Ethical issues with psychedelic-assisted treatments in psychiatry: A systematic scoping review.

Based on promising preliminary results from clinical trials, it seems likely that psychedelic substances (classic serotonergic psychedelics, such as psilocybin, and entactogens, such as MDMA) will be introduced into psychiatry as psychedelic-assisted therapy. This also raises a range of ethical questions that urgently need to be addressed before widespread roll-out in society. This scoping review fills a gap in the literature by providing an overview of these ethical issues using a systematic search, presentation, and descriptive analysis of ethical issues in psychedelic-assisted treatments. Ethical issues in genomics research on neurodevelopmental disorders: a critical interpretive review.

BACKGROUND: Genomic research on neurodevelopmental disorders (NDDs), particularly involving minors, combines and amplifies existing research ethics issues for biomedical research. We performed a review of the literature on the ethical issues associated with genomic research involving children affected by NDDs as an aid to researchers to better anticipate and address ethical concerns. RESULTS: Qualitative thematic analysis of the included articles revealed themes in three main areas: research design and ethics review, inclusion of research participants, and communication of research results. Shortcomings of protocols of drug trials in relation to sponsorship as identified by Research Ethics Committees: analysis of comments raised during ethical review.

BACKGROUND: Submission of study protocols to research ethics committees (RECs) constitutes one of the earliest stages at which planned trials are documented in detail. Previous studies have investigated the amendments requested from researchers by RECs, but the type of issues raised during REC review have not been compared by sponsor type. The objective of this study was to identify recurring shortcomings in protocols of drug trials based on REC comments and to assess whether these were more common among industry-sponsored or non-industry trials. METHODS: Retrospective analysis of 226 protoco

Ethical Design of Data-Driven Decision Support Tools for Improving Cancer Care: Embedded Ethics Review of the 4D PICTURE Project.

Oncology patients often face complex choices between treatment regimens with different risk-benefit ratios. The 4D PICTURE (Producing Improved Cancer Outcomes Through User-Centered Research) project aims to support patients, their families, and clinicians with these complex decisions by developing data-driven decision support tools (DSTs) for patients with breast cancer, prostate cancer, and melanoma as part of care path redesign using a methodology called MetroMapping. There are myriad ethical issues to consider as the project will create data-driven prognostic models and develop conversation

Understanding E-Consent in Anaesthesia: A Review of Clinical, Legal, and Ethical Dimensions.

The integration of electronic consent (e-consent) into anaesthetic practice has accelerated since the Coronavirus Disease 2019 (COVID-19) pandemic, offering new opportunities to enhance patient autonomy, documentation fidelity, and clinical efficiency. This review examines the clinical, legal, and ethical dimensions of e-consent, situating it within the statutory and common law frameworks, such as the Mental Capacity Act 2005 and the principles established in Montgomery v Lanarkshire Health Board. It further interrogates the challenges posed by digital exclusion, cybersecurity vulnerabilities,

Incarceration and electrophysiology care: A review of clinical, ethical, and systems-level challenges.

BACKGROUND: Incarcerated individuals bear a disproportionate burden of cardiovascular disease and face significant barriers to specialty care, including in cardiac electrophysiology (EP). The impact of incarceration on EP care remains underrecognized and incompletely characterized. OBJECTIVE: To characterize how incarceration disrupts the delivery of cardiac EP care, identify clinical and system-level barriers, and propose opportunities to improve access and quality. METHODS: We conducted a narrative review of the literature on incarceration, cardiovascular disease, and EP care. RESULTS: Hi

Ethical considerations of genetic and genomic testing in pediatric oncology: A narrative review.

Genomics-and genomic testing in particular-has transformed oncology, facilitating both targeted therapies and personalized care. In pediatric oncology, unique clinical and ethical considerations arise. Compared with adults, children and adolescents are affected by more limited evidence regarding test performance, variant interpretation, and the clinical utility of genomically informed interventions. Nevertheless, genomic findings may have implications beyond the patient, affecting their parents, siblings, and other relatives and raising questions around consent, assent, privacy, and psychosoci

Brain Tumors in Pregnancy: A Review of Pathophysiology, Clinical Management, and Ethical Dilemmas.

BACKGROUND: Central nervous system (CNS) tumors during pregnancy are rare but present significant diagnostic, therapeutic, and ethical challenges. These include both primary and metastatic lesions, which share overlapping clinical features and management complexities. Their clinical course is influenced by gestational physiological changes, which can mask symptoms and delay diagnosis, thereby increasing maternal and fetal risks. OBJECTIVE: This review aims to synthesize current evidence on the epidemiology, pathophysiology, clinical presentation, diagnostic strategies, treatment options, prog

Ethical aspects of sudden cardiac arrest research using observational data: a narrative review.

Sudden cardiac arrest (SCA) accounts for half of all cardiac deaths in Europe. In recent years, large-scale SCA registries have been set up to enable observational studies into risk factors and the effect of treatment approaches. The increasing scale and variety of data sources, coupled with the implementation of a new European data protection legal framework, causes researchers to struggle with how to handle these 'big data'. Data protection in the SCA setting is especially complex since patients become at least temporarily incapacitated, and are thus unable to provide prospective informed co

Invited review: ruminating conscientiously: scientific and socio-ethical challenges for US dairy production.

