Intracardiac Electrogram Targets for Ventricular Tachycardia Ablation.

The pathogenesis of ventricular tachycardia (VT) in most patients with a prior myocardial scarring is reentry involving compartmentalized muscle fibers protected within the scar. Often the 12-lead ECG morphology of the VT itself is not available when treated with a defibrillator. Consequently, VT ablation takes on an interesting challenge of finding critical targets in sinus rhythm. High-density recordings are essential to evaluate a substrate based on whole electrogram voltage and activation delay, supplemented with substrate perturbation through alternate site pacing or introducing an extra

Utility of Intracardiac Echocardiography to Guide Transseptal Catheterization for Different Electrophysiology Procedures.

"Intracardiac echocardiography (ICE) has evolved into an indispensable tool in the armamentarium of cardiac electrophysiologists not only for understanding the internal cardiac anatomy but also for establishing transseptal access and for monitoring electrophysiology procedures. ICE aids in live monitoring of every step of the procedure including early detection of complications. Access to the left atrium through transseptal catheterization is a vital step to perform atrial fibrillation and accessory pathway ablations, ablation of left ventricular tachycardias, left atrial appendage closures, l

Principal Component-Referenced Multipolar Mapping to Localize an Arrhythmic Source From Various Depths of the Myocardium.

BACKGROUND: Intramural premature ventricular contraction (PVC) mapping is limited by the ability of unipolar and bipolar electrograms (EGMs) to discriminate near from far-field activity. OBJECTIVES: In this study, the authors hypothesized that multipolar EGMs with improved far-field rejection would provide superior source detection. METHODS: A Langendorff perfused healthy swine heart model was utilized in which a left ventricular free wall PVC was generated using a variable depth plunge needle. EGMs were recorded from the ventricular surface using either a customized array (n = 5) or the OPT

Mapping and Ablation of Fascicular Tachycardias (Reentrant and Nonreentrant).

Fascicular ventricular tachycardia (FVT) usually involves the left fascicular system; namely the left posterior fascicle, anterior fascicle, and rarely the upper septal fascicle. It may also involve the right Purkinje arborization. This tachycardia can be seen in normal heart or in the setting of structural heart diseases. Monomorphic FVT can be reentrant or nonreentrant and verapamil-sensitive left FVT is the second most common type of idiopathic ventricular tachycardia (VT) after right ventricular outflow tract VT. This article focuses on the practical approach for both reentrant and nonreen

Strategy and Outcome of Catheter Ablation for Persistent Atrial Fibrillation - Impact of Progress in the Mapping and Ablation Technologies.

Pulmonary vein (PV) antrum isolation (PVAI) is effective in treating paroxysmal atrial fibrillation (AF) but is less so for persistent AF. A recent randomized study on the ablation strategies for persistent AF demonstrated that 2 common atrial substrate modifications, creation of linear lesions in the left atrium and ablation of complex fractionated electrogram sites, in addition to PVAI did not improve the outcome compared with stand-alone PVAI, suggesting the necessity of a more individualized, selective approach to persistent AF. There are emerging technologies, including high-resolution ma

Electrophysiological Analysis of human Pluripotent Stem Cell-derived Cardiomyocytes (hPSC-CMs) Using Multi-electrode Arrays (MEAs).

Cardiomyocytes can now be derived with high efficiency from both human embryonic and human induced-Pluripotent Stem Cells (hPSC). hPSC-derived cardiomyocytes (hPSC-CMs) are increasingly recognized as having great value for modeling cardiovascular diseases in humans, especially arrhythmia syndromes. They have also demonstrated relevance as in vitro systems for predicting drug responses, which makes them potentially useful for drug-screening and discovery, safety pharmacology and perhaps eventually for personalized medicine. This would be facilitated by deriving hPSC-CMs from patients or suscept

Mechanism and mapping of atrial arrhythmia in the modified Fontan circulation.

The Fontan circulation has been a major success in the management of patients with functionally single-ventricle cardiac lesions, although arrhythmia remains one of the major complications in the long term and historically has been difficult to treat. The electrophysiologic substrate within the right atrium is complex, with areas of scarring and/or anatomic structures acting as central barriers to conduction in reentrant circuits, which may be mechanistically and anatomically different from typical atrial flutter. Developments in three-dimensional mapping have improved our understanding of dif

Temporary transvenous cardiac pacing: a survey on current practice.

