INTRODUCTION: Otolith function, which is dependent on linear velocity and acceleration, may be expected to change in underwater divers, who are submerged in a medium that is denser than air. The purpose of the present study was to examine possible changes in the sacculocollic reflex of professional divers and to investigate whether there might be diving-induced adaptation of the saccular response.
METHODS: We used the vestibular evoked myogenic potential (VEMP) response to evaluate saccular function in 12 professional divers shortly after a dive and after an interval of at least 24 h. The con

The trigeminal nerve and heart are strongly related through somato-autonomic nervous reflexes that induce rapid changes in cardiovascular function. Several trigeminal reflexes have been described, but the diving and trigeminocardiac reflexes are the most studied. The heart is a target organ dually innervated by the sympathetic and parasympathetic systems. Thus, how cardiac function is regulated during the trigeminal reflexes is the result of the combination of an increased parasympathetic response and increased, decreased, or unaltered sympathetic activity. Various hemodynamic changes occur as

The authors discuss about a patient who, while undergoing a routine procedure to drain a subcutaneous abscess within his forehead, suffered cardiac arrest that we conclude was caused by an activation of the diving response. This reflex affects homeostatic function which alters respiration and preferentially distributes oxygen stores to the heart and brain. Under some conditions, however, this reflex can also trigger cardiovascular collapse and death. The diving reflex is can begin with triggering receptors that are sensitive to cold water, submersion, or pressure within the nasal cavity and ot

A vagal origin of sudden death has been hypothesised in humans, but it has not yet been clearly demonstrated. Two vagal reflexes have been widely investigated: the diving reflex and the fear-induced central reaction, which are responsible for diving bradycardia and alarm bradycardia, respectively. The latter occurs in humans mainly in the context of emotional presyncope/syncope. A simultaneous occurrence of these two vagal reflexes has been observed in animals that are threatened while diving, and heart rates (HR) as low as two to six beats/min have been reported. In experiments carried out in

The mammalian diving response (DR) is a remarkable behavior that was first formally studied by Laurence Irving and Per Scholander in the late 1930s. The DR is called such because it is most prominent in marine mammals such as seals, whales, and dolphins, but nevertheless is found in all mammals studied. It consists generally of breathing cessation (apnea), a dramatic slowing of heart rate (bradycardia), and an increase in peripheral vasoconstriction. The DR is thought to conserve vital oxygen stores and thus maintain life by directing perfusion to the two organs most essential for life-the hea

BACKGROUND: Profound bradycardia during ophthalmic surgery is a rare but potentially serious event. Little is known about the predictability of the oculocardiac reflex.
METHODS: Four vagotonic maneuvers were performed on six patients who had profound oculocardiac reflex (3- to 10-second asystole) during eye surgery, the results of which were compared with 30 previously studied control subjects. Electrocardiographs were monitored during the following vagotonic maneuvers: diving response (apneic facial immersion), Valsalva maneuver, ocular compression, and carotid sinus massage.
RESULTS: The d

The diving response, an autonomic reflex characterized by apnea, bradycardia and increased peripheral vascular resistance, is initiated when animals submerge underwater. Neurons located within the trigeminal nucleus caudalis, specifically the ventral medullary dorsal horn (MDH), become activated and produce Fos after repetitive diving. It is assumed nerves innervating the nasal passages are important for activating these neurons during diving. The present study investigated the anatomical route by which nasal stimulation during diving can produce activation of MDH neurons. In rats trained to r

Healthy teens and adults performed four vagotonic maneuvers. A large series of strabismus surgery patients had deliberately quantified tension on extraocular rectus muscles during general anesthesia. The mean bradycardia was greatest for diving response (apneic facial exposure to cold) and Valsalva maneuver and least for pressure on the globe and carotid sinus massage. Bradycardia occurred for every subject for the non-surgical maneuvers, however, extraocular muscle tension frequently caused no change in heart rate or even tachycardia. The inter-subject variance in percent heart rate change wa

Stimulation of the trigeminal nerve evokes a dramatic decrease in heart rate and blood pressure, and this reflex has generally been termed the trigeminocardiac reflex. A subset of the trigeminocardiac reflex is the diving reflex in which the nasal mucosa is stimulated with water or air-borne chemical irritants. Activation of the diving reflex evokes a pronounced bradycardia, mediated by increased parasympathetic cardiac activity, and is the most powerful autonomic reflex. However, exaggeration of this protective response could be detrimental and has been implicated in Sudden Infant Death Syndr

The goal of our study was to develop a simple and practical method for simulating diving in humans using facial cold exposure and apnea stimuli to measure neural and circulatory responses during the stimulated diving reflex. We hypothesized that responses to simultaneous facial cold exposure and apnea (simulated diving) would be synergistic, exceeding the sum of responses to individual stimuli. We studied 56 volunteers (24 female and 32 male), average age of 39 years. All subjects were healthy, free of cardiovascular and other diseases, and on no medications. Although muscle sympathetic nerve

