Background: The emergence of etomidate-containing e-cigarettes represents a significant and rapidly evolving public health challenge, marking a transition of this clinical anesthetic into illegal use of an anesthetic agent. Originally utilized as a short-acting intravenous anesthetic for intubation induction, etomidate's high lipophilicity allows it to be easily aerosolized for inhalation via vaping devices. Misuse is primarily driven by its rapid-onset effects, including relaxation, dissociation, and transient euphoria, as well as a pervasive "illusion of safety" among users who perceive hosp

Etomidate is the agent of choice for the induction of anesthesia in patients with cardiovascular illness, hypotension, or shock. Notwithstanding its numerous attributes, etomidate's most notable adverse impact is adrenocortical suppression. Due to these concerns, the creation of an etomidate alternative that maintains its beneficial effects while mitigating adverse outcomes became essential. This article will focus on the new etomidate analog, methoxyethyl etomidate hydrochloride, referred to as ET-26.

Etomidate, an intravenous hypnotic used for anaesthesia and critical care, is known for its undesirable side effects, including pain on injection, myoclonus, and adrenocortical depression. Despite its continued clinical use because of its haemodynamic stability and rapid onset and offset of effect, alternatives like propofol, ketamine, and remimazolam offer fewer drawbacks. Recent efforts to improve etomidate through chemical modifications, such as methoxyethyl etomidate hydrochloride (ET-26), have shown limited success, with persistent issues like involuntary muscle movements and adrenocortic

Etomidate is a synthetic imidazole anesthetic that exerts hypnotic effects by potentiating the action of the inhibitory neurotransmitter γ-aminobutyric acid (GABA) or directly activating the anionic GABA (GABAA) receptor. It stands out among many anesthetics because of its multiple advantages, such as good hemodynamic stability and minimal inhibition of spontaneous respiration. However, its low water solubility and side effects, such as adrenal cortex inhibition and myoclonus, have limited the clinical application of this drug. To address these issues, extensive research has been conducted on

This review addresses the practical usage of intravenous etomidate as a medical therapy in Cushing's syndrome. We reviewed the relevant literature, using search terms 'etomidate', 'Cushing's syndrome', 'adrenocortical hyperfunction', 'drug therapy' and 'hypercortisolaemia' in a series of public databases. There is a paucity of large randomised controlled trials, and data on its use rely only on small series, case study reports and international consensus guideline recommendations. Based on these, etomidate is an effective parenteral medication for the management of endogenous hypercortisolaemi

Etomidate does not depress the upper airway reflexes, making it difficult to insert a laryngeal mask airway (LMA) when using it for anaesthesia. This study investigated the effect of adding remifentanil to etomidate for LMA insertion. Fifty adult patients, undergoing cystoscopy, were randomized to two groups. The propofol-remifentanil group (n=25) received propofol anaesthesia induction (2.5 mg/kg) and a remifentanil bolus of 0.5 microg/kg, followed by a 2-min remifentanil infusion of 0.05 microg/kg per min. The etomidate-remifentanil group (n=25) received etomidate anaesthesia induction (0.3

Twenty patients were enrolled in a prospective, randomized, open vial, within-patient, crossover design study of methohexital and etomidate anesthesia for electroconvulsive therapy (ECT). Methohexital (1.0 mg/kg) and etomidate (0.3 mg/kg) were each given for two ECTs. While there were no differences in hemodynamics between the etomidate and methohexital groups, etomidate had a 24% longer mean wakeup time than methohexital. There was no difference between etomidate and methohexital in recovery room stay, induction time, or seizure duration. There was no difference in cardiac rhythms. More patie

Plasma concentrations of etomidate have been measured in six patients who required intermittent positive pressure ventilation following surgery. There was an approximately linear relationship between the plasma concentration and the rate of infusion of etomidate. The drug exhibited linear pharmacokinetics over a 10-fold variation in plasma concentration. The decrease in the plasma concentration after discontinuing the infusion was consistent with a three-compartment pharmacokinetic model. The plasma terminal half-life was found to be about 5.5 h, and the clearance calculated to be 0.025 litre

An etomidate infusion regimen for hypnosis as part of balanced, totally intravenous anesthesia was designed to maintain plasma etomidate concentrations above the awakening concentration of 300 ng/ml while avoiding dose-related side effects. The etomidate infusion regimen of 0.1 mg/kg/min for 3 min, 0.02 mg/kg/min for 27 min, and 0.01 mg/kg/min for the remainder of the anesthesia was used together with intravenous bolus doses of fentanyl, droperidol, and pancuronium. This was evaluated in 11 patients and the kinetics of etomidate were reexamined. The anesthetic technique seemed clinically satis

Fourteen, ASA class 1 or 2, outpatients of either sex, admitted for elective arthroscopy of the knees, were anesthetized by continuous infusion of etomidate. After a mean infusion time +/- SD of 38.2 +/- 11.6 min., the awakening time was 4.96 +/- 3.29 min. Five patients (37.7%) had mild pain on the injection side and 3 (21.5%) showed myoclonic movements. Postanesthetically 6 patients (42%) were nauseated and 4 (28.5%) were vomiting. After 130.4 +/- 38.33 min. all patients could be sent home. It is concluded that continuous infusion of etomidate is unless these side effects a good alternative a

The induction characteristics of etomidate, a new i.v. hypnotic agent, were studied in 400 patients. Two hundred were premedicated with atropine and anaesthesia was induced with 0.2, 0.25, 0.3 or 0.35 mg/kg of etomidate. The remainder received one of four standard premedications and anaesthesia was induced with etomidate 0.3 mg/kg. Involuntary muscle movements occurred in more than 60% of patients receiving atropine alone. The frequency was reduced in the second group, but remained unacceptable in over 8% of patients. The incidence of other excitatory phenomena, such as cough and hiccup, was 1

OBJECTIVE: While etomidate is reported as a procedural sedative in adults, its use in children has not been extensively reported. The authors describe their experience with etomidate for procedural sedation in children with extremity fractures and major joint dislocations.
METHODS: This was a retrospective descriptive chart review. The setting was a university-based emergency department (ED) that follows national guidelines for procedural sedation. Subjects were children less than 18 years old who received etomidate prior to fracture reduction or major joint dislocations. Standardized data we

Joint dislocations are common presenting complaints in emergency departments (EDs). Dislocations of major joints, such as the shoulder, elbow, and hip, are often difficult to gently reduce because of the challenge in obtaining sufficient relaxation of large muscle groups. This is the first reported use of etomidate to assist in the reduction of a major joint in an outpatient setting. We discuss the case of an elderly woman with total hip arthroplasty who experienced four spontaneous posterior hip dislocations in a 5-month period. Narcotics and benzodiazepines failed to facilitate reduction at
