The modern staged repair of bladder exstrophy in the female: a contemporary series.

OBJECTIVE: Many changes have occurred in the treatment of bladder exstrophy over the last few years and several repairs are now offered, but there is a lack of long-term follow-up data. The purpose of this study was to evaluate long-term outcomes in a select group of female patients in whom modern staged repair was undertaken. PATIENTS AND METHODS: From an institutionally approved database 41 patients were identified. All had undergone primary bladder exstrophy closure in 1988-2005, at a mean age of 2 months (range 4h to 3 months), with or without an osteotomy by a single surgeon, and all wer

The natural history of daytime urinary incontinence in children: a large British cohort.

AIM: Few studies have looked at the prevalence of daytime incontinence in a longitudinal cohort of children. This study set out to determine the prevalence of daytime incontinence and relationships between daytime incontinence and bedwetting, faecal incontinence and urgency in a large cohort of British children. METHODS: Parents of children taking part in the Avon Longitudinal Study of Parents and Children (ALSPAC) were asked questions concerning the child's daytime wetting, bedwetting and faecal incontinence at different time points, 4.5, 5.5, 6.5, 7.5 and 9.5 years. The difference between t

The "clam": indications and complications.

We have reviewed 78 patients who underwent "clam" enterocystoplasty as part or all of their lower urinary tract reconstruction. Nearly half (32) were non-neuropathic; the remainder were either overtly neuropathic (mainly spina bifida) or suspected neuropathic (mainly the older primary enuretics). Only 5 were non-walkers--all neuropathic. Most patients were operated on for incontinence but 12 had upper tract damage related either to urinary diversion or to poor bladder compliance in the early phase of filling; 69 patients became dry, voiding spontaneously (30), by activation of an artificial ur

Parental experiences with outpatient care for daytime urinary incontinence in children: a mixed methods study.

INTRODUCTION: Daytime urinary incontinence (DUI) is a prevalent condition in children that can have a negative impact on both their quality of life and their parents' psychosocial wellbeing. Parents play a central role in care and often initiate referrals. Despite this, parental experiences are underexplored. OBJECTIVE: The aim of this study was to explore how parents perceive and manage their child's DUI and the associated healthcare process. STUDY DESIGN: A mixed-methods study was set up. In the qualitative phase, semi-structured interviews were conducted with parents of children aged 4-12

Comparative analysis of the frequency of lower urinary tract dysfunction among institutionalised and non-institutionalised children.

OBJECTIVES: To evaluate the level of symptoms of lower urinary tract dysfunction (LUTD) in orphans in institutions, and compare these data with schoolchildren of the same age group who were not institutionalised, as LUTD in children is related to factors such as social isolation and low self-esteem, with other psychological changes also being cited, although it is unknown whether these problems are primary or secondary to the symptoms of LUTD. PATIENTS AND METHODS: Children institutionalised in orphanages with no parental presence and who are isolated from a family environment are probably mo

Urinary incontinence in children.

BACKGROUND: Urinary incontinence (bedwetting, enuresis) is the commonest urinary symptom in children and adolescents and can lead to major distress for the affected children and their parents. Physiological and non-physiological types of urinary incontinence are sometimes hard to tell apart in this age group. METHODS: This article is based on selected literature retrieved by a PubMed search and on an interdisciplinary expert consensus. RESULTS AND CONCLUSION: Nocturnal enuresis has a variety of causes. The main causative factors in monosymptomatic enuresis nocturna (MEN) are an impaired abil

Delayed presentation of posterior urethral valves: a not so benign condition.

PURPOSE: Posterior urethral valves are usually detected during infancy by prenatal sonography. Rarely they may be diagnosed during later childhood, adolescence or even adulthood. Less is known about presentation and outcome in these older patients. We reviewed our experience at 4 institutions with the late presentation of posterior urethral valves. MATERIALS AND METHODS: A 13-year retrospective review revealed the late presentation of posterior urethral valves in 47 patients 5 to 35 years old (mean age 8). Data collected included presenting symptomatology, radiographic findings and renal func

Sleep Parameters and Motor Activity in Children With Nocturnal Enuresis and Daytime Urinary Incontinence.

INTRODUCTION: Nocturnal enuresis (NE) and daytime urinary incontinence (DUI) are common in childhood. They are associated with sleep disorders, which, in turn, can affect physical activity. The aim of the present study was to investigate sleep quality and motor activity in children with NE and DUI by using actigraphy as an objective assessment method. METHODS: Thirty-four children with NE or DUI, aged 5-13 years, and 33 controls were included in the study. Of the patients, 16 (47.1%) children had isolated NE and 18 (52.9%) DUI (isolated or combined with NE). Sleep quality and motor activity w

[Excretory disorders in children and adolescents].

