[Tuberculous meningoencephalitis in post-embryo transfer status].

A 30-year old pregnant woman, who received an embryo transfer, suffered since the 12th week of pregnancy from therapy-resistant attacks of fever for several weeks. With progressive deterioration, the increasingly somnolent patient showed by now clear signs of meningism and acid-fast bacilli were found in the cerebrospinal fluid. NMR showed a finding consistent with bacterial meningoencephalitis. During therapy with INH, Rifa and EMB, the clinical course and the CSF findings showed rapid improvement. Nevertheless, spontaneous abortion occurred in the 23rd week of pregnancy. Previous history rev

Techniques for preparation prior to embryo transfer.

RATIONALE: Embryo transfer (ET) is the final and most vulnerable step in in vitro fertilisation (IVF) treatment. Pregnancy rates after ET may be influenced by several factors, including straightening the utero-cervical angle, cervical and endometrial preparation, and techniques for traversing the cervix. Recent research has focused on improving the ET technique in the hope of increasing IVF success rates. This review, first published in 2009, examined preparation techniques, as it remains unclear whether these simple interventions make ET an easier procedure with higher success rates and fewer

An appraisal of current embryo transfer strategies.

Embryo transfer, one of the most essential procedures in assisted reproductive technology, plays a vital role in the success of in-vitro fertilization and intracytoplasmic sperm injection. During the last decades, the strategies for embryo transfer have changed dramatically. In this review, we evaluate the efficacy and safety of several current embryo transfer strategies including fresh versus frozen embryo transfer, cleavage- versus blastocyst-stage embryo transfer, and single- versus double-embryo transfer. Available evidence indicates that the freeze-only strategy improves the live birth ra

Racial disparities in frozen embryo transfer success.

PURPOSE: To compare pregnancy and birth outcomes after frozen embryo transfers (FETs) among White, Black, and Asian women and evaluate the effect of patient, protocol, and cycle characteristics on success. METHODS: A retrospective chart review identified women who underwent an autologous FET at an academic fertility center between January 2013 and March 2020. RESULTS: White, Black, and Asian women completed 1,181 (71.7%), 230 (14.0%), and 235 (14.3%) cycles, respectively. Black women were significantly less likely to achieve a positive hCG level (AOR 0.66, 95% CI 0.49-0.90), clinical pregnan

Heterotopic pregnancy after a single embryo transfer.

Heterotopic pregnancy is a rare and life-threatening condition which is defined as coexistent intrauterine and ectopic gestation. The risk of ectopic and heterotopic pregnancy is increasing due to the increased risk of multiple pregnancies with the aid of assisted reproductive technologies. However, it hardly happens in the setting of single embryo transfer, since single embryo transfer significantly reduces the incidence of multiple pregnancies. Surprisingly, we experienced a case of heterotopic pregnancy after a single embryo transfer caused by coincidental natural pregnancy during assisted

Antibiotics prior to embryo transfer in ART.

BACKGROUND: Embryo transfer (ET) involves the placement of one or more embryos into the uterine cavity, usually by passing a catheter through the cervical os. ET is the final step in an assisted reproductive technology (ART) cycle, where a woman has undergone controlled ovarian stimulation, egg retrieval and in vitro fertilisation of her eggs. Despite the transfer of high quality embryos, many ETs do not result in a pregnancy. There are many factors which may affect the success of ET, including the presence of upper genital tract microbial colonisation. The administration of antibiotics prior

Artificial insemination and embryo transfer in mares.

Mares can be artificially inseminated with chilled or frozen semen to increase the revenue from their offspring. Embryo transfer can be used to produce more than one foal from a single mare per season. Recent advances in using equine follicle-stimulating hormone to induce superovulation in mares have stimulated research on preserving equine embryos. Equine embryos are usually collected on day 7 or 8 after ovulation, and younger (day 6.5) embryos are typically cryopreserved. Cryopreservation improves the ability of veterinary clinicians to preserve embryos for implantation in recipient mares an

[Elective single embryo transfer - the Scandinavian experiment].

Limiting the number of embryos transferred from three to two does not reduce the high risk of twin pregnancy (between 21 and 40%). Scandinavian centers have proposed in the 2000s the elective single embryo transfer (eSET) as the only means to reduce maternal, neonatal and psychological consequences related to multiple births. Pooled results from prospective randomized controlled trials and prospective cohort studies comparing eSET and transfer of two embryos (DET) in a selected population have confirmed the almost complete disappearance of twins when eSET was effective but the compromising eff

Bioengineering studies of the embryo transfer procedure.

Embryo transfer (ET) is the final manual intervention in extracorporeal fertilization in which an embryo is transferred into the uterus by a transcervical catheter. The low rates of embryo implantation within the uterus are attributed, among other factors, to the ET technique, which depends on a multitude of anatomical, physiological, and mechanical aspects. We developed computational and experimental models to simulate ET to examine the contribution of mechanical features to the success of this procedure. The experimental model allowed laboratory simulations of the dispersion of the catheter

Single embryo transfer - state of the art.

