Extra-Articular Endoscopy.

With the advent of endoscopy in the last 2 decades, a number of procedures, and modifications to them, have been developed and have advanced exponentially. The list of indications was extended over time because of several reasons: better understanding of the pathophysiology, better diagnostics, and advances in endoscopic technology. In this review article, we summarize the most frequently performed extra-articular endoscopic procedures on the extremities. As there are several methods, some have been described briefly, whereas others have been described in greater detail, such as suprascapular

Wireless capsule endoscopy.

Wireless capsule endoscopy is a new technique that allows complete exploration of the small bowel without external wires. Its role has been analyzed in many small bowel diseases such as obscure gastrointestinal bleeding, Crohn's disease and gastrointestinal polyposis syndromes with promising results. Studies on other pathologies (i.e. small bowel tumour, celiac disease) are under evaluation to define the role of this technique.

Video capsule endoscopy.

PURPOSE OF REVIEW: The small bowel has been a difficult organ for gastroenterologists to evaluate. Small bowel radiographs and CT scanning have not had a high diagnostic rate, especially in patients presenting with small bowel bleeding. The video capsule "endoscope" has allowed for the direct visualization of the small bowel mucosa. This new technique provides direct viewing of the entire small bowel much like conventional endoscopy. RECENT FINDINGS: A series of studies have shown that video capsule endoscopy (VCE) is superior to barium radiography studies, CT scanning, and push enteroscopy f

Therapeutic pancreatic endoscopy.

In view of increasingly accurate noninvasive diagnostic imaging modalities for pancreatic diseases, endoscopic retrograde cholangiopancreatography (ERCP) should be mainly restricted to therapeutic indications. Acute pancreatitis is still the most common complication of ERCP. Prevention measures should focus in particular on well-defined risk groups. Temporary pancreatic duct drainage, preferably using small-diameter endoprostheses, can reduce the incidence of post-ERCP pancreatitis in at-risk individuals. By contrast, pharmacological prevention does not appear to be effective. ERCP in conjunct

Therapeutic biliary endoscopy.

Almost all biliary diseases are now amenable to endoscopic treatment. Progress in therapeutic biliary endoscopy is currently focusing on identifying the best treatment options to be used in each case in relation to the available evidence: how should a malignant stricture of the common bile duct or hilum be drained? How should an external biliary fistula be treated? What should one do with the gallbladder after extraction of common bile duct stones? Should sedation or general anesthesia be offered? Progress has been made in the technique of endoscopic ampullectomy and in developing new devices

Therapeutic pancreatic endoscopy.

The role of endoscopic retrograde cholangiopancreatography (ERCP) in the management of pancreatic diseases is continuing to evolve. This article reviews recent publications spanning a wide range of topics related to therapeutic pancreatic endoscopy: Over the last 12 months, several case series have added to the literature on the short-term and long-term effectiveness of endoscopic therapy of pseudocysts, pancreatic abscesses and fistulas. Identification of a communication between pancreatic duct and a pseudocyst has been suggested to predict response to percutaneous drainage. The importance of

Therapeutic pancreatic endoscopy.

Pharmacological treatment and/or temporary pancreatic stenting can reduce the risk of pancreatitis after endoscopic retrograde cholangiopancreatography. Potential candidates are patients who are at high risk for this complication. Extensive diagnostic work-up is needed for patients with acute recurrent pancreatitis before treatment can be planned. Endoscopic pancreatic sphincterotomy or temporary stenting, or both, appear to be beneficial in idiopathic recurrent pancreatitis. Short-term stent placement in the dorsal duct, with or without papillotomy of the minor papilla, is a promising approac

Diagnostic laryngeal endoscopy.

Diagnostic laryngeal endoscopy is a crucial part of the evaluation of patients with dysphonia. Multiple methods are available, and often several are required to provide a comprehensive physiologic and anatomic evaluation. The pros and cons of each method are discussed and the development of a standardized examination protocol is reviewed.

Therapeutic pancreaticobiliary endoscopy.