Nocturnal enuresis unaccompanied by other excretory problems is due to a genetic CNS development disorder and responds well to cognitive-behavioral therapy and alarm interventions. If diurnal urinary incontinence is also present, various subtypes are differentiated, for the treatment of which cognitive-behavioral therapy must be supported by drugs or behavioral therapy. In the etiology of encopresis, genetic and psychosocial factors must be considered, as well as, in particular, chronic constipation. Generally, cognitive-behavioral therapeutic management is recommended. Measures aimed at regul

The impact of DSM-5 and guidelines for assessment and treatment of elimination disorders.

Elimination disorders are very common in children: 10 % of 7-year-olds wet at night (nocturnal enuresis), 2-3 % during daytime (diurnal urinary incontinence) and 1-3 % soil (faecal incontinence). In the past decades, many subtypes of elimination disorders have been identified with different symptoms, aetiologies, comorbid disorders and specific treatment options. The aim of the paper is to present a short overview of the proposed DSM-5, the ICCS and the Rome-III classification systems, of assessment and of treatment. The DSM-5 criteria no longer reflect current research data and a revision is

Female epispadias managed by bladder neck plication via a perineal approach.

Female epispadias is a rare genitourinary anomaly characterized by urinary incontinence and typical appearance of the external genitalia. Traditional surgical approaches have involved a staged genitoplasty and interval bladder neck reconstruction. We describe further experience of a promising single-stage technique using a cystoscopically guided bladder neck plication via a perineal approach with combined genitoplasty.

Total reconstruction of the lower urinary tract using bowel and the artificial urinary sphincter.

We report 3 cases in which the lower urinary tract was reconstructed using bowel. In each instance the cuff was placed around the bowel to provide a continence mechanism. The 2 patients in whom the sigmoid colon was used have sporadic diurnal incontinence with nocturnal enuresis. Urodynamic evaluation has shown that this is secondary to high pressures generated within the closed sigmoid loop. The ileocecocolonic segment is preferable for reconstruction of the lower urinary tract, since the pressure developed within the bowel is significantly lower. The use of the artificial urinary sphincter a

Eosinophilic cystitis in a 4-year-old boy: successful long-term treatment with cyclosporin A.

A 4-year-old Jewish boy presented with dysuria, urinary dribbling, increased urinary frequency, and new onset of diurnal enuresis. An infiltrating solid mass involving the entire bladder wall was found. Biopsy revealed "tumor-forming" eosinophilic cystitis, a rare bladder lesion of unclear cause. Antitoxocariasis treatment was unsuccessful. High-dose corticosteroids failed. The child's clinical condition and bladder sonographic findings continued to deteriorate. Treatment with cyclosporin A was given for 8 months, with a complete clinical, radiologic, and histopathologic cure and no side effec

[Endocrine theory of idiopathic nocturnal enuresis].

Body fluid homeostasis is maintained by the kidney. Such an accurate control in achieved via the secretion of antidiuretic hormone (ADH), the secretion of which is regulated by hypothalamic osmoreceptors. Both urine flow rate and the excretion of most electrolytes have a diurnal rhythm; they increase during daytime and decrease during nighttime. Such a rhythm seems to be absent in some subjects who suffer from bedwetting because of relative polyuria. In these cases, the polyuria is associated with a decreased nocturnal secretion of ADH and the subsequent excretion of dilated urine. A deficit i

[Orthopedic foot abnormalities as a important sign of lumbosacral spinal lipoma in an 11-year-old boy].

A case of an 11-year-old boy with giant lumbosacral spinal lipoma is presented. Hobbling on the left foot and its dropping was observed at the age of 9 after which left equinovarus foot and excavated foot deformities developed. Diurnal and nocturnal enuresis, infection of the urinary tract, and bilateral vesico-ureteral reflux were found. At birth, the patient had cleft palate and cleft upper lip, and bending of the interbuttocks groove, which is suggestive of occult spina bifida. It seems that in the case of orthopedic foot abnormalities developing in a previously healthy child, MRI investiga

Characteristics of Children Successfully Treated for Daytime Urinary Incontinence.

OBJECTIVE: A stepwise approach is recommended for the treatment of daytime urinary incontinence, with standard urotherapy as the first-line treatment, followed by pharmacological treatment when standard urotherapy is unsuccessful. This study was aimed at characterizing and comparing children with an overactive bladder and daytime urinary incontinence who became continent solely on standard urotherapy and those who achieved continence on a combination of standard urotherapy and pharmacological treatment. METHODS: This retrospective study included 221 patients with complete continence during th

Terodiline. A review of its pharmacological properties, and therapeutic use in the treatment of urinary incontinence.

Terodiline has both anticholinergic and calcium antagonist properties and, as a result, effectively reduces abnormal bladder contractions caused by detrusor instability. When administered to adult patients with urge incontinence (generally as a 25mg twice-daily dose) terodiline reduces diurnal and nocturnal micturition frequency and incontinence episodes. In studies also assessing cystometric parameters, bladder volume at first urge and bladder capacity are increased. Children with diurnal enuresis respond similarly to a daily 25mg dose. Several studies have shown that terodiline 50 mg/day is
