An innovative surgical approach for solid pseudopapillary neoplasm with duodenal invasion in a pediatric patient: a case report.

BACKGROUND: Pediatric pancreatic tumors, especially with duodenal invasion, are exceptionally rare and a strategy for their treatment has not been established. A pancreaticoduodenectomy is often the desired treatment, but may be over-invasive for solid pseudopapillary neoplasm (SPN). This study reports an innovative surgical approach for SPN with duodenal invasion using pancreatic enucleation and endoscopically guided partial duodenectomy. CASE PRESENTATION: An 11-year-old girl complained of malaise and presented with severe anemia; imaging revealed a tumor of undetermined origin, involving t

Synchronous Pancreatic Serous Cystic Neoplasm and Duodenal Neuroendocrine Tumor: Case Report and Review of the Literature.

Simultaneous presence of pancreatic serous cystic neoplasms and neuroendocrine neoplasms is rare. We present a case with the incidental finding of a duodenal neuroendocrine tumor (NET) with 2 lymph node metastases in a Whipple resection specimen performed to remove a pancreatic cystic neoplasm that postoperatively turned out to represent a serous cystic neoplasm (SCN). The patient was a 75-year-old female. She presented with loss of appetite and weight. Preoperative contrast-enhanced computed tomography scan of the abdomen showed a multicystic lesion in the head of pancreas. On histologic exam

Neurofibromatosis 1 presenting with multiple duodenal GISTS associated with a somatostatin-producing D cell neoplasm.

The co-existence of a duodenal somatostatin-producing D cell neoplasm and multiple duodenal gastrointestinal stromal tumours (GISTs) in a 61-year-old woman with neurofibromatosis type 1 is reported. Histologically, the D cell neoplasm showed a glandular pattern with psammoma bodies and was metastatic to regional lymph nodes and liver at the time of surgery. Tumour cells were monomorph and showed intense and diffuse immunoreactivity for somatostatin, focal positivity for calcitonin, while were negative for other gastroenteropancreatic hormones including insulin, glucagon, pancreatic polypeptide

Synchronous duodenal neuroendocrine neoplasm and congenital factor XIII deficiency: case report and review of the literature.

BACKGROUND: Neuroendocrine neoplasms (NENs) are uncommon, with duodenal NENs (dNENs) being particularly rare in clinical practice. Congenital factor XIII deficiency (FXIIID) is also an extremely rare hematological disease in which poor wound healing may occur due to coagulopathy. The concurrent occurrence of these two rare diseases has not been reported before, which increases the difficulty of diagnosis and treatment. This is the first report of dNEN concomitant with Congenital FXIIID, which can present as a reference for clinicians who may encounter similar situations in the future. CASE DE

Resection of a recurrent solid pseudopapillary neoplasm of the pancreas after duodenal sparing pancreaticoduodenectomy: A case report.

INTRODUCTION: Solid Pseudopapillary Neoplasm (SPN) is a rare pancreatic neoplasm with low malignant potential and a relative indolent course. Complete resection of the SPN is curative for most cases and has a high survival rate. Recurrences, though rare, can still occur despite adequate resection. Pancreaticoduodenectomy is commonly performed to treat pancreatic head SPNs. In recent years, duodenum-preserving pancreatic head resection (DPPHR) has been reported as a less radical and acceptable alternative. CASE PRESENTATION: We are reporting a case of 26-year old female who presented with a 7

[Recurrent duodenal ulcer bleeding as the first manifestation of a solid pseudopapillary neoplasm of the pancreas with hepatic metastases].

HISTORY AND ADMISSION FINDINGS: In a 17-year-old girl recurrent duodenal ulcer bleeding had led to severe anemia. INVESTIGATIONS: Sonography and computed tomography revealed a partially cystic tumour of the pancreatic head and suspicious hepatic lesions. TREATMENT AND PATHOLOGICAL DIAGNOSIS: A partial duodenopancreatectomy was performed and two liver metastases were resected. Histological examination of the resected pancreatic specimen revealed a solid pseudopapillary neoplasm of the pancreas (SPN) with hepatic metastases. CLINICAL COURSE AND PROGNOSIS: The seven remaining liver metastases we

Duodenal neuroendocrine neoplasms on enhanced CT: establishing a diagnostic model with duodenal gastrointestinal stromal tumors in the non-ampullary area and analyzing the value of predicting prognosis.

