[Sonographic image of intrarenal and intra-arterial fungus thrombi in a newborn infant with Candida infection].

Shortly after birth a preterm infant suffering from aspiration syndrome and subsequent Pseudomonas aeruginosa sepsis showed signs of renal insufficiency and mycotic infection: Yeast cells were identified in several urinalyses; there was also an increasing anti-Candida IgM antibody titer. At the same time sonographic examinations revealed an increasing echogenicity of the renal cortex and echogenic masses of variable size which did not cause acoustic shadows in both enlarged kidneys. A few days later, we found a right-sided hydronephrosis caused by an intraureteric prevesical mass of equal echo

[Cerebrogenic disorders of cardiac rhythm and conductivity in epilepsy patients].

51 epileptic patients aged 16-32 years without any signs of cardiovascular pathology were examined. The primary generalized epilepsy was diagnosed in 24 cases while a partial one--in 27 patients. Were examined a state of the heart (echo- and electrocardiography, Holter's monitoring of ECG), of the brain (computed echoencephalography, visual evoked potentials) and of the autonomic nervous system (variability of cardiac rhythm, autonomic tonus). A control group included 15 healthy individuals. Significantly higher prevalence of the disorders of the cardiac rhythm and conduction was found in pati

A case of massive fetal cardiac rhabdomyoma: ultrasound features and management.

We report the case of a massive fetal cardiac rhabdomyoma recently occurred at our clinic. A woman at 23 weeks of gestational age was referred to our center for a fetal cardiac echogenic mass of 26 mm detected at the second-trimester screening ultrasound. During pregnancy, though, the mass progressively increased its dimensions until reaching 48 mm in diameter at 37 weeks of gestation. Fetal echoencephalography and brain magnetic resonance did not show any further fetal anomalies, but molecular genetic testing at amniocentesis revealed a heterozygotic missense variant of gene TSC2 associated w

Sonographic lenticulostriate vasculopathy in infants: some associations and a hypothesis.

PURPOSE: To describe the causes of infantile lenticulostriate vasculopathy (LSV) as demonstrated by sonography and propose the pathogenesis of these findings. METHODS: Five hundred eighty-six infants were examined via echoencephalography because of seizures, psychomotor retardation, dysmorphism, congenital malformation, microcephaly, macrocephaly, bulging of anterior fontanel, consciousness disturbance, or prematurity. We directed our attention on the sonographic study to the basal ganglionic and thalamic areas. Twenty-eight of the 586 patients underwent color Doppler studies. RESULTS: In 34

The spectrum of leukomalacia using cranial ultrasound.

The spectrum of leukomalacia using cranial ultrasound is discussed. Transient densities not evolving into cystic lesions may represent a mild degree of leukomalacia when persisting for at least a week. A unilateral parenchymal density may be due to bleeding into an ischaemic area, but can also be due to a venous infarction. Cystic leukomalacia can be confidently diagnosed using appropriate equipment and performing sequential scans. A distinction should be made between cysts in the periventricular white matter and cysts in the deep white matter, as the latter carries a higher risk for cerebral

An Atlas of Neonatal Neurovascular Imaging Anatomy as Depicted with Microvascular Imaging: The Intracranial Veins.

Cerebral ultrasound is the preferred technique for evaluating the infant brain, offering real-time bedside assessment without exposure to ionizing radiation. Microvascular imaging (MVI) ultrasound, an advanced Doppler technique, enables the visualization of microvascular slow-flow structures without the need for contrast agents. This innovative approach has the potential to provide valuable insights into the vasculature and hemodynamics of the developing brain. The utility of MVI has been investigated in various neurologic conditions affecting neonates and infants, demonstrating its promise as

The face-brain relationship: a case of the face not predicting the brain.

An infant with midline facial cleft and a severe brain malformation, not typical of holoprosencephaly, is reported. The CNF abnormalities suggested a malformation in the same category, but distinguishable from the various forms of holoprosencephaly sequence.

