Efficacy and Safety of Hyperbaric Oxygen Therapy for Retinal Artery Occlusion: A Systematic Review and Meta-Analysis.

BACKGROUND: Retinal artery occlusion (RAO) is a vision-threatening condition with limited therapeutic options. Hyperbaric oxygen therapy (HBOT) has emerged as a potential treatment to enhance retinal oxygenation and salvage ischemic tissue, though its efficacy and safety remain debated. METHODS: We followed the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines and the Cochrane Handbook for Systematic Reviews of Interventions. Databases were searched through November 2024 for studies comparing HBOT with control in RAO patients. Risk of bias was assessed usi

A retrospective review of divers treated for inner ear decompression sickness at Fiona Stanley Hospital hyperbaric medicine unit 2014-2020.

INTRODUCTION: Inner ear decompression sickness (IEDCS) is increasingly recognised in recreational diving, with the inner ear particularly vulnerable to decompression sickness in divers with a right-to-left shunt, such as is possible through a persistent (patent) foramen ovale (PFO). A review of patients treated for IEDCS at Fiona Stanley Hospital Hyperbaric Medicine Unit (FSH HMU) in Western Australia was performed to examine the epidemiology, risk factors for developing this condition, the treatment administered and the outcomes of this patient population. METHODS: A retrospective review of

The Gradient Perfusion Model Part 3: An extraordinary case of decompression sickness.

INTRODUCTION: Decompression sickness (DCS) has been associated with unusual circumstances such as breath-hold diving, shallow depths, and short bottom times. We report a case of DCS with an extraordinary cause and course. MATERIALS AND METHODS: A 72-year-old healthy Hispanic female was referred to our 24/7 Hyperbaric Medicine Unit for emergency hyperbaric oxygen recompression treatment (HBO2 RCT) after developing lower-extremity paralysis following a hyperbaric air exposure in a homemade hyperbaric chamber. RESULTS: After an uneventful exposure to hyperbaric air at a maximum 72-foot depth (3

CNS function and dysfunction during exposure to hyperbaric oxygen in operational and clinical settings.

Hyperbaric oxygen (HBO2) is breathed during hyperbaric oxygen therapy and during certain undersea pursuits in diving and submarine operations. What limits exposure to HBO2 in these situations is the acute onset of central nervous system oxygen toxicity (CNS-OT) following a latent period of safe oxygen breathing. CNS-OT presents as various non-convulsive signs and symptoms, many of which appear to be of brainstem origin involving cranial nerve nuclei and autonomic and cardiorespiratory centers, which ultimately spread to higher cortical centers and terminate as generalized tonic-clonic seizures

Return to an extreme environment exposure after pulmonary infection: SCUBA diving as a model?

The human body implements several adaptive mechanisms to perform in extreme environments. Increasingly, individuals return to such conditions after pulmonary infections, raising concerns about potential long-term respiratory consequences. This study aims to investigate the return to SCUBA diving after pulmonary infection, using COVID-19 as a model to provide evidence for safe reintegration into extreme environments, with a focus on vascular, oxidative and vascular stress. Twenty-three expert SCUBA divers who had symptomatic COVID-19 (SARS-CoV-2 Alpha variant) were enrolled as studied group (CO

Validation of Hyperbaric Pressure System with Xenon Anesthesia for Drosophila melanogaster.

Under hyperbaric conditions, the elevated concentrations of the ambient gases may elicit a range of behavioral responses in various animals. A substantial portion of these behaviors represents changes in cognitive states that are yet poorly understood. One notable example is general anesthesia. The behavioral effect of several gases under hyperbaric conditions in general anesthesia, or narcosis, has been a long-lasting scientific inquiry, despite the overwhelmingly frequent use of the drugs in medicine. Xenon, classified as a noble gas and an anesthetic agent, is regarded as one of the safest

Decompression sickness after a highly conservative dive in a diver with known persistent foramen ovale: Case report.

A diver returned to diving, 15 months after an episode of neuro-spinal decompression sickness (DCS) with relapse, after which she had been found to have a moderate to large provoked shunt across a persistent (patent) foramen ovale (PFO), which was not closed. She performed a single highly conservative dive in line with the recommendations contained in the 2015 position statement on PFO and diving published jointly by the South Pacific Underwater Medicine Society and the United Kingdom Sports Diving Medical Committee. An accidental Valsalva manoeuvre shortly after surfacing may have provoked in

Bipolar spectrum disorders in divers: risks, recognition, and recommendations.