OBJECTIVE: To identify CT features and establish a diagnostic model for distinguishing non-ampullary duodenal neuroendocrine neoplasms (dNENs) from non-ampullary duodenal gastrointestinal stromal tumors (dGISTs) and to analyze overall survival outcomes of all dNENs patients. MATERIALS AND METHODS: This retrospective study included 98 patients with pathologically confirmed dNENs (n = 44) and dGISTs (n = 54). Clinical data and CT characteristics were collected. Univariate analyses and binary logistic regression analyses were performed to identify independent factors and establish a diagnostic m

Feasibility of pancreatic duct stent placement before endoscopic submucosal dissection for superficial duodenal neoplasms adjacent to the papilla.

BACKGROUND: Endoscopic submucosal dissection (ESD) for superficial non-ampullary duodenal epithelial tumors (SNADETs) is technically challenging and is associated with a risk of adverse events, particularly when lesions are located near the major papilla. Pancreatic duct (PD) stent may reduce the risk of post-ESD pancreatitis; however, no standard strategy has been established. This study aimed to evaluate the effectiveness and safety of PD stent placement combined with ESD for SNADETs near the major papilla. METHODS: This was a retrospective study of duodenal ESD after prophylactic PD stent

Primary tumor size as a prognostic factor in duodenal neuroendocrine neoplasms ≤20 mm: a population-based SEER analysis.

OBJECTIVE: The prognostic value of primary tumor size in duodenal neuroendocrine neoplasms (dNENs) ≤20 mm remains uncertain. Although size is used to guide management, its independent effect on survival is unclear. DESIGN AND METHODS: We performed a population-based study using the US SEER database (1975-2021), including adults with histologically confirmed dNENs ≤20 mm. Disease-specific survival (DSS) was the primary endpoint. Associations between tumor size, stage, lymph-node involvement, and DSS were evaluated using chi-squared tests, logistic regression, Kaplan-Meier curves, and Cox model

Short- and long-term outcomes of laparoscopic endoscopic cooperative surgery for duodenal neoplasms: a retrospective single-center study.

BACKGROUND: Non-ampullary duodenal tumors are relatively rare; however, advancements in esophagogastroduodenoscopy have improved their detection. Endoscopic resection is effective for treating adenomas and early-stage cancers without lymph node involvement. However, challenges include assessing tumor invasion depth, achieving complete resection, and managing postoperative complications such as delayed perforation. Consequently, laparoscopic endoscopic cooperative surgery (D-LECS) was introduced, but data on its long-term outcomes remain limited. METHODS: This retrospective study reviewed 18 c

Non-ampullary duodenal mixed neuroendocrine-non-neuroendocrine neoplasm initially diagnosed as adenocarcinoma: a case report.

INTRODUCTION: Mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) are rare tumors composed of neuroendocrine and non-neuroendocrine components. Their occurrence in the duodenum is extremely uncommon. Preoperative diagnosis is difficult because the neuroendocrine component may not be detected in biopsy specimens. We report a rare case of non-ampullary duodenal MiNEN initially diagnosed as adenocarcinoma. CASE PRESENTATION: A 56-year-old man was found to have a tumor in the duodenal bulb during routine endoscopic examination. Biopsy revealed moderately to poorly differentiated adenocarci

Successful Non-surgical Treatment for a Duodenal Perforation Caused by the Rupture of an Intraductal Papillary Mucinous Neoplasm of the Pancreas.

An intraductal papillary mucinous neoplasm (IPMN) was incidentally detected in a 91-year-old woman 12 years before her current admission. The patient declined additional examinations and surgical intervention. Over the subsequent years, she experienced recurrent acute pancreatitis and ultimately developed a gastrointestinal perforation due to the rupture of the IPMN. Considering her age, general condition, and personal wishes, conservative management was selected, which allowed her to successfully resume oral intake until death from cancer. This rare clinical course provides valuable insights

Controversies in NEN: An ENETS position statement on the endoscopic management of localised gastric, duodenal and rectal neuroendocrine neoplasms.

