Endolymphatic duct/sac enhancement on gadolinium magnetic resonance imaging of the inner ear: preliminary observations and case reports.

Twelve patients with auditory and/or vestibular symptoms were examined with a gadolinium magnetic resonance imaging (Gd-MRI) study. They all were found to have enhancement of only the endolymphatic duct/sac structures of the inner ear. Most of these findings were unilateral, but a few were bilateral. The patients underwent auditory, vestibular, and blood tests in an attempt to clarify the reason for this abnormal enhancement. Several specific etiologies were identified, but several cases had unknown causes. In this article we review the concept of inflammation of the inner ear structures with

A Synchrotron and Micro-CT Study of the Human Endolymphatic Duct System: Is Meniere's Disease Caused by an Acute Endolymph Backflow?

Background: The etiology of Meniere's disease (MD) and endolymphatic hydrops believed to underlie its symptoms remain unknown. One reason may be the exceptional complexity of the human inner ear, its vulnerability, and surrounding hard bone. The vestibular organ contains an endolymphatic duct system (EDS) bridging the different fluid reservoirs. It may be essential for monitoring hydraulic equilibrium, and a dysregulation may result in distension of the fluid spaces or endolymphatic hydrops. Material and Methods: We studied the EDS using high-resolution synchrotron phase contrast non-invasive

A Micro-CT and Synchrotron Imaging Study of the Human Endolymphatic Duct with Special Reference to Endolymph Outflow and Meniere's Disease.

Meniere's disease remains enigmatic, and has no treatment with sufficient evidence. The characteristic histopathological finding is endolymphatic hydrops, suggesting either an overproduction or decreased reabsorption of endolymph in the human inner ear. This study presents the first analysis of the vascular plexus around the human endolymphatic duct using micro computed tomography and coherent synchrotron radiation with phase contrast imaging. Using a software program, data were processed by volume-rendering with scalar opacity mapping to create transparent three-dimensional reconstructions. A

A Three-Year Follow-Up of a Patient With Large Vestibular Aqueduct Syndrome Who Underwent Bilateral Endolymphatic Duct Blockage Surgery.

Large vestibular aqueduct syndrome (LVAS) is a congenital malformation characterized by an abnormally large vestibular aqueduct, diagnosed primarily via CT scans. Patients with LVAS often experience progressive hearing loss and recurrent vertigo, with treatment strategies mirroring those for Meniere's disease. Traditional surgical interventions such as endolymphatic sac decompression (ESD) are common; however, the efficacy of endolymphatic duct blockage (EDB) as an alternative still remains under investigation. We present the case of a female in her 20s who was diagnosed with Meniere's disease

Endolymphatic Sac and Duct as an Anatomic Repair to Achieve Complete Exposure of the Internal Auditory Canal Through the Retrosigmoid Approach: Anatomic Study.

BACKGROUND: The retrosigmoid approach (RSA) is used in a wide variety of diseases in the cerebellopontine angle, and according to the findings, it can preserve hearing function during the resection of vestibular schwannomas. Complete exposure of the fundus of the internal auditory canal (IAC) could jeopardize the integrity of the otic capsule and subsequently cause profound hearing loss and chronic disabling vertigo. Great variability has been described in labyrinthine structures and to date there are no clear data on the distances of these structures to safely expose the IAC fundus. METHODS:

Menière's symptoms resulting from bilateral otosclerotic occlusion of the endolymphatic duct: an analysis of a causal relationship between otosclerosis and Menière's disease.

An association between otosclerosis and Menière's disease has been proposed on both a clinical and temporal bone histopathologic basis for well over three-quarters of a century. Controversy persists over a causal relationship between these two entities, however, and the underlying pathophysiologic mechanisms relating capsular otosclerosis with Menière's disease remain speculative. The first case of total, bilateral endolymphatic duct occlusion resulting from extensive capsular otosclerosis is presented in a woman manifesting Menière's symptoms. The severe endolymphatic hydrops resulting from o

Experimental endolymphatic hydrops in the rat.

In the rat, the epithelial lining of the endolymphatic duct and sac, and the contents of the sac, showed remarkable differences in comparison with these structures in the guinea pig and man. Obliteration of the endolymphatic duct resulted in endolymphatic hydrops of varying severity in 55% of the rats, after survival times varying from one to five months. The presence of a positive periodic acid-Schiff-stain for coagulum accumulating in the endolymphatic duct after prolonged saccus obliteration suggests active involvement of the endolymphatic duct in the migration of endolymph toward the endol

Submillimeter magnetic resonance imaging of the temporal bone in Meniere's disease.

The pathoetiology of Meniere's disease remains elusive. Histopathologic and imaging studies have implicated congenital or developmental abnormality of the endolymphatic duct as a likely predisposing factor to the development of endolymphatic hydrops and clinical Meniere's disease. Recently, improved high-resolution magnetic resonance imaging (MRI) protocols have allowed better demonstration of the soft tissues of the endolymphatic duct and sac. This study is a prospective evaluation of the ability of submillimeter MRI to detect the endolymphatic duct in Meniere's disease patients and control s

Endolymphatic hydrops in otologic syphilis: a temporal bone study.

