Chronic daily administration of oral etoposide in refractory lymphoma.

In a phase II study, 25 patients with previously treated lymphoma received oral etoposide for 21 consecutive days. All patients were considered incurable with standard therapy. Etoposide was administered at 50 mg/m2 per day: courses were repeated every 28-35 days, depending on myelosuppression. 15 patients (60%) had partial responses (95% CI 41-77%), while 10 patients had no response. Median time to disease progression was 5 months (range 2-13 months). Oral etoposide was active against indolent and aggressive (intermediate and high grade) lymphomas; however, median time to progression was only

Retinal toxicity associated with cisplatin and etoposide in pediatric patients.

Cisplatin is an effective chemotherapeutic agent used in the treatment of many pediatric solid tumors. Retinal toxicity is a side effect of the drug reported in adults, but is not well described in pediatric patients. We present the cases of two children treated with cisplatin and etoposide who experienced retinal toxicity documented by visual evoked response (VER) and electroretinogram (ERG). significantly, both patients had abnormal renal function. The mechanism of visual toxicity induced by cisplatin is unknown but may result from central nervous system (CNS) accumulation of drug after repe

Effect of etoposide and cyclophosphamide on an experimental pulmonary infection in mice.

The effects of monocytopenia and granulocytopenia on the proliferation of Klebsiella pneumoniae and the efficacy of treatment with ceftriaxone were studied in an experimental lung infection in mice. Treatment with etoposide led to monocytopenia, and cyclophosphamide to granulocytopenia and monocytopenia. Both drugs gave a decrease in the number of alveolar macrophages during the infection and a diminished influx of granulocytes into the alveolar space. The mice treated with etoposide did not differ from controls with respect to either the proliferation of Kl. pneumoniae during the infection or

[Comparison of Cisplatin plus Etoposide and Carboplatin plus Etoposide in Chemoradiotherapy for Limited-Stage Small Cell Lung Cancer-A Single-Institution Retrospective Study].

BACKGROUND: The Japanese Guidelines for Diagnosis and Treatment of Lung Cancer recommend concurrent chemoradiotherapy (CRT) with cisplatin plus etoposide (PE) for limited-stage small cell lung cancer (LS-SCLC). When cisplatin administration is difficult, carboplatin plus etoposide (CE) with either sequential or concurrent thoracic radiotherapy is selected. While overseas studies have shown no significant difference in overall survival between PE and CE, evidence in Japan remains limited. METHODS: We retrospectively compared progression-free survival (PFS) between patients treated with PE and

Goodbye etoposide? Taking the leap to ruxolitinib in haemophagocytic lymphohistiocytosis.

Fang et al. report a retrospective analysis of paediatric patients with haemophagocytic lymphohistiocytosis (HLH) associated with autoimmune/autoinflammatory disorders treated with ruxolitinib. Responses were impressive and rapid, and ruxolitinib was well tolerated. This study demonstrates that a subset of patients with HLH can be treated with JAK inhibition without the need for cytotoxic chemotherapy. Further work will be needed to better define patient selection for therapy, as some patient groups and HLH triggers, such as malignancy-associated HLH, may be better suited for etoposide-based t

Severe multiorgan toxicities after the first cycle of standard-dose etoposide-cisplatin therapy for metastatic nonseminoma successfully managed with etoposide-carboplatin: a case report.

BACKGROUND: The combination of bleomycin, etoposide, and cisplatin is the standard regimen for good-risk metastatic nonseminomatous germ cell tumors, with etoposide and cisplatin used when pulmonary risk is present. Etoposide and cisplatin therapy is generally well tolerated, and severe toxicities are particularly rare after the first standard-dose cycle. We report a case of early-onset, simultaneous multiorgan failure after a single cycle of etoposide and cisplatin therapy, successfully managed with etoposide-carboplatin. A yeast-based platform for etoposide production via yatein bioconversion.

Etoposide, a semisynthetic derivative of podophyllotoxin originally isolated from the rhizomes of mayapple (Podophyllum peltatum) and indian Podophyllum (P. hexandrum) plants, is one of the most powerful chemotherapeutic agents used to treat various types of solid tumors and blood malignancies. Despite its clinical importance, its supply is recurrently constrained due to a heavy reliance on plant extraction, where low natural precursor abundance and increasing climate-related pressures limit production scalability. Developing alternative manufacturing routes has therefore become a major object

Gut bacterial O-demethylation modulates systemic exposure to oral etoposide.

Gut microbial O-demethylation has been reported for plant-derived dietary compounds containing O-methylated aromatic(s). However, the significance of gut microbial O-demethylation in drug metabolism and disposition remains unexplored. This study examined 64 clinically used oral drugs containing one or more methoxylated aromatics for gut microbial O-demethylation using high-resolution mass spectrometry (HRMS). For 35 of the tested drugs, including the anticancer agent etoposide, we detected metabolites corresponding to O-demethylation (i.e., a mass difference of -14 and its multiples) when indi

Ten-day schedule oral etoposide therapy in advanced childhood malignancies.

PURPOSE: The activity of etoposide (VP-16) has been demonstrated to be schedule-dependent. Several studies have been conducted on the efficacy and safety of different schedules of VP-16 both in adults and in children, but the optimal schedule has not been determined. METHODS: In the current study, the feasibility and effectiveness of prolonged oral VP-16 in children with high-risk malignancies were evaluated. Between April 1995 and February 1999, 15 pretreated patients with high-risk tumors received oral VP-16. The schedule of therapy was oral VP-16 50 mg/m2/day for 10 consecutive days and 1-

Primary oral etoposide therapy in gestational trophoblastic disease. An update.

Sixty patients who developed persistent or metastatic gestational trophoblastic disease (GTD) received primary oral etoposide therapy (VP 16-213). Twelve patients had metastatic GTD. Fifty-nine patients achieved biochemical remission. One patient had marked nausea and vomiting and the therapy was switched to a methotrexate/folinic acid regimen. Three patients developed relapse of GTD, giving a relapse rate of 5.1%. Etoposide is an active drug against choriocarcinoma. Its use should not be restricted to drug-resistant GTD.

Engineered etoposide-prednisolone co-nanocrystals for synergistic antitumor efficacy enhancement.

The present work involves the development of new organic crystalline nanomedicines, namely, co-nanocrystals (co-NCs), that combine a chemotherapeutic agent with an anti-inflammatory active pharmaceutical ingredient (API), both in their nanocrystalline forms. These mixed systems were formulated to improve their anticancer efficiency. For this purpose, formulations composed of etoposide (ETO) and prednisolone (PRD) NCs were prepared. The co-formulations were optimized with the stabilizing agent poloxamer P407. The average size obtained was 191.9 ± 6.5 nm with a polydispersity index of 0.28 ± 0.0

Secondary acute myeloid leukemia after etoposide therapy for haemophagocytic lymphohistiocytosis.

Haemophagocytic lymphohistiocytosis (HLH) is an uncommon disease with a high fatality rate. Etoposide is an important component of current HLH treatment regimes. Two patients with HLH developed etoposide-related secondary acute myeloid leukemia (sAML) following therapy for HLH. Etoposide, an epipodophyllotoxin, is a topoisomerase II inhibitor that interacts with DNA to potentiate leukaemogenesis. The risk of developing sAML is estimated to be between 1% and 5%, 2-20 years after exposure to etoposide but may also be related to cumulative drug doses, treatment schedules, host factors and co-admi