Gastric, duodenal and rectal neuroendocrine tumours (NETs) are increasingly detected due to advances in endoscopic imaging. While international guidelines provide criteria for endoscopic management, several aspects remain controversial due to limited high-quality evidence. This position paper, developed by an expert panel, aims to clarify these unresolved issues and provide consensus-based recommendations. The primary objective of this position paper is to critically analyse and address key controversies in the endoscopic management of gastro-duodenal-rectal NETs. These include the optimal sel

The Value of Ultrasound and Contrast-Enhanced Ultrasound in a Young Asymptomatic Patient With Duodenal Neuroendocrine Neoplasms: A Case Report.

Conventional ultrasound(US) plays an important role in check-up, while contrast-enhanced ultrasound(CEUS) is a valuable tool for distinguishing tumor characteristics and guiding subsequent diagnosis and treatment. We present a rare case of duodenal neuroendocrine neoplasms (NENs) with liver metastasis in a young male, whose liver masses were detected by US during a check-up and then subsequently underwent Sonazoid-CEUS examination in the outpatient department. The lesions in the liver showed rapid hyper-enhancement in the arterial phase followed by early washout in the portal venous phase, fin

Two case of bilateral approach in laparoscopic pancreas-sparing distal duodenectomy for duodenal neoplasms arising from the distal duodenum.

INTRODUCTION: Laparoscopic pancreas-sparing distal duodenectomy is a less invasive surgical therapy; however, the anatomical complexity of the duodenum increases the difficulty of laparoscopic procedures. We introduce our technique for laparoscopic pancreas-sparing distal duodenectomy for distal duodenal tumors. PRESENTATION OF CASES: A first patient was 47-year-old woman who had 30 mm of duodenal tumor which located in third portion of duodenum. A second patient was 66-year-old man who had 35 mm of submucosal tumor which located in the third portion of duodenum. Laparoscopic pancreas-sparing

An Aggressive Primary Retroperitoneal Diffuse Large B-Cell Lymphoma Mimicking a Pancreatic Neoplasm, Presenting as Duodenal Stenosis.

Diffuse Large B-Cell Lymphoma (DLBCL) is the most common histological subtype of Non-Hodgkin's Lymphoma (NHL). Primary retroperitoneal DLBCL is uncommon and has seldom been reported. Extrinsic compression of the duodenum due to lesions originating from the retroperitoneum is also rare. We present a case of a 39-year-old man who presented with inability to tolerate oral intake, abdominal pain, an upper abdominal mass and postprandial bilious vomiting caused by a large DLBCL arising from the retroperitoneum causing extrinsic compression of the duodenum. The cause of compression was initially pre

Fine-needle aspiration biopsy of an intra-abdominal inflammatory mass secondary to a penetrating duodenal ulcer mimicking neoplasm.

Peptic ulcers complicated by penetration into the surrounding tissue and presenting as an intra-abdominal mass simulating a malignancy are uncommon. We report on a case of a 56-yr-old physician with a 40-yr history of peptic disease. Due to recent and transitory right flank pain, an ultrasound then a computed tomography scan demonstrated a 4-cm retropancreatic mass. A fine-needle aspiration biopsy (FNAB) of the mass showed an acute and chronic inflammatory exudate. After a course of medical treatment for peptic ulcer disease and 5 mo after the biopsy, the mass remarkably decreased in size. In

Neuroendocrine tumor chromogranin A response following synthetic somatostatin analog (lanreotide): Early observations from an isolated duodenal neoplasm .

Neuroendocrine tumors (NETs) of duodenal origin are an unusual subset among all NETs, comprising only about 3% of this neoplasm class. In general, NETs are characterized by overexpression of somatostatin receptors and carry an excellent prognosis with early diagnosis and intervention. Chromogranin A (CgA), a protein originating in secretory vesicles of neurons and endocrine cells, has gained wide usage in NET diagnosis and surveillance. Lanreotide is a synthetic octapeptide somatostatin analog with potent anti-proliferative action which has been approved by the FDA (U.S.) and EMA (E.U.) for NE
