The Impact of Intraoperative Electrocorticography on Seizure Outcome After Resection of Pediatric Brain Tumors: A Cohort Study.

BACKGROUND: Intraoperative electrocorticography (ECoG) has been utilized in patients with tumor-associated seizures; however, its effectiveness for seizure control remains controversial. OBJECTIVE: To evaluate clinical outcomes in pediatric patients undergoing lesionectomy with or without ECoG. METHODS: Patients undergoing brain tumor resection at Boston Children's Hospital were examined retrospectively (2005-2014). Inclusion criteria involved diagnosis of a supratentorial tumor, ≥2 unequivocal seizures, and ≥6 mo follow-up. Patients with isolated cortical dysplasia or posterior fossa tumors

A 360° electronic device for recording high-resolution intraoperative electrocorticography of the brain during awake craniotomy.

OBJECTIVE: Epilepsy is common among patients with supratentorial brain tumors; approximately 40%-70% of patients with glioma develop brain tumor-related epilepsy (BTRE). Intraoperative localization of the epileptogenic zone during surgical tumor resection (real-time data) may improve intervention techniques in patients with lesional epilepsy, including BTRE. Accurate localization of the epileptogenic signals requires electrodes with high-density spatial organization that must be placed on the cortical surface during surgery. The authors investigated a 360° high-density ring-shaped cortical ele

Shedding light on interictal epileptic spikes: An in vivo study using fast optical signal and electrocorticography.

OBJECTIVE: Interictal epileptic spikes (IESs), apart from being a key marker of epileptic neuronal networks, constitute a nice model of the widespread endogenous phenomenon of neuronal hypersynchronization. Many questions concerning the mechanisms that drive neurons to hypersynchronize remain unresolved, but synaptic as well as nonsynaptic events are likely to be involved. In this study, changes in optical properties of neural tissues were observed in rats with penicillin-induced IES using fast optical signal (FOS) concomitantly with electrocorticography (ECoG). METHODS: In this study, near-i

[An anesthetic experience of epileptic focus resection in a nine month-old girl under monitoring by electrocorticography].

We report a successful electrocorticography (ECoG) monitoring for a epileptic cortical focus resection in a 9-month-old girl with uncontrollable epileptic seizures. Anesthesia was induced and maintained with nitrous oxide, oxygen, and sevoflurane with vecuronium. After the craniotomy, ECoG monitoring was used to locate the epileptic cortical lesion. During the procedure of subpial resection and gyrectomy, ECoG was repeatedly recorded. It was necessary to maintain adequate depth of anesthesia throughout the procedure to clarify the epileptic waves. For this purpose, sevoflurane (2.5%) with and

Effect of Dexmedetomidine on Electrocorticography During Epilepsy Surgery under Isoflurane or Propofol Anesthesia: A Prospective, Randomized Trial.

INTRODUCTION: Avoiding or minimizing the interference of anesthetic agents with electrocorticography (ECoG) signals during ECoG-guided epilepsy surgery is vital to the successful resection of the epileptogenic area. Most agents in routine use have widely variable effects like suppression, enhancement, or no impact on the ECoG signals. Dexmedetomidine is reported to have no influence, or minimal depressant effect, on the signals, but studies evaluating its effect on intraoperative ECoG are limited. This study evaluates the effect of dexmedetomidine on ECoG signals during ECoG-guided epilepsy su

Efficacy of intraoperative electrocorticography for assessing seizure outcomes in intractable epilepsy patients with temporal-lobe-mass lesions.

PURPOSE: The findings of previous studies have been controversial regarding the optimal surgical procedures required for effective seizure control. In particular, there are varying views as to whether or not lesionectomy of a temporal-lobe-mass lesion is a satisfactory process or whether removal of additional seizure foci is necessary. In this study, we evaluated the efficacy of additional removal of electrically positive foci using intraoperative electrocorticography on mass lesions related to temporal lobe epilepsy. METHODS: Thirty-five medically intractable epilepsy patients with temporal-

Neurophysiologic findings of neuronal migration disorders: intrinsic epileptogenicity of focal cortical dysplasia on electroencephalography, electrocorticography, and magnetoencephalography.

