Practice and theory of "delayed" embolectomy. A 22-year perspective.

A study was made of delayed embolectomy in 45 patients (55 limbs). In 5 patients (11%) the cause of arterial embolization was rheumatic heart disease, and 40 patients (89%) suffered from atherosclerotic cardiovascular disease. The study was divided into 3 phases: I (1960-1964), II (1965-1974) and III (1975-1981). In phases II and III surgery was carried out using the Fogarty catheter technique. In 5 patients the embolus was located in the upper extremity. Seven patients died and 8 major amputations were performed within 30 days of surgery. Use of the Fogarty catheter technique and persistent a

Salvage pulmonary embolectomy following cardiac arrest: a 10-year experience.

INTRODUCTION: Acute pulmonary embolism (PE), particularly massive PE, presents significant mortality risk, often necessitating emergency intervention such as surgical embolectomy. The prognosis for patients undergoing such interventions, especially after cardiac arrest, remains poor due to hemodynamic complications. This study aims to evaluate the efficacy of salvage pulmonary embolectomy in patients experiencing cardiac arrest due to massive PE, focusing on survival outcomes and influential risk factors. METHODS: Conducted at a single center over 10 years, this retrospective study involved 2

Catheter-directed interventions versus surgical embolectomy in massive pulmonary embolism.

OBJECTIVE: Catheter-directed intervention (CDI) use in massive pulmonary embolism (PE) is rarely studied due to guideline recommendations for systemic thrombolysis (stPA). Nevertheless, surgical embolectomy (SE) and CDI remain well-accepted alternatives in massive PE management, particularly when patients have contraindications to or do not improve after stPA. We hypothesized that CDI and SE have comparable outcomes in the treatment of massive PE. METHODS: We conducted a retrospective review of patients presenting with massive PE who underwent CDI or SE at a multihospital health care system (

Surgical embolectomy for paradoxical cerebral embolism with massive pulmonary embolism.

The patient was a 68-year-old woman with a history of multiple cerebral emboli. The patient presented with dysarthria, dysphagia, and left facial paralysis. Emergent MRI demonstrated multiple emboli in the brain stem and left occipital lobe. Echocardiography and chest CT demonstrated floating emboli entrapped in the foramen ovale and pulmonary emboli in both main pulmonary arteries. Emergent pulmonary embolectomy and extraction of the emboli on the foramen ovale were successfully performed without neurological exacerbation.

Retrograde Pulmonary Embolectomy for Acute Pulmonary Embolism: A Simplified Technique.

Surgical embolectomy in acute pulmonary embolism is usually reserved for patients with massive pulmonary embolism presenting with cardiogenic shock, or for whom thrombolysis is absolutely contraindicated or has failed. Incomplete removal of thrombotic material lodged in the distal pulmonary arterial bed is considered an important cause of persistent pulmonary hypertension. Retrograde pulmonary embolectomy is an adjunct to conventional pulmonary embolectomy, resulting in more complete embolectomy, specifically of material lodged in the distal pulmonary arterial bed. We describe our simplified t

Emergent cervical surgical embolectomy for extracranial internal carotid artery occlusion.

BACKGROUND: Acute extracranial internal carotid artery (ICA) occlusion by a huge cardiogenic embolus is rare, but can be catastrophic. METHODS: Seven patients with acute ischemic stroke due to embolic occlusion of extracranial ICA who underwent emergent cervical surgical embolectomy were retrospectively reviewed. Diagnosis was made in six patients with magnetic resonance imaging (MRI) with optional digital subtraction angiography (DSA), while computed tomography (CT) and DSA were used in one patient with an implanted pacemaker. Clinical outcomes, including recanalization rate, recanalization

Anaesthetic management of emergency caesarean section followed by pulmonary embolectomy.

This report discusses the anaesthetic management of a patient undergoing emergency Caesarean section followed by an open pulmonary embolectomy with cardiopulmonary bypass. This case was complicated by the major physiological alterations associated with pregnancy and delivery, massive blood loss, cardiac dysrhythmias, hypotension, and cardiac arrest. Both patients, mother and child, made a full recovery.

Urgent open embolectomy for cardioembolic cervical internal carotid artery occlusion.

Acute ischemic stroke attributable to cervical internal carotid artery (ICA) occlusion is frequently associated with severe disability or death and is usually caused by atherosclerosis. By contrast, the cardioembolic cervical ICA occlusion is rare, and feasibility of urgent recanalization remains unclear. We present the first study in the literature that focuses on urgent open embolectomy for the treatment of cardioembolic cervical ICA occlusion. A retrospective review of the charts for patients undergoing open embolectomy was performed. Between April 2006 and September 2007, 640 consecutive p

Outcome predictors of open embolectomy in middle cerebral artery occlusion.

