BACKGROUND AND OBJECTIVE: To establish the nasogastric enteral nutrition tolerance in patients with severe acute pancreatitis.
PATIENTS AND METHOD: A total of 12 patients with severe acute pancreatitis (> or = 3 Ranson criteria; C-reactive protein > 210 mg/dl) and adverse clinical course were included during 2006. When we verified the disease severity, nasogastric (10 F) enteral nutrition was initiated. We used a low fat semi-elemental feed (Dietgrif) in a slow infusion rate. We evaluated the enteral nutrition tolerance and the adverse events.
RESULTS: The patient (4 women and 8 men) mean ag

Enteral nutrition (EN) by means of oral nutritional supplements (ONS) and tube feeding (TF) offers the possibility to increase or to insure nutrient intake in case of insufficient oral food intake. The present guideline is intended to give evidence-based recommendations for the use of ONS and TF in patients with liver disease (LD). It was developed by an interdisciplinary expert group in accordance with officially accepted standards and is based on all relevant publications since 1985. The guideline was discussed and accepted in a consensus conference. EN by means of ONS is recommended for pat

We report a case of intussusception during enteral nutrition after esophagectomy. The case was a 60-year-old patient who underwent subtotal esophagectomy via the right thoraco-abdominal approach, reconstructed with esophagogastrostomy. He underwent tube feeding through gastrostomy after surgery. The tip of the feeding tube was emplaced in the jejunum. He complained of intermittent abdominal pain and the drainage volume through the gastric decompression tube was increased on the 2nd week from surgery. There was no abnormal finding on abdominal CT or or radiography of the digestive tract, and so

Controversy persists as to the optimal means of providing adjuvant nutritional support. The aim of this study was to compare enteral nutrition (EN) and parenteral nutrition (TPN) in terms of adequacy of nutritional intake, septic and nonseptic morbidity, and mortality. This was a prospective pragmatic study, whereby the route of delivery of nutritional support was determined by the attending clinician's assessment of gastrointestinal function. Patients considered to have inadequate gastrointestinal function were given TPN (group 1), while those deemed to have a functioning gastrointestinal tra

Artificial nutritional support does not alter the natural course in patients with malignant disease. The outcome of these patients is mainly determined by the type and stage of the underlying tumor. Progress of the underlying disease is often paralleled by malnutrition which in turn facilitates complications and may reduce survival and quality of life. Nutritional support can be applied to maintain body weight, immune function and quality of life. Enteral nutrition can be applied with a functioning gastrointestinal tract and has been proven to be superior compared to parenteral nutrition. Main

Enteral nutrition (EN) support was used in 88 patients with severe abdominal diseases, each with 2 to 7 (mean 3.8) surgical complications. Hospitalization ranged from 43 to 760 days (mean 122 days), during which the patients underwent 2 to 8 surgeries (mean 3.4). EN lasted for 9 to 409 days (mean 57.4 days), including TEN in 47 patients, TPN+EN in 12, and TEN transferring from TPN in 29, respectively. Patients' body weight, plasma proteins, and serum transferrin increased significantly by the end of EN support with the improvement starting from the second week. We conclude that EN should be us

Radiological confirmation of enteral feeding tube placement is widely regarded as the gold standard for verifying the position of blind-inserted feeding tubes before initiating enteral nutrition. Misplacement of feeding tubes into the respiratory tract or incorrect positioning within the gastrointestinal tract can lead to severe complications, including aspiration pneumonia, pneumothorax, pulmonary injury, and death. Although bedside methods such as pH testing are useful for ongoing monitoring, radiographic imaging remains the most accurate technique for confirming initial tube placement. This

PURPOSE OF REVIEW: Several observational studies have pointed out the negative effects of an energy deficit in long-staying ICU patients. Morbidity as well as mortality seem to be linked to such an energy deficit, which is often built up during the first week of stay. A combination of enteral and parenteral nutrition may be a strategy to avoid the development of an energy deficit.
RECENT FINDINGS: A cumulated energy deficit is demonstrated to be connected to an increased infectious morbidity. The question is whether or not addition of parenteral nutrition to the enteral nutrition may prevent

Immunonutrition, once thought to reduce infections in critically ill patients, faced scrutiny in the early 2000s following large trials that showed limited benefits and potential harm, particularly from formulas supplemented with arginine and glutamine. Newer evidence on immune-enhancing diets (IEDs) has prompted a shift toward individualized, precision-based nutrition strategies. Omega-3 fatty acids remain promising, especially in patients with traumatic brain injury or critical illness-induced muscle-wasting. Current guidelines recommend selective use of IEDs, such as in patients with cancer

Exclusive enteral nutrition involves the use of a complete liquid diet, with the exclusion of normal dietary components for a defined period of time, as a therapeutic measure to induce remission in active Crohn's disease (CD). This very efficacious approach leads to high rates of remission, especially in children and adolescents newly diagnosed with CD. This intervention also results in mucosal healing, nutritional improvements and enhanced bone health. Whilst several recent studies have provided further elaboration of the roles of exclusive enteral nutrition in the management of CD, other rep

Enteral nutrition (NE) is a technique of artificial nutrition that enables management by digestive tract of a defined mixture of nutrients and water, by means of probes implanted nasally or by enterostomies (eg: gastrostomy). Whenever the patient present limitations for voluntary oral ingestion or swallowing of the nutrients, and digestive capacity permitted to absorb nutrients, will draw the administration through a tube. Concern for the nutritional status of the patients is a more present reality among health professionals have demonstrated the direct relationship between malnutrition and mo

Older adults living in healthcare communities (HCCs) have multiple comorbidities and are at increased risk of malnutrition and unintended weight loss. Aging affects nearly every system as well as body composition and structure, causing physiological changes that can affect nutrition status. A significant percentage (56%) of residents who live in nursing facilities require extensive help to eat and have dental problems such as ill-fitting dentures, missing teeth, and swallowing problems, which can lead to inadequate caloric intake and unintended weight loss. Alzheimer disease or dementia is pre

PURPOSE OF REVIEW: Patients suffering from dementia or significant cognitive impairment (SCI) due to neurologic injury routinely receive percutaneous endoscopic gastrostomy (PEG) due to swallowing difficulty or lack of appetite. This review discusses current data and opinion regarding the risks and benefits of PEG in these populations.
RECENT FINDINGS: The current data regarding PEG placement in patients with dementia or SCI due to neurologic injury do not confirm either improvement or worsening of survival. Significant risk factors for poor prognosis after PEG include sex, hypoalbuminemia, a

Generally hospitalization is regarded as a prerequisite for patients with cancer who require terminal care. Recently, special attention has been directed to home care for these patients from the standpoints of patients quality of life (QOL) and health economics. In the case of home care for patients with terminal cancer, effective pain control, careful nutrition administration, psychological care for the patient and his/her family members and emergency care are indispensable. Due to the number and extent of these requirements, home care for terminal patients is not yet common. We have been pro

The physical and chemical compatibility of an enteral diet (standard Precitene) with a series of liquid preparations included in the hospital's formulary was evaluated. These preparations are included in the following groups: vitamins, antibiotics and chemotherapeutic drugs, analgesics, antiepileptic drugs, neuroleptics, bronchodilators, expectorants, antihistamines, immunosuppressors and short chain triglycerides. Phase separation, particle growth pH variation, osmolality increase and viscosity increase were studied. The results obtained indicate that the majority of the drugs studied are com