We define specific neurophysiologic characteristics for focal cortical dysplasia, a neuronal migration disorder. We reviewed data from published reports and our patients with focal cortical dysplasia. Our patients underwent preoperative scalp video-electroencephalography (EEG), magnetic resonance imaging (MRI), magnetoencephalography, and intraoperative or extraoperative electrocorticography monitoring. Scalp EEG showed trains of rhythmic epileptiform spike or sharp waves. Positive spikes correlated with early seizure onset, MRI lesion around the rolandic fissure, hemiparesis, and a less favor

Integration of intraoperative ultrasound and depth-electrode electrocorticography for resection guidance in epilepsy surgery: technical workflow and feasibility.

BACKGROUND: Complete resection of the epileptogenic zone (EZ) is the strongest predictor of seizure freedom in drug-resistant epilepsy (DRE). However, even in MRI-positive cases with anatomo-electro-clinical concordance, the EZ may not be clearly delineated, complicating intraoperative decision-making. Intraoperative ultrasound (ioUS) provides real-time anatomical feedback, while depth-electrode intraoperative electrocorticography (iECoG) enables electrophysiological delineation of epileptogenic tissue beyond the cortical surface, sampling deep regions not accessible to subdural electrodes. Propofol anesthesia-induced spatiotemporal changes in cortical activity with loss of external and internal awareness: An electrocorticography study.

OBJECTIVE: To understand the underlying mechanism of consciousness, investigating spatiotemporal changes in the cortical activity during the induction phase of unconsciousness is important. Loss of consciousness induced by general anesthesia is not necessarily accompanied by a uniform inhibition of all cortical activities. We hypothesized that cortical regions involved in internal awareness would be suppressed after disruption of cortical regions involved in external awareness. Thus, we investigated temporal changes in cortex during induction of unconsciousness. METHODS: We recorded electroco

Advancing intraoperative brain mapping: First-in-human evaluation of flexible high-density electrocorticography grids for epilepsy surgery.

Mapping epileptiform activity, specifically high frequency oscillations (HFOs), with intraoperative electrocorticography (ECoG) may guide epilepsy surgery. The most relevant recording takes place following the surgical resection to identify any residual epileptic activity. Recording HFOs is, however, challenging due to their focal and low-amplitude nature. We designed and evaluated a novel soft, flexible, and stretchable high-density electrode grid (SOFT ECoG) tailor-made for epilepsy surgery to enhance HFO detection and enable recordings from within resection cavities. In this first-in-human

Spectral entropy variability of intraoperative electrocorticography predicts outcome after epilepsy surgery in people with focal cortical dysplasia.

OBJECTIVE: Epilepsy surgery in people with focal cortical dysplasia (FCD) requires accurate removal of all epileptogenic tissue, and outcome is difficult to predict. We explored whether spectral entropy, a fast computable electroencephalographic (EEG) feature, could estimate epileptic activity in intraoperative electrocorticography (ioECoG) and forecast postsurgical outcomes. METHODS: We included people with FCD pathology who underwent ioECoG-assisted resective surgery. We analyzed ioECoG recordings acquired before and after resection. We computed spectral entropy across eight frequency bands

Post-section recruitment of epileptiform discharges in electrocorticography during callosotomy in 48 patients with Lennox-Gastaut syndrome.

We aimed to clarify the relationships between post-sectional recruitment of epileptiform discharges (PRED) seen in electrocorticograms (ECoG) during callosotomy in patients with Lennox-Gastaut syndrome (LGS) and postoperative seizure outcome. We retrospectively analyzed ECoG obtained during subtotal callosotomy (6 cm) in 48 patients with LGS (32 males, 16 females; age range, 1-20 years; mean age, 7.6 years) from July 1993 to November 1996 and compared recorded findings with postoperative seizure outcome. At analysis, all patients had been followed postoperatively for more than 10 years. Of 48

Occipital gamma-oscillations modulated during eye movement tasks: simultaneous eye tracking and electrocorticography recording in epileptic patients.

