BACKGROUND: The newer atypical antipsychotics have gained prominence as first-line oral agents to treat severe agitation, but their use in the psychiatric emergency service (PES) setting has been limited because of the unavailability of parenteral formulations. The advent of parenteral formulations of atypical antipsychotics, beginning with ziprasidone in 2002, might afford an alternative treatment option.
OBJECTIVE: The objective of this article was to determine the effectiveness and tolerability of i.m. ziprasidone used to treat agitation in elderly patients.
METHODS: In this data analysis

BACKGROUND: Substance use is a major contributor to psychiatric emergencies, often complicating the management of acute agitation. Despite the high prevalence of co-occurring substance use in emergency psychiatric settings, evidence guiding pharmacologic interventions remains limited. This study examined whether active substance use, as measured by urine drug screening (UDS), predicted clinical and safety outcomes among patients requiring intramuscular (IM) medication for agitation in an emergency psychiatric setting.
METHODS: This retrospective chart review included 103 patients presenting t

INTRODUCTION: The successful treatment of adolescents almost always requires parents' involvement in the treatment process. Thus, parental involvement will impact further treatment, especially concerning the acute management of self-harming behavior of their children. Parental burden or low parental motivation for treatment can significantly affect the success of the intervention. Therefore, this study aimed at investigating how especially motivational factors of the adolescents and parents, as well as stressors of the parents, affect the course of non-suicidal self-injury (NSSI) after an acut

The present study investigated the predictors of an increased number of visits from individuals with some of the diagnoses noted in chapter F14 of ICD-10, from calls to the emergency psychiatric unit of a general hospital in São Paulo state, Brazil, in the period 2011-2012. Poisson regression models were carried out for the outcome variable, accounting for number of subsequent visits to the psychiatric emergency unit. For the analysis of this outcome we took into account the exposure time of each individual in the study. Our findings point to a population at risk for frequent psychiatric emerg

BACKGROUND: Adolescents experiencing psychiatric emergencies often require intensive interventions to prevent hospitalisation and support their return to education, employment or training. Intensive Community Care Services aim to provide an alternative to inpatient care.
AIMS: To evaluate the effectiveness of Intensive Community Care Services compared to treatment as usual in reducing the time to start or return to education, employment or training for young people with psychiatric emergencies.
METHOD: A multicentre, parallel-group, single-blinded, randomised controlled trial, including an i

On the basis of the rules of the Hamburg Law on Aid and Protective Measures in Mental Diseases, a psychiatric emergency service operates at night and on weekends. This emergency service is called upon to decide on the need for compulsory hospitalization and to prevent the same, if possible, via therapeutic alternatives. The article illustrates by documents an initial phase of the activities of this public service. Within the framework of a preliminary study, 63 recorded incidents of service by four physicians are presented who collected data on the situation encountered by them at the time of

Attendances at Accident & Emergency (A & E) departments by patients with mental health problems are on the increase. An emphasis on care in the community for patients with psychiatric disorders, and a society which is placing increased demands on the vulnerable, has meant that A & E departments are increasingly having to manage patients who have self-harmed and patients with acute mental health problems. Arguably, it is no longer acceptable or necessary for all patients presenting with self-harm to be admitted to hospital or to have a full evaluation by a psychiatrist. Other options exist wher

BACKGROUND: Telepsychiatry (TP), a live video meeting, has been implemented in many contexts and settings. It has a distinct advantage in the psychiatric emergency department (ED) setting, as it expedites expert assessments for psychiatric patients. However, limited knowledge exits for TP's effectiveness in the ED setting, as well as the process of implementing TP in this setting.
OBJECTIVE: This scoping review aimed to review the existing evidence for the administrative and clinical outcomes for TP in the ED setting and to identify the barriers and facilitators to implementing TP in this set

Patients who present to the emergency department (ED) with mental illness or behavioral complaints merit workup for underlying physical conditions that can trigger, mimic, or worsen psychiatric symptoms. However, there are wide variations in quality of care for these individuals. Psychiatry and emergency medicine specialty guidelines support a tailored, customized approach to patients. Our group has long advocated a dynamic comanagement approach for medical clearance in the ED, and this article summarizes best-practice approaches to the medical clearance of patients with psychiatric illness, t

The integration of psychiatric treatment into emergency services is critical to improving the management of severe mental health emergencies. Emergency departments (EDs) are frequently the initial point of contact for patients with mental emergencies, but they are not always prepared to offer adequate care. This systematic review aims to analyze the effectiveness of collaborative care models (CCMs), psychiatric consultation-liaison (C-L) teams, and telepsychiatry in enhancing psychiatric treatment in emergencies. We searched numerous databases, including PubMed, PsycINFO, EMBASE, and the Cochr

