Factitious Disorder Presenting with Refractory Enterocutaneous Fistula that Underwent 7 Surgeries.

The formation of a post-appendicectomy fistula is rare but devastating. Major etiological factors include leakage from the appendiceal stump, neoplasm of the appendix and/or cecum, infection, inflammatory bowel disease, and distal obstruction. The management of enterocutaneous fistula involves enteral nutrition, drainage, antibiotic coverage, as well as surgical excision and segmental resection of the involved bowel. Here we present a case where a young female suffered from refractory enterocutaneous fistula after appendicectomy. Despite extensive treatments, her symptoms persisted and contrad

Fatal non-thrombotic pulmonary embolization in a patient with undiagnosed factitious disorder.

BACKGROUND: Factitious fever is extremely challenging to diagnose in patients with complicated chronic medical problems, and represents as much as 10% of fevers of unknown origin. Factitious fever caused by self-injecting oral medications through indwelling central catheters is a diagnostic challenge. CASE PRESENTATION: We present a 32-year-old Caucasian female with history of short gut syndrome, malnutrition requiring total parental nutrition, and pancreatic auto-islet transplant with fever of unknown origin. Multiple episodes of bacteremia occurred with atypical pathogens, including α-hemol

Unexplained recurrent oral bleeding in an adolescent suggestive of factitious disorder: a case report.

BACKGROUND: Factitious disorder is characterized by the intentional production or feigning of symptoms to assume the sick role without obvious external gain. It poses significant diagnostic and treatment challenges, particularly in pediatric populations where symptoms may mimic organic illness and caregiver involvement is common. CASE PRESENTATION: We describe the case of a 16-year-old Indian female individual with 13 months of unexplained recurrent oral bleeding. Comprehensive multidisciplinary evaluation, including hematologic panels, imaging, and multiple otolaryngologic procedures, reveal

Self-inflicted non-healing genital ulcer: a rare form of factitious disorder.

Dermatitis artefacta is a factitious dermatological disorder with many forms of presentation that may occur on any part of the body. A diagnosis of dermatitis artefacta is often reached after rigorous and repeated investigations. Here we present the case of a 49-year-old single man complaining of a 4- month history of ulceration on the dorsal surface of the glans penis. In view of the unusual appearance of the lesion and the negative findings from clinical investigations, a diagnosis of dermatitis artefacta was made and the patient was referred for psychiatric evaluation. He was started on 20

Factitious disorder comorbid with borderline personality disorder and dysthymia: from medically unexplained physical symptoms to functional neurological disorder.

INTRODUCTION: In this study, we introduce the concept of comorbidity between factitious disorder (FD), borderline personality disorder (BPD), dysthymia (DY), medically unexplained physical symptoms (MUPS) and functional neurological disorder (FND) characterising patients who may tend to exaggerate physical or psychiatric symptoms of presentation to a general or psychiatric hospital with a constellation of signs that do not receive confirmation from further clinical and instrumental assessments. The similarities between these syndromes and the constant presence of borderline personality in the

Case Report: Area of focus - involuntary admission for severe Factitious Disorder imposed on self.

Factitious Disorder is clinically challenging to diagnose owing to its reliance on unequivocal evidence of deception. The nature of the disorder and lack of definitive treatment may limit utilisation of mental health legislation, despite its potential fatality. Here, we report a case of a young lady with recurrent, unexplained and life-threatening episodes of iron-deficiency anaemia. Lack of reliable informant to clarify suspicion of Factitious Disorder as well as overshadowing of legal and ethical dilemmas further complicate the clinical management. An involuntary admission, as a last resort,

The girl who cried wolf: A literature review and case report of pediatric factitious disorder.

Pediatric presentations of factitious disorder (Munchausen syndrome) remain underdiagnosed and poorly understood compared to adult cases. The purpose of this study is to review the current literature on child and adolescent factitious disorder in order to better understand the differences between pediatric and adult presentations of this disorder. We also present the case of an adolescent girl with factitious disorder; her hospital course draws attention to the excessive healthcare expenditures and risk of iatrogenic complications associated with this diagnosis. We utilized MEDLINE and Google

Munchausen Syndrome by Proxy: balancing ethical and clinical challenges for healthcare professionals Ethical consideration in factitious disorders.

