Nonthoracotomy esophagectomy for corrosive esophagitis with gastric perforation.

Surgical treatment of corrosive esophagitis with gastric perforation is a high risk operation. Nonthoracotomy esophagectomy plus total gastrectomy was performed upon seven patients and compared with cervical esophagostomy plus total gastrectomy performed upon five patients. Two patients in the first group and all of the patients in the second group died. Transpleural esophagectomy for these critically ill patients is usually dangerous with high morbidity rates, while nonthoracotomy esophagectomy is a procedure which has lower risks and more advantages for removing the damaged esophagus of the

"Thoracic gastrostomy"--a new technique for feeding gastrostomy in wide-gap esophageal atresia and tracheo-esophageal fistula.

INTRODUCTION: Initial management of wide-gap esophageal atresia and tracheo-esophageal fistula, especially in developing countries, often involves ligation of the fistula, proximal esophagostomy and a gastrostomy. The conventional gastrostomy requires an abdominal incision and has several complications. The authors present an alternative technique of 'gastrostomy' that does not require an abdominal incision. METHOD: Through a standard right thoracotomy, the fistula is divided and tracheal end closed. If primary anastomosis is not possible due to wide gap, the lower esophageal pouch is mobilis

[Application of Gambee's single layer anastomosis in cervical esophagogastrostomy].

To decrease the incidence of postoperative leakage, we used the Gambee's method of single layer anastomosis in cervical esophagogastrostomy for carcinoma of the hypopharynx and superior segment of the esophagus. Since December 1985, this method has been used in 76 cases resulting leakage in only 2 cases (2.6%). The technique of Gambee's single layer anastomosis is described in this paper. It is very important to keep the mucosa and submucosa layers in close contact to promote healing of the anastomosis. It is concluded that Gambee's single layer anastomosis is a good and safe operative techniq

Esophageal atresia and tracheoesophageal fistula: Effect of delayed thoracotomy on survival.

Eighty-four infants with esophageal atresia and/or tracheosophageal (TE) fistula were treated from 1972 to 1977. Twenty-eight percent were premature and 24% weighed less than 2.0 kg. Major symptoms included excess salivation (56 patients), respiratory distress (28 patients), cyanosis (26 patients), and choking (nine patients). Pneumonia and or atelectasis occurred in 58% and associated anomalies in 68%. Seventy-three of 84 patients (87%) had proximal esophageal atresia and distal TE fistula (type C defect). Operation was carried out in 79 patients. Gastrostomy was performed in 75 patients, oft

Tracheal agenesis in a child who survived for 6 years.

The authors report on a child with tracheal agenesis who survived for 6 years and 10 months. A female infant was born with respiratory distress and an absence of audible cry. With mask ventilation, gastrostomy and division of the esophagus was performed just after birth. She was well ventilated with an endotracheal tube inserted through the cervical esophagostomy after thoracic esophageal banding. An accidental extubation caused her anoxic encephalopathy at 9 months of age. She had been alive without artificial ventilatory support until she died of acute esophageal bleeding.

[Wide gap esophageal atresia without esophagotracheal fistula (author's transl)].

Standard treatment of infants with "wide gap" esophageal atresia usually requires a staged procedure consisting of gastrostomy at birth (with or without cervical esophagostomy) and then a bowel interposition between the oesophagus and stomach at 1 year of age. Because this approach procedures significant morbidity and swallowing dysfunction, an alternative method for treating the wide gap atresia is recommended. The upper and lower esophageal pouch can be elongated by preoperative bougienage. This technique allows successful primary esophageal reconstruction in infants with wide gap esophageal

Effect of antrectomy on the nervous phase of gastric secretion in the dog.

A method is described for complete isolation of the stomach in the dog with vagal innervation intact. This involves esophagostomy, double mucosal closure of the pylorus and a Maydl gastric fistula combined with gastrojejunostomy. The latter is occluded during periods of study. In this preparation the responses to sham feeding and to insulin-induced hypoglycemia were reduced approximately 10-fold, reiterating the significant synergistic effect of gastrin on vagal stimulation of the parietal cell mass. However, significant acid secretion could still be induced in this preparation by both sham fe

[Early esophageal replacement in patients with esophageal atresia].