Every practitioner active in the field of assisted reproduction treatment is aware of the risks and complications related to twin and higher-order multiple pregnancies. Introduction of single embryo transfer (SET) into IVF/intracytoplasmic sperm injection (ICSI) is one of the possible ways of reducing the rate of twin pregnancy. Careful selection of patients, in combination with elective SET, has been shown to decrease the twin pregnancy rate while maintaining a stable ongoing pregnancy rate. The combination of a woman younger than 38 years of age, in her first or second IVF/ICSI cycle and wit

Technical note: porcine non-surgical embryo transfer.

The cervix is a barrier for transferring porcine embryos non-surgically. A non-surgical embryo transfer instrument composed of four parts has been developed that allows the delivery of embryos into the lumen of one uterine horn. Part 1, a modified artificial insemination spirette, is used to produce a cervical lock and to assist in manipulating Part 2 through the cervix. Part 2 is made of stainless steel tubing with a unique curved tip that can be manipulated through the folds of the cervix. Part 3 is a testing bar to ensure that the tip of Part 2 reaches the bifurcation of the uterus. Part 4,

Endoscopic embryo collection and embryo transfer into the oviduct and the uterus of pigs.

We describe the first complete embryo transfer program, including flushing of embryos from the oviducts via the uterine horns, transfer of embryos into the Fallopian tubes or the uterine horns and recording of the number of piglets born live. The described procedure is minimally invasive and allows the use of pigs simultaneously for embryo collection and production of normal pregnancies. A 30 degrees forward oblique endoscope provided optimal visualization of the reproductive organs and free access to the organs for embryo flushing and transfer. In contrast to surgical and nonsurgical methods,

Every second counts: the association of embryo transfer duration with live birth following 2267 single, euploid, frozen embryo transfer.

PURPOSE: To identify the impact of the duration of the embryo transfer procedure on pregnancy outcomes. METHODS: This was a retrospective cohort study at a single center including single euploid frozen embryo transfers from April to December 2022. Embryo transfer time was defined as the total time (seconds) from when the embryologist handed the catheter with the loaded embryo to the physician, to the release of the embryo into the uterus. Time measurements were divided into quartiles: (1) 4-27 s, (2) 28-38 s, (3) 39-54 s, and (4) 55-1028 s. The primary outcome was the probability of live birt

Effect of sequential embryo transfer on in vitro fertilization and embryo transfer outcomes: a systematic review and meta-analysis.

BACKGROUND: Sequential embryo transfer has been recognized as a strategy to increase pregnancy rates in women undergoing in vitro fertilization and embryo transfer (IVF-ET). However, its impact on assisted reproductive outcomes remains to be substantiated by robust evidence. This systematic review aims to summarize and analyze the available evidence to investigate the effect of sequential embryo transfer on assisted reproductive outcomes. METHODS: A comprehensive literature search was executed across the Pubmed, Cochrane Library, Web of Science, and Scopus databases in accordance with the Pre

Improving reproductive outcomes in frozen embryo transfer over fresh embryo transfer in women with endometrioma: A historical cohort study.

BACKGROUND: Endometrioma, a common manifestation of endometriosis, often indicates the severity of the disease. In vitro fertilization and embryo transfer (ET) are key therapeutic strategies for infertility associated with endometriosis. However, the optimal type of ET (frozen or fresh) and its impact on pregnancy success rates remain debated, with limited studies available. OBJECTIVE: This historical cohort study aimed to compare fertility and neonatal outcomes, focusing on live birth rate (LBR), clinical pregnancy, and implantation rates in women with endometrioma-associated infertility, be

Effectiveness and safety of consecutive single embryo transfer compared to double embryo transfer: results from the UK HFEA registry.

STUDY QUESTION: How does two-consecutive single embryo transfer (2xSET) affect reproductive outcomes of IVF and ICSI compared to double embryo transfer (DET)? SUMMARY ANSWER: Two-consecutive SET may provide greater or comparable live birth rate (LBR); with lower multiple birth, preterm birth, and pregnancy loss or neonatal death rates compared to DET. WHAT IS KNOWN ALREADY: Elective SET in IVF/ICSI is widely encouraged over DET to minimize the risk of multiple births and associated morbidities. Despite this, multiple birth rates following IVF remain higher than the 10% target across Europe a

Endometrial echo patterns of embryo transfer day affect pregnancy outcomes in frozen embryo transfer cycles: a retrospective clinical study.

BACKGROUND: The echo pattern constitutes an ultrasonic index that reflects the characteristics of the endometrium across various phases of the menstrual cycle. However, consensus of endometrial echo pattern and pregnancy outcomes is lacking in clinical application. METHODS: The retrospective cohort study analyzed the data from the electronic records of women who underwent frozen embryo transfer (FET) with hormone replacement treatment (not only one cycle per patient) between July 2020 to August 2021 at Reproduction Medicine Center of Jbnminling Hospital of Medical College of Nanjing Universit

Blastocyst-stage embryos provide better frozen-thawed embryo transfer outcomes for young patients with previous fresh embryo transfer failure.

Older patients or patients with a reduced ovarian response have a low number of embryos, which limits the opportunity for embryo selection. However, for young patients undergoing frozen-thawed embryo transfer (ET), it remains unclear whether embryo stage affects pregnancy outcomes. In the present study, a total of 2952 patients undergoing their first frozen-thawed ET were divided into two groups: patients who had experienced one failed fresh ET (Group A) and patients who had not received fresh ET because of the high risk of ovarian hyperstimulation syndrome (OHSS) (Group B). Our results show t
