Drug-eluting stents for treatment of focal infrapopliteal lesions.

We have investigated the role of drug-eluting stents on patency rates after treatment of focal infrapopliteal lesions in patients with intermittent claudication and critical limb ischemia. Reports indicate that drug-eluting stents reduce the risk of restenosis after percutaneous infrapopliteal artery revascularization. A Pub Med, EMBASE, Cochrane database review search of non-randomized studies investigating patency rates, target lesion revascularisation rates, limb salvage rates and mortality rates in an up to 3-year follow-up period after drug-eluting stent placement was conducted. Catastrophic early drug eluting stents thrombosis and aspirin hypersensitivity.

Stent thrombosis is a feared complication of percutaneous coronary intervention. Promises and problems, late complications and early stent thrombosis have been reported after drug eluting stents implantation too. Moreover some patients with imperative cardiologic indications for combination therapy with aspirin and clopidogrel (stent placement and/or acute coronary syndrome) have a history of allergy to aspirin. We present a case of catastrophic early drug eluting stents thrombosis in a 79-year-old Italian woman with aspirin hypersensitivity.

Drug eluting stents: managing coronary artery stenosis following PTCA.

Drug eluting stents (DES) release drugs that inhibit tissue growth in narrowed coronary arteries in an effort to prevent restenosis, a renarrowing of the artery. Several types of DES are under investigation in clinical trials; however, none are currently approved for use in Canada. Preliminary trial data are encouraging, demonstrating greater lumen diameter and reduced restenosis with DES versus uncoated stents. If DES prove to be more effective than uncoated stents in the treatment and/or prevention of restenosis, this technology may diffuse rapidly. The total health care costs, including the

Polymer-free drug-eluting stents for coronary artery disease.

Polymer-free drug-eluting stents (PF-DES) were designed with the expectation of avoiding late restenosis and thrombosis related to the polymer used in traditional DES platforms. Furthermore, due to similarities with bare metal stents after drug elution, PF-DES has been considered as particularly suitable for patients at high bleeding risk. A variety of PF-DES platforms have been clinically tested. Despite their theoretical advantages, PF platforms showed comparable clinical outcomes with modern permanent- or biodegradable polymer-based DES up to 5 years after implantation. The Newest Generation of Drug-eluting Stents and Beyond.

There has been a great evolution in the development of coronary stents in order to avoid both restenosis and thrombosis. Improvements have led to improvements in the design and conformation of metallic or resorbable structures, with an adequate balance between trackability and radial force, the development of antiproliferative drugs and the polymers to control release and allow adequate endothelialisation and an optimal duration of the antiplatelet regimen. Some suggestions are provided about the ideal characteristics of future coronary stents.

Newly available and recent advances in drug-eluting stents.

The introduction of drug-eluting stents has revolutionized the treatment of coronary artery disease. First-generation stents, however, were associated with relatively high revascularization rates and the risk of stent thrombosis. This has led to a worldwide search for improvements in stent technology that will reduce adverse cardiac events following stent implantation while providing optimal treatment. Here, the authors discuss recent advances in stent technology from improvements in stent platform and stent polymer to newer mechanisms of drug delivery and the incorporation of proendothelizing

Inflammatory mechanisms of adverse reactions to drug-eluting stents.

In-stent restenosis and stent thrombosis represent the main adverse reactions to coronary stents and individual susceptibility appears to play an important role in their onset. In particular, inflammatory status, classically assessed by C-reactive protein levels, predicts the risk of in-stent restenosis after bare-metal stent implantation but not after drug-eluting stent (DES) implantation. On the other hand, C-reactive protein seems to predict the risk of stent thrombosis after treatment with DES but not with bare-metal stent. If DES have considerably reduced, as compared to bare-metal stent,

Are drug-eluting stents the future of SFA treatment?

Drug-eluting stent (DES) technology was developed to prevent early thrombosis and late luminal loss to potentially improve long-term patency rates. Although favorable DES results have recently become available with the Zilver PTX and STRIDES studies, the high price of DES is a major drawback for this technology to become the golden standard for peripheral endovascular therapy in de novo femoro-popliteal (FP) lesions. Nevertheless, DES has the potential to make the difference and to establish itself as an important treatment option in patients presenting with TASC C&D FP lesions who are at high

Drug-eluting stents in octogenarians: early and intermediate outcome.

