The point of epiphyseal penetration affects rotational stability of screw fixation in slipped capital femoral epiphysis: A biomechanical study.

The epiphyseal tubercle, a posterosuperior projection of the epiphysis into the metaphysis, serves as the axis of rotation in slipped capital femoral epiphysis (SCFE) and a source of physeal stability. We hypothesized that in a biomechanical model of single screw fixation of stable SCFE, a screw passing through the epiphyseal tubercle (the axis of rotation) would confer less rotational stability than a centrally placed screw. Three femurs were selected from a sample population of 8- to 15-year-old healthy hips to represent three stages of maturation: a "young" femur with a prominent epiphyseal

A Novel Classification System for Slipped Capital Femoral Epiphysis Based on the Radiographic Relationship of the Epiphyseal Tubercle and the Metaphyseal Socket.

UNLABELLED: Recent studies have suggested that the epiphyseal tubercle serves as a fulcrum for rotation in slipped capital femoral epiphysis (SCFE). However, radiographic evidence of the rotational mechanism is limited. In this study, we describe a novel radiographic staging system for SCFE based on the anatomic relationship between the epiphyseal tubercle and the metaphyseal socket. METHODS: We reviewed the cases of 469 patients with SCFE who were treated at our institution between 2000 and 2017. SCFE was classified according to our proposed staging system using the preoperative lateral radi

Mid-term results of concomitant epiphyseal fixation and trochanteric osteotomy for severe chronic slipped capital femoral epiphysis.

The management of severe forms of slipped capital femoral epiphysis (SCFE) has been the subject of intense debate in the literature, and controversy remains as to whether the proximal femoral epiphysis should be realigned by intracapsular or extracapsular osteotomies or just fixated in situ. The aim of this study is to evaluate the late results of treatment of severe unreduced slipped capital femoral epiphyses by combined epiphyseal stabilisation in situ using a single cancellous screw and biplane corrective trochanteric osteotomy. Eighteen hips with severe chronic slipped capital femoral epip

Lateral Proximal Femoral Epiphyseal Vessel Avulsion in Slipped Capital Femoral Epiphysis: A Newly Identified Mechanism of Avascularity: A Case Report.

CASE: We report 2 cases of Loder-unstable slipped capital femoral epiphysis (SCFE). Both patients had initial pinning followed by surgical hip dislocation where complete avulsion of the lateral epiphyseal vessels from the nutrient foramina was identified. Both patients underwent epiphysiodesis to revascularize the femoral head but developed osteonecrosis with collapse 9 months after their injury. CONCLUSION: Avulsion of the lateral epiphyseal vessels currently represents a nonreversible mechanism of avascularity in SCFE. Although Loder previously theorized the existence of this mechanism, it

Capital Femoral Epiphyseal Cupping and Extension May Be Protective in Slipped Capital Femoral Epiphysis: A Dual-center Matching Cohort Study.

BACKGROUND: Peripheral cupping of the capital femoral epiphysis over the metaphysis has been reported as a precursor of cam morphology, but may also confer stability of the epiphysis protecting it from slipped capital femoral epiphysis (SCFE). The purpose of this study was to investigate the relationship between a novel morphologic parameter of inherent physeal stability, epiphyseal cupping, and the development of SCFE in a dual-center matched-control cohort study. METHODS: We performed a dual-center age-matched and sex-matched cohort study comparing 279 subjects with unilateral SCFE and 279

Smaller Epiphyseal Tubercle and Larger Peripheral Cupping in Slipped Capital Femoral Epiphysis Compared with Healthy Hips: A 3-Dimensional Computed Tomography Study.

BACKGROUND: The inner surface of the capital femoral epiphysis is important for growth plate stability. However, abnormalities of epiphyseal morphology associated with the pathogenesis of slipped capital femoral epiphysis (SCFE) remain poorly understood. This study compares the 3-dimensional anatomy of the epiphyseal tubercle and peripheral cupping in hips with SCFE and normal hips. METHODS: We created 3-dimensional models of the capital femoral epiphysis with use of computed tomography (CT) imaging from 51 patients with SCFE and 80 subjects without hip symptoms who underwent CT because of ab

Epiphysiolysis in rickets.

Presented is a case demonstrating slipped capital femoral and capital humeral epiphyses. The physician should keep in mind not only that slipped capital femoral epiphyses may indicate renal osteodystrophy but that epiphysiolysis elsewhere is increasingly a recognized finding in this entity.

Slipped capital femoral epiphysis associated with hyperparathyroidism. A case report.

A case of slipped capital femoral epiphyses is reported which occurred in association with primary hyperparathyroidism. Only two similar cases have been found in the English literature. The patient was a man, 16 years of age, who presented with bilateral slipped epiphyses. Investigation showed that he had primary hyperparathyroidism due to a parathyroid adenoma. At a single operation, the epiphyses were pinned in situ, and the adenoma removed. The result was satisfactory.

