Cognitive outcome of cerebral fat embolism.

Cerebral fat embolism is an uncommon but serious complication of long-bone fracture. We report a young adult patient who sustained fat embolism after a femoral fracture. He developed stupor and coma within 24 hours from his injury. His acute recovery was characterized by marked frontal dysfunction. A comprehensive neuropsychological evaluation 4 months later revealed overall normal cognitive function, except for mild residual frontal dysfunction and weakness of verbal memory.

Autopsy diagnosis of fat embolism syndrome.

The fat embolism syndrome (FES) is considered a clinical diagnosis. It typically occurs within several days following major traumatic injury, usually involving fractures of the pelvis and/or lower extremities. Fat embolism syndrome is characterized by the onset of respiratory, neurological, cutaneous, and hematologic manifestations and is thought to be related to intravascular embolization of fat, presumably arising from within the fractured bone marrow space. In its most severe form, FES can be lethal. The presence of fat emboli within the microvasculature of the lungs, brain, and sometimes o

MRI findings in cerebral fat embolism.

The purpose of this retrospective study was to demonstrate the MRI features of cerebral manifestations in patients with fat embolism syndromes in comparison with cerebral CT (CCT). Magnetic resonance imaging was performed according to standard protocols revealing multiple small non-confluent hyperintense intracerebral lesions larger than 2 mm on proton-density and T2-weighted images to various extents in three of four patients with clinically suspected cerebral fat embolism. Cerebral CT was negative in all patients. Our findings confirm that MRI can detect cerebral fat embolism with a higher s

Fat embolism syndrome: A clinical appraisal.

Over a four-year period (1987-1990), 28 cases of fat embolism syndome (FES) were prospectively evaluated for clinical, hematological and biochemical manifestations and prognostication. In a total of 2,112 patients admitted to the Intensive Care Unit (ICU), 1.3% were diagnosed as having FES which was also seen in 2.6% of the patients with polytrauma. FES followed multiple fractures in 60.7% of cases; 17.9% of patients had an isolated fracture of the femur. Other predisposing fractures included closed head injury (7.1%), lipectomy (7.1%), and soft tissue injury (3.6%). The mean free interval was

Fat embolism, intravascular coagulation, and osteonecrosis.

A triad of intraosseous fat embolism, intravascular coagulation (both thrombosis and hemorrhage), and osteonecrosis was pathologically demonstrated to coexist for the first time in humans. Specimens were evaluated from the earliest nontraumatic (18 hours) and traumatic (29 hours) femoral head lesions yet reported, and the cause and early pathogenesis were confirmed in a third case. An absolute overload of subchondral fat emboli, with hypercoagulability, stasis, and endothelial damage by free fatty acids, appears to cause end-organ death by triggering intravascular coagulation. This intermediar

Fat embolism in severely injured patients.

Forty-five patients with severe blunt injuries were examined during eight posttraumatic days. At random, 14 patients were given three doses of methylprednisolone intravenously; 10 mg/kg at 8-hour intervals. Fat embolism syndrome was diagnosed in 13/45 patients, only one of whom had received corticosteroid (p = 0.03). Shock, acidosis and elevated plasma catecholamines showed no correlation with the occurrence of fat embolism syndrome. Platelet counts immediately after trauma were significantly lower in the fat embolism patients than in the other trauma patients, indicative of early platelet agg

Fat embolism in sickle cell disease.

Fat embolism is a known complication of marrow infarction in patients with sickle cell disease (Hb S/S and Hb S/C) disease. It should be considered in sickle cell crisis when there is deterioration in respiratory function, a fall in arterial PO(2), a normoblastaemia, and a thrombocytopenia in the peripheral blood. We now report a patient in whom the diagnosis was confirmed in life by identifying fat in the sputum, demonstrating disseminated intravascular coagulation, and using a double isotope technique to distinguish recent from old marrow infarction. Use of these features enabled an early di

Extremely Acute-Onset Cerebral Fat Embolism.

Cerebral fat embolism (CFE) causes the neurological involvement observed in fat embolism syndrome, which is a post-traumatic complication seen mostly after long bone fractures and usually presents 24-72 h after the injury. An early 80s female who sustained an isolated traumatic fracture of the left distal femur without dislocation was alert on admission but fell into a coma 55 min after the injury. Brain computed tomography showed no abnormalities. Brain magnetic resonance imaging was performed approximately 5 h after the accident, and diffusion-weighted images revealed hyperintense, dot-like

Therapeutic aspects of fat embolism syndrome.

Signs and symptoms of clinical fat embolism syndrome (FES) usually begin within 24-48 hours after trauma. The classic triad involves pulmonary changes, cerebral dysfunction, and petechial rash. Clinical diagnosis is key because laboratory and radiographic diagnosis is not specific and can be inconsistent. The duration of FES is difficult to predict because it is often subclinical or may be overshadowed by other illnesses or injuries. Medical care is prophylactic or supportive, including early fixation and general ICU management to ensure adequate oxygenation and ventilation, hemodynamic stabil

The fat embolism syndrome. A review.

While fat embolism occurs in most (more than 90%) patients with traumatic injury, the fat embolism syndrome (FES) occurs in only 3%-4% of patients with long-bone fractures. FES involves multiple organ systems and can cause a devastating clinical deterioration within hours. The major clinical features of FES include hypoxia, pulmonary edema, central nervous system depression, and axillary or subconjunctive petechiae. Improvements have been made in supporting the respiratory compromise and adult respiratory distress syndrome that these patients develop. Aggressive measures to improve the pulmona

[Fat embolism; a survey (author's transl)].

As far as diagnostics, prophylaxis and therapy are concerned, this clinical entity has not lost its acuteness. However it can no longer be considered only as a mechanical problem (impression of fat corpuscles out of the broken bone into the capillaries), instead it has to be regarded as an epiphenomenon of shock in combination with the respiratory distress syndrome and disorders of the microcirculation. A successful anti-shock regimen including effective stabilisation of fractures are at present the best means to prevent fat embolism. The effect of medicaments after manifestation of fat emboli

Residual lesions of cerebral fat embolism.

Necropsy findings of a case which survived for three months after massive cerebral fat embolism are presented. The lesions are characterized by numerous patchy necroses and areas of demyelination in the white matter of cerebral and cerebellar hemispheres and brain stem, accompanied by atrophy of white matter and ventricular enlargement. Attention is drawn to the significance of these lesions for posttraumatic coma or dementia.

Blood alcohol and fat embolism syndrome.

In an analysis of the cases of 100 consecutive patients with diaphyseal fractures in the major bones of the lower limb, the incidence of fat embolism syndrome was 17 per cent. The blood alcohol level was determined at the time of admission. A raised level of alcohol in the blood was associated with a lower incidence of fat embolism.

Fat embolism secondary to yellow fat disease in an Appaloosa horse.

A 4-year-old female Appaloosa horse was referred to the Department of Comparative Pathology, Veterinary Faculty, with a history of colic and acute respiratory distress. At necropsy, gross lesions consisted of extensive firm, multinodular, hemorrhagic foci in fat tissues with yellow-brown discoloration. The most affected areas were peritoneal fat and perirenal, epicardial, and subcutaneous adipose tissues. Other findings were hepatic lipidosis and multiple 1-1.5 cm hemorrhagic foci scattered in both lungs. Histopathological examination revealed severe degeneration and necrosis of adipose tissue