BACKGROUND: Large randomized controlled trials have demonstrated that percutaneous coronary intervention with the routine use of drug-eluting stents is safe and effective. However, octogenarians are usually excluded from these trials. METHODS: We analyzed 2453 consecutive patients who underwent DES implantation at the Mayo Clinic (Rochester, MN). The patients were classified in 2 age groups: patients > or = 80 years of age < or = 79 years of age. RESULTS: Patients > or = 80 years old had significantly more adverse baseline characteristics including more comorbid conditions and more severe ex

Clinical outcomes with drug-eluting stents following atheroablation therapies.

BACKGROUND: Prior studies of atheroablation (directional atherectomy, rotational atherectomy and laser angioplasty) have demonstrated either no advantage or worse outcomes relative to conventional balloon angioplasty. Because these techniques are still required in a minority of patients, we hypothesized that the use of drug-eluting stents (DES) would minimize the rate of major adverse cardiac events (MACE) after atheroablation. METHODS: From 2,252 percutaneous coronary intervention procedures, 212 patients were extracted using case control matching and were analyzed to compare the rate of MAC

Drug-eluting stents to prevent reblockage of coronary arteries.

Restenosis limits the success of percutaneous transluminal coronary interventions. Coronary artery stenting decreases restenosis, improves outcomes, and is currently the most commonly used percutaneous coronary intervention in the United States. However, in-stent restenosis continues to occur at an unacceptable rate. In-stent restenosis is a neointimal hyperplastic response resulting primarily from vascular smooth muscle cell proliferation. Treatment with anti-proliferative agents presents a logical approach to eradicating restenosis, however, these drugs are highly toxic. Coating stents with

Drug-eluting stents in peripheral vascular disease: eliminating restenosis.

Transcatheter endovascular therapy for peripheral atherosclerotic disease has become more popular. In general, good results have been reported in focal aortoiliac disease. However, the long-term patency of angioplasty in longer, more distal lesions has been less satisfactory. Stenting has not been shown to improve long-term patency compared to angioplasty alone. Drug-eluting stents have shown promise in preventing coronary restenosis, and preliminary results in peripheral arterial disease are encouraging. This review article will discuss the current status of endovascular therapy of aortoiliac

Bare Metal Stents Versus Drug Eluting Stents: Where Do We Stand in 2015?

The development of bare metal stent (BMS) was a major advancement over plain old balloon angioplasty (POBA) in the management of symptomatic coronary artery disease. BMS prevented restenosis by attenuating early arterial recoil and contraction; both seen commonly after POBA. However, the rate of clinically indicated target lesion repeat revascularization due to a process of in-stent restenosis (ISR) at 1 year remained relatively high (10 to 20 %), often due to excessive neointimal growth (Fischman et al. Preventing leg amputations in critical limb ischemia with below-the-knee drug-eluting stents: the PaRADISE (PReventing Amputations using Drug eluting StEnts) trial.

OBJECTIVES: We investigated the efficacy and safety of using balloon expandable drug-eluting stents (DES) to prevent amputations in patients with below-the-knee critical limb ischemia. BACKGROUND: Critical limb ischemia patients have a 1-year amputation rate of 30% and a mortality rate of 25%. Most patients with critical limb ischemia have severe below-the-knee arterial disease that limits the use of bypass surgery or balloon angioplasty. METHODS: In all, 106 patients (118 limbs) were treated with DES in this prospective, nonrandomized trial. No patients were excluded because of comorbiditie

Bayesian network meta-analysis of nitinol stents, covered stents, drug-eluting stents, and drug-coated balloons in the femoropopliteal artery.

OBJECTIVE: Several randomized controlled trials (RCTs) have shown the superiority of some of these technologies over balloon angioplasty, but direct comparisons between these treatment options are lacking. The authors conducted a network meta-analysis of RCTs comparing bare nitinol stents, covered nitinol stents, paclitaxel- or sirolimus-eluting stents (PES or SES), and paclitaxel-coated balloons (PCB) with plain balloon angioplasty or with each other in the femoropopliteal artery (PROSPERO registry: CRD42013004845).
