Seizures, Cultural Beliefs, and Ethical Tensions.

This case narrative explores the ethical challenges that arise when culturally rooted surrogate decision-making conflicts with standard biomedical care. A 20-year-old Somali man presented with suspected autoimmune encephalitis requiring urgent immunosuppressive treatment to prevent permanent brain injury. Lacking decisional capacity due to encephalopathy, his mother served as his surrogate.Guided by religious and cultural beliefs, his family attributed his condition to demonic possession and declined medical treatment in favor of a spiritual healing ritual. This decision generated profound mor

Is there a role for ethics in addressing healthcare incivility?

In a healthcare setting, a multitude of ethical and moral challenges are often present when patients and families direct uncivil behavior toward clinicians and staff. These negative interactions may elicit strong social and emotional reactions among staff, other patients, and visitors; and they may impede the normal functioning of an institution. Ethics Committees and Clinical Ethics Consultation Services (CECSs) can meaningfully contribute to organizational efforts to effectively manage incivility through two distinct, yet inter-related channels. First, given their responsibility to promote a

Evaluating non-disclosure of errors and healthcare organization: a case of bioethics consultation.

Sometimes medical errors should not be disclosed. We report a case of semen samples exchange, during a homologous artificial insemination procedure, where a bioethics consultation was required. The bioethics consultation addressed ethical and legal elements in play, supporting non-disclosure to some of the subjects involved. Through a proper methodology, gathering factual and juridical elements, a consultant can show when a moral dilemma between values and rights-privacy versus fatherhood, in our case-is unsubstantial, in a given context, because of the groundlessness of the value or the right

Organizational Ethics in Practice: Examples from a Large Pediatric Healthcare System.

AbstractOrganizational ethics (OE) in healthcare has gained prominence, particularly during the COVID-19 pandemic. This article provides detailed guidance for establishing OE services and conducting OE consultations, drawing on examples from Seattle Children's Hospital. We define OE, compare it with clinical ethics, and describe consultation processes and procedures. Examples illustrate OE's role in guiding institutional decisions, promoting equity, and managing resource constraints. We conclude with recommendations for OE program development and integration.

Research Priority Setting Is Not Research: A Call for Ethical Clarity in Australia.

In Australia, research priority setting (RPS) may be misclassified as research requiring ethics approval, despite its purpose as a collaborative, pre-research involvement activity. Treating RPS as research creates delays, diverts resources and undermines partnership principles central to inclusive, community-centred priority setting. Recent revisions to the National Statement on Ethical Conduct in Human Research clarify that pre-research consultation does not require ethics review, creating an opportunity for consistent national practice. We call for institutions to recognise RPS as involvemen

Assessing ethical competence and ethical climate: development and psychometric testing of the Clinical Ethics Support Measurement Instrument (CES-MI).

BACKGROUND: Clinical ethics support (CES) is increasingly recognised as essential in healthcare, yet its evaluation remains challenging due to the complexity of CES practices and the latent, psychosocial nature of relevant outcomes. The European Moral Case Deliberation Outcomes Instruments (Euro-MCD original and 2.0) represents important steps in operationalising CES outcomes. However, there remains a need for a further theoretically grounded and psychometrically robust instrument. This study aimed to revise the Euro-MCD 2.0 and to develop and psychometrically test a new measurement instrument

Even Resourceful Parents Lack Access to Ethics-Informed Resources.

AbstractEthics resources have the potential to help parents of children with medical complexity understand and navigate ethics concerns relevant to their child's care. This study conducted 28 semi-structured individual interviews with parents of children with chronic critical illness to learn about current ethics resources use patterns. Parents universally attested to encountering ethics concerns relevant to their child's care. These parents described ethics resources as helpful for navigating ethics challenges, including obtaining information, clarifying goals, advocating for access, and impr

Comparing the safety and quality of video versus telephone telehealth using simulated consultations on fictional medications for an Australian community sample: a randomised controlled trial protocol.

INTRODUCTION: Internationally, routine telehealth consultations have been extensively implemented in primary care in telephone or video-based formats. Despite this widespread implementation, few studies have compared information exchange quality in telephone and video-based consultations from consumer and clinician perspectives, nor have they examined related indicators of safety. This protocol reports on a randomised controlled trial assessing safety and quality of telehealth consultations across telephone versus video-based modalities for simulated consultations about a fictional medication.

Ethical challenges in the care of children and families affected by life-limiting neuromuscular diseases.

OBJECTIVE: We sought to understand how neuromuscular clinicians respond to the ethical challenges that arise in caring for children with life-limiting neuromuscular diseases. METHODS: We conducted a national survey of interdisciplinary professionals who care for children with Duchenne Muscular Dystrophy and Spinal Muscular Atrophy Type-1 to document their knowledge, attitudes, beliefs and reported practices with regard to prominent ethical challenges, and their suggestions for ethics interventions that would assist them in improving clinical practice. RESULTS: 157 participants completed pape

Guiding organisational decision-making about COVID-19 asymptomatic testing in workplaces: mixed-method study to inform an ethical framework.