BACKGROUND: Temporary transvenous cardiac pacing (TTCP) is a standard procedure in current practice, despite limited coverage in consensus guidelines. However, many authors reported several complications associated with TTCP, especially development of infections of cardiac implantable electronic devices (CIED). The aim of this survey was to provide a country-wide picture of current practice regarding TTCP. METHODS: Data were collected using an online survey that was administered to members of the Italian Association of Arrhythmology and Cardiac Pacing. RESULTS: We collected data from 102 phy

Transesophageal electrophysiology study in the diagnosis of dual atrioventricular nodal nonreentrant tachycardia.

"Double fire" is generally characterized by 1:2 atrioventricular conduction of sinus beats traveling down fast and slow pathways that result in double ventricular response. When this phenomenon repeats rapidly, dual atrioventricular nodal nonreentrant tachycardia (DAVNNT) occurs. We report a case of an irregular tachycardia with a comprehensive record that includes an electrocardiogram, a transesophageal electrophysiology study, and an intracardiac electrophysiology study. This is the first report of transesophageal electrophysiology study in the diagnosis of DAVNNT. A diagnosis of DAVNNT was

Left Ventricular Summit Arrhythmias: State-of-the-Art Review of Anatomy, Mapping, and Ablation Strategies.

The left ventricular summit (LVS) is the most common site of epicardial arrhythmias. Ablation of LVS arrhythmias continue to pose a challenge to the electrophysiologist because of its complex and intimate anatomical location. In this review, we undertake a detailed examination of the intricate anatomy of the LVS alongside a comprehensive synthesis of mapping and ablation strategies used to treat LVS arrhythmias.

Feasibility of transthoracic echocardiographic guidance for multicatheter electrophysiological mapping studies in horses.

BACKGROUND: Improved characterization of arrhythmias is based on minimally invasive catheterizations. However, these catheterizations have been poorly explored in horses because apart from 3-dimensional (3D) mapping systems, continuous guidance of the catheter's position with adequate detail is difficult using current imaging modalities. HYPOTHESIS: Position multiple electrophysiology catheters simultaneously at predetermined strategical positions in the heart using transthoracic echocardiographic guidance. ANIMALS: Eight adult healthy horses. METHODS: Observational study. Two electrophysio

T-wave alternans: reviewing the clinical performance, understanding limitations, characterizing methodologies.

Accurate recognition of individuals at higher immediate risk of sudden cardiac death (SCD) is still an open question. The fortuitous nature of acute cardiovascular events just does not seem to fit the well-known model of ventricular tachycardia/fibrillation induction in a static arrhythmogenic substrate by a synchronous trigger. On the mechanism of SCD, a dynamical electrical instability would better explain the rarity of the simultaneous association of a correct trigger and an appropriate cardiac substrate. Several studies have been conducted trying to measure this cardiac electrical instabil

Electrophysiological modulation of cardiomyocytic tissue by transfected fibroblasts expressing potassium channels: a novel strategy to manipulate excitability.

BACKGROUND: Traditional pharmacological therapies aiming to modify the abnormal electrophysiological substrate underlying cardiac arrhythmias may be limited by their relatively low efficacy, global cardiac activity, and significant proarrhythmic effects. We suggest a new approach, in which transfected cellular grafts expressing various ionic channels may be used to manipulate the local electrophysiological properties of cardiac tissue. To examine the feasibility of this concept, we tested the hypothesis that transfected fibroblasts expressing the voltage-sensitive potassium channel Kv1.3 can m

The arrhythmogenic substrate--diagnostic and therapeutic implications: hypertrophic cardiomyopathy.

Hypertropic cardiomyopathy provides an important model for the study of mechanisms of sudden death. Initiating factors for sudden cardiac death may include sinus tachycardia and paroxysmal supraventricular arrhythmias. The haemodynamic effects of such triggers may be modulated depending on the patients exercise haemodynamics, peripheral vasomotor control and autonomic function. Myocardial disarray provides the underlying histological substrate for the lethal ventricular arrhythmias that represent the final common pathway. Attempts at individual risk stratification for sudden cardiac death usin

Anaesthetic management of paediatric patients undergoing electrophysiology study and ablation for supraventricular tachycardia: A focused narrative review.

Every year, 80,000-100,000 ablation procedures take place in the United States and approximately 1% of these involve paediatric patients. As the paediatric population undergoing catheter ablation to treat dysrhythmia is constantly growing, involvement of anaesthesiologists in the cardiac electrophysiology laboratory is simultaneously increasing. Compared with the adult population, paediatric patients need deeper sedation or general anaesthesia (GA) to guarantee motionlessness and preserve comfort. As a result, the anaesthesiologist working in this setting should keep in mind heart physiopathol
