Psychiatric symptoms and dissociation in conversion, somatization and dissociative disorders.

OBJECTIVE: Conversion, dissociation and somatization are historically related in the long established concept of hysteria. Somewhere along the way they were separated due to the Cartesian dualistic view. The aim of the present study was to compare these pathologies and investigate whether symptoms of these pathologies overlap in their clinical appearance in a Portuguese sample. METHOD: Twenty-six patients with conversion disorder, 38 with dissociative disorders, 40 with somatization disorder, and a comparison group of 46 patients having other psychiatric disorders answered questions about dis

Comparing the symptoms and mechanisms of "dissociation" in dissociative identity disorder and borderline personality disorder.

A total of 75 patients were diagnosed with the Structured Clinical Interview for DSM-IV Dissociative Disorders-Revised as having dissociative identity disorder (DID), and 100 patients were diagnosed with the Structured Interview for DSM-IV Personality as having borderline personality disorder (BPD). Both groups were administered the Multidimensional Inventory of Dissociation (MID). DID patients had significantly higher MID scores than BPD patients, different distributions of MID scores, and different MID subscale profiles in 3 ranges of MID scores (0-15, 15-30, 30-45). The core MID symptoms-ex

Group-based psychoeducation for dissociative disorders: Reducing PTSD and dissociative symptoms while enhancing emotion regulation and adaptive capacity.

OBJECTIVE: This study evaluated the effectiveness of the Finding Solid Ground (FSG) program, delivered through psychoeducational groups, as an adjunct to individual therapy for adults with dissociative disorders (DDs). METHOD: In this naturalistic study, seventy-two adults with DDs were recruited to participate in a 31-week FSG psychoeducational program across multiple outpatient sites while continuing individual psychotherapy. Participants completed assessments at baseline, midpoint, and endpoint, measuring dissociative symptoms, PTSD symptoms, emotion regulation, and adaptive capacities. An

Discriminating among diagnostic categories using the Dissociative Disorders Interview Schedule.

The Dissociative Disorders Interview Schedule was administered to 1,308 subjects in eight diagnostic categories, including 296 with dissociative identity disorder. The study tested three hypotheses: (1) the Mahalanobis distance between dissociative identity disorder and each of seven other diagnostic categories would be large, (2) the closest diagnostic category to dissociative identity disorder would be dissociative disorder not otherwise specified, and (3) nondissociative diagnostic categories would be closer to each other than any one to dissociative identity disorder. All three hypotheses

Dissociative experiences and eating disorders in a female college sample.

The present study evaluates the relationship between abuse experiences, dissociation and eating disorders (ED) in an Italian female college sample. In particular, the study aims at comparing the dissociative effects of abuse experiences in ED and normal subjects. Dissociative experiences were assessed by Dissociation Questionnaire (DIS-Q), which appeared to be an internally consistent and valid instrument. The presence of ED in 491 female college students was assessed by a two-stage epidemiological procedure. The factor structure of the DIS-Q in our sample allowed us to identify specific featu

Internal containment in the treatment of patients with dissociative disorders.

Both external and internal containment techniques are useful in treating patients with multiple personality disorder and other dissociative disorders. The author focuses on internal containment using various individualized strategies for helping these patients cope with life and work through past traumas. Because the visual imagery typically used in these strategies varies significantly from patient to patient, the author does not intend to provide an exhaustive description. The techniques are illustrated by case examples from the author's clinical practice.

Hypnotic suggestibility in dissociative and related disorders: A meta-analysis.

Elevated responsiveness to verbal suggestions is hypothesized to represent a predisposing factor for the dissociative disorders (DDs) and related conditions. However, the magnitude of this effect has not been estimated in these populations nor has the potential moderating influence of methodological limitations on effect size variability across studies. This study assessed whether patients with DDs, trauma- and stressor-related disorders (TSDs), and functional neurological disorder (FND) display elevated hypnotic suggestibility. A systematic literature search identified 20 datasets. A random-e

Group treatment for complex dissociative disorders: a randomized clinical trial.

BACKGROUND: Patients with complex dissociative disorders (CDD) report high levels of childhood- abuse experiences, clinical comorbidity, functional impairment, and treatment utilization. Although a few naturalistic studies indicate that these patients can benefit from psychotherapy, no randomized controlled trials have been reported with this patient-group. The current study evaluates a structured protocolled group treatment delivered in a naturalistic clinical setting to patients with CDD, as an add-on to individual treatment. METHODS: Fifty nine patients with CDD were randomized to 20 sessi

[Factitious disorders in dermatology: Value of the dissociative state concept].