BACKGROUND: Workplace programmes to test staff for asymptomatic COVID-19 infection have become common, but raise a number of ethical challenges. In this article, we report the findings of a consultation that informed the development of an ethical framework for organisational decision-making about such programmes. METHODS: We conducted a mixed-method consultation - a survey and semi-structured interviews during November-December 2020 in a UK case study organisation that had introduced asymptomatic testing for all staff working on-site in its buildings. Analysis of closed-ended survey data was

A case of deceptive mastectomy.

This paper poses the question, "what are providers' obligations to patients who lie?" This question is explored through the lens of a specific case: a 26-year-old woman who requests prophylactic bilateral mastectomy with reconstruction reports a significant and dramatic family history, but does not want to undergo genetic testing. Using a conversational-style discussion, the case is explored by a breast surgeon, genetic counselor/medical geneticist, clinical psychologist, chair of a hospital ethics committee and director of a clinical ethics consultation service.

Clinical and cost-effectiveness of a personalised guided consultation versus usual physiotherapy care in people presenting with shoulder pain: a protocol for the PANDA-S cluster randomised controlled trial and process evaluation.

INTRODUCTION: Musculoskeletal shoulder pain is a common reason for people to be treated in physiotherapy services, but diagnosis can be difficult and often does not guide treatment or predict outcome. People with shoulder pain cite a need for clear information, and timely, tailored consultations for their pain. This trial will evaluate the introduction of a personalised guided consultation to help physiotherapists manage care for individuals with shoulder pain. METHODS AND ANALYSIS: This is a cluster randomised controlled trial to evaluate the clinical and cost-effectiveness of introducing a

The role of ethicists in pediatric hematology/oncology: Current status and future needs.

As pediatric hematology/oncology (PHO) becomes more complex and sub-subspecialized, dedicated PHO ethicists have emerged as sub-subspecialists focused on addressing ethical issues encountered in clinical and research practices. PHO physicians and other clinicians with advanced training in bioethics contribute to the field through ethics research, education, and ethics consultation services. Furthermore, there exists a newer generation of PHO trainees interested in bioethics. This review details the experiences of current PHO ethicists, providing a blueprint for future educational, research and

[The clinical ethics committee (CEC) in the area of conflict between hospital certification, moral pragmatics and scientific approach].

During the last decade numerous consultative bodies for bioethical and medical ethical issues have been established. In this study we will introduce the clinical ethics committee (CEC), which can be mainly brought into action for three purposes: discussing moral problems in a hospital's everyday work, developing guidelines for the clinic, and giving further education to the hospital's staff. Starting with the denominational hospitals at the end of the 1990s, CECs have been established in the meantime at a large number of German clinics, often in an interrelation with hospital certification. Becoming in-formed: genetic counselling, ambiguity and choice.

The paper presents findings from an ethnography of dysmorphology, a specialism in genetic medicine, to explore genetic counselling as a process through which parents 'become informed.' Current professional and policy debate over the use of genetic technology in medicine emphasises the need for informed choice making, and for genetic services that provide parents with what is referred to as 'non-directive genetic counselling.' In the paper the process of becoming informed is shown to be very specific and to have its own effects. Specifically, genetics is performed in dysmorphology as a space of

The New Organization of Ethics Committees in Italy: What is the Future of Clinical Ethics?

BACKGROUND: In Italy, clinical ethics is not well institutionalized. On February 7, 2023, the Italian Ministry of Health published four long-awaited decrees regarding the reorganization of ethics committees. AIM: The aim of this article is twofold: firstly, we aim to briefly summarize the development of clinical ethics in Italy from a legislative point of view; secondly, we aim to examine how Italian regions are implementing the part of the new decrees on the organization of ethics committees that concerns clinical ethics. METHODS: As for the first aim, we conducted a critical interpretive r

Husbands' involvement in abortion in Vietnam.

This study analyzes the involvement of men in abortion in Vietnam, where induced abortion is legal and abortion rates are among the highest in the world. Twenty men were interviewed in 1996 about the role they played in their wives' abortions and about their feelings and ethical views concerning the procedure. The results showed that both husbands and wives considered the husband to be the main decisionmaker regarding family size, which included the decision to have an abortion, but that, in fact, some women had undergone an abortion without consulting their husbands in advance. Parents and in

Jandu Yani U 'For All Families' Triple P-positive parenting program in remote Australian Aboriginal communities: a study protocol for a community intervention trial.

INTRODUCTION: The population-based (Lililwan) study of fetal alcohol spectrum disorder (FASD) revealed a high prevalence of FASD in the remote communities of the Fitzroy Valley, Western Australia (WA) and confirmed anecdotal reports from families and teachers that challenging child behaviours were a significant concern. In response, Marninwarntikura Women's Resource Centre initiated a partnership with researchers from The University of Sydney to bring the positive parenting program (Triple P) to the Valley. Triple P has been effective in increasing parenting skills and confidence, and improvin
