A postulated role of the contact system in anaphylactic reactions to insect stings was investigated. During prospective, in-hospital sting challenge, we collected serial blood samples from five normal volunteers and 16 patients with a history of insect-sting anaphylaxis. Activation of the contact system was assessed by measuring plasma levels of factor XIIa-C1-inhibitor and kallikrein-C1-inhibitor complexes as well as those of cleaved high molecular weight kininogen (HK). Fibrin deposits drive thromboinflammation and neutrophil extracellular trap (NET) formation (NETosis) in various pathophysiological situations, but the underlying mechanisms remain unclear. Similarly, it is unknown how, irrespective of diabetes, hyperglycemia exacerbates thromboinflammation. Using a microfluidic model of thromboinflammation based on the perfusion of partially recalcified citrated human blood at a venous flow rate over immobilized fibrin, we investigated these mechanisms. This work describes the purification, gene cloning and expression of infestin, a thrombin inhibitor from midguts of Triatoma infestans. Infestin is located in the midgut and its purification was performed by anion-exchange and affinity chromatographies. The N-terminal sequence and the sequence of tryptic peptides were determined. BACKGROUND: Milvexian, an oral factor (F)XIa inhibitor, is undergoing phase 3 evaluation for acute coronary syndrome. Such patients often require percutaneous coronary intervention (PCI). INTRODUCTION: Factor XII (FXII) is a liver-derived plasma zymogen that autoactivates on anionic surfaces to FXIIa, which drives the contact blood coagulation pathway, kallikrein-kinin signaling, fibrinolysis, and classical complement. Although congenital FXII(a) deficiency is largely asymptomatic, dysregulated activity is linked to thrombosis, hereditary angioedema (HAE), and neuroinflammation, making FXII(a) an attractive therapeutic target.

The subsite specificity of human lung and skin tryptase (trypsin-like enzyme) has been studied at pH 7.5 using 17 amino acid and dipeptide thioester substrates and 14 tripeptide 4-nitroanilide substrates. The reactivity and specificity of the human tryptases were compared with bovine trypsin and other trypsin-like enzymes. Eight patients with sickle cell anemia (SS hemoglobin) were found to have decreased plasma levels of prekallikrein compared to normal control subjects or patients with other types of anemia. The prekallikrein levels in the patients with sickle cell anemia were found to decrease further during a sickle cell crisis. These results suggest that components of the kallikrein-kinin system are profoundly affected in patients with sickle cell anemia, and during crises may play a role in the clinical presentation of patients.

The hypotension induced in the pentobarbital anesthesized rat by the i.v. administration of an active Hageman factor fragment (Hff) is significantly attenuated by naloxone. This effect is specific because the opiate antagonist does not modify the hypotension elicited by rat urinary kallikrein, bradykinin or nitroglycerin. These results suggest that the contact activation of endogenous Hageman factor could result in the generation of vasoactive opioid peptides derived from circulating large molecular weight precursors.