OBJECTIVE: The purpose of this paper was to analyse the outcome of the patients with acute middle cerebral artery (MCA) occlusion treated by open embolectomy. METHODS: A clinical chart review was retrospectively conducted for 30 patients who had MCA occlusion and were treated with open embolectomy. According to the Glasgow Outcome Scale, the patients' outcome at discharge is divided in two groups: favorable outcome (good recovery and moderate disability) or unfavorable outcome (severe disability, vegetative state and death). The following variables between the favorable and unfavorable outcom

Innovative Use of Interventional Hardware in Surgical Embolectomy in Small Infants.

Surgical embolectomy for thrombosis and ischemia following femoral arterial cannulation in small infants is rare. The nonavailability of embolectomy balloons in small sizes and the small size of the native vessels pose a challenge in repair. We describe the use of a coronary angioplasty balloon for the purpose of embolectomy in a 2.5-kg infant with femoral arterial occlusion and limb ischemia.

A case of acute superior mesenteric embolism successfully treated by embolectomy.

Superior mesenteric artery embolism is undoubtedly fatal unless operative intervention is promptly performed. The first case successfully managed by embolectomy in Taiwan is reported in this communication. The key to successful management lies in the early suspicion in patients with atrial fibrillation or recent myocardial infarction, presenting with sudden abdominal pain and an unremarkable physical examination. Abdominal angiography is strongly recommended; however, immediate laparatomy should not be postponed if angiography is not available. Early embolectomy is the only useful means of tre

Retriever first embolectomy (ReFirE): An alternative approach for challenging cervical access.

After endovascular treatment became the standard of care procedure for acute ischaemic stroke with large artery occlusion in 2015 the number of performed interventions has increased dramatically. Especially because age is no exclusion criterion for endovascular treatment, a relevant number of patients with difficult to access carotid arteries has to be treated. In these patients a direct puncture of the carotid is a valuable tool but is associated with severe complications and an initial learning curve. We therefore developed the so called retriever first embolectomy (ReFirE) technique in whic

[Treatment of retinal arterial branch occlusion with transluminal Nd:YAG laser embolectomy].

BACKGROUND: There is currently no effective therapy for retinal artery branch occlusion (RABO). Transluminal Nd:YAG embolectomy (TYE) was developed to achieve rapid reperfusion; however, it is still a subject of controversy. A Nd:YAG laser is focused on the occluded vessel, the vessel wall is ruptured and the embolism dislocates to the vitreous humor through the opening in the artery. METHOD: We retrospectively examined the results of five patients treated with TYE. RESULTS: The age of the patients was 26-75 years (mean age 58 years, median 68 years). Initial visual acuity was hand movements

Factors affecting limb salvage and mortality in patients undergoing femoral embolectomy.

The management and outcome of 131 acute femoral arterial occlusions in 126 patients over a period of 7 years is presented. The emboli were of cardiac origin in 82% of cases; 96% of the patients were treated with thromboembolectomy. The overall mortality rate was 26% with a limb salvage rate of 88% amongst the survivors. There was an increased risk of mortality in patients having both atrial fibrillation and myocardial infarction, saddle emboli and those having delayed embolectomy after 24 h. Only the latter was found to be statistically significant. One-third of the patients who died had a fai

Hyperkalaemic cardiac arrest following intravenous hydralazine and propranolol therapy post-embolectomy.

A case of hyperkalaemic cardiac arrest occurring in a 50-year-old woman after bilateral femoral embolectomies is presented. The postoperative course was complicated by acute renal failure, metabolic acidosis, hypotension and low cardiac output. The possible contribution of hydralazine and propranolol therapy to the rapid development of hyperkalaemia is discussed.

Experimental microsurgical embolectomy after middle cerebral artery embolization in the dog.

The effects of microsurgical embolectomy were investigated clinicopathophysiologically in 60 dogs after occlusion of the middle cerebral artery (MCA) trunk with a silicone cylinder embolus. One group of animals served as a control (non-embolectomized group), and in the other two groups the embolus was removed 3 or 6 hours after occlusion (3-hour or 6-hour embolectomy group). In the non-embolectomized animals, major neurological deficits with deep cerebral infarction were observed. Regional cerebral blood flow (CBF) in the basal ganglia decreased most prominently. Sensory evoked potentials also
