BACKGROUND: Persistent type II endoleaks (ELIIp) occur in 8-23% of patients submitted to endovascular aneurysm repair (EVAR) and may lead to aneurysm progression and rupture. Intraoperative embolization of the abdominal aortic aneurysm (AAA) sac is effective to prevent their occurrence, however a method to achieve complete sac thrombosis has not been standardized yet. Aim of our study was to identify factors associated with prevention of ELIIp after intraoperative embolization, in order to optimize technical details.
METHODS: Patients at high risk for ELIIp, who underwent EVAR with AAA - sac

PURPOSE: To report a complication of renal stent crushing during open aneurysmorrhaphy performed 10 years after fenestrated endovascular aneurysm repair (FEVAR).
CASE REPORT: A 67-year-old male patient underwent elective FEVAR of a juxtarenal aortic aneurysm. Uncovered balloon-expandable stents were placed through the fenestrations for the superior mesenteric and right renal arteries; the left renal artery received a Jostent covered balloon-expandable stent. The uncovered right renal stent was noted to be crushed on the first imaging after FEVAR, but was left untreated; the right kidney was t

Vascular complications of Behçet's disease include occlusion of vessels and arterial aneurysm formation, which is prone to rupture. Early type II endoleak repair following an endovascular stent-graft procedure for a ruptured descending aortic aneurysm in a 31-year-old man with Behçet's disease is described. Endovascular stent-grafts provide an important alternative in complicated cases, but the possibility of endoleak should be born in mind.

PURPOSE: The primary objective of this study was to evaluate long-term outcomes in patients who underwent endovascular treatments for type II endoleaks (T2ELs) following endovascular aneurysm repair (EVAR).
METHODS: This single-center, retrospective study included patients who developed early T2EL after EVAR for abdominal aortic aneurysm. Patients were divided into those who did and did not receive intervention for T2EL. Study outcomes included 10-year overall survival (OS), open conversion-free survival, and T2EL regression rates without open conversion.
RESULTS: Of the 385 patients who und

OBJECTIVE: Endoleaks may be seen at case completion of endovascular abdominal aortic aneurysm repair (EVAR), and the presence of an endoleak may impact outcomes. However, the clinical implications of various endoleaks seen during follow-up is not well-described. Therefore, we studied the impact of endoleaks at completion and at follow-up on mid-term outcomes.
METHODS: We reviewed patients who underwent EVAR from 2003 to 2016 within the Vascular Quality Initiative-Medicare database and identified patients with endoleak at procedure completion and during follow-up, excluding those presenting wi

PURPOSE: The exact significance of type 2 endoleaks (T2ELs) and the indication and efficacy of treatment are widely debated. We report our experience with managing T2ELs in a tertiary Asian centre.
MATERIALS AND METHODS: This was a retrospective study of patients who underwent endovascular abdominal aortic aneurysm repair (EVAR) between February 2006 and December 2016. Patients with T2ELs were identified, and their data were analysed.
RESULTS: A total of 156 patients underwent EVAR, of which 67 (42.9%) developed T2ELs. Seven were lost to follow-up. The remaining 60 patients had a mean follow

Endovascular treatment has become the preferred method of repair of abdominal and thoracic aortic aneurysms, and comes with a unique complication in the form of endoleaks (type I-IV). Type II endoleaks are the focus of this review. They are the most common form of endoleak detected in CT surveillance following endovascular repair. They are observed in 9% to 30% of patients after abdominal endovascular repair (EVR), and 1.4% following thoracic endovascular aortic repair (TEVR). They are classified as primary or secondary, depending on when they are identified following EVR. Typically, retrograd

Situs inversus totalis (SIT) is a rare congenital condition characterized by mirror image transposition of thoracic and abdominal organs. The coexistence of SIT and abdominal aortic aneurysm presents unique anatomical and surgical challenges. We reported a case of a 60-year-old male with SIT and a history of endovascular aneurysm repair who developed multiple endoleaks (T1bEL, T2EL, and T3EL). A hybrid procedure combining open surgical repair and endovascular techniques was successfully employed to address the complex endoleaks (ELs). The procedure included ligation of collateral vessels, sten

OBJECTIVE: Little evidence is available supporting the optimal treatment of type II endoleaks associated with aortic sac growth. Previous studies have lacked comparisons between treatment methods and long-term follow-up. The purpose of the present study was to review our center's experience with the treatment of type II endoleaks comparing Onyx (a liquid embolization agent consisting of ethylene vinyl alcohol; Medtronic, Minneapolis, Minn) embolization and coil embolization.
METHODS: A retrospective review of prospectively collected data from a vascular surgery database was performed to ident