HYPOTHESIS: Endolymphatic hydrops in temporal bones with otologic syphilis directly relates to osteitis, new bone formation, and/or resorption along the course of the endolymphatic system. BACKGROUND: Ménière's disease and otosyphilis both cause progressive endolymphatic hydrops, and the mechanism of hydrops formation in each disease process remains unclear. Traditionally, osteitic changes of the labyrinthine capsule were thought to lead to endolymphatic hydrops in syphilitic temporal bones. More recently, authors have suggested that microgummata and inflammatory change obliterating the endol

Cochlear blood flow in endolymphatic hydrops.

There is no animal model for Meniere's disease but by obliteration of the endolymphatic duct, endolymphatic hydrops may be achieved in several animal species. In order to measure the cochlear blood flow in ears with endolymphatic hydrops the endolymphatic duct was obliterated in 9 guinea pigs. The blood flow was measured with the microsphere method and the cochlear histology was studied. The regional and total blood flow was determined in the serially sectioned cochleas 2, 4 and 8 months after obliteration of the endolymphatic duct. No change in regional or total cochlear blood flow was observ

Differential expression of transthyretin in papillary tumors of the endolymphatic sac and choroid plexus.

Aggressive papillary tumors of the temporal bone, occurring sporadically or as part of von Hippel-Lindau disease, have been shown to originate within the endolymphatic sac or duct. Also implicated as a potential precursor from which some of these tumors may arise is ectopic choroid plexus epithelium. To aid in the differentiation between papillary tumors of endolymphatic sac and duct origin and those arising from choroid plexus, an immunohistochemical study using stains for transthyretin (TTR), cytokeratins, S-100 protein, epithelial membrane antigen (EMA), and glial fibrillary acidic protein

High-resolution MR imaging of the inner ear: findings in Menière's disease.

Symptoms in Menière's disease are characterized by hydrops of the endolymphatic system with recurrent rupture of the membranous labyrinth. The primary cause of the increased endolymphatic volume appears to be an imbalance between secretion and resorption of the endolymph which may be due to an obstruction of the membranous endolymphatic duct and sac, located in the vestibular aqueduct. The membranous endolymphatic duct and sac are not expected to be visualized using conventional tomography and high-resolution computed tomography (HR-CT), whereas, these are identified with high resolution MRI (

A Revised Surgical Approach to Induce Endolymphatic Hydrops in the Guinea Pig.

Endolymphatic hydrops is an enlargement of scala media that is most often associated with Meniere's disease, though the pathophysiologic mechanism(s) remain unclear. In order to adequately study the attributes of endolymphatic hydrops, such as the origins of low-frequency hearing loss, a reliable model is needed. The guinea pig is a good model because it hears in the low-frequency regions that are putatively affected by endolymphatic hydrops. Previous research has demonstrated that endolymphatic hydrops can be induced surgically via intradural or extradural approaches that involve drilling on

Magnetic resonance imaging of the temporal bone in patients with Ménière's disease.

Ménière's disease (MD) is still controversial in several aspects. The vestibular aqueduct, the osseous channel that carries the endolymphatic duct and sac, has previously been studied by tomography and computed tomography, with shortening and narrowing of this structure observed. These findings are apparently correlated to the development of the endolymphatic hydrops present in MD and related to its episodic symptoms. In studying the endolymphatic duct, the key structure in the pathology of this disease, magnetic resonance imaging (MRI) studies of the temporal bone were performed in 12 patient

Ménière disease caused by an anomalous vein of the vestibular aqueduct.

The precise cause of Ménière disease remains unclear. Multiple causes have been proposed with most experimental evidence pointing to impaired fluid resorption by the endolymphatic duct and sac as the final common pathway in development of hydrops. We report a unique case of Ménière disease secondary to compression of the endolymphatic duct and sac by an anomalous vein of the vestibular aqueduct. The resulting mechanical obstruction led to excessive accumulation of endolymph with clinical signs of Ménière disease. We review the literature and discuss proposed pathogenesis of disease. The findin

Degeneration of vestibular sensory cells caused by ablation of the vestibular aqueduct in the gerbil ear.

The vestibular aqueduct of the gerbil has a unique anatomic feature that makes it possible to selectively obliterate the endolymphatic sac with or without interfering with its venous drainage. In animals in which only the endolymphatic sac was ablated, endolymphatic hydrops was slight in the cochlea and was absent in the vestibular labyrinth. The cochlear and vestibular sensory cells were normal. In animals in which both the endolymphatic duct and the vein were obliterated, hydrops was slight, with the exception of a few cochleas that showed moderate hydrops. The sensory cells of the posterior

Animal models of endolymphatic hydrops.

Endolymphatic hydrops in the animal can be produced by various methods. The most promising is obliteration of the endolymphatic duct in the guinea pig. The extent of hydrops is similar to that of Ménière's disease specimens, but the animals are asymptomatic. A better model, utilizing a less invasive method and having the ability to elicit episodic vestibular symptoms, is needed. There is a reasonable doubt that one of the causes for Ménière's disease lies in the abnormal endolymphatic duct and sac. A wide ablation of such pathologic tissue may provide additional pathogenetic information.
