Blue toe syndrome can occur due to distal embolization from proximal lesions such as an aortic thrombus. We describe the case of a patient who presented with chronic limb threatening ischemia due to a flow-limiting infrarenal aortic thrombus, with gangrene from distal embolization to the left fifth toe, and was successfully treated with endovascular aortic stent graft insertion. Distal embolization during instrumentation was successfully prevented by using a partially deployed Wallstent (Boston Scientific) as an embolic protection device. The reconstrainable Wallstent device can be considered

OBJECTIVES: The use of cerebral embolic protection (CEP) during transcatheter aortic valve implantation (TAVI) has been studied in several randomised trials. We aimed to perform a systematic review and Bayesian meta-analysis of randomised CEP trials, focusing on a clinically relevant reduction in disabling stroke.
METHODS: A systematic search was applied to three electronic databases, including trials that randomised TAVI patients to CEP versus standard treatment. The primary outcome was the risk of disabling stroke. Outcomes were presented as relative risk (RR), absolute risk differences (AR

BACKGROUND: Cerebral embolic protection devices (EPDs) were developed to mitigate the risk of stroke during transcatheter aortic valve replacement (TAVR), but their benefit remains unproven. In the PROTECTED-TAVR trial (Stroke Protection With Sentinel During Transcatheter), EPD use did not reduce periprocedural stroke (primary study outcome) but led to a 62% reduction in the secondary end point of disabling stroke. Given these results, the impact of EPDs during TAVR remains unclear.
METHODS: We used STS/ACC TVT registry data to examine the association between EPD use and a proxy for disabling

BACKGROUND AND AIMS: Cerebral embolic protection (CEP) devices are employed to capture embolic debris and reduce the risk of stroke during transcatheter aortic valve replacement (TAVR). Evidence is mixed regarding the safety and efficacy of CEP. We aimed to summarize the safety and effectiveness of CEP use during TAVR.
METHODS: Electronic databases, including PubMed, PubMed Central, Scopus, Cochrane Library, and Embase, were searched using relevant search terms for articles relating to CEP. All relevant data from 20 studies were extracted into a standardized form. Statistical analyses were pe

The importance of angioplasty and stenting in the treatment of carotid artery disease cannot be underestimated. Successful carotid stenting does not only depend on the operator's skills and experience but also on an adequate selection of cerebral protection devices, and carotid stents can help avoid neurological complications. A broad spectrum of carotid devices is currently on the market, and because each of them has its own advantages and disadvantages, it is virtually impossible to claim that one specific device is the best. The individual characteristics of each specific protection system

BACKGROUND: Though distal filter protection during carotid stenting reduces the risk of cerebrovascular events, periprocedural stroke remains a risk despite their broad usage. This observation may be related to the pore size of common filters. The FiberNet distal filter system is unique by its very small pore size (40 µm) as well as its low profile and flexibility. Little data is available regarding the clinical performance and safety of this device.
PURPOSE: The aim was the evaluation of the safety of the FiberNet embolic protection system during carotid artery stenting.
METHODS: All consec

OBJECTIVES: The authors hypothesized that the origin of the right coronary artery (RCA) is a direct continuation of the major aortic arch branches (MAAB) takeoff plane, which may have implications for brachiocephalic interventions and next generation transcatheter aortic valve intervention (TAVI) embolic protection devices (EPDs).
METHODS: In this single-center, retrospective, cross-sectional study, the authors analyzed computed tomographic angiography (CTA) images from 92 patients undergoing TAVI evaluation to determine the spatial relationship between the origin of the RCA and the MAAB take

BACKGROUND: Transcarotid artery revascularization (TCAR) has become an increasingly utilized surgical option for carotid disease, relying on flow reversal for distal embolic protection (DEP). However, anatomical variations, such as an isolated middle cerebral artery hemisphere, and severe carotid stenosis may compromise the safety of flow reversal intraoperatively. The authors describe the use of an adjunctive DEP device during TCAR as a "bailout" strategy for high-risk patients.
OBSERVATIONS: The authors present the case of an adult patient with high-grade symptomatic carotid stenosis who wa

BACKGROUND: Some studies have shown evidence that the prophylactic use of bethanechol chloride (BC) may be useful in preventing the incidence and/or severity of xerostomia (XT). However, the indication of BC in irradiated patients with XT needs to be better characterized. The study aimed to evaluate the influence of BC on XT, salivary flow rate, and salivary composition in patients previously submitted to head and neck radiotherapy.
MATERIAL AND METHODS: Forty five irradiated patients complaining of XT used 50 mg/day of BC for 3 months, and the salivary parameters were evaluated in 4 Phases (

Indwelling pacemaker or defibrillator leads are often extracted with use of a cutting laser sheath. We report embolization of a tined pacemaker lead tip into the azygous vein, with the use of a distal embolization protection device to retrieve the fragment. Creative and non-traditional use of a filter-based device may assist in retrieval of embolized material.

BACKGROUND: Carotid artery stenting (CAS) is frequently used in the management of carotid artery stenosis. Although the use of embolic protection devices (EPDs) is common, the association with lower mortality has been questioned. We aimed to assess the association between CAS with and without EPD and periprocedural stroke and mortality.
METHODS: We included patients from the nationwide Vascular Quality Initiative registry who underwent CAS between 2015 and 2019. Patients undergoing transcarotid artery revascularization were excluded. Outcomes after CAS with either no EPD attempted or failed v

A 77-year-old female with peripheral artery disease underwent brachiocephalic artery stenting for right upper extremity claudication. Given a very high atherosclerotic burden seen on CT, a dual-filter embolic protection device (Sentinel, Boston Scientific) was deployed from the right radial artery to protect the right common carotid and subclavian arteries, and therefore the vertebral artery, during the stenting procedure. This case report demonstrates a novel use of this dual-filter device to provide both carotid and vertebral artery embolic protection during brachiocephalic artery interventi

The evidence regarding the impact of cerebral embolic protection devices (EPDs) on outcomes following transcatheter aortic valve replacement (TAVR) is limited. The objective of this study was to evaluate in-hospital outcomes with the use of cerebral EPDs in TAVR. We performed a comprehensive EMBASE and PUBMED search to investigate randomized control studies or propensity score-matched retrospective studies which assessed patients undergoing TAVR with or without EPD up to April 2021. Endpoints of interest were in-hospital mortality, stroke, acute kidney injury, pacemaker implantation, major ble

We report a case of a stroke and upper limb ischemia in a 27-year-old female secondary to a right cervical rib and retrograde thromboembolization. Follow-up showed complete patency of the vessels after thrombectomy and internal carotid artery stenting followed by transbrachial embolectomy performed with a cerebral protection device. The cervical rib was surgically removed to prevent additional events.

OBJECTIVES: The aim of this study was to investigate the efficacy and safety of intraprocedural embolic protection (EP) during transcatheter aortic valve replacement (TAVR).
BACKGROUND: Randomized controlled trials (RCTs) investigating the efficacy of EP devices during TAVR were relatively underpowered.
METHODS: A systematic review and study-level meta-analysis was performed of randomized controlled trials that tested the efficacy and safety of EP during TAVR. Trials using any type of EP and TAVR vascular access were included. Primary imaging efficacy endpoints were total lesion volume and n