Transcranial Doppler: clinical and experimental uses.

Blood flow velocity in the basal intracranial arteries can be reliably recorded using transcranial Doppler (TCD) ultrasonography. The utility of Doppler ultrasound in detecting stenosis of arteries has therefore been extended to include the intracranial basal arteries. This has been useful in detecting intracranial stenosis from a variety of causes including atherosclerosis and vasospasm induced by subarachnoid hemorrhage. Changes in cerebral hemodynamics during significantly increased intracranial pressure have also been detected, and have been useful in warning of compromise of the cerebral

Neonatal neurosonography.

Neonatal neurosonography is used commonly to evaluate the central nervous system in the neonatal intensive care setting. The procedure can be performed at the bedside in these critically ill patients who may suffer from hemodynamic and thermoregulatory instability and often require mechanical ventilation. This article reviews current recommendations regarding neurosonography technique, pathophysiology, and imaging of intracranial insults including hemorrhage, white matter injury, infarction, and hypoxic-ischemic encephalopathy.

Antenatal origin of neurologic damage in newborn infants. II. Multiple gestations.

Necrosis of the cerebral white matter may be identified in living infants with echoencephalography. Echoencephalographic studies were performed in 89 twins and 12 triplets at less than 36 weeks of gestation to determine the incidence and complications associated with antenatal necrosis of the cerebral white matter. Antenatal necrosis of the cerebral white matter was identified when brain atrophy or cavities in the white matter were present by day 3 of life. Fourteen infants (13.8%) were considered to have antenatal necrosis of the cerebral white matter. The incidence of antenatal necrosis of t

Neonatal ventriculomegaly: Pathophysiology and management guided with cranial ultrasonography.

Neonatal ventriculomegaly often, but not always, follows intraventricular haemorrhage in infants born preterm. Serial cranial ultrasonography (CUS) is a very useful tool to evaluate the mechanism behind ventricular dilatation, to differentiate several types of cerebrospinal fluid retention, and to guide treatment. This review examines neonatal ventriculomegaly and its definition, pathophysiology, treatment, and prognosis from the perspective of CUS assessment. It also outlines the consensus statements formulated by the EurUS.Brain group, which are based on rounds of expert opinions on neonatal

New non-invasive sonographic modality for intracranial pressure/volume monitoring.

INTRODUCTION: The wide use of intracranial pressure and cerebral perfusion pressure monitoring has improved the management of patients with severe head injuries. The rare but worrying complications associated with the application of such monitoring makes the idea of a non-invasive method of monitoring very attractive. MATERIALS AND METHODS: A new non-invasive ultrasonographic technology was used to measure cerebral perfusion pressure in 27 normal volunteers. The average monitoring time was 45.3+/-0.2 min, and the average perfusion pressure recorded was 77.4+/-0.3 mmHg. No complications were r

Prenatally acquired cytomegalovirus encephalopathy.

A case is reported of congenital cytomegalovirus infection. A baby girl was born at an estimated gestational age of 35 weeks to a 26-year-old Hispanic women. There was maternal cocaine abuse and poor prenatal care, and variable decelerations, abruptio placentae, and meconium-stained fluid occurred in the intrapartum period. Cytomegalovirus was isolated from the urine, with growth occurring in 48 hours, and the neonate had characteristic neurodevelopment handicaps.

Prophylactic ibuprofen for the prevention of intraventricular hemorrhage among preterm infants: a multicenter, randomized study.

OBJECTIVE: Ibuprofen enhances cerebral blood flow autoregulation and was shown to protect neurologic functions after oxidative stresses in an animal model. For these reasons, we hypothesized that the prophylactic use of ibuprofen would reduce the occurrence of intraventricular hemorrhage (IVH) and its worsening toward grades 2 to 4 among preterm infants. To confirm this hypothesis, we planned the present prospective study. METHODS: This was a double-blind, randomized, controlled trial in which preterm infants with gestational ages of <28 weeks received ibuprofen or placebo within the first 6
