Clinical aspects of exhaled nitric oxide.

There has been intense research into the role nitric oxide (NO) plays in physiological and pathological mechanisms and its clinical significance in respiratory medicine. Elevated levels of exhaled levels of exhaled NO in asthma and other inflammatory lung diseases lead to many studies examining NO as potential markers of airway inflammation, enabling repeated noninvasive and standardized monitoring of airway inflammation. In airway inflammation, NO is not merely a marker but may have anti-inflammatory and pro-inflammatory effects. Significant correlation has been found between exhaled NO and s

Exhaled nitric oxide in childhood asthma.

UNLABELLED: Endogenous synthesis of nitric oxide (NO) and its presence in exhaled air was observed in various species including humans. Particularly high levels were found in adults with bronchial asthma, possibly because of the underlying pulmonary inflammatory activity. We studied oral and nasal exhaled NO by chemiluminescence in 47 children aged between 6 and 10 years. Thirty children had bronchial asthma, 17 were healthy controls. In asthmatic children oral exhaled NO was 13.4 +/- 1.4 parts per billion (ppb) (mean +/- SEM), nasal exhaled NO was 21.7 +/- 1.5 ppb. In healthy controls oral ex

Reperfusion injury and exhaled hydrogen peroxide.

Reactive oxygen species have been implicated in the pathophysiology of lung injury associated with the sequence of ischemia-reperfusion. To study this, we measured the exhaled breath hydrogen peroxide concentration [H2O2] in human and canine models of reperfusion lung injury. Our models were patients subjected to cardiopulmonary bypass (CPB) (Group 1), patients undergoing pulmonary thromboendarterectomy (Group 2), canine single lung transplant (Group 3), and patients subjected to peripheral ischemia resulting from aortic cross-clamping or tourniquet application (Group 4). In addition, we studi

Exhalation-synchronous robotic abdominal compression for user-centered respiratory assistance and training in neurological patients.

BACKGROUND: Patients with respiratory weaknesses due to neurological disorders or aging often face usability challenges with conventional mechanical ventilators, which directly move air in and out of the lungs via the ventilation mask. To address these challenges, Exo-Abs was developed to support a wide range of respiratory functions through synchronous abdominal compression. Its control previously relied on continuous full-phase measurements from multiple sensors to ensure control performance across a wide dynamic range. However, accommodating long-term usage scenarios remained challenging, a

Differential Response of Pentanal and Hexanal Exhalation to Supplemental Oxygen and Mechanical Ventilation in Rats.

High inspired oxygen during mechanical ventilation may influence the exhalation of the previously proposed breath biomarkers pentanal and hexanal, and additionally induce systemic inflammation. We therefore investigated the effect of various concentrations of inspired oxygen on pentanal and hexanal exhalation and serum interleukin concentrations in 30 Sprague Dawley rats mechanically ventilated with 30, 60, or 93% inspired oxygen for 12 h. Pentanal exhalation did not differ as a function of inspired oxygen but increased by an average of 0.4 (95%CI: 0.3; 0.5) ppb per hour, with concentrations d

Exhalation of volatile organic compounds during hemorrhagic shock and reperfusion in rats: an exploratory trial.

Ischemia and reperfusion alter metabolism. Multi-capillary column ion-mobility spectrometry (MCC-IMS) can identify volatile organic compounds (VOCs) in exhaled gas. We therefore used MCC-IMS to evaluate exhaled gas in a rat model of hemorrhagic shock with reperfusion. Adult male Sprague-Dawley rats (n  =  10 in control group, n  =  15 in intervention group) were anaesthetized and ventilated via tracheostomy for 14 h or until death. Hemorrhagic shock was maintained for 90 min by removing blood from the femoral artery to a target of MAP 35  ±  5 mmHg, and then retransfusing the blood over 60 min

Inspiratory flow rates during hard work when breathing through different respirator inhalation and exhalation resistances.

There has been a long-standing debate regarding the adequacy of airflow rates used in respirator certification testing and whether these test flow rates underestimate actual values. This study investigated breath by breath inspiratory peak flow rate, minute ventilation, and instantaneous flow rates of eight young, healthy volunteers walking on a treadmill at 80-85% of maximal aerobic capacity until exhaustion while wearing an air-purifying respirator with one of eight combinations of inhalation and exhalation resistance. An analysis of variance was performed to identify differences among the e

Quantify impacted scope of human expired air under different head postures and varying exhalation rates.

Many researches indicate human respiration flow and background ventilation are two important aspects leading to possible respiratory disease spread. However, current studies on respiration flow and the resulted exhaled pollutant dispersion are limited, because different head postures, respiration mode, breath rate, room ventilation and so on, can exert profound impacts that are not understood very clearly. To evaluate the role of head postures on transmission of human exhaled pollutants, this study uses a computational fluid dynamics (CFD) program to study the exhalation flow of a sitting adul

Human brain activation during phonation and exhalation: common volitional control for two upper airway functions.

Phonation is defined as a laryngeal motor behavior used for speech production, which involves a highly specialized coordination of laryngeal and respiratory neuromuscular control. During speech, brief periods of vocal fold vibration for vowels are interspersed by voiced and unvoiced consonants, glottal stops and glottal fricatives (/h/). It remains unknown whether laryngeal/respiratory coordination of phonation for speech relies on separate neural systems from respiratory control or whether a common system controls both behaviors. To identify the central control system for human phonation, we

Estimating nasal nitric oxide from measured fractional exhaled nitric oxide and nasal fractional exhaled nitric oxide.