OBJECTIVE: The clinical significance of type II endoleaks is not well understood. Some evidence, however, indicates that some type II endoleaks might result in aneurysm enlargement and rupture. To identify factors that might contribute to aneurysm expansion, we analyzed the influence of several variables on aneurysm growth in patients with persistent type II endoleaks after endovascular aortic aneurysm repair (EVAR).
METHODS: In a series of 348 EVARs performed during a 10-year period, 32 patients (9.2%) developed type II endoleaks that persisted for more than 6 months. Variables analyzed incl

BACKGROUND: Compared with open repair of abdominal aortic aneurysms, endovascular aneurysm repair (EVAR) is less invasive and results in better perioperative outcomes and lower perioperative mortality risk. However, complications, such as endoleaks, can be frequent, leading to interventions on the stent graft. Managing endoleaks remains controversial.
CASE SUMMARY: Type I endoleaks require secondary intervention because of a high rupture risk, whereas type II endoleaks are treated if the aneurysm sac grows. The most effective treatment for type II endoleaks is still debated. Transcatheter emb

Background Endovascular aneurysm repair (EVAR) has evolved into treatment of choice for infrarenal abdominal aortic aneurysms (AAA). Type II endoleaks, although frequently benign, can lead to sac enlargement and rupture. Management of these endoleaks by endovascular means can be quite challenging and may require complex techniques and assistance of interventional radiologists, not always available in all vascular units. This is a single-center study of management of type II endoleaks with transarterial coil embolization performed by vascular surgeons and with minimum requirements regarding the

OBJECTIVE: Presentation of a clinical case of an aorto-esophageal fistula secondary to thoracic aorta pseudoaneurysm, complicated by early type Ia endoleak after endovascular repair.
CLINICAL CASE: A 64 years old male patient, with a history of arterial hypertension, smoking, alcohol abuse and ischemic heart disease with previous coronary revascularization was observed because of chest pain and abundant hematemesis, with Angio-CT revealing a 77mm pseudoaneurysm of the descending thoracic aorta, close to the left subclavian artery, with mass effect on the esophagus and trachea and signs of fis

BACKGROUND: The aim of this study was to report the occurrence of a type IIIb endoleak after endovascular repair of a thoracic aortic aneurysm caused by endoanchor dislocation.
CASE REPORT: An 84-year-old female patient underwent thoracic endovascular repair for aneurysmal disease of her thoracic aorta. The procedure included primary left subclavian artery revascularization and the placement of endoanchors to enhance fixation of the endograft within the aortic arch. Dislocation of one of the endoanchors resulted in a graft defect leading to a type IIIb endoleak and aortic diameter expansion.

We describe a patient who survived a ruptured abdominal aortic aneurysm without any surgical intervention. The patient had previously had endovascular repair of the aneurysm and surveillance of a stable persistent type II endoleak. This case highlights the difficulties surrounding type II endoleak, its natural history, and the ongoing controversies of its management.

INTRODUCTION: One of the most common complications of endovascular repair of abdominal aortic aneurysm is type II endoleak - retrograde branch flow.
CASE REPORT: A 76-year-old man with abdominal aortic aneurysm, 7.1 cm in diameter and aneurysm of the right common iliac artery, 3.2 cm in diameter was admitted to our Department with abdominal pain. The patient had no chance of having open repair of abdominal aortic aneurysm because of high perioperative risk (cardiac ejection fraction of 23%, chronic pulmonary obstructive disease). Multislice computed angiography also revealed a large inferior

ObjectivesPrimary ruptured abdominal aortic aneurysm (rAAA) survival has been described in a few case reports; however, to our knowledge, there is no existing literature regarding rAAA survival secondary to isolated type II endoleak (T2EL). We aim to report a rare case of non-operative management for a T2EL-associated rAAA.MethodsAn 89-year-old man with a history of endovascular aneurysm repair (EVAR) and persistent T2EL presented with acute abdominal pain and hypotension. Computed topography angiography (CTA) confirmed rAAA with retroperitoneal haematoma. Given his advanced age, comorbidities

Type III endoleaks are characterized by a problem with the endograft structure, such as fracture of the metallic structure, separation, or rupture. They constitute a rare complication, occurring in 2.1% of patients after treatment of abdominal aortic aneurysm by endovascular repair, with higher incidence in first and second generation endografts, and can occur early (after 30 days) or later. This type III classification is subdivided into IIIa -modular separation of components - and IIIb - mesh fracture or rupture involving the endograft. This case report describes an asymptomatic patient who
