Long-Term Electromyographic Monitoring of the Stapedius Reflex via Implanted Electrodes in Sheep: Toward Objective Autonomous Cochlear Implant Fitting.

Objective fitting measures offer a means to circumvent the subjectivity of cochlear implant programming, with the stapedius reflex representing one robust predictor of the maximum comfortable loudness level. With the present study, it was investigated whether long-term electromyographic measurements of the stapedius muscle using implanted electrodes are feasible. In nine sheep, myoelectrical activities were recorded intraoperatively and synchronized with middle ear admittance as a reference signal. For acoustic stimulation pure tones with different frequencies were used. The electrodes were pl

Cochlear Implant Electrode Impedances: A 5-Year Study.

BACKGROUND: Electrode impedance (EI) monitoring is vital for understanding electrode-tissue interface dynamics and ensuring consistent neural stimulation in cochlear implant (CI) recipients. METHODS: This retrospective cohort study examined long-term EI patterns in 79 CI recipients (136 implanted ears) over a 5-year period. Impedance measurements were obtained intraoperatively and at 1 and 5 years post-implantation across 12 electrodes, categorized into apical (electrodes 1-4), middle (5-8), and basal (9-12) regions. RESULTS: The apical region showed stable impedance throughout the 5-year an

Advances in Electrode Design and Physiological Considerations for Retinal Implants.

Until now, the ultimate solution for blind people has not been achieved, because challenges still exist. Retinal implants have emerged as a promising solution for restoring vision in individuals suffering from retinal degenerative diseases such as retinitis pigmentosa and age-related macular degeneration. Central to the efficacy of these implants is the design and functionality of the electrode arrays responsible for stimulating retinal neurons. This review evaluates the evolution of retinal implants, with particular emphasis on electrode specifications, physiological considerations for electr

Ictal Semiology Important for Electrode Implantation and Interpretation of Stereoelectroencephalography.

Scalp video-electroencephalography (video-EEG) monitoring should be analyzed thoroughly to preoperatively evaluate stereoelectroencephalography (SEEG). Formulating the working hypotheses for the epileptogenic zone (EZ) considering "anatomo-electroclinical correlations" is the most crucial step, which determines the placement of SEEG electrodes. If these hypotheses are insufficient, precise EZ identification may not be achieved during SEEG recording. In ictal semiology analysis, temporal and spatial patterns with reference to ictal EEG changes are emphasized. In frontal lobe epilepsy, seizures o

Dynamic current steering with phantom electrode in cochlear implants.

Phantom electrode (PE) stimulation can extend the lower limit of pitch perception with cochlear implants (CIs) by using simultaneous out-of-phase stimulation of the most apical primary electrode and the adjacent basal compensating electrode. The total electrical field may push the excitation pattern beyond the most apical electrode to elicit a lower pitch, depending on the ratio of current between the compensating and primary electrodes (i.e., the compensation coefficient σ). This study tested the hypothesis that dynamic current steering of PE stimuli can be implemented by varying σ over time

Functional and Histological Effects of Chronic Neural Electrode Implantation.

OBJECTIVES: Permanent injury to the cranial nerves can often result in a substantial reduction in quality of life. Novel and innovative interventions can help restore form and function in nerve paralysis, with bioelectric interfaces among the more promising of these approaches. The foreign body response is an important consideration for any bioelectric device as it influences the function and effectiveness of the implant. The purpose of this review is to describe tissue and functional effects of chronic neural implantation among the different categories of neural implants and highlight advance

Cochlear implant electrode array misplaced in Hyrtl's fissure.

Hyrtl's fissure is a cleft that is present in the developing fetal petrous temporal bone and extends from the area inferior to the round window to the meninges of the posterior fossa. Persistent Hyrtl's fissure, due to incomplete ossification, is considered a rare temporal bone malformation, and is a known cause of perilabyrinthine cerebrospinal fluid fistula. Very few cases are reported as being at risk of complication of cochlear implant surgery. Here we report the case of an 8-year-old boy with misplacement of an electrode array in Hyrtl's fissure. The diagnosis was made postoperatively, si

Implantation and testing of subretinal film electrodes in domestic pigs.