Bipolar disorder is a recurrent psychiatric condition characterised by episodic mood disturbances, residual functional impairment, and high rates of psychiatric and medical comorbidity. While many individuals achieve symptomatic remission, persistent deficits in cognition, emotional regulation, and insight may remain, raising concerns for participation in safety-critical activities such as scuba diving. This systematic review synthesised evidence from psychiatric, occupational, aviation, and diving medicine literature to examine the clinical course of bipolar disorder, treatment considerations

Oxygen treatment and retrieval pathways of divers with diving-related conditions in Townsville, Australia: a 15-year retrospective review.

INTRODUCTION: First aid for injured divers includes oxygen delivery prior to definitive care. Delay to specialist assessment and/or hyperbaric oxygen treatment (HBOT) may be due to dive site remoteness and limited access to facilities. Townsville has the only hyperbaric facility along the Great Barrier Reef. Analysis of oxygen therapy and retrieval pathways of divers treated in Townsville may assist with establishing future education strategies and resource allocation. METHODS: Data were retrospectively collected on divers assessed at the Townsville hyperbaric medicine unit from November 2003

Testing the Emergency Evacuation Hyperbaric Stretcher for Fit in Small Military and Civilian Helicopters.

INTRODUCTION: Hyperbaric oxygen (HBO2) therapy is essential for the treatment of diving injuries in the field, but typically requires larger recompression chambers with limited porta-bility. The Emergency evacuation hyperbaric stretcher (EEHS) offers a portable solution for prehospital HBO2, particularly in remote or austere environments. However, it's compatibility with smaller evacuation aircraft remains uncertain. METHODS: A commercially available EEHS (Hyperlite 1) was tested for fit and operational computability in both a U.S. Coast Guard Air-bus MH-65 "Dolphin" and an Airbus H145 helico

Early nitrogen wash-out for inside attendants during hyperbaric oxygen therapy: a novel oxygen distribution regimen.

INTRODUCTION: Hyperbaric oxygen therapy (HBOT) is widely used as treatment of decompression sickness (DCS). The patient breathes oxygen at an elevated oxygen partial pressure of up to 280 kPa, whereas any inside attendant (IA) normally breathes air. This results in nitrogen uptake and tissue loading in the IA, which must be considered during decompression. The US Navy Treatment Table 6, and its equivalent used by the Swedish Armed Forces, prescribe periods of oxygen breathing prior to and during decompression to reduce the risk of DCS for the IA. However, since these oxygen periods are concent

Dysbaric osteonecrosis: Case series osteonecrosis in divers.

INTRODUCTION: This is the first published study examining decompression illness (DCI) among divers in Algeria. DCI is a complex syndrome caused by bubbles of inert gas that form in the body due to an improper decompression process, following exposure to increased pressure underwater. Algerian diving conditions pose significant challenges to divers. The study focuses on red coral diving, a popular and long-standing practice in eastern Algeria. We present three case reports describing dysbaric osteonecrosis of the humeral head with varying degrees of severity and presentation. Dysbaric osteonecr

Hyperbaric oxygen therapy modulates immune effector responses and reshapes peripheral immune tolerance: a narrative review.

Hyperbaric oxygen therapy (HBOT) refers to an intervention in which patients inhale near-100% oxygen at pressures exceeding 1 atmosphere absolute to increase plasma and tissue oxygen partial pressure. HBOT has been applied clinically across a broad range of conditions, including infections, inflammation, hypoxia-related injury, and malignancies. However, its immunological effects are often reduced to a binary notion of "immune enhancement" or "immunosuppression". Moreover, substantial heterogeneity in treatment parameters and immune endpoints across studies has limited the development of a uni

Mild and Traditional Hyperbaric Oxygenation: Clinical Uses and Safety Considerations.

INTRODUCTION: Unlike traditional hyperbaric oxygen treatment, which involves the inhalation of 100% oxygen by individuals enclosed in a pressurized treatment chamber at greater than sea level, mild hyperbaric oxygen treatment is performed in a pressurized chamber using shallower treatment depths and lower oxygen concentrations. METHODS: This narrative review examines the physiological differences between mild and traditional hyperbaric oxygen treatment, the clinical uses of mild hyperbaric oxygen treatment, and the safety considerations of both treatment modalities. RESULTS: The systemic hyp

Tension Pneumothorax During Hyperbaric Oxygen Therapy With Pigtail Pleural Catheter: A Case Report and Experimental Flow Analysis.

INTRODUCTION: Tension pneumothorax is a life-threatening complication during hyperbaric oxygen treatment (HBO₂). The presence of a pleural catheter is assumed to mitigate this risk, but its efficiency under hyperbaric conditions remains unclear. CASE REPORT: We describe a 37-year-old female undergoing HBO₂ with a pigtail pleural catheter in situ for secondary pneumothorax. The patient developed respiratory and cardiovascular symptoms during decompression. To investigate gas flow dynamics, we conducted an experimental analysis measuring flow resistance in pigtail catheters under chamber pressu