BACKGROUND: Nasal nitric oxide (nNO) is an effective screening indicator for primary ciliary dyskinesia (PCD) and a potential biomarker for nasal diseases; however, Measurement of nNO using standard methods is not available everywhere. Therefore, we aimed to estimate nNO values from measured fractional exhaled NO (FeNO) and nasal FeNO (nFeNO) levels using a widely used FeNO analyzer. METHODS: In this single-center cross-sectional study, the proportion of NO from the lower airway mixed with nFeNO was calculated by simultaneously measuring nFeNO and nNO in 15 healthy participants. Additionally,

Comparison of exhaled and nasal nitric oxide and exhaled carbon monoxide levels in bronchiectatic patients with and without primary ciliary dyskinesia.

BACKGROUND: Primary ciliary dyskinesia (PCD) is associated with chronic airway inflammation resulting in bronchiectasis. METHODS: The levels of exhaled nitric oxide (eNO), carbon monoxide (eCO) and nasal NO (nNO) from bronchiectatic patients with PCD (n=14) were compared with those from patients with non-PCD bronchiectasis without (n=31) and with cystic fibrosis (CF) (n=20) and from normal subjects (n=37) to assess the clinical usefulness of these measurements in discriminating between PCD and other causes of bronchiectasis. RESULTS: Exhaled NO levels were lower in patients with PCD than in

The pupillary respiratory-phase response: pupil size is smallest around inhalation onset and largest during exhalation.

Respiration shapes brain activity and synchronizes sensory and exploratory motor actions, with some evidence suggesting that it also affects pupil size. However, evidence for a coupling between respiration and pupil size remains scarce and inconclusive, hindered by small sample sizes and limited controls. Given the importance of pupil size in visual perception and as a reflection of brain state, understanding its relationship with respiration is essential. In five experiments using a pre-registered protocol, we systematically investigated how respiratory phase affects pupil size across differe

Effects of condensate in the exhalation limb of neonatal circuits on airway pressure during bubble CPAP.

BACKGROUND: Bubble CPAP is frequently used in spontaneously breathing infants with lung disease. Often bubble CPAP systems lack pressure alarms and pressure-release valves. We observed a large volume of condensate in the exhalation limb of a patient circuit and conducted a series of experiments to test the hypothesis that accumulated condensate could affect delivered pressures. METHODS: An anatomically accurate nasal airway model of a preterm infant was attached to a spontaneously breathing lung model. A bubble CPAP system was attached to the nasal airway with bi-nasal short prongs, and the r

Caffeine, quercetin and alizarin stimulate the exhalation of metabolic products of [14C]-N-nitrosodiethylamine in mice.

Naturally occurring plant products belonging to different chemical classes namely alizarin, an anthraquinone, caffeine, a methylxanthine derivative and quercetin, a flavonol were studied for their effect on elimination of metabolites of [14C]-N-nitrosodiethylamine (14C-NDEA) through respiration in mice. Treatment with caffeine, quercetin and alizarin at doses of 200, 9 and 9 microg/ml respectively, in drinking water enhanced the exhalation of 14CO2, one of the major end products of NDEA metabolism. Radioactive CO2 exhaled in 60 min increased by 2, 1.61 and 1.4-folds in animals treated with caf

Role of exhaled nitric oxide as a predictor of atopy.

BACKGROUND: The fractional exhaled nitric oxide (FeNO) is a quantitative, noninvasive and safe measure of airways inflammation that may complement the assessment of asthma. Elevations of FeNO have recently been found to correlate with allergic sensitization. Therefore, FeNO may be a useful predictor of atopy in the general population. We sought to determine the diagnostic accuracy of FeNO in predicting atopy in a population-based study. METHODS: We conducted a cross-sectional study in an age- and sex- stratified random sample of 13 to 15 year-olds in two communities in Peru. We asked particip

Exhaled breath condensate in pulmonary arterial hypertension.

Over the last decade, several new agents have been developed for the treatment of pulmonary arterial hypertension (PAH), and blood biomarkers have been developed which aim to monitor such treatment, and which correlate well with physiological parameters, symptoms and mortality. However, little is known regarding biomarkers collected using non-invasive methods such as exhaled breath condensate (EBC). EBC biomarkers show potential as a rapid, repeatable and easy method of sampling the pulmonary vasculature in severely ill patients. The current study aimed to investigate EBC biomarkers in patient

Characterizing exhaled airflow from breathing and talking.

UNLABELLED: The exhaled air of infected humans is one of the prime sources of contagious viruses. The exhaled air comes from respiratory events such as the coughing, sneezing, breathing and talking. Accurate information on the thermo-fluid characteristics of the exhaled airflow can be important for prediction of infectious disease transmission. The present study developed a source model to provide the thermo-fluid conditions of the exhaled air from the breathing and talking processes. The source model is a set of equations obtained from the measurements of the flow rate, flow direction, and ar

Are exhaled breath condensates useful in monitoring asthma?

Exhaled breath condensate (EBC) has emerged as a novel noninvasive technique for assessment of airway inflammation, and it also provides useful information on the airway lining fluid composition. Examples of markers that can be identified in the EBC of patients with asthma include pH, eicosanoids, nitrogen oxides and related products, markers of oxidative stress, certain cytokines, chemokines, and growth factors. There is some evidence that certain markers in EBC differ between patients with asthma and controls, and some markers may correlate with asthma severity and lung function, but there a
