Diagnosis and Management of Fecal Incontinence.

PURPOSE OF REVIEW: The purpose of this review is to highlight current and newer therapeutic approaches to treat fecal incontinence in patients who do not respond to conservative measures. RECENT FINDINGS: Neurostimulation techniques, injection of bulking agents, and radiofrequency energy delivery to the anal canal have been proposed and tested for fecal incontinence over the last decade. Sacral stimulation is both effective and durable and is now the most popular of the invasive techniques whereas percutaneous tibial stimulation, radiofrequency energy, and bulking agents are either less effec

Fecal incontinence: the role of the urologist.

Fecal incontinence is the involuntary loss of solid or liquid stool. While the true prevalence of fecal incontinence is difficult to discern, it is estimated that almost 9 % of non-institutionalized women in the US experience this condition. Disorders leading to fecal urgency alone are usually related to rectal storage abnormalities while incontinence is often a result of anatomic or neurologic disruption of the anal sphincter complex. Many risk factors exist for fecal incontinence and include female sex, increasing age, higher body mass index (BMI), limited physical activity, smoking, presenc

Constipation and fecal incontinence in the elderly.

The successful management of constipation and fecal incontinence in the elderly requires an understanding of colorectal function, careful delineation of the patient's complaint, and in selected patients, specialized studies of colonic and anorectal function. This article reviews (1) the prevalence and nature of lower bowel dysfunction in the elderly population: (2) colonic and anorectal physiologic changes that are associated with aging; (3) the causes, consequences, and approaches to the management of constipation in the elderly; and (4) the causes of fecal incontinence in this age group and

Management of Fecal Incontinence in Children Without Functional Fecal Retention.

The management of the fecal incontinence in children is difficult, and its social consequences are usually devastating. The general objectives of any bowel program are to produce social continence, predictability, and eventually independence. How to achieve those goals depends in part on the underlying condition. In children, fecal incontinence can occur from a variety of conditions. The most common is overflow incontinence from functional fecal retention, but it can also occur in otherwise healthy children with functional nonretentive fecal soiling or in children with organic causes of fecal

[Surgical Therapy Options for Fecal Incontinence].

Incontinence - especially anal incontinence - poses a significant burden for those affected. It severely impacts the quality of life, leads to limitations in daily life, and can result in social isolation. The aetiology of anal incontinence is heterogeneous and often multifactorial. Depending on the specific genesis, the therapy requires both a high degree of personal responsibility and discipline from the patients, as well as acceptance of the condition and its causes. For the responsible professionals, it is essential to incorporate not only professional expertise but also empathy and patien

Anal Sphincter Defect and Fecal Incontinence.

Anal sphincter defects can lead to fecal incontinence. The relationship between the extent of defect and continence is controversial. Magnetic resonance imaging (MRI) of the pelvis can assess anal sphincter defects. Transrectal ultrasonography (TRUS) is used to assess sphincter integrity in adults. We present a 17-year-old male with history of sexual abuse, rectal prolapse, and fecal incontinence. MRI showed a small defect that did not explain his clinical presentation. TRUS identified more extensive defects which were not picked up by MRI. The patient had rectopexy, and his rectal prolapse an

Fecal Incontinence: Etiology, Diagnosis, and Management.

INTRODUCTION: Fecal incontinence is a debilitating condition affecting primarily the elderly. Many patients suffer in silence resulting in both underdiagnosis and undertreatment often culminating in an overall poor quality of life. METHODS: We sought to review the etiology, diagnosis, and treatment of fecal incontinence based on current literature. Additionally, newer treatment methods such as Solesta will be evaluated. RESULTS: There are many diagnostic modalities available to assess the degree and severity of the patient's incontinence; however, a thorough history and physical exam is crit

Overt rectal prolapse and fecal incontinence.

PURPOSE: Rectal prolapse is frequently associated with fecal incontinence; however, the relationship is questionable. The study was designed to evaluate fecal incontinence in a large consecutive series of patients who suffered from rectal prolapse, focusing on both past history, anal physiology, and imaging. METHODS: Eighty-eight consecutive patients who suffered from an overt rectal prolapse (72 women, 16 men; mean age, 51.1 +/- 19.5 years) as a main symptom were analyzed; 48 patients also experienced fecal incontinence compared with 40 without incontinence. Logistic regression analyses were

[Sacral nerve stimulation in fecal incontinence].

