Dynamic Modified Early Warning Score for predicting postoperative mortality in acute type A aortic dissection: a retrospective study.

BACKGROUND: The Modified Early Warning Score (MEWS) is a valuable tool for predicting in-hospital mortality, yet it is seldom applied to acute type A aortic dissection (AAAD). This study aimed to evaluate the predictive accuracy of MEWS at different times within the first 24 hours after AAAD surgery, and to investigate the association between MEWS trajectories and postoperative mortality. METHODS: This retrospective study included 279 patients who underwent AAAD surgery from January 2020 to December 2022. MEWS was calculated at 0, 4, 8, 12, 16, 20, and 24 hours postoperatively. Predictive val

Respiratory National Early Warning Score for 28-day mortality prediction in suspected sepsis patients in the emergency department.

BACKGROUND: Most early warning scores were derived in heterogeneous acutely ill populations and are not specifically tailored to patients with suspected sepsis in the emergency department (ED). In sepsis, respiratory rate (RR) is frequently elevated as part of the pathophysiologic response, suggesting that sepsis-focused tools may require recalibrated RR thresholds. We therefore aimed to develop and evaluate the Respiratory National Early Warning Score (R-NEWS), a sepsis-oriented modification emphasizing respiratory parameters, to improve prediction of 28- and 7-day mortality among ED patients

Clinical performance of early warning scoring systems for identifying sepsis among anti-hypertensive agent users.

BACKGROUND: Little is known about the accuracy of the quick Sequential Organ Failure Assessment (qSOFA) and the National Early Warning Score (NEWS) in identifying sepsis patients with a history of hypertension on anti-hypertensive agents, which affect vital signs as components of the scoring systems. We aimed to examine the ability of qSOFA and NEWS to predict sepsis among anti-hypertensive agent users by comparing them with non-users. METHODS: We retrospectively identified adult patients (aged ≥18years) with suspected infection who presented to an emergency department (ED) of a large tertiar

Early Warning Scores for Identification of Patients at Risk of Clinical Deterioration in the Emergency Department: A Narrative Literature Review.

INTRODUCTION: Emergency department (ED) overcrowding may delay recognition and treatment of patients at risk of clinical deterioration. In this context, early warning scores (EWSs) may support the early identification of high-risk patients and help prioritize timely assessment and intervention. AIM: This narrative literature review aimed to identify the most effective EWSs for detecting clinical deterioration in ED patients and to determine which best predicts short-term mortality. METHODS: The search was conducted using the Medline, Web of Science, Scopus, and Cochrane databases between Apr

Consensus for the Development of a New Early Warning Score for Predicting Patients' Clinical Deterioration in Angola: A Delphi Study.

Background: Nearly 30 years since its inception, the early warning scores (EWSs) remain pivotal, yet variations have emerged for hospital and prehospital use. Aggregated scores, reflecting multiple physiological parameters, outperform single-parameter systems in assessing acute illness severity, though consensus on optimal approaches is lacking. Resource-limited countries, including Angola, lack adapted EWSs, emphasizing the need for cost-effective and adaptable solutions to enhance patient care. Objective: To explore the perspectives of Angolan experts to identify physiological parameters sui

National early warning score 2 plus non-invasive capnography and perfusion index to estimate poor outcomes in emergency departments.

BACKGROUND: The study of the inclusion of new variables in already existing early warning scores is a growing field. The aim of this work was to determine how capnometry measurements, in the form of end-tidal CO2 (ETCO2) and the perfusion index (PI), could improve the National Early Warning Score (NEWS2). METHODS: A secondary, prospective, multicenter, cohort study was undertaken in adult patients with unselected acute diseases who needed continuous monitoring in the emergency department (ED), involving two tertiary hospitals in Spain from October 1, 2022, to June 30, 2023. A protocol for developing early warning score models from vital signs data in hospitals using ensembles of decision trees.

INTRODUCTION: Multiple early warning scores (EWS) have been developed and implemented to reduce cardiac arrests on hospital wards. Case-control observational studies that generate an area under the receiver operator curve (AUROC) are the usual validation method, but investigators have also generated EWS with algorithms with no prior clinical knowledge. We present a protocol for the validation and comparison of our local Hamilton Early Warning Score (HEWS) with that generated using decision tree (DT) methods. METHODS AND ANALYSIS: A database of electronically recorded vital signs from 4 medica

ELTS Score and Early "Warning" Define a Two-Stage Risk Framework for Failure-Free Survival in Chronic-Phase CML.

Risk stratification to predict treatment failure in chronic-phase chronic myeloid leukemia (CP-CML) remains an important challenge in the tyrosine kinase inhibitor (TKI) era. We conducted a pooled analysis of 710 newly diagnosed CP-CML patients enrolled in the prospective JALSG CML212 and New TARGET studies, approximately 80% of whom received second-generation TKIs (2G-TKIs) as first-line therapy, to identify predictors of failure-free survival (FFS). In this study, we incorporated the ELN 2025-defined "Warning" category as an initial adverse state in the definition of FFS events and evaluated

The variable relationship between the National Early Warning Score on admission to hospital, the primary discharge diagnosis, and in-hospital mortality.

BACKGROUND: Patients with an elevated admission National Early Warning Score (NEWS) are more likely to die while in hospital. However, it is not known if this increased mortality risk is the same for all diagnoses. The aim of this study was to determine and compare the increased risk of in-hospital mortality associated with an elevated NEWS and different primary discharge diagnoses in unselected emergency admissions to a UK university teaching hospital. METHODS: A non-interventional observational study of 122,321 consecutive, unselected, adult patients with complete data admitted as an emerge
