Sensorimotor seizures of pediatric onset with unusual posteriorly oriented rolandic spikes.

BACKGROUND: Benign childhood epilepsy with centro-temporal spikes (BECCT) is usually associated with anteriorly oriented rolandic discharges. We report an unusual type of childhood-onset epilepsy with sensorimotor seizures associated with posteriorly oriented rolandic discharges. METHODS: Among 942 patients who underwent simultaneous electroencephalography and magnetoencephalography (MEG) for epilepsy evaluation, we found 7 patients with sensorimotor seizures aged under 13 years at onset, with no abnormal magnetic resonance imaging findings, and posteriorly oriented rolandic discharges detect

Febrile convulsions and rolandic discharges.

Clinical and electroencephalographic assessments were performed on 39 children with febrile convulsions showing EEG evidence of rolandic discharges and 23 epileptic children showing rolandic discharges with a preceding history of febrile convulsions. Ten families in which EEGs from the proband and his (or her) sibling or cousin demonstrated rolandic discharges were also described. The results of these studies stress a close genetic relationship between febrile convulsions with rolandic discharges and focal seizures or generalized convulsions that occur during sleep, namely, benign epilepsy of

[Idiopathic focal epilepsies of infancy and childhood].

We studied 1036 children with epileptic seizures, aged from 1 to 18 years, during 2004-2008. One hundred and six patients were diagnosed with idiopathic focal epilepsy (IFE). The following forms of IFE were singled out: benign seizures of infancy (familial and non-familial) - Watanabe--Vigevano syndrome - 5,7%, occipital epilepsy of childhood with early manifestation (Panayiotopoulos syndrome) -26,4%, occipital epilepsy of childhood with late manifestation (Gastaut syndrome) - 12,3%, benign epilepsy of childhood with central-temporal spikes (rolandic epilepsy) - 51%, benign focal epilepsy with

Benign familial neonatal convulsions.

A pedigree of so-called benign familial neonatal convulsions was reported. Nine members experienced convulsions in the neonatal period and/or in early infancy. All members except one had good prognosis. One member had mild mental retardation and epilepsy with rolandic EEG foci. It was suggested that some common genetic factors may be involved in the etiology of benign familial neonatal convulsions, convulsions of unknown etiology in infancy (Fukuyama) and epilepsy with rolandic EEG foci. A review of literature revealed that about 14% of cases with benign familial neonatal convulsions eventuall

Abnormal Ventral Somatomotor Network Homogeneity in Patients With Temporal Lobe Epilepsy.

BACKGROUND: Abnormalities of functional connectivity in the somatomotor network have been thought to play an essential role in the pathophysiology of epilepsy. However, there has been no network homogeneity (NH) study about the ventral somatomotor network (VSN) in patients with temporal lobe epilepsy (TLE). Therefore, we explored the NH of the VSN in TLE patients in this study. METHODS: The sample included 52 patients with left temporal lobe epilepsy, 83 patients with right temporal lobe epilepsy, and 68 healthy controls. The NH method was utilized to analyze the resting-state functional magn

Atypical "benign" partial epilepsy of childhood or pseudo-lennox syndrome. Part II: family study.

Atypical benign partial epilepsy of childhood (ABPE = Pseudo-Lennox syndrome) shows semiologic parallels to Lennox-Gastaut syndrome, however--besides the lack of tonic seizures--it has an entirely different etiology and prognosis. Recently Hahn et al [17] investigated the long-term evolution of 43 cases with ABPE. Symptomatology, EEG findings, and course were found to overlap with Rolandic epilepsy, Landau-Kleffner syndrome and ESES. The incidence of seizures in relatives was determined in the whole series investigated by Hahn et al [17]. Five of 56 siblings suffered from seizures (3 Rolandic

Genotype and phenotype analysis of epilepsy caused by ADGRV1 mutations in Chinese children.