INTRODUCTION: Sacral nerve stimulation for the treatment of faecal incontinence has gained increasing use in Europe over the last two years. Experience with the first patients treated in Denmark is described here. MATERIAL AND METHODS: Fourteen patients with severe faecal incontinence were given sacral nerve stimulation. The first treatment was temporary, and if this was successful they had a device for permanent stimulation implanted. RESULTS: The result of the test stimulation was good in ten of the 14 patients and a permanent system was implanted. After a median of 4.5 months' stimulation

Ischemic fecal incontinence and rectal angina.

In 36 patients who consulted for fecal incontinence or rectal pain, or both, there was grossly visible scarring of the rectum and biopsy revealed mucosal atrophy and fibrosis. A steal from the hemorrhoidal arteries to the iliac vessels was demonstrated in 3 subjects. Maximum tolerable volumes within a rectal balloon were smaller than in control subjects, both in men (192 vs. 273 ml) and in women (142 vs. 217 ml) (p less than 0.01). The rectoanal inhibitory reflex was abnormal in all but 1 patient. Specific abnormalities were a decreased amplitude or a prolonged duration of the reflex. It was t

Fecal Incontinence in Adults: New Therapies.

Fecal incontinence (FI) is the involuntary and recurrent loss of stool. FI significantly affects both physical and social well-being, while imposing a substantial economic burden. The global prevalence of FI is approximately 8%. Risk factors include advanced age, diarrhea, anal sphincter damage from obstetric trauma or anorectal surgery, pelvic floor abnormalities (such as rectal prolapse), inflammatory bowel disease, and neurological conditions. Despite greater understanding of the impact of FI and advances in diagnostic techniques and treatment options, management remains inconsistent, likel

Fecal incontinence risk factors and pregnancy.

MATERIALS AND METHODS: We conducted an analysis on 231 pregnant women. A proctologist examined the patients three times: in the 1st trimester (within the first 15 weeks), in the 3rd trimester (29-40 weeks), and 12 months after childbirth. RESULTS: The total number of fecal incontinence observations among women included in the study was 66 cases (28.6%), detected at the final visit. Risk factors for fecal incontinence with a high probability were age over 36 years (P = 0.001), low physical activity (P = 0.034), three or more pregnancies resulting in childbirth (P = 0.022), history of hemorrhoi

[Recent advances in fecal incontinence treatment].

RECENT ADVANCES IN FECAL INCONTINENCE TREATMENT. Anal incontinence is a chronic condition that affects nearly 10% of the general population. When anal leakage concerns the stool and is frequent, the impact on the quality of life is very important. Recent advances in non-invasive medical treatments and in operative approaches make it possible to provide for most patients an anorectal comfort compatible with a social life. The three main challenges for the future lie in the organization of screening for this condition which is still taboo and for which patients do not easily confide, in a better

[Fecal incontinence - symptoms are guiding therapy].

Fecal incontinence (FI) is an escalating medical problem. Due to increasing life expectancy FI becomes more evident, especially in the geriatric population. The prevalence varies from 4 % in the adult population reaching > 50 % in the octogenerians, yet most probably associated with a significant based error due to underreporting. Diagnosis is depending primarily upon history taking and gathered information from the patient and caregivers. Additional technical diagnostic procedures are of secondary importance. In most cases no single cause for FI can be identified, mostly multifactorial disord

Fecal incontinence after transanal endoscopic microsurgery.

PURPOSE: Transanal endoscopic microsurgery (TEM) procedure could potentially influence the development of fecal incontinence later in life. The aim of our study was to assess long-term functional outcomes after TEM and to determine possible variables related to incontinence. METHODS: Patients, enrolled in a prospectively collected TEM operation database, were interviewed using a postal questionnaire. The questionnaire consisted of EuroQol (EQ)-5D-5L quality of life questionnaire, Wexner fecal incontinence grading scale, and additional questions about other perianal operations and obstetric hi

Critical analysis of fecal incontinence scores.

INTRODUCTION: Objectively evaluating the lack of bowel control (fecal incontinence) continues to be a challenge. Many have attempted to measure the severity of fecal incontinence and to evaluate its impact on the quality of life by developing standardized scoring systems. Some of these systems have been validated but none have achieved widespread use and all have limitations in evaluating pediatric patients. METHODS: A review of the literature was performed looking for validated scoring systems of fecal incontinence that are currently used for either adult or pediatric patients. The identifie
