[Late study results following megacolon operations].

The basic pathophysiologic disturbance in Hirschsprung's disease is a functional obstruction caused by defective intramural nerve supply and by internal anal sphincter achalasia. Therapy consists in resecting the dysganglionic bowel segment. In each case however an aganglionic segment of different length and an internal analsphincter with a different degree of achalasia remain in situ. Therefore the postoperative results are dependant on an equilibrium between the proximal normal innervated colon and the length and function of the remaining aganglionic and achalic parts of the rectum and anal

Childhood constipation: evaluation and management.

Constipation is a common problem in children. It is also a long-term problem persisting for many months to years in children. Approximately 95% of childhood constipation is functional in nature without any obvious cause. Evaluation of a child with constipation requires a thorough history and physical examination. Hirschsprung's disease is an important cause of constipation arising in infancy and requires a thorough diagnostic evaluation and surgical treatment. Treatment of functional constipation in children requires a well-designed plan and a team approach involving the child, parents, and a

What is the role of stimulant laxatives in the management of childhood constipation and soiling?

BACKGROUND: Constipation is extremely common in childhood and may lead to overflow soiling/encopresis. Standard treatment of the more severe case is to empty the bowels of impacted faeces by the use of oral or rectal laxatives and then maintain regular bowel movements by the continuation of osmotic and stimulant laxatives. OBJECTIVES: The objective of the review was to determine the effect of stimulant laxative treatment in children with chronic constipation who may also suffer from soiling / encopresis. SEARCH STRATEGY: The Cochrane database of randomised controlled trials was searched. Add

[Attention deficit - hyperactivity disorder and enuresis in children and adolescents].

Frequency of comorbid disorders and neuropsychological state, executive functions (EF), were studied in two groups of patients aged from 5 to 14 years: 53 patients with attention-deficit hyperactivity disorder (ADHD) in the association with enuresis and 71 patients with ADHD without enuresis. The most cases of enuresis (50 out of 53 patients) were represented by primary nocturnal enuresis. The significant increase of total number of ADHD cases with comorbidity for oppositional-defiant disorder, anxiety disorder, tics or encopresis (77.7%) was found in the first group compared to the second one

Heterocyclic antidepressants in nonpsychiatric disorders.

Heterocyclic antidepressants have been used successfully in the treatment of migraine, enuresis and encopresis, peptic ulcer disease, irritable bowel syndrome, chronic pain, narcolepsy, sleep apnea and attention deficit disorder. The mechanism of their therapeutic effects in these conditions is still unclear. Serotonergic, noradrenergic, anticholinergic and antihistaminic properties and rapid-eye-movement sleep suppression have been implicated.

Chronic constipation in Turkish children: clinical findings and applicability of classification criteria.

We aimed to evaluate general features of children with chronic constipation and classified them according to the Iowa criteria and Rome II criteria in order to analyze applicability of these criteria in our population. The medical records of 485 children who were referred for chronic constipation over a six-year period were evaluated retrospectively. We found that 7.7% of the cases had an organic pathology, and short segment Hirschsprung disease was the leading cause. Other children (92.3%) were classified as functional constipation, with a mean age of 6.4 +/- 4 years and with slight male domi

Use of polyethylene glycol solution in functional and organic constipation in children.

A brief review is made of the use of high dose polyethylene glycol solution in the treatment of functional constipation and encopresis in children. Experience with low dose polyethylene glycol for the treatment of neurogenic constipation in children with severe brain damage is also reported. Treatment with polyethylene glycol caused a significant increase in bowel frequency and a decrease in gastrointestinal transit time. Side-effects, consisting of nausea, vomiting and irritability, have limited the use of this treatment in a few children.

[Anorectal manometry. Practical use in a pediatric surgical clinic].

Over a series of 158 cases of anorectal manometry the authors describe the technique used in a general pediatric surgery department. Results are reported in Hirschsprung's disease specially in the neonatal period, chronic constipation, fecal incontinence after anorectal malformations and meningomyelocele, encopresis. Biofeedback was performed in 28 patients. Technique varies with the etiologies and results are reported. Anorectal manometry to be effective must be simple, reliable, safe and non invasive. The reported technique seems effective for this goal.

Is it possible to predict the response to therapy in enuretic children? The PiFe score.

INTRODUCTION: Nocturnal enuresis (NE) is defined as the involuntary passage of urine during sleep in children over the age of five. Although the precise mechanisms of NE are unclear, factors like delayed development, genetic influences, excessive nighttime urine production, disrupted sleep, and bladder dysfunction play a role. This study aims to evaluate the role of comorbidities in NE and develop a scoring system to predict treatment response, with clinical applications. MATERIAL AND METHODS: We recruited 374 patients aged 5-18 years undergoing pharmacological treatment (single or combined)

Treatment of the occult tethered spinal cord for neuropathic bladder: results of sectioning the filum terminale.

PURPOSE: Occult tethered cord syndrome applies to patients with signs and symptoms consistent with a caudal spinal cord malformation despite normal neuroimaging. Although several reports of successful surgical treatment exist, controversy remains with respect to patient selection and efficacy. We present a large series with excellent clinical followup, neuroimaging and urodynamic characterization. MATERIALS AND METHODS: We present our experience with 36 patients at a single institution with preoperative clinical findings, neuroimaging and urodynamics available. Postoperative outcomes were ass

A child death as a result of physical violence during toilet training.

Enuresis and delayed bladder control are a common source of psychosocial concern for both parents and children. Different cultures have different norms with regard to parenting attitudes. The fact that in Turkey, parents consider enuresis or encopresis as a sign of laziness, misbehavior, or disobedience rather than a medical disorder may cause children to be exposed to physical and emotional violence and maltreatment by the family as a corrective strategy. We present a case in this paper which had a fatal outcome due to physical violence against a child as an educational measure and a toilet t

Neuroblastoma and Poland syndrome in a 15-year-old boy.

A 15-year-old boy with neuroblastoma associated with Poland syndrome is presented. He was admitted with a 2-month history of progressive back pain and a 3-day history of weakness of the lower extremities, encopresis and enuresis. On physical examination, in addition to paraplegia, absence of the pectoralis major muscle was diagnosed on the right side of his chest. A large heterogeneous mass in the right side of the thorax was revealed on computerized tomography. Neuroblastoma was diagnosed on histopathological analysis of the mass. To the authors' knowledge this is the first case of neuroblast

[Excretory disorders in children and adolescents].

Nocturnal enuresis unaccompanied by other excretory problems is due to a genetic CNS development disorder and responds well to cognitive-behavioral therapy and alarm interventions. If diurnal urinary incontinence is also present, various subtypes are differentiated, for the treatment of which cognitive-behavioral therapy must be supported by drugs or behavioral therapy. In the etiology of encopresis, genetic and psychosocial factors must be considered, as well as, in particular, chronic constipation. Generally, cognitive-behavioral therapeutic management is recommended. Measures aimed at regul