OBJECTIVE: To investigate the genotype and phenotype of epilepsy caused by ADGRV1 variants in Chinese children. METHODS: A total of 625 patients with epilepsy who had undergone whole-exon gene sequencing or epilepsy and related paroxysmal disease gene panel sequencing were recruited. Variants were evaluated for susceptibility pathogenicity based on their frequency in the Genome Aggregation Database (≤ 0.001). We used six algorithms (sorting intolerant from tolerant (SIFT), PolyPhen-2, Mutation Taster, CADD, REVEL and Splice AI) that predicted that the ADGRV1 variant would have a harmful impac

Benign childhood focal epilepsies: assessment of established and newly recognized syndromes.

A big advance in epileptology has been the recognition of syndromes with distinct aetiology, clinical and EEG features, treatment and prognosis. A prime and common example of this is rolandic epilepsy that is well known by the general paediatricians for over 50 years, thus allowing a precise diagnosis that predicts an excellent prognosis. However, rolandic is not the only benign childhood epileptic syndrome. Converging evidence from multiple and independent clinical, EEG and magnetoencephalographic studies has documented Panayiotopoulos syndrome (PS) as a model of childhood autonomic epilepsy,

Dipole analysis in panayiotopoulos syndrome.

Panayiotopoulos syndrome (PS) is a type of benign childhood partial epilepsy, which has a good prognosis despite the fact that it is frequently associated with abundant multifocal spikes on the electroencephalography (EEG). We investigated whether stable dipoles, as seen in rolandic epilepsy, were also present in PS. We performed dipole analysis of the interictal spike discharges seen in the interictal EEGs of eight children with PS. We chose more than 10 spikes for each kind of spike, and investigated whether or not more than three of these spikes showed consistently stable dipole locations.

Ictal electroencephalography in a case of benign centrotemporal epilepsy.

The purpose of this research was to publish a complete ictal electroencephalographic (EEG) record of a child with benign centrotemporal epilepsy. The record includes a brief description of the patient's epilepsy syndrome, analysis of interictal and ictal EEG records, and a description of corresponding clinical ictal events. Ictal EEG activity appeared over the right rolandic cortex and spread to adjacent sites. Initial rhythmic activity was followed by bilateral series of sharply contoured slow waves. The electric field of the seizure discharge changed during the seizure. Tangentially oriented

[Occipital spike-wave with and without benign epilepsy in the child].

We describe 31 children with runs of high amplitude and variable frequency spike-and-wave discharges on the occipito-postero-temporal regions at the EEG. This characteristic EEG pattern can be seen on the posterior areas of one or both hemispheres only when the eyes are closed. A functional spike-wave focus of temporo-rolandic localization also occurred in 13 cases. Clinically, 10 children were seizure-free with minimal brain dysfunction in 8 of them. Of these, only 4 patients suffered from febrile convulsions while the 17 other patients were undergoing treatment for visual, motor or vegetativ

Magnetoencephalography localizing spike sources of atypical benign partial epilepsy.

RATIONALE: Atypical benign partial epilepsy (ABPE) is characterized by centro-temporal electroencephalography (EEG) spikes, continuous spike and waves during sleep (CSWS), and multiple seizure types including epileptic negative myoclonus (ENM), but not tonic seizures. This study evaluated the localization of magnetoencephalography (MEG) spike sources (MEGSSs) to investigate the clinical features and mechanism underlying ABPE. METHODS: We retrospectively analyzed seizure profiles, scalp video EEG (VEEG) and MEG in ABPE patients. RESULTS: Eighteen ABPE patients were identified (nine girls and

Topographical EEG analysis of rolandic spikes.