We determined the spatio-temporal dynamics of cortical gamma-oscillations modulated during eye movement tasks, using simultaneous eye tracking and intracranial electrocorticography (ECoG) recording. Patients with focal epilepsy were instructed to follow a target moving intermittently and unpredictably from one place to another either in an instantaneous or smooth fashion during extraoperative ECoG recording. Target motion elicited augmentation of gamma-oscillations in the lateral, inferior and polar occipital regions in addition to portions of parietal and frontal regions; subsequent voluntary

Bilateral temporal lobe epilepsy: How many seizures are required in chronic ambulatory electrocorticography to estimate the laterality ratio?

OBJECTIVE: This study was undertaken to measure the duration of chronic electrocorticography (ECoG) needed to attain stable estimates of the seizure laterality ratio in patients with drug-resistant bilateral temporal lobe epilepsy (BTLE). METHODS: We studied 13 patients with drug-resistant BTLE who were implanted for at least 1 year with a responsive neurostimulation device (RNS System) that provides chronic ambulatory ECoG. Bootstrap analysis and nonlinear regression were applied to model the relationship between chronic ECoG duration and the probability of capturing at least one seizure. La

Neural Correlates of Vocal Auditory Feedback Processing: Unique Insights from Electrocorticography Recordings in a Human Cochlear Implant User.

There is considerable interest in understanding cortical processing and the function of top-down and bottom-up human neural circuits that control speech production. Research efforts to investigate these circuits are aided by analysis of spectro-temporal response characteristics of neural activity recorded by electrocorticography (ECoG). Further, cortical processing may be altered in the case of hearing-impaired cochlear implant (CI) users, as electric excitation of the auditory nerve creates a markedly different neural code for speech compared with that of the functionally intact hearing syste

Brain-Computer interface control of stepping from invasive electrocorticography upper-limb motor imagery in a patient with quadriplegia.

Introduction: Most spinal cord injuries (SCI) result in lower extremities paralysis, thus diminishing ambulation. Using brain-computer interfaces (BCI), patients may regain leg control using neural signals that actuate assistive devices. Here, we present a case of a subject with cervical SCI with an implanted electrocorticography (ECoG) device and determined whether the system is capable of motor-imagery-initiated walking in an assistive ambulator. Methods: A 24-year-old male subject with cervical SCI (C5 ASIA A) was implanted before the study with an ECoG sensing device over the sensorimotor

Is intraoperative electrocorticography (ECoG) for long-term epilepsy-associated tumors (LEATs) more useful in children?-A Randomized Controlled Trial.

OBJECTIVES: The utility of intraoperative electrocorticography (ECoG)-guided resective surgery for pediatric long-term epilepsy-associated tumors (LEATs) with antiseizure medication (ASM) resistant epilepsy is not supported by robust evidence. As epilepsy networks and their ramifications are different in children from those in adults, the impact of intraoperative ECoG-based tailored resections in predicting prognosis and influencing outcomes may also differ. We evaluated this hypothesis by comparing the outcomes of resections with and without the use of ECoG in children and adults by a randomi

Can intraoperative electrocorticography be used to minimize the extent of resection in patients with temporal lobe epilepsy associated with hippocampal sclerosis?

OBJECTIVE: Tailored surgery to extensively resect epileptogenic lesions using intraoperative electrocorticography (ioECoG) may improve seizure outcomes. However, resection of large areas is associated with decreased memory function postoperatively. The authors assessed whether ioECoG could provide useful information on how to minimize the focus resection and obtain better seizure outcomes without memory deterioration. They examined the postoperative seizure-free period and memory alteration in a retrospective cohort of patients with mesial temporal lobe epilepsy (TLE) due to hippocampal sclero

Study of neurovascular coupling functions for transient focal cerebral ischemia in rats using electrocorticography functional photoacoustic microscopy (ECoG-fPAM).

Recently, the functional photoacoustic microscopy (fPAM) system has been proven to be a reliable imaging technique for measuring the total hemoglobin concentration (HbT), cerebral blood volume (CBV) and hemoglobin oxygen saturation (SO2) in single cerebral blood vessels of rats. In this study, we report for the first time the combination of electrocorticography (ECoG) recordings and fPAM (ECoG-fPAM) to investigate functional hemodynamic changes and neuro-vascular coupling in single cortical arterioles of rats with electrical forepaw stimulation after photothrombotic stroke.
