dissection in a new classification system: Clinicopathological features of 62 cases.

Herein we present a single institution 5-year experience with ascending aorta dissection biopsies depicting the clinicopathological features of 62 cases. To detect microscopic abnormalities in the aortic wall beside the dissection itself, we applied a new histopathological classification scheme from the consensus documents issued by The Society for Cardiovascular Pathology and The Association for the European Cardiovascular Pathology in 2015 and 2016. The most common finding was medial degeneration (MD) - 61 cases (98.4%); atherosclerosis (AS) was found in 19 aortae (30.6%), and chronic aortit

Layer-specific biomechanical and histological properties of normal and dissected human ascending aortas.

Recent studies have attempted to characterize the layer-specific mechanical and microstructural properties of the aortic tissues in either normal or pathological state to understand its structural-mechanical property relationships. However, layer-specific tissue mechanics and compositions of normal and dissected ascending aortas have not been thoroughly compared with a statistical conclusion obtained. Eighteen ascending aortic specimens were harvested from 13 patients with type A aortic dissection and 5 donors without aortic diseases, with each specimen further excised to obtain three tissue s

Traumatic Dissection of the Ascending Aorta Complicated by Multiple Injuries Following a Traffic Accident.

Trauma to the ascending aorta may progress to a cardiac tamponade, which is often life-threatening. Here, we report on a case of traumatic dissection of the ascending aorta, complicated by multiple injuries. A 24-hour follow-up period was provided to evaluate the traumatic bleeding, and a large tear that extended over three-quarters of the circumference of the aortic intima was identified. An ascending aorta and partial arch replacement were successfully performed. The timing of cardiopulmonary bypass surgery in cases of multiple traumas must be sufficiently flexible to accommodate the conditi

Endovascular stenting of the ascending aorta for visceral malperfusion in a patient with type A aortic dissection.

A type A aortic dissection is a challenging condition for both cardiothoracic and vascular surgeons. Although open surgery remains the gold standard, there is considerable interest in the use of endovascular techniques for patients who present with malperfusion. We present the case of an unstable 55-year-old man with visceral malperfusion from a type A dissection who was stabilized using an endovascular technique as a bridge to open surgery. A bare metal thoracic endograft was used in the ascending aorta to rapidly restore perfusion. This hybrid approach to the problem of malperfusion in type