Despite the predominantly positive depictions of dairy production, public concern about farm animal production practices in the United States is currently higher than at any point in recent history. Many standard industry practices, including some used by the US dairy industries, are increasingly challenged not just on scientific grounds, but also on ethical grounds. Concerns include the environmental impacts and sustainability of modern farm animal production practices, food safety and security, and the increasingly complex issue of animal welfare. As the impetus increases to achieve broad st

Clinical review: Ethical and medical considerations of androgen deprivation treatment of sex offenders.

CONTEXT: Blood testosterone codetermines the threshold for erotosexual imagery and sexual activity. Androgen deprivation may therefore have a place in the treatment of unacceptable sexual behavior. Although androgen deprivation can be effective for sex offenders, their basic human rights must be respected; otherwise such treatment constitutes a violation of their physical integrity and is ethically unacceptable. As experience in treating prostate cancers demonstrates, androgen deprivation may have serious side effects. Endocrinologists are qualified to advise on and monitor androgen deprivatio

Ethical issues in neurosurgery - A scoping review from the EANS Ethico-legal committee.

BACKGROUND: Ethical considerations are integral to neurosurgical decision-making, yet emerging technologies, demographic shifts, and global crises continuously introduce new challenges. Key ethical concerns include patient autonomy, prioritization, the value of life, research ethics, and personality-altering procedures. Despite their importance, formal ethics training for neurosurgical residents is often lacking, and guideline application remains inconsistent. This scoping review summarizes current literature on ethical issues in neurosurgery, identifies key topics and assessment methods, and

Ethical challenges in EEG neurofeedback: a systematic review of gaps, risks, and responsibilities.

This systematic review examines the ethical landscape of electroencephalographic neurofeedback (EEG-NFB). In accordance with the PRISMA 2020 guidelines, a comprehensive search of six databases was conducted between December 2024 and February 2025, yielding 23 studies that met the predefined inclusion criteria. A qualitative thematic synthesis identified seven recurring ethical challenges: (1) concerns regarding informed consent and autonomy, shaped by uncertainty surrounding efficacy and the influence of placebo and contextual effects; (2) issues related to non-maleficence and the psychologica

Ethical, Legal, and Social Implications of Newborn Screening in Africa: A Scoping Review.

Newborn screening initiatives have the potential to mitigate childhood morbidity in Africa, but they also have special ethical, legal, and social implications (ELSI) that are influenced by issues with the health system, cultural diversity, and limited resources. This scoping review explores the ELSI of newborn screening across Africa to identify key challenges, gaps, and future research needs. A systematic search identified 27 peer-reviewed studies published between 2008 and 2025, covering 12 African countries. Data were extracted on study characteristics, disease types, and ELSI dimensions fr

Clinical psychedelic research in adolescents: a scoping review and overview of ethical considerations.

The potential use of psychedelic-assisted therapy for adolescents with mental illness has sparked both interest and concern. Modern psychedelic research has focused on adults, and adolescents younger than 18 years are typically excluded due to ethical and legal challenges. To explore whether adolescents have been included in 21st century psychedelic research, we conducted a scoping review of the medical literature from January, 2000, to April, 2025. Three trial registrations and one trial plan showed involvement of participants younger than 18 years, but none of these trials were completed and

Long-Term Ventilation in Neuromuscular Patients: Review of Concerns, Beliefs, and Ethical Dilemmas.

BACKGROUND: Noninvasive mechanical ventilation (NIV) is an effective treatment in patients with neuromuscular diseases (NMD) to improve symptoms, quality of life, and survival. SUMMARY: NIV should be used early in the course of respiratory muscle involvement in NMD patients and its requirements may increase over time. Therefore, training on technical equipment at home and advice on problem solving are warranted. Remote monitoring of ventilator parameters using built-in ventilator software is recommended. Telemedicine may be helpful in reducing hospital admissions. Anticipatory planning and pa

Ethical considerations and palliative care in patients with amyotrophic lateral sclerosis: A review.

Amyotrophic lateral sclerosis (ALS) is not a curable disease, but it is treatable. By definition, much of the care provided to ALS patients is palliative, even though active life-sustaining strategies are available to prolong survival. Healthcare professionals must develop communication skills that help patients cope with the inexorable progression of the disease and the inevitability of death. Symptomatic treatments as well as respiratory insufficiency and nutritional life-sustaining therapies must be regularly evaluated as the disease progresses, without losing sight of the burden placed on

Ethical and management dilemmas in the care of the newborn at the limit of viability: a narrative review.

There are variations among healthcare providers regarding the gestational age considered the threshold of viability. Currently, the World Health Organization sets the lower limit of viability at 22 weeks of gestation, or a birth weight of 500 g, or a birth length of 25 cm. Neonates are not autonomous. The present study aimed to evaluate the factors involved in healthcare professionals' management decisions in cases of neonates born at the margins of viability. A narrative review was conducted, including articles published between 2005 and 2025. We included 55 manuscripts and used the SANRA qua