INTRODUCTION: When primary anastomosis is not feasible in esophageal atresia esophageal replacement is one of the possible options. We report our experience with this approach in patients with long-gap esophageal atresia. MATERIALS AND METHODS: From 1991 to 2002 we treated 50 children with esophageal atresia. Nine required esophageal replacement because of long-gaps. Six were boys and three girls. Six had isolated atresia, and three had regular atresia with TEF. Associated malformations in 8 patients were: cardiovascular (2), anorectal (2), urogenital (5), intestinal duplication (1), and cost

[Surgical treatment of esophageal atresia--6 years of experience].

Surgical treatment of fourteen infants with esophageal atresia over the past six years was reviewed retrospectively. Two patients had esophageal atresia without tracheoesophageal fistula (Gross Type A), the other 12 patients had distal tracheoesophageal fistula (Gross Type C). Associated anomalies were found in three patients: one had a patent ductus arterious, another had trisomy 18, and the third had polydactylia. According to Waterston's risk group classification, there were 7 cases in Group A, 5 in Group B and 2 in Group C. The choice of operative method mainly depended on the distance bet

Laparoscopic gastric pull-up for long gap esophageal atresia.

BACKGROUND: Esophageal replacement (ER) is indicated for long gap esophageal atresia (LGEA) when anastomosisis not possible, especially in cases without fistula or when elongation techniques have failed. The authors show their techniques and analyze preliminary results of the laparoscopic gastric pull-up (LGPU) for ERin LGEA. METHODS: Four children with LGEA admitted for ER (three type A, one type C) underwent LGPU. Using three ports, including the gastrostomy site, surgical steps included releasing the stomach while preserving the right gastric and gastroepiploic arteries, pyloromyoplasty, a

Successful Treatment of Esophageal Necrosis Secondary to Acute Type B Aortic Dissection.

Esophageal necrosis after descending aortic dissection has been described but with no reports of successful treatment. A 66-year-old man with aortic dissection extending from the left subclavian artery through the common iliac arteries subsequently experienced esophageal necrosis. He underwent thoracic esophagectomy, cervical end esophagostomy, and open gastrostomy tube placement. Gastrointestinal continuity was established with a gastric tube conduit in the substernal plane. An oral diet was tolerated after reconstruction. Treatment of esophageal necrosis after aortic dissection may require e

Cineradiography identifies esophageal candidiasis in progressive systemic sclerosis.

Cineradiography of the esophagus showed signs of esophageal candidiasis in 11 out of 71 patients with progressive systemic sclerosis (PSS) - both in diffuse scleroderma and the CREST syndrome. Culture of esophageal brushings confirmed the presence of Candida albicans in eight of these 11 patients. Antimycotic treatment decreased the cineradiographic signs of candidiasis and the degree of dysphagia. Since impaired esophageal motility and treatment with immunosuppressive drugs may predispose to candida esophagitis, and since dysphagia will decrease after antimycotic treatment esophageal mycosis

Strategy for ventricular septal defect closure after prior gastric pull-through.

A 9-year-old boy was referred with a perimembranous ventricular septal defect. At birth, he had undergone a right thoracotomy with ligation of a tracheoesophageal fistula, cervical esophagostomy, and feeding gastrostomy. At 2 years of age, he had gastric tube reconstruction with a pull-through retrosternally, anterior to the heart, and an end-to-end esophagogastric anastomosis. Via a right anterolateral thoracotomy through the previous scar, the entire gastric tube was mobilized away from the sternum to facilitate a median sternotomy. With the patient supine, a median sternotomy was performed

Application of ECMO to the treatment of benign double tracheoesophageal fistula: report of a case.

This report presents the extracorporeal membrane oxygenation (ECMO)-assisted surgical as a treatment of benign double tracheoesophageal fistula. The patient was a 43-year-old woman who presented the airway obstruction for 3 weeks after the esophagus metal stent implantation for the tracheoesophageal fistula 1 year ago. The airway obstruction was due to the expansion and piercing of the metal stent through the upper part of the esophagus into the tracheal cavity. In view of the failure of endotracheal intubation, we finally used ECMO-assisted surgery to remove the stent. And at the same time, c