The remarkable advances achieved in the performance of therapeutic endoscopic retrograde cholangiopancreatography have placed endoscopy in the forefront in the schema for treating pancreaticobiliary disorders. Since the introduction of sphincterotomy and its impact on the management of stone disease, we have seen rapid development of alternative methods for treating obstructive jaundice, neoplasia of the biliary tree, and benign disorders. In addition, endoscopists are treating strictures and fistulas of the bile duct and pancreas, gallbladder disease, and more spectacularly, gallstone pancrea

[Infectious colitis. Endoscopy].

Colon and rectum localizations of an disease or a parasitosis depend essentially on the nature of the pathogenous agent and the host resistance. Acute enterocolitis is secondary to enterotoxinogenous germs (such as cholera vibrio), invasive germs (such as shigella), penetrating germs (such as salmonella); viruses are seldom concerned. Parasitic colitis include mostly amibiasis and bilharziosis. Infectious and parasitic enterocolitis may be transmitted sexually. On the other hand, certain venereal diseases have intestinal manifestations. Finally, in AIDS, timely gastro-intestinal infections dev

Intraoperative biliary endoscopy.

We examined the incidence of "missed" biliary calculi in a teaching center where intraoperative biliary endoscopy is performed routinely. During a six-year period, 1,280 procedures were performed for benign biliary disease. Two hundred eight patients (16%) underwent common duct explorations with choledocholithiasis discovered in 152 patients (73%). Choledochoscopy, using a rigid endoscope equipped with the Hopkins rod-lens system, was performed in 148 patients. When the scope was used, missed stones were seen on a follow-up T-tube cholangiogram in only 2.3% of the patients undergoing choledoch

Intraoperative gastrointestinal endoscopy.

A four year experience with the adaptation of the flexible fiberoptic endoscope to the intraoperative environment is presented in 30 patients. The technique of intraoperative endoscopy was utilized in a wide variety of difficult gastrointestinal surgical problems to include the location of the site and cause of bleeding of obscure etiology; resolution of intraoperative dilemmas without the necessity of opening abdominal viscera; resection of lesions during operations conducted for other pathological processes; and enhancement of diagnosis at laparotomy. There were no complications from the use

Endoscopy-related bacteremia.

A prospective study was undertaken to investigate the frequency of bacteremia after endoscopy of the gastrointestinal tract. In one out of 100 patients undergoing gastroscopy and in one out of 25 patients undergoing coloscopy, positive blood cultures could be demonstrated. In both cases anaerobe lactobacilli were recovered. No bacteremia was demonstrated in 25 patients undergoing endoscopic retrograde cholangiopancreatography (ERCP).

Comparative Analysis of Laser Assisted Endoscopie and Conventional Endoscopie Dacryocystorhinostomy.

BACKGROUND: Dacryocystorhinostomy (DCR) is the standard surgical treatment for epiphora caused by obstructions distal to the common canaliculus. Endoscopic DCR (EDCR) and laser assisted DCR (LAEDCR) are becoming increasingly popular alternatives. METHOD: 69 cases of EDCR were compared with 18 cases of LAEDCR. The success rates were noted at 01 week, 01, 03 and 06 months. The operating time required, incidence of adjuvant procedures, complications and post operative morbidity were recorded. RESULTS: At the end of 06 months, 100% and 95% success was achieved with LAEDCR and EDCR respectively.

Capsule endoscopy of the upper gastrointestinal tract -- the need for a second endoscopy.

BACKGROUND AND AIMS: Capsule endoscopy (CE) has been recognised as an important investigational tool in the diagnosis of gastrointestinal disease. Studies have shown that CE can identify lesions in the upper gastrointestinal tract as well as in the small intestine. However, contrary to conventional oesophagogastroscopy results, the role of CE findings in the diagnostic algorithm of disorders such as gastrointestinal bleeding has not been well understood. We evaluated the frequency of lesions identified in the oesophagus and the stomach as well as their significance. METHODS: We retrospectivel
