A Modified Technique in Rhinoplasty: A Septal Extension Graft Complex Using Septal Cartilage, Ethmoid Bone, and Auricular Cartilage.

BACKGROUND: Due to the small and weak septal cartilage of Asians, it is a challenge to obtain ideal tip projection and nose lengthening in this population by performing septal extension grafts with only septal cartilage. OBJECTIVES: The aim of this study was to introduce a septal extension graft complex incorporating nasal septal cartilage, the perpendicular plate of the ethmoid bone, and bilateral auricular cartilage, and to examine its effectiveness in terms of morphological and mechanical support. METHODS: Septal cartilage was harvested under an endoscope according to standard techniques.

Orbital roof cartilage and bone in human fetuses with special reference to changing territories among the ala minor of the sphenoid, frontal bone, and ethmoid.

The orbital roof in adults consists mainly of the frontal bone (FB), with the ala minor of the sphenoid at the posterior margin. In fetuses, these bones have been observed to overlap at the junction. The present study examined sagittal, frontal, and horizontal histological sections from 38 human fetuses at a gestational age (GA) of approximately 7-39 weeks. At GA 7-8 weeks, the ala minor extended anterolaterally from the orbitosphenoid, covering the posteromedial quadrant of the roof until GA 9 weeks and reaching to almost the center of the roof at late-term. Simultaneously, the FB appeared in

Psammomatoid Ossifying Fibroma of the Ethmoid Sinus with Secondary Intracranial Aneurymal Bone Cyst: A Case Report and Literature Review.

Juvenile psammomatoid ossifying fibroma (JPOF) is a rare, slowly progressive tumor of the extragnathic craniofacial bones, with a tendency toward locally aggressive behavior and recurrence. The pathognomonic histopathologic feature is the presence of spherical ossicles, which are similar to psammoma bodies. Very few cases in association with secondary aneurysmal bone cyst (ABC) formation have been reported in literature. Treatment consists of complete surgical removal. However, incomplete excision has been associated with a high local recurrence rate. The prognosis is good because malignant ch

Congenital cranial fasciitis of the ethmoid bone with extensive intracranial and facial involvement: a case report and review of the literature.

Cranial fasciitis is a rare fibroproliferative condition predominantly affecting children. An unusual case of cranial fasciitis in a full-term male newborn presenting with significant right malar swelling is reported. Initial imaging revealed a large frontal extra-axial mass displacing both frontal lobes and extending to the right infratemporal fossa and orbit, subsequently diagnosed via a transoral biopsy. Two-stage surgery involving a multidisciplinary team was performed to completely remove both the intracranial and extracranial components of the lesion. Long-term follow-up MRI indicated no

A Safe Technique for Excising the Perpendicular Plate of the Ethmoid Bone in Patients with Crooked Nose: A Finite Element Analysis.

BACKGROUND: Correction of the crooked nose, especially the perpendicular plate of the ethmoid bone, has the potential to cause skull base injury. At present, the safe and effective method for perpendicular plate resection has not been clearly defined through biomechanics. METHOD: CT scan data of 48 patients with crooked nose and deviated nasal septum were divided into C-type, angular deformity-type, and S-type based on the morphology of the 3D model. Different types of finite element models of the nasal bony septum and skull base were established. The osteotomy depth, angle, and force mode of

Aneurysmal bone cyst on top of fibro-osseous lesion of the ethmoid sinus with orbital and intracranial extension in a child.

OBJECTIVE: Aneurysmal bone cysts (ABCs) rarely involve the cranium. We report a case arising in the ethmoid sinus with orbital and intracranial invasion. Imaging suggested an associated fibro-osseous lesion. The lesion was completely resected. Histology confirmed the imaging diagnosis of ABC on top of an ossifying fibroma. A multidisciplinary approach is essential for optimal surgical outcomes. METHODS: We report a case of an 8-year-old boy with a 5 week history of painless, increasing prominence of the left eye. Clinical examination revealed non-pulsatile left proptosis. Visual acuity and oc

Chondromyxoid fibroma of the nasal bone with extension into the frontal and ethmoidal sinuses: report of one case and a review of the literature.

Chondromyxoid fibroma is a rare benign tumor that usually occurs in the long bones. A 50-year-old patient presented with chondromyxoid fibroma of the nasal bone with extension into the frontal and ethmoidal sinuses. This is the fourth case reported to date in the literature. The clinical manifestations of the tumor were very limited, and the appearance at rhinoscopy was misleading. Radiologic imaging showed a soft tissue lesion invading the adjacent bony structures and the dura mater. Surgery was performed by a combined team of otorhinolaryngologists and neurosurgeons, and total excision of th

Computed tomography of the anterior skull base in Kallmann syndrome reveals specific ethmoid bone abnormalities associated with olfactory bulb defects.

CONTEXT: Kallmann syndrome (KS) is characterized by congenital hypogonadotropic hypogonadism (CHH) and an impaired sense of smell related to defective development of the olfactory system. OBJECTIVE: The aim of the study was to use high-resolution computed tomography (CT) to detect specific abnormalities in the ethmoid bone region surrounding the olfactory bulbs in patients with KS. PATIENTS: Thirty-seven KS patients were compared to normosmic CHH (nCHH) patients (n = 15) and controls (n = 30) of similar age. DESIGN AND METHODS: We conducted a prospective study in a single referral center. S

Surgical management of frontal bone and naso-orbito-ethmoidal fractures: a 7-year multi-center retrospective review.