Munchausen syndrome by proxy is a relatively rare behavioral disorder affecting a child's primary caregiver, typically the mother. Ethical dilemmas that physicians may face in such situations mainly concern the medical options for best protecting the child's welfare, that are important, in clinical pediatric practice, because critical conflicts might arise between health professionals and parents. In such cases, the physician's primary obligation is to protect the children involved, whose family environment may be essential to their wellbeing. Ev- ery ethical choice should be tailored to a giv

Factitious disorder imposed on self: A retrospective study of 2232 cases from health insurance databases.

OBJECTIVE: Patients with factitious disorder imposed on self (FDIS) seek medical care for deliberately falsified problems. Although a large amount of work has been published, the scientific literature lacks robust data on FDIS. The present study aimed to estimate the annual mean of in-hospital FDIS codings in France, describe the sociodemographic characteristics of subjects with FDIS, assess healthcare utilisation and medical nomadism, and describe the pathologies most frequently associated with FDIS. METHOD: Subjects with at least one coding of FDIS in French health insurance databases betwe

A brain origin for factitious disorder (Munchausen's) with malingering? A single case with an old frontal lobe lesion.

We describe a patient who presented with gender identity dysphoria and stroke-like symptoms who we diagnosed with Munchausen's syndrome (factitious disorder). We discuss whether a brain lesion in the left frontal cortex is a possible risk factor, and support this hypothesis through neuropsychological investigation, EEG abnormalities, and a personality assessment. This case report supports previous suggestions that underlying brain disease/lesions might be risk factors for Munchausen's syndrome (factitious disorder).

Pediatric condition falsification (Munchausen syndrome by Proxy) as a continuum of maternal factitious disorder (Munchausen syndrome).

Pediatric condition falsification (PCF), also known as Munchausen syndrome by Proxy or Medical Child Abuse, is a somewhat rare form of child abuse and neglect. Its association with a history of adult factitious disorder (AFD) or Munchausen syndrome in the perpetrator is also well known. Exogenous insulin injection to cause hypoglycemia both in the context of PCF and AFD has been reported in the literature. However, the coexistence of both conditions via insulin injection in the same family has not been reported in the literature. This paper presents a family, in which the mother was diagnosed

Dermatitis Artefacta, a Form of Factitial Disorder Imposed on Self, Misdiagnosed as Pyoderma Gangrenosum for Eight Years.

Dermatitis artefacta is a rare psychological disorder in which patients self-inflict cutaneous lesions to satisfy an emotional need. Due to the nature of this disease, patients can present with a wide array of sometimes very severe skin lesions. Here, we describe a case of dermatitis artefacta initially misdiagnosed as pyoderma gangrenosum and treated as such for eight years. The patient reported a long history of cutaneous ulcers on her extremities and trunk, with resultant extensive scarring. Upon presentation, she displayed rapidly progressing necrotizing skin lesions on her bilateral dista

[Factitious disorder revealed by polymicrobial septic arthritis of a lumbar facet joint diagnosed by percutaneous biopsy].

Septic arthritis of a lumbar facet joint is rare with few reports in the literature. Clinically, septic arthritis of a lumbar facet joint can mimic spondylodiscitis. Imaging is usually required for diagnosis. Bacteriological diagnosis is needed to optimize treatment with antibiotics. Most of the previously reported cases were due to staphylococcus aureus. We report one case due to rare bacteria which lead to a diagnosis of factitious disorder. Precise bacteriological diagnosis was obtained by CT-guided percutaneous biopsy.

Medical child abuse and factitious disorder imposed on another: two diagnoses, one child-a narrative review for paediatricians.

UNLABELLED: Medical child abuse (MCA) and factitious disorder imposed on another (FDIA) do not describe the same person. MCA is a diagnosis given to a child harmed by unnecessary medical care initiated by a caregiver; FDIA is a psychiatric diagnosis given to the adult who falsifies illness in another. This review summarises the clinical features, diagnosis and management of MCA in children and relates MCA to FDIA and to the British concepts of fabricated or induced illness (FII) and perplexing presentations (PP). Structured narrative review of PubMed, last updated in September 2026, using the