Epidemiology of slipped capital femoral epiphysis in a population with a high proportion of New Zealand Maori and Pacific children.

AIM: To describe the epidemiology of slipped capital femoral epiphysis in NZ Maori and Pacific children residing in Auckland compared with NZ European children. METHODS: The charts and radiographs of 211 children admitted with 307 slipped capital femoral epiphyses to Starship Children's Hospital between 1988 and 2000 were reviewed. RESULTS: The average age at first presentation was 132.6 +/-16.7 months in girls (range 95 to 170 months) and 149.5 +/- 19.3 months in boys (range 99 to 190 months), p <0.05. The age at presentation was not statistically different between the three ethnic groups.

Atypical and typical (idiopathic) slipped capital femoral epiphysis. Reconfirmation of the age-weight test and description of the height and age-height tests.

BACKGROUND: The age-weight test was described to aid the clinician in defining demographic predictors of an atypical slipped capital femoral epiphysis. We wished to retest the accuracy and applicability of the age-weight test and height differences in children with atypical and typical slipped capital femoral epiphyses. METHODS: A retrospective review of the records for all children with slipped capital femoral epiphysis from 1998 through 2003 was performed. Gender, race, chronological age, weight, height, the duration of symptoms, and the laterality of the slip were recorded. The slip angle

Current practice in the management of acute/unstable slipped capital femoral epiphyses in the United Kingdom and the Netherlands: results of a survey of the membership of the British Society of Children's Orthopaedic Surgery and the Werkgroep Kinder Orthopaedie.

A questionnaire was sent to all members of the British Society for Children's Orthopaedic Surgery and the Werkgroep Kinder Orthopaedie to identify points of agreement/disagreement on the management of the acute unstable slip of the upper femoral epiphysis and to compare these European results with those from a similar North American survey. Sixty-five per cent responded. Overall, both countries evaluated cases similarly and believed in their urgent management. Sixty-six per cent did not reposition the slip. Significant differences were observed in attitude towards single screw usage, prophylac

Bilateral unstable slipped capital femoral epiphysis: a look at risk factors.

Unstable slipped capital femoral epiphysis can have disastrous complications including osteonecrosis and chondrolysis. It has been shown that 20% to 80% of patients may develop a contralateral slip ≤18 months after diagnosis. The purpose of this article is to report and characterize patients who developed bilateral unstable slips. After Institutional Review Board approval, the patients included were only those with bilateral unstable slipped capital femoral epiphyses. A minimum 2-year follow-up was required. Seven patients, all female, were included in the study, with an average age of 11.4 ye

Slipped proximal humeral epiphysis: a complication of radiotherapy to the shoulder in children.

Slipped proximal humeral epiphyses as a sequela of radiotherapy were identified in two children treated for Ewing's sarcoma in whom the shoulder was included in the field of treatment. The radiation doses were slightly greater than those in children developing slipped capital femoral epiphyses following radiotherapy. The lesser stress about the shoulder compared to the hip is believed to be the reason this is an infrequent complication of radiotherapy.

Single versus double screw fixation for treatment of slipped capital femoral epiphysis: a biomechanical analysis.

Slipped capital femoral epiphyses (SCFE) were created in 10 pair of bovine femurs. Each pair was pinned with a single screw on one side and two screws contralaterally. The specimens were reloaded to failure. Double pin fixation yielded only a 33% increase in stiffness as compared with single pin fixation. Resistance to further slip was not proportional to screw number. The stiffness of neither double nor single screw fixation approximated that of the intact physis. Single screw fixation in slipped epiphyses is recommended because the small gains in stiffness with a second screw do not offset t

Ultrasonography in slipped capital femoral epiphysis. Diagnosis and assessment of severity.

We used ultrasonography to study 26 hips with slipped capital femoral epiphyses. In recently slipped epiphyses the ultrasound image revealed a step at the anterior physeal outline (mean 6.4 mm), diminished distance between the anterior acetabular rim and the femoral metaphysis (mean 4.3 mm) and an effusion. As metaphyseal remodelling progressed the physeal step decreased. The femoral neck appeared straighter in hips which had been symptomatic for longer than three weeks. It was possible to measure posterior epiphyseal displacement without projectional errors and the method was accurate in diag

Epiphyseal separation of the proximal humerus after birth trauma.

The purpose of this study was to report a new entity of epiphyseal slipping, which is a slipping of the upper humeral epiphysis in neonates due to birth trauma, and reporting the results of their management. Eight neonates presented with pseudo-paralysis with associated shoulder swelling and pain on passive movements of the upper limb; the radiographs revealed slipping of the proximal humeral epiphyses in six cases and associated shoulder dislocations in the other 2 cases. Failed attempts of closed reduction were done for all cases; they were managed through open reduction of the slipping and
