Validation of Parkinson's Disease Ascertainment in the Veterans Administration Electronic Medical Record.

BACKGROUND: Electronic medical record (EMR)-based studies hold great potential for epidemiologic investigations of Parkinson's disease (PD) causal factors and phenomenology, but diagnostic misclassification may obscure or bias inferences. OBJECTIVES: The aims were to determine the validity of PD diagnostic codes in the Veterans Administration (VA) national electronic medical databases and develop recommendations for maximizing ascertainment accuracy. METHODS: We investigated a cohort of 146,776 veterans who utilized VA healthcare between 1999 and 2021. We reviewed the medical records of indi

Anaphylaxis to cow's milk protein in a probiotic not detected by the electronic medical record.

A 13-year-old female with a history of congenital left lower leg lymphedema, multiple food allergies, including an immunoglobulin E mediated severe cow's milk allergy, and well-controlled moderate persistent asthma was hospitalized with left lower leg erysipelas and Group A Streptococcus septicemia. While hospitalized, immediately after exposure to cow's milk protein as an inactive ingredient within a probiotic, she developed anaphylaxis with respiratory failure requiring intubation. This is only the third reported case of anaphylaxis due to a probiotic. Additionally, it raises issues inherent

pyPheWAS: A Phenome-Disease Association Tool for Electronic Medical Record Analysis.

Along with the increasing availability of electronic medical record (EMR) data, phenome-wide association studies (PheWAS) and phenome-disease association studies (PheDAS) have become a prominent, first-line method of analysis for uncovering the secrets of EMR. Despite this recent growth, there is a lack of approachable software tools for conducting these analyses on large-scale EMR cohorts. In this article, we introduce pyPheWAS, an open-source python package for conducting PheDAS and related analyses. This toolkit includes 1) data preparation, such as cohort censoring and age-matching; 2) tra

A novel method for estimating transgender status using electronic medical records.

PURPOSE: We describe a novel algorithm for identifying transgender people and determining their male-to-female (MTF) or female-to-male (FTM) identity in electronic medical records of an integrated health system. METHODS: A computer program scanned Kaiser Permanente Georgia electronic medical records from January 2006 through December 2014 for relevant diagnostic codes, and presence of specific keywords (e.g., "transgender" or "transsexual") in clinical notes. Eligibility was verified by review of de-identified text strings containing targeted keywords, and if needed, by an additional in-depth

Validating pathophysiological models of aging using clinical electronic medical records.

Bioinformatics methods that leverage the vast amounts of clinical data promises to provide insights into underlying molecular mechanisms that help explain human physiological processes. One of these processes is adolescent development. The utility of predictive aging models generated from cross-sectional cohorts and their applicability to separate populations, including the clinical population, has yet to be completely explored. In order to address this, we built regression models predictive of adolescent chronological age from 2001 to 2002 National Health and Nutrition Examination Survey (NHA

Are patient-reported medical histories enough for safe dental care? A comparison with electronic medical records in dental practice.

OBJECTIVES: Accurate medical information is essential for safe dental treatment planning and for tailoring dental care to the patient's systemic condition. The patient-reported medical history questionnaire is a well-established and recommended practice in dental clinics, guiding the dentist in deciding whether to obtain additional information from the patient's treating physician. We aimed to evaluate the reliability of this approach through comparison with corresponding medical records. METHODS: We retrospectively compared 276 self-reported medical questionnaires with corresponding electron

Using linked electronic medical record-pharmacy data to examine lupus medication adherence: A retrospective cohort study.

INTRODUCTION: Medication nonadherence is common in systemic lupus erythematosus (SLE) and associated with morbidity and mortality. We explored the reliability of pharmacy data within the electronic medical record (EMR) to examine factors associated with nonadherence to SLE medications. METHODS: We included patients with SLE who were prescribed ≥1 SLE medication for ≥90 days. We compared two datasets of pharmacy fill data, one within the EMR and another from the vendor who obtained this information from pharmacies and prescription benefit managers. Adherence was defined by medication possessio

Pharmacological Prophylaxis for Venous Thromboembolism Among Hospitalized Patients With Acute Medical Illness: An Electronic Medical Records Study.

Patients hospitalized with acute medical illness have an elevated risk of venous thromboembolism (VTE). American College of Chest Physicians guidelines list various chronic illnesses, sepsis, advanced age, history of VTE, and immobility as risk factors and recommend prophylactic anticoagulation using fondaparinux, low-molecular weight heparin, or low-dose unfractionated heparin. The objectives of this study were to examine pharmacological prophylaxis against VTE among hospitalized medically ill patients and to assess demographic and clinical correlates related to VTE prophylaxis. A retrospecti

A Large Language Model Pipeline for Stroke Staging Using Electronical Medical Records.

OBJECTIVE: Identifying stroke patients at different disease stages is a prerequisite for clinical research using electronic medical records (EMRs), whereas an artificial intelligence-based model that can be directly applied remains lacking. We therefore develop a large language model (LLM) pipeline for stroke staging model (StrokeSM) in retrospective clinical research. METHODS: StrokeSM was developed using a Chinese national stroke database comprising EMRs from 33,637 patients. A total of 2000 patients were randomly selected from the Tianjin regional stroke database for external validation. S

Identification of antibiotic allergies in children using prescribing patterns in the electronic medical record.

BACKGROUND: By age three, 5%-75% of children have a documented antibiotic allergy (AL). Despite this prevalence, recognition remains low because of limited clinician knowledge and time. Our study evaluates a novel approach to identifying AL using electronic medical record (EMR) prescription patterns. METHODS: An EMR report was developed to identify paediatric encounters where adrenaline and/or an antihistamine was administered within 48 h of the last antibiotic dose. The first 100 cases were reviewed to refine exclusion criteria. Exclusions included diagnoses of eczema, or rash; antihistamine

Digital Health for Asthma Management: Electronic Medication Monitoring for Adherence as a Case Example.

Digital health is an umbrella term for components of health care using computer platforms, software, connectivity, and sensors to augment the recording, documentation, and communication of clinical information. The functions of digital health may be viewed in three domains: (1) the repository for patient information, (2) monitoring devices, and (3) communication tools. Monitoring devices have provided robust information as diagnostic and prognostic tools in office and hospital settings. In this review, as a case study, we will discuss the research and our direct clinical experience of electron
