Creating rare epilepsy cohorts using keyword search in electronic health records.

OBJECTIVE: Administrative codes to identify people with rare epilepsies in electronic health records are limited. The current study evaluated the use of keyword search as an alternative method for rare epilepsy cohort creation using electronic health records data. METHODS: Data included clinical notes from encounters with International Classification of Diseases, Ninth Revision (ICD-9) codes for seizures, epilepsy, and/or convulsions during 2010-2014, across six health care systems in New York City. We identified cases with rare epilepsies by searching clinical notes for keywords associated w

Careful: An administrative child welfare and electronic health records linked dataset.

Between 2012 and 2017, N = 2814 youth between the ages of 4 and 20 were in child protective services (CPS) custody in Hamilton County, Ohio, and placed in out-of-home care. Child welfare administrative records were extracted and linked to electronic health records for all encounters at Cincinnati Children's Hospital Medical Center, with n = 2787 (99.1%) of records successfully linked prior to de-identifying the data for research purposes. Child welfare administrative data fields in the dataset include demographics, dates of entry into and exit from protective custody and out-of-home care, reas

Representing patient preference-related concepts for inclusion in electronic health records.

In recent years shared decision making between patients and their health care providers and the inclusion of patient preferences in patient care have been, in theory, embraced as models for good clinical practice. Patients' experiences, values, and preferences are increasingly acknowledged as important pieces of evidence for appropriate health care decision making. To effectively use information about patient preferences in patient care, this information, which is gathered through a process of preference elicitation, needs to be integrated with other types of information, e.g., diagnoses, trea

Network analysis of longitudinal electronic health records using linear mixed models.

BACKGROUND: The accelerating development of healthcare data stored in electronic health records (EHRs) has created novel opportunities for biomedical research. Clinical data contains rich, heterogeneous, and longitudinal information of diverse cohorts of population. These data can be analyzed to uncover complex patterns that describe disease progression, comorbidities, and patient trajectories. Significant analytical challenges arise when identifying relationships among clinical variables, often due to high dimensionality and interdependence of temporal data, which requires adaptable methodolo

Early attention deficit hyperactivity disorder prediction from longitudinal electronic health records.

Attention-deficit/hyperactivity disorder (ADHD) is a common neurodevelopmental condition that can negatively impact long-term outcomes for individuals. Early diagnosis is critical, yet demographic and clinical disparities can delay detection. Using electronic health records (EHR) from a cohort of over 720,000 patients, we pretrained an EHR foundation model. We then fine-tuned it to predict the likelihood of ADHD diagnosis and timing from birth until age nine years in a pediatric cohort of over 140,000 patients. By age five years, the model achieved a time-dependent area under the receiver oper

Subtyping Alzheimer's disease and Parkinson's disease using longitudinal electronic health records.

Neurodegenerative diseases such as Alzheimer's disease (AD) and Parkinson's disease (PD) are clinically heterogeneous, hampering the success of nonselective treatment strategies. Here we apply a transformer-based unsupervised clustering framework to longitudinal electronic health record data from over 100,000 patients across two UK cohorts, Clinical Practice Research Datalink Aurum and UK Biobank, to identify, validate and characterize subtypes of AD and PD. We uncover five reproducible subtypes for each condition, characterized by distinct comorbidity patterns, symptom trajectories, outcomes

Regional Electronic Health Records in Denmark: Research Potential for Clinical Epidemiology.

Denmark has a well-established tradition of utilizing national registries for epidemiological research. Healthcare services in Denmark are managed by five administrative regions and documented in electronic health records (EHRs). While certain EHR data are mandatorily transferred to national registries, the national registries capture only a fraction of the information available within regional EHR systems. In this review, we describe and discuss the context, content, and research potential of regional EHR data in Denmark for observational studies and pragmatic clinical trials. Throughout, exa

Phenotyping the Transfusion Management Experience of Trainees Using Electronic Health Records.

BACKGROUND: Clinical experience is foundational to graduate medical education but is difficult to quantify. Self-reported case logs and scheduled rotations provide limited detail on residents' actual experiences. The analysis of electronic health record (EHR) data may provide a more comprehensive view of residents' actual clinical experiences. We aimed to quantify and characterize residents' direct clinical experiences with intraoperative transfusion management as a proof of concept for the utility of EHR data in anesthesiology graduate medical education. METHODS: This retrospective observati

Reliability of Electronic Health Records in Recording Veterans' Tobacco Use Status.

INTRODUCTION: The prevalence of tobacco use in the Veteran population and among Veterans Health Administration patients remains high, resulting in significant health and economic consequences. This problem has generated many tobacco research studies and clinical interventions, which often rely upon tobacco use status data previously recorded in electronic health records (EHR). Therefore, the consistency and reliability of these data are critical. The Veterans Health Administration uses an extensive EHR system where tobacco use status can be documented either as free text (FT) or as health fact

Defining severity of personality disorder using electronic health records: short report.

Severity of personality disorder is an important determinant of future health. However, this key prognostic variable is not captured in routine clinical practice. Using a large clinical data-set, we explored the predictive validity of items from the Health of Nation Outcome Scales (HoNOS) as potential indicators of personality disorder severity. For 6912 patients with a personality disorder diagnosis, we examined associations between HoNOS items relating to core personality disorder symptoms (self-harm, difficulty in interpersonal relationships, performance of occupational and social roles, an

Predicting childhood obesity using electronic health records and publicly available data.

BACKGROUND: Because of the strong link between childhood obesity and adulthood obesity comorbidities, and the difficulty in decreasing body mass index (BMI) later in life, effective strategies are needed to address this condition in early childhood. The ability to predict obesity before age five could be a useful tool, allowing prevention strategies to focus on high risk children. The few existing prediction models for obesity in childhood have primarily employed data from longitudinal cohort studies, relying on difficult to collect data that are not readily available to all practitioners.
