A Review of Research on Acceptance and Commitment Therapy for Anxiety and Obsessive-Compulsive and Related Disorders.

This article reviews acceptance and commitment therapy (ACT) for anxiety disorders, obsessive-compulsive disorder (OCD), trichotillomania, excoriation disorder, hoarding, and body dysmorphic disorder (BDD). Measurement of psychological inflexibility and its relationship to pathology is reviewed. Outcome work in all disorders listed is reviewed with results supporting ACT for anxiety disorders, OCD, and trichotillomania, but there is limited evidence for excoriation disorder, hoarding, and BDD. Future research should focus on processes of change in addition to outcomes.

An open clinical trial of fluvoxamine treatment of psychogenic excoriation.

The purpose of this study was to examine the safety and efficacy of fluvoxamine in the treatment of psychogenic (neurotic) skin excoriation. Fourteen subjects with psychogenic excoriation were given fluvoxamine in a 12-week, open-label trial after completion of the Structured Clinical Interview for DSM-IV. All subjects met DSM-IV criteria for at least one comorbid psychiatric disorder, with mood disorder the most common. Most subjects' excoriation had features of an impulse control disorder. Both completers (N = 7) and the entire group had significant improvement on the modified Yale-Brown Obs

A selective review of glutamate pharmacological therapy in obsessive-compulsive and related disorders.

Glutamate, an excitatory central nervous system neurotransmitter, is emerging as a potential alternative pharmacological treatment when compared to gamma-aminobutyric acid (GABA)-, dopamine-, and serotonin-modulating treatments for neuropsychiatric conditions. The pathophysiology, animal models, and clinical trials of glutamate modulation are explored in disorders with underlying inhibitory deficits (cognitive, motor, behavioral) including obsessive-compulsive disorder, attention deficit hyperactivity disorder, Tourette syndrome, trichotillomania, excoriation disorder, and nail biting. Psycho-cutaneous disorders: an epidemiologic study.

BACKGROUND AND AIMS: Psycho-dermatology addresses the interaction between mind and the skin. Effective management of at least one third of the patients attending the skin department depends, to some extent, upon the recognition of emotional and psychologic factors. The aim of this study was to provide epidemiologic data about common psycho-dermatoses and their prevalence in dermatologic patients in Iran. MATERIALS AND METHODS: Patients suspicious to have psychocutaneous problem with primary dermatologic chief complaint were included. After detailed dermatologic evaluation, all patients were v

A Case of Skin Picking Disorder of a Patient with a History of Childhood Abuse.

Skin picking (excoriation) disorder is the recurrent excoriation of one's own skin, resulting in noticeable skin damage. People pick their skin for different reasons. For the majority of patients, first skin picking is associated with a history of childhood abuse and personal problems. Subjects who moderately to severely cause injurious self-harm are more likely to have a history of exposure to domestic violence and childhood abuse than those who do not self-harm. At the same time, these conditions could be related to the etiology for majority of other psychiatric disorders. We report herein,

Trigeminal trophic syndrome in a pediatric patient with diffuse intrinsic pontine glioma.

A 13-year-old girl with a history of diffuse intrinsic pontine glioma (DIPG) suffered from progressively worsening facial ulcerations secondary to paresthesia-induced self-excoriation. She was diagnosed with trigeminal trophic syndrome (TTS) induced by DIPG and struggled to heal her lesions in the background of this excoriation disorder. A multidisciplinary approach that included mood disorder management with sertraline and amitriptyline helped diminish paresthesia, improve her quality of life, and promote healing of the ulcers despite the progression of her DIPG. This case highlights the mult

Phenomenology and therapeutic options for dermatotillomania.

Dermatotillomania (also called psychogenic excoriation, neurotic excoriation, acne excoriée, skin picking and pathological or compulsive skin picking) is characterized by excessive scratching or picking of normal skin or skin with minor surface irregularities. Until recently, there have been few therapeutic options for patients with this disorder, which causes substantial distress and functional impairment in many patients. Recent pharmacological studies have found that some selective serotonin reuptake inhibitors may be effective in the treatment of this disorder. Furthermore, phenomenologica

Body-Focused Repetitive Behavior Disorders: From Competing Paradigms Toward Iterative Integration.

Body-focused repetitive behaviors (BFRBs), such as hair-pulling and skin-picking, have been described by physicians from Hippocrates onward. The descriptions have not been theory-neutral; clinicians and researchers have applied specific conceptual frameworks to understand such behaviors and to intervene accordingly. These include what might be termed "turns" to psychoanalysis, cognitive behaviorism, psychopharmacology, nosology, neuroscience, consumer advocacy, and global mental health. It is timely to consider lessons learned from this range of conceptual frameworks, each of which has both st

Body-focused repetitive behaviors and the dermatology patient.

Body-focused repetitive behaviors (BFRBs) are psychiatric disorders that involve recurrent pulling and picking one's own body resulting in skin lesions with varying degrees of severity. For that reason, the interface with dermatology is important. Currently, the classified BFRBs are trichotillomania and excoriation disorder. Both trichotillomania and excoriation disorder appear to be more common than previously thought. Besides that, most patients are unlikely to seek mental health treatment for their condition. Thus, many patients will instead seek dermatologic help due to the cosmetic damage

Psychosomatic study of self-excoriative behavior among male acne patients: preliminary observations.