BACKGROUND: Factitious disorders in dermatology consist of intentionally self-inflicted skin lesions that vary in morphology and distribution and occur on surfaces readily accessible to the patient's hands. They tend to be a chronic condition that waxes and wanes according to the circumstances of the patient's life. Patient management poses a particular challenge to the clinician and the prognosis is considered poor. The aetiopathogenesis of factitious disorders in dermatology is not completely understood. We present a case in which we suggested the occurrence of factitious behaviour during a

Dissociative disorders: pathways to diagnosis, clinician attitudes and their impact.

OBJECTIVES: To study the attitudes and experiences of Australian clinicians with dissociative disorders and the paths to diagnosis and experiences of patients. METHOD: The attitudes of Australian clinicians to dissociative disorders and the experiences of patients were assessed by questionnaires. The clinicians were mental health specialists and a small number of general medical practitioners. The patients had all been diagnosed with a dissociative disorder. RESULTS: Of the 250 clinicians, 21% reported experience with more than six cases on average of any one of the dissociative disorders, 3

The paradox of "detachment disorders": binding-disruptions of dissociative process.

This case is discussed in the contexts of affect theory, attachment theory, and dissociative experience. Dissociative adaptations to living that become entrenched resistances during psychotherapeutic inquiry give way to a scrupulous attention to the characteristics of dissociative experiencing. Somatosensory free association and appreciation of experiential aspects of depersonalization, derealization, and dissociative amnesia open new areas of negotiation between patient and therapist. Paradigms from infant attachment and adult attachment research parsimoniously explicate psychodynamic formula

Pseudoseizures and dissociative disorders: a common mechanism involving traumatic experiences.

Patients with psychogenic non-epileptic seizures (pseudoseizures) have been diagnosed as having conversion disorder or dissociative disorder. Pseudoseizure patients frequently report a history of physical and sexual abuse, and traumatic experience is considered part of the mechanism for producing dissociation. Pseudoseizures may be a manifestation of dissociative disorder, especially when a history of sexual or physical abuse is documented. A common mechanism involving traumatic experience may be present in both pseudoseizures and dissociative disorders. A complete neurodiagnostic evaluation a

Self-Injury and Suicide Attempt in Relation with Trauma and Dissociation among Adolescents with Dissociative and Non-Dissociative Disorders.

OBJECTIVE: To explore the role of trauma and dissociation over self-injurious behaviors (SIB) and suicide attempts (SA) in adolescents. METHODS: A total of 207 adolescents participated in the study. After conducting diagnostic interview, participants were divided into five groups as subjects with dissociative disorders (DD), attention deficit hyperactivity disorder (ADHD), major depressive disorder (MDD) and anxiety disorders (AD), and a control group (CG) without any psychiatric disorder. ADHD, MDD and AD groups were considered as non-dissociative disorders (non-DD group) in the present stud

Hyperglycemia associated dissociative fugue (organic dissociative disorder) in an elderly.

Inadequate glycemic control in patients with diabetes is known to be associated with psychiatric disorders such as depression, anxiety disorder, and cognitive impairment. However, dissociative syndrome has not been reported so far. Here we are reporting a case of repeated dissociative fugue associated with hyperglycemia, in an elderly with type II diabetes. Possible neurobiological mechanism has been discussed.

Etiology, pathogenesis, and therapy according to Pierre Janet concerning conversion disorders and dissociative disorders.

Pierre Janet's works on conversion disorders or dissociative disorders has mainly fallen to the wayside in favour of Freud's works. In the first part of this paper, Janet's conception of hysteria is discussed and his place in French psychiatry described. Different aspects of Janet's diathesis-stress approach are presented (particularly the pathogenic concept of fixed ideas), which refer not only to a conception of hysteria but also to traumatic (stress) disorders and other psychological disturbances. The second part of the paper details the varieties of Janetian therapeutic treatments of these

[Dissociative disorder and self-injury].

Both the number of patients with dissociative disorder and that of those with self-injury have been increasing since the end of the twentieth century, suggesting that dissociation and self-injury might be closely related. When dissociative disorder coexists with self-injury, it implies self-punishment and a wish to be understood by others. Although many cases of self-injury observed since 2000 lacked traumatic experiences and were not accompanied by pathological dissociative symptoms, the patients did have dissociative tendencies. According to the results of our study examining self-injury in
