Siderotic glaucoma without detectable intraocular foreign body in a pseudophakic eye: a case report.

BACKGROUND: Ocular siderosis is induced by a retained intraocular foreign body (IOFB) containing iron and can present as siderotic glaucoma. We report a rare case of histopathologically proven siderotic glaucoma in a middle-aged blacksmith with a preceding history of ocular trauma but no radiologically detectable IOFB. CASE PRESENTATION: A 42-year-old blacksmith presented with an elevation of intraocular pressure (IOP) in left eye showing iris heterochromia and brownish deposits throughout the trabecular meshwork (TM). Preoperative ophthalmic examination did not reveal any retained IOFBs. Ele

[Regarding the so called posterior approach for posterior eye wall foreign body removal].

Disadvantages of the posterior approach for the optic nerve head foreign body removal, especially with associated intraocular hemorrhage, have been analyzed. In such circumstances the transvitreal approach is of an evident benefit since it also allows removing the intraocular hemorrhage which is a potential source of severe vitreoretinal proliferation.

Evaluation of retinal integrity in eyes with retained intraocular metallic foreign body by ERG and EOG.

In three eyes with a retained intraocular metallic foreign body, the neural retina and retinal pigment epithelial (RPE) integrity were evaluated by means of ERG (a-wave, b-wave, oscillatory potentials) and non-photic EOG responses from the RPE (hyperosmolarity response, Diamox response, bicarbonate response). All three cases were judged in an early stage of retinal impairment on the basis of a well-preserved or supernormal b-wave amplitude and showed good postoperative prognosis. Out of three non-photic responses from the RPE, only the bicarbonate response was preoperatively reduced in two out

A comparative clinical study of Solcoseryl Eye-Gel and Cysteine Eye-Gel 2.4% in the treatment of foreign-body injuries of the cornea.

Solcoseryl, a protein-free haemodialysate, promotes tissue regeneration and improves utilization of oxygen in the cell. In a randomized, double-blind, clinical study, Solcoseryl Eye-Gel was compared with Cysteine Eye-Gel in the treatment of foreign-body injuries of the cornea in a total of 99 eyes. In order to facilitate objective evaluation of the effects of the treatment, the area of the lesion before the start of treatment and on the following day was determined by means of slit-lamp photographs. Healing of the lesion and relative reduction of the area of the wound were observed much more f

Eye irrigation as a first-line treatment and diagnostic method for emergency department patients who complain of ocular foreign bodies.

This prospective study aimed at determine whether eye irrigation removes ocular foreign bodies (FBs) and whether ocular pain predicts FBs. Emergency department patients complaining of ocular FBs were enrolled. In the irrigation group (n = 52), pain was evaluated with a visual analog scale before and after irrigation, and the presence of FBs was determined under a slit-lamp. In the nonirrigation group (n = 27), the evaluations were performed upon arrival. The corneal FB retention rate was found significantly lower in the irrigation (13/52, 25%) than in the nonirrigation groups (13/27, 48%; P =

Long-term follow-up and the role of vitrectomy in the treatment of perforating eye injuries without intraocular foreign bodies.

The long-term results of 292 consecutive eyes with a perforating eye injury without an intraocular foreign body treated with or without pars plana vitrectomy are reported. With the mean follow-up time of 31.0 months 45 eyes (15%) were enucleated and another 47 eyes (16%) were blind (visual acuity less than 0.05). The retina was attached in 87% of the non-enucleated eyes. The visual outcome was considerably better in eyes with anterior segment injuries (ASI) compared to posterior segment injuries (PSI). Eyes with primary vitreous haemorrhage had poorer visual prognosis than eyes without haemorr

Chandelier-assisted scleral buckling in an eye with longstanding inert foreign body with fresh rhegmatogenous retinal detachment.

Intraocular foreign bodies (IOFBs) present in varied manners which in turn necessitate their removal in majority of the cases. A stone foreign body can remain inert inside the eye for years. Retinal detachment in eyes following penetrating trauma with an IOFB is common, but a combination of fresh rhegmatogenous retinal detachment in an eye with a longstanding inert stone foreign body is extremely rare. We report a case of a 50-year-old male with rhegmatogenous retinal detachment with a longstanding stone foreign body, where we managed such a scenario with a chandelier-assisted, sutureless, scl

Transscleral Extraction of an Intraocular Foreign Body from the Posterior Segment of the Eye without Pars Plana Vitrectomy.

PURPOSE: Penetrating eye trauma with an intraocular foreign body is very frequent, especially in men in their productive age. Pars plana vitrectomy would be the standard surgical method at our department. However, in indicated cases (metallic intraocular bodies in the posterior eye segment in young patients with well transparent ocular media without detached ZSM and without any evident vitreoretinal traction) transscleral extraction of the intraocular foreign body is performed using the exo magnet, eventually endo magnet with a minimal PPV without PVD induction under the visual control of endo

[Pars plana vitrectomy with extraction of intraocular foreign body in patients with siderosis of the eye: report of two cases].

INTRODUCTION: Penetrating injury is characterized by the existence of entry wound only, and it can be with or without an intraocular foreign body (IOFB). IOFB can lead to a mechanical injury of the eye and to cause infection or to manifest other toxic effects on intraocular structures. Iron and copper can dissolve and cause siderosis, i.e. chalcosis of the eye. Ocular siderosis is diagnosed by clinical and electroretinogram (ERG) findings. OUTLINE OF CASES: The first patient was a 37-year-old male who was injured by a metal foreign body. He presented at the Clinic two years after the injury.

Plain film, CT and MRI sensibility in the evaluation of intraorbital foreign bodies in an in vitro model of the orbit and in pig eyes.

Detection and characterization of intraorbital foreign bodies (IFB) is fundamental in acute trauma setting, preventing inflammatory sequelae or complications related to IFB movements when a MRI study is planned. Papers concerning plain film and CT sensibility in IFB detection show controversial results. For this reason we investigated plain film, CT and MRI sensibility in the evaluation of IFB. For an in vitro model, specimens of dry and fresh wood, glass, iron, plastic and graphite were immersed in animal lard and in a 0.9% sodium chloride plus 3.5 g/dl human serum albumin solution. More to it than meets the eye: a case of retained intraorbital foreign body removed using transcutaneous orbital endoscopic surgery.

The possibility of a retained foreign body should always be considered when a patient presents with a history of orbital trauma, especially when the patient is unresponsive or temporarily responsive to treatment. Not all cases of retained foreign body present with decreased vision or restricted mobility or fever. The entry wound is also not apparent on examination in all cases. In summary, meticulous history-taking, thorough examination, high index of suspicion along with a low threshold for imaging studies are essential to make a timely diagnosis of a retained intraorbital foreign body.