OBJECTIVE: Frontal and naso-orbito-ethmoidal (NOE) fractures are rare but important injuries due to their anatomical complexity and proximity to vital structures. This study aims to describe the patient factors, procedural factors, and postoperative outcomes in the surgical management of these fractures. STUDY DESIGN: Retrospective review of patients aged 18 years or older who were surgically treated for these fractures at two Australian tertiary hospitals (2014-2020). Patient factors (demographics, mechanism of injury, fracture pattern, concomitant injuries); procedural factors (operation ti

Decreased postoperative pain after reduction of fractured nasal bones using a nerve block of the anterior ethmoidal nerve.

There is consensus that all patients should experience minimal pain following reduction of a fractured nasal bone. The issue requiring further study is what technique will provide patients with the greatest pain relief following the reduction of nasal fractures. This study investigated the use of an anterior ethmoidal nerve block as preemptive analgesia for the management of postoperative pain associated with reduction of nasal bone fractures. The medical documents of 85 patients were reviewed for a retrospective case-controlled comparative study. Patients in the nerve block group (n=45) were

Sinonasal osteosarcoma of the ethmoid bone with decade-later recurrence presenting as early orbital apex syndrome: an 18-year case report.

INTRODUCTION: Primary ethmoid osteosarcoma is an exceptionally rare craniofacial malignancy, characterized by aggressive local invasion of adjacent structures. While most recurrences occur within the first 2-3 years, late relapse after long-term disease-free survival is exceedingly uncommon. CASE PRESENTATION: We describe an 18-year clinical course of a patient first diagnosed with ethmoidal chondroblastic osteosarcoma in childhood. He was treated with chemotherapy and radiation therapy and went into remission. Seven years later, the cancer recurred in the sinuses and lungs. He underwent surg

Septal Cartilage/Ethmoid Bone Composite Graft: A New and Improved Method for the Correction Underdeveloped Nasal Septum in Patients with Short Noses.

BACKGROUND: Septal extension grafts are an effective means of extending nasal length in patients with a short nose. However, such grafts can be challenging in patients who only have small quantities of weak septal cartilage, such as some East Asian patients. We developed a rhinoplasty technique using ethmoid bone to create a cartilage-bone complex to overcome this issue, allowing adequate nasal lengthening. METHODS: Sixty-four women with short noses and inadequate septal cartilage determined by preoperative computed tomography underwent septal extension grafting with a cartilage-bone complex

The ethmoid bone is the ideal graft to strengthen nasal septum L-strut among different grafts: An evaluation based on finite element analysis.

BACKGROUND: In the rhinoplasty of Asian short nose deformity, the surgeon cuts a portion of septal cartilage and retains the nasal septum L-strut, but a deviated nasal septum is formed after the surgery under forces of soft tissue contracture. Although various autologous grafts are used to reinforce the L-strut, there is a lack of objective "mechanical theory" to support the clinical findings in other studies at the present time. METHODS: A simplified and geometrical finite element model of L-strut reinforced with clinically common materials, including auricular cartilage, nasal septal cartil

New landmarks in endonasal surgery: from nasal bone to anterior cribriform plate including branches of anterior ethmoidal artery and nerve and terminal nerve.

BACKGROUND: Despite the development of anterior skull base surgery, the anatomy of the nasal bone and anterior cribriform plate remains unclear. A recent study confirmed 2 distinct foramina in the anterior part of cribriform plate: the ethmoidal slit (ES) and the cribroethmoidal foramen (CF). The aim of this study was to specify their content, their anatomic relationship to the frontal sinus and skull base, and their potential value in skull base surgery. METHODS: Dissections were performed on 36 cadaver heads. Macro- and microscopic examinations were carried out. Microcomputed tomography sca

Localization and migration route of Cephalopina titillator (Diptera: Oestridae) larvae in the head of infested camels (Camelus dromedarius).

The localization and migration routes of Cephalopina titillator larvae were studied in the heads of 45 naturally infested camels. Larvae were located in the pharyngeal cavity (95.6%), the labyrinth of the ethmoid bone (71.1%), the turbinates (28.9%) and the lower nasal meatus (6.7%). Out of 261 first-instar larvae, 73.7% were found in the labyrinth of the ethmoid bone and 26.3% in the turbinates, while 88.9% of second-instar larvae were observed in the pharyngeal cavity and 11.1% in the labyrinth of the ethmoid bone. Most third-instar larvae (91.9%) were found in the pharyngeal cavity, with a

The frontal sinus and the ethmoidal labyrinth.

In the "standard" anatomic description, the frontal bone and cribriform plate of the ethmoid bone form the base of the anterior cranial fossa. We studied the development of the ethmoidal bone as well as its relations to the frontal bone in macerated disarticulated skull bones and macerated skull bases of 35 individuals between 9 and 35 years of age. In 19 cases the ethmoidal cells were completely or partly uncovered by the frontal bone. In 6 of 19 cases the frontal bone did not cover any of the ethmoidal cells; in 10 further cases the frontal bone covered only the anterior and in 3 cases the a