Previous studies have shown that the bradycardic response to diving is maintained or enhanced during exercise. However, the integrative mechanism by which diving-induced bradycardia supersedes exercise-induced tachycardia remains unclear. This study aimed to elucidate the contributions of central and peripheral afferent mechanisms to the diving response in humans under controlled laboratory conditions. Thirty-two healthy participants [17 males, 15 females; mean age: 22 ± 3 yr; body mass index (BMI): 24 ± 4 kg/m2] were exposed to simulated diving via trigeminal nerve stimulation (TGS) under thr

Understanding the physiology and genetics of human hypoxia tolerance has important medical implications, but this phenomenon has thus far only been investigated in high-altitude human populations. Another system, yet to be explored, is humans who engage in breath-hold diving. The indigenous Bajau people ("Sea Nomads") of Southeast Asia live a subsistence lifestyle based on breath-hold diving and are renowned for their extraordinary breath-holding abilities. However, it is unknown whether this has a genetic basis. Using a comparative genomic study, we show that natural selection on genetic vari

Trigeminocardiac reflex (TCR) is a brainstem reflex that manifests as sudden onset of hemodynamic perturbation in blood pressure (MABP) and heart rate (HR), as apnea and as gastric hypermotility during stimulation of any branches of the trigeminal nerve. The molecular and clinical knowledge about the TCR is in a constant growth since 1999, what implies a current need of a review about its definition in this changing context. Relevant literature was identified through searching in PubMed (MEDLINE) and Google scholar database for the terms TCR, oculocardiac reflex, diving reflex, vasovagale resp

In light of the overwhelming popularity of self-contained underwater breathing apparatus (SCUBA) diving, general dental practitioners should be prepared to address complications arising as a result of diving and to provide patients with accurate information. The aim of this article was to introduce the concepts of diving medicine and dentistry to the dentist, and to supply the dental practitioner with some diagnostic tools as well as treatment guidelines. The literature was reviewed to address diving barotrauma (pressure-induced injury related to an air space) to the head, face and oral region

Marine mammals have adapted to forage while holding their breath in a suite of aquatic habitats from shallow rivers to deep oceans. The key to tolerate such extensive apnea is the dive response, which comprises bradycardia and peripheral vasoconstriction. Although initially considered an all-or-nothing reflex [1], numerous studies on freely diving marine mammals have revealed substantial dynamics of the dive response to meet the impending dive demands of depth, duration and exercise [2]. Such adjustments are not only autonomic responses, but are under acute cognitive control in pinnipeds [3] l

INTRODUCTION: Gas narcosis impairs divers when diving deeper. Pupillometry is sensitive to alcohol intoxication and it has been used in anaesthesia to assess nitrous oxide narcosis. It is a potential novel method to quantify narcosis in diving. The aim of this study was to evaluate pupillometry for objective measurement of narcosis during exposure to hyperbaric air or nitrous oxide.
METHOD: Pupil size in 16 subjects was recorded directly at surface pressure and during air breathing at 608 kPa (equivalent to 50 metres' seawater depth) in a hyperbaric chamber. Another 12 subjects were exposed t

Lowering heart rate while diving helps marine mammals regulate blood pressure while redistributing blood flow and conserving oxygen during extended dives. However, the classic characterization of this dive response-as a pronounced and abrupt reduction in heart rate observed during forced dives in laboratory settings-contrasts with the higher minimum heart rates and oscillatory patterns observed in freely diving marine mammals in the wild. To assess this apparent discrepancy in cardiovascular control, we measured the heart rates of three Steller sea lions (Eumetopias jubatus) using subcutaneous

Cardiovascular responses to diving are characterized by two opposing responses: tachycardia resulting from exercise and bradycardia resulting from the apnea. The convergence of bradycardia and tachycardia may determine the cardiovascular responses to diving. The purpose of this study was to investigate the interaction of breath holding and muscle mechanoreflex on cardiovascular responses in breath-hold divers (BHDs) and non-BHDs. We compared the cardiovascular responses to combined apnea and the mechanoreflex in BHDs and non-BHDs. All participants undertook three trials-apnea, passive leg cycl

Most terrestrial animals demonstrate an autonomic reflex that facilitates survival during prolonged submersion under water. This diving response is characterized by bradycardia, apnea and selective increases in peripheral vascular resistance. Stimulation of the nose and nasal passages is thought to be primarily responsible for providing the sensory afferent signals initiating this protective reflex. Consequently, the primary objective of this research was to determine the central terminal projections of nerves innervating the external nose, nasal vestibule and nasal passages of rats. We inject

The dive response, or the 'master switch of life', is probably the most studied physiological trait in marine mammals and is thought to conserve the available O2 for the heart and brain. Although generally thought to be an autonomic reflex, several studies indicate that the cardiovascular changes during diving are anticipatory and can be conditioned. The respiratory adaptations, where the aquatic breathing pattern resembles intermittent breathing in land mammals, with expiratory flow exceeding 160 litres s-1 has been measured in cetaceans, and where exposure to extreme pressures results in alv