We performed topographic EEG investigation with instant voltage mapping in 57 patients with a Rolandic spike focus including 35 patients with benign Rolandic epilepsy. A pronounced maximal negativity of "Rolandic" spikes could be demonstrated over central or mid-temporal electrodes, with a spread to parietal or upper frontal areas with a dipole formation (centrotemporal negativity, frontal positivity) and involvement of midline. There was a moderate correlation between spike amplitude or duration with spread to adjacent areas. No other focal abnormalities such as focal slowing could be reveale

Dysfunction of the Heteromeric KV7.3/KV7.5 Potassium Channel is Associated with Autism Spectrum Disorders.

Heterozygous mutations in the KCNQ3 gene on chromosome 8q24 encoding the voltage-gated potassium channel KV7.3 subunit have previously been associated with rolandic epilepsy and idiopathic generalized epilepsy (IGE) including benign neonatal convulsions. We identified a de novo t(3;8) (q21;q24) translocation truncating KCNQ3 in a boy with childhood autism. In addition, we identified a c.1720C > T [p.P574S] nucleotide change in three unrelated individuals with childhood autism and no history of convulsions. This nucleotide change was previously reported in patients with rolandic epilepsy or IGE

ADHD and ADHD-related neural networks in benign epilepsy with centrotemporal spikes: A systematic review.

BACKGROUND: Attention-deficit/hyperactivity disorder (ADHD) and benign epilepsy with centrotemporal spikes (BECTS or rolandic epilepsy) present with a very high level of comorbidity. We aimed to review the existing literature focusing on two aspects: the possible role of epileptic activity in the damage of ADHD-related neural networks and the clinical approach to patients presenting with both conditions. MATERIAL AND METHODS: A systematic review was performed using Sapienza Library System and PubMed. The following search terms have been considered: attention networks, ADHD, attention systems,

The relationship between sleep-activated interictal epileptiform discharges and intelligence in children.

This study investigates the relationship between interictal epileptiform discharges (IEDs) during sleep in children with benign rolandic epilepsy (BRE) and cognitive test scores as measured by the Wechsler Intelligence Scale for Children-Fourth Edition (WISC-IV) with the hypothesis that, as IEDs increase, cognitive test scores decrease. Studies have shown that generalized seizures and status epilepticus can negatively impact cognition (Dodrill 2004), that children with epilepsy have lower cognitive function on average than children without epilepsy and that children with epilepsy and abnormal

Benign childhood epileptic syndromes with occipital spikes: new classification proposed by the International League Against Epilepsy.

In a recent proposal, the Commission on Classification and Terminology of the International League Against Epilepsy recognized early-onset childhood epilepsy with occipital spikes (Panayiotopoulos type), differentiating it from the only other type of childhood epilepsy with occipital spikes previously accepted: late-onset childhood epilepsy with occipital spikes (Gastaut type). The importance of this newly recognized syndrome of benign childhood partial seizures is that it is very common-only 2.4 times less frequent than benign rolandic epilepsy, and of equally excellent prognosis. It is chara

Migraine and the benign partial epilepsies of childhood: evidence for an association.

There is an increasing body of evidence to suggest that benign rolandic epilepsy and benign occipital epilepsy of childhood are frequently associated with migraine. This however has not been universally recognized. Clearly further investigation of this association is required. The purpose of this report is to examine the varied diagnostic criteria for common migraine utilized by epileptologists and other neurologists, to review clinical migraine epilepsy relationships, EEG abnormalities in migraine and epilepsy, and the association of seizures with vascular headache in patients with various fo

[Bilateral anterior opercular syndrome as a manifestation of a non-convulsive epileptic state].

INTRODUCTION: Foix-Chavany-Marie syndrome, or bilateral anterior opercular syndrome, is characterised by facio-pharyngo-glosso-masticatory diplegia with 'automatic-voluntary dissociation', which consists in the abolition of voluntary movements while involuntary movements and reflexes are preserved. It is produced by bilateral involvement of the anterior or frontal opercular region. In adults it is related to ischemic lesions. In childhood it presents congenitally in perisylvian dysplasias and as an acquired disorder in encephalitis or can be episodic in symptomatic or idiopathic epilepsies suc