By definition, an electronic subretinal visual prosthesis requires the implantation of stimulation electrodes in the subretinal space of the eye. Polyimide film electrodes with flat contacts were implanted subretinally and used for electrical stimulation in acute experiments in anaesthetised domestic pigs. In two pigs, the film electrode was inserted through a sclerostomy into the vitreous cavity and, subsequently, via a retinotomy into the subretinal space around the posterior pole (ab interno approach). In three other pigs the sclera and pigment epithelium were opened for combined ab interno

Linear intracerebral calcification after depth electrode implantation.

We report linear calcifications along the trajectories of previously implanted depth electrodes in 2 patients. A 20-year-old man and a 38-year-old woman with medically intractable complex partial seizures (CPS), underwent bilateral frontal and mesiotemporal depth electrode implantation as part of their epilepsy surgery workup. Brain computed tomography (CT) at that time was normal (except for cerebrellar atrophy in one case). One patient had a left anterotemporal lobectomy (ATL), and the other declined operation. Subsequent CT scans showed linear calcifications 1-2 cm long in the occipital lob

Technique for implanting intramuscular electrodes in the diaphragm by videolaparoscopy in pigs.

PURPOSE: To describe a novel approach for implanting intramuscular electrodes in the diaphragm through videolaparoscopy. METHODS: We used twelve pigs for this videolaparoscopic technique, which permits at the same time to explore the diaphragm, to locate its motor points and to fix the electrodes in the diaphragm bilaterally. In this technique we used three trocars: one portal for a 10-mm 0° viewing angle laparoscope, one portal for the manipulation of structures and another for electrode implantation. RESULTS: All animals survived the procedure without pneumothorax/capnothorax or other comp

Robotic implantation of gastric electrical stimulation electrodes for gastroparesis.

BACKGROUND: Gastric electrical stimulation (GES) is a low-morbidity treatment option that may be effective for refractory symptoms in patients with gastroparesis of diabetic or idiopathic etiology. During surgery to initiate GES, two electrodes are tunneled in the gastric antrum in a precise location. If these electrodes pass through the mucosa and into the gastric lumen (determined by endoscopy) they must be repositioned, often multiple times. During this procedure, extensive suturing to anchor the electrodes is necessary once properly placed. Robotic surgical systems may provide surgeons wit

Audiologic outcomes with the penetrating electrode auditory brainstem implant.

OBJECTIVE: The penetrating electrode auditory brainstem implant (PABI) is an extension of auditory brainstem implant (ABI) technology originally developed for individuals deafened by neurofibromatosis type 2. Whereas the conventional ABI uses surface electrodes on the cochlear nuclei, the PABI uses 8 or 10 penetrating microelectrodes in conjunction with a separate array of 10 or 12 surface electrodes. The goals of the PABI were to use microstimulation to reduce threshold current levels, increase the range of pitch percepts, and improve electrode selectivity and speech recognition. PATIENTS AND

Electrode complications in 100 adults with multichannel cochlear implants.

At switch-on (first post-operative stimulation of the implant) and during subsequent reprogramming, electrodes can, in some patients, be found to be non-functional or to be performing sub-optimally for a number of reasons. This paper examines the reasons for the poor performance of these electrodes by means of a retrospective analysis of 100 patient records. All of these patients received the Nucleus multichannel device. The most common reason for an electrode to require de-activation was found to be facial nerve stimulation, with poor sound quality and pain also being very common. Other reaso

Cochlear implant and traumatic lesions secondary to electrode insertion.

On the basis of data reported in the literature, the authors have attempted to define the relationship between the functional results of cochlear implants and possible traumatic damage caused by the insertion of electrodes and their support into the cochlear bony walls. These findings show that traumatic conditions result in functional damage only when they involve the body of Corti's ganglion cells or the central part of their axon, whereas functional results are not influenced by traumatic damage to the peripheral part of the axon. Traumatic damage sustained by other non-nervous structures a