BACKGROUND: Studies of the psychosomatic correlates of self-excoriative behavior in acne have involved mainly women with acne excorieé. Little is written about the psychosomatic factors that affect men with acne. Excessive self-excoriation of acne lesions is an important clinical factor because it can prolong the course of the disease and exacerbate the deeper inflammatory process with an increase in the severity of scarring. METHODS: Thirteen men (mean +/- SE: age: 22.2 +/- 1.4 years) with mild to moderate facial acne, whose self-excoriative behavior was not severe enough to result in acne e

Skin Picking Successfully Treated With Atomoxetine in Comorbid Skin Picking and Attention-Deficit/Hyperactivity Disorder: A Case Report.

"Skin picking disorder" (SPD: also known as neurotic excoriation, psychogenic excoriation, or dermatillomania) is classified in the "obsessive-compulsive and related disorders" category in the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, and characterized by unintentional, repetitive skin picking behaviors. Atomoxetine is a selective noradrenaline reuptake inhibitor used in the treatment of attention-deficit/hyperactivity disorder (ADHD). In this case report, we present a 9-year-old girl with the comorbid diagnosis of ADHD and SPD treated successfully with atomoxetine.

Psychiatric Features in Neurotic Excoriation Patients: The Role of Childhood Trauma.

INTRODUCTION: Neurotic excoriation is a psychodermatological disease of primary psychological/psychiatric genesis, responsible for self-induced dermatological disorders. Childhood traumatic events are closely related with self-injurious behaviors. The aim of this study is to evaluate the psychiatric features of neurotic excoriation and to investigate the effect of childhood traumatic events on the disease. METHODS: Thirty-eight neurotic excoriation patients who did not receive any psychiatric treatment within the past year and 40 healthy individuals having similar sociodemographic features we

Skin Picking Disorder in a Woman with Polyarteritis Nodosa. Case Report.

INTRODUCTION: Excoriation (skin picking) disorder is included in the DSM-5 in the obsessive compulsive and related disorders category. It is defined as the recurrent urge to touch, scratch, scrape, scrub, rub, squeeze, bite or dig in the skin, leading to skin lesions. It is a rare disorder (1.4-5.4% of the population) and occurs mainly in women. CASE REPORT: this article reports the case of a 31-year-old female patient, initially assessed by dermatology and orthopaedics for the presence of infected ulcerated lesions on her lower limbs, with other superficial lesions from scratching on her che

Psychocutaneous syndromes: a call for revised nomenclature.

We propose abandoning three outdated, pejorative diagnostic terms: 'trichotillomania', 'delusions of parasitosis', and 'neurotic excoriation'. These insensitive terms are offensive to patients and become a barrier to treatment. We suggest the following alternative patient-centred nomenclature: 'neuromechanical alopecia', 'pseudoparasitic dysaesthesia', and (simply) 'excoriation'. We briefly review clinical and historical aspects of these disorders.

Interoception abnormalities and skin-picking disorder: Systematic review and narrative synthesis of preliminary evidence.

A growing body of research suggests that disrupted interoception plays an important role in various mental disorders, including Obsessive-Compulsive and Related Disorders. However, interoceptive abnormalities in Skin Picking Disorder (SPD) remain unexplored. This study aimed to evaluate and synthesize existing evidence regarding interoceptive disruptions in SPD, adopting a broad definition of interoception encompassing certain types of skin-mediated signals. We searched five databases (PubMed, Scopus, EBSCO, Web of Science, Embase) for articles reporting associations between SPD and any intero

Primary psychiatric conditions: dermatitis artefacta, trichotillomania and neurotic excoriations.

Primary psychiatric conditions encountered in dermatology include dermatitis artefacta, trichotillomania (TTM) and neurotic excoriations. For these disorders, the primary pathologic condition involves the psyche; therefore, any cutaneous findings are self-induced. Herein, we review common primary psychiatric conditions in dermatology - dermatitis artefacta, neurotic excoriations and TTM - and examine their epidemiology, clinical presentation, differential diagnosis and treatment strategies. For all primary psychiatric disorders, the most effective underlying strategy is to first establish a st

Resting-State Functional Connectivity of Supplementary Motor Area Associated with Skin-Picking Symptom Severity.

Pathological skin picking (excoriation) is a relatively common disorder. Although it has been hypothesized to share a similar pathophysiological basis as other obsessive-compulsive (OC) spectrum disorders, to date, little work has specifically examined the precise neurobiological mechanisms involved in excoriation. Disruption in functional circuits involving the right inferior frontal gyrus (rIFG) and supplementary motor area (SMA) may be particularly relevant to skin-picking pathology as these regions have been implicated in other OC-spectrum disorders for their roles in response inhibition a

Neurotic excoriations: a diagnosis of exclusion.

Patients with psychiatric disease may use the skin as a means of communication during times of increased emotional distress. Furthermore, a high incidence of skin disorders among patients with a primary psychiatric condition, including depression, schizophrenia, and anxiety, has been demonstrated, with neurotic excoriation being one of the most commonly diagnosed. Despite the strong association and incidence of psychogenic excoriation in patients with a primary psychiatric disorder, it is important for primary care physicians and dermatologists alike to realize that these patients may have tru