The reported findings of the European Consultation-Liaison Workgroup (ECLW) Collaborative Study describe consultation-liaison service delivery by 56 services from 11 European countries aggregated on a C-L service level. During the period of 1 year (1991), the participants applied a standardized, reliability tested method of patient data collection, and data were collected describing pertinent characteristics of the hospital, the C-L service, and the participating consultants. The consultation rate of 1% (median; 1.4% mean) underscores the discrepancy between epidemiology and the services deliv

Psychiatric disorders are best managed by interdisciplinary teams of psychiatrists, psychiatric nurses, social workers, and psychologists. Some Emergency Departments have access to such psychiatric services either in the hospital or in associated clinics or community mental health centers. Unfortunately, many are not staffed with mental health professionals or such expertise is available only on a limited basis, particularly on evenings and weekends. Therefore, one or more members of the Emergency Department staff should have specialized psychiatric training and experience. We have reviewed th

In the last few decades, there has been a remarkable increase in the number of children and adolescents presenting with psychiatric complaints to the emergency department in the USA. In Europe, however, less is known about the paediatric psychiatric emergency population. This study provides a clinical and demographic profile of this population and its service use in a European context. From 2003 to 2008, we registered 989 paediatric psychiatric patients consulting the psychiatric emergency services (PES) of a large university hospital in Belgium. During this period the number of patients incre

ACCESSIBLE SUMMARY: Studies have indicated that individuals who are homeless access hospital emergency departments more frequently and may have different needs than individuals who are housed. Successful interventions have been developed and tested to reduce discharge to homelessness for psychiatric inpatients but have not been similarly tested for discharge from emergency departments. This study was developed to provide baseline data on this issue to inform future emergency department interventions. Findings from the current study suggest that discharge from emergency departments to homelessn

Psychiatric prehospital emergencies account for an increasing proportion and involve multiple professional disciplines. Faced with a population with somatic, psychiatric, and social challenges, various interprofessional models have been developed to improve care, limit coercion, and reduce the burden on emergency departments. They operate at different levels: upstream triage by emergency call centers, multidisciplinary co-response combining law enforcement, emergency services, and mental health, or integration of psychiatric expertise within a prehospital team with/without systematic police in

Emergency department visits for pediatric mental health crises have risen sharply in recent years, while inpatient psychiatric bed capacity has declined, resulting in prolonged patient boarding. This article describes the development and implementation of behavioral health observation (BHO) models to address this gap in care. BHO provides time-limited, intensive psychiatric assessment and crisis stabilization with a multidisciplinary team focused on symptom reduction, diagnostic clarification, and structured disposition planning for patients. The article outlines patient selection, interdiscip

AIM AND BACKGROUND: We estimated health service utilisation attributable to methamphetamine use, its national impact and examined other predictors of health service utilisation among dependent methamphetamine users.
METHOD: Past year rates of health service utilisation (number of attendances for general hospitals, psychiatric hospitals, emergency departments, general practitioners, psychiatrists, counsellors or psychologists, and dentists) were estimated for three levels of methamphetamine use (no use, < weekly, ≥ weekly) using panel data from a longitudinal cohort of 484 dependent methamphet

OBJECTIVE: The Behavioral Health Emergency Assistance Response Division (B-HEARD) program is an emergency medical services (EMS)-based alternative to police-led responses for nonviolent mental health crises in New York City. This study aimed to examine B-HEARD's effects on nonviolent mental health-related EMS call rates.
METHODS: Data from the EMS Incident Dispatch Data file (January 2019-December 2024) were analyzed. The sample included 76 New York Police Department precincts, of which 31 adopted B-HEARD during the study period; the remaining 45 precincts did not adopt the program. A stagger

OBJECTIVE: Specific ethnic groups appear to be disproportionally represented in emergency compulsory admissions. This may be due to a parallel higher risk of psychopathology, but different pathways to care in patients from ethnic minorities may also be an explanatory factor. In this article we concentrate on the influence of ethnic background, pathways to psychiatric emergency services and the amount of past psychiatric treatment as predictors of emergency compulsory admission.
METHODS: All the patients coming into contact consecutively with the Psychiatric Emergency Service Amsterdam (PESA)

OBJECTIVE: Emergencies in child and adolescent psychiatry are prevalent, and pediatric psychiatric emergency departments play a crucial role in acute intervention during mental health-related crises. This retrospective cross-sectional study aimed to analyze the characteristics of children and adolescents who were admitted to a pediatric psychiatric emergency department.
METHOD: The data were collected from the records of 8803 patients (females 56.6%, Mage = 15.07 ± 2.6) admitted to our pediatric psychiatry emergency department between 2014 and 2021. Diagnoses were made using the International
