Editorial Commentary: It Is Imperative to Fix Symptomatic Hip Gluteus Medius Tears at Time of Femoroacetabular Impingement: Why Ignore This Pain in the Butt?

The hip can have a multitude of different pathologies leading to different symptoms. Greater trochanteric pain syndrome, historically attributed to bursitis, has been largely found to be associated with lesions of the gluteus medius and minimus tendons, and the prevalence of gluteus medius pathology in patients with femoroacetabular impingement (FAI) is as high as one-third of the FAI population. If a patient is found to have significant clinical symptoms of both FAI and a gluteus medius tear, it is imperative to fix both pathologies. The most important diagnostic predictor in hip arthroscopy

Less hip range of motion is associated with a greater alpha angle in people with longstanding hip and groin pain.

PURPOSE: A higher alpha angle has been proposed to correlate with lower hip range of motion, but the association in people with longstanding hip and groin pain is currently unclear. The aims were to: (1) assess the association between range of motion and alpha angle in patients with longstanding hip and groin pain; (2) examine if a cut-off value in range of motion variables could identify patients with an alpha angle above or below 60°. METHODS: Seventy-two participants were consecutively recruited from an orthopaedic department after referral for hip- and groin-related pain. Passive hip rang

Femoral Neck Over-Resection After Hip Arthroscopy Treated with Vascularized Fibula Autograft: A Case Report.

CASE: We describe a case of femoroacetabular impingement syndrome treated with labral repair and arthroscopic femoral osteoplasty in a 30-year-old man. The case was complicated by resection of 50% of the femoral neck resulting in high risk of an impending femoral neck fracture. The patient was treated with a vascularized fibula autograft and internal fixation. CONCLUSION: Complications of femoral neck over-resection can threaten the viability and structural integrity of the hip joint but may successfully be treated with vascularized fibula autograft.

Ossification of the acetabular rim: a highly prevalent finding in asymptomatic non-osteoarthritic hips of all ages.

OBJECTIVE: To estimate the prevalence of acetabular rim ossifications in the adult population with asymptomatic, morphologically normal hips at CT and to determine whether the presence of these ossifications is associated with patient- or hip-related parameters. METHODS: We prospectively included all patients undergoing thoracoabdominal CT over a 3-month period. After exclusion of patients with a clinical history of hip pathology and/or with signs of osteoarthritis on CT, we included a total of 150 hips from 75 patients. We analyzed the presence and the size of ossifications around the acetab

Editorial Commentary: Hip Preservation and Opioids.

Approximately one-third of patients undergoing arthroscopic hip preservation surgery for femoroacetabular impingement syndrome and labral tears are on preoperative opioid medications. The single most important predictor for prolonged chronic postoperative opioid use is preoperative use. Despite the well-documented high success rates in nonarthritic, nondysplastic individuals undergoing hip arthroscopy, up to half of those individuals on preoperative opioids may still be on opioids at 1 to 2 years of follow-up. Mental wellness disorders (e.g., depression, anxiety, substance abuse) significantly

Editorial Commentary: Hip Arthroscopy for Femoroacetabular Impingement in Systemic Inflammatory Disease: Pirates Gold in Uncharted Waters.

Hip pain in patients with systemic inflammatory diseases may arise from synovitis, cartilage degeneration or bony abnormalities like femoroacetabular impingement or acetabular dysplasia. With advances in early diagnosis and medical management of the autoimmune response, these often-young patients may benefit from correction of underlying bony abnormalities like femoroacetabular impingement. Short-term improvement in patients' pain and function can be achieved with hip arthroscopic treatment. However, the long-term efficacy of hip arthroscopy in systemic inflammatory disease needs further resea

Editorial Commentary: Chicken or the Egg? The Ligamentum Teres and Degenerative Hip Disease.

The ligamentum teres has traditionally been described as a vestigial remnant or redundant structure with little contribution to hip biomechanics or function. However, interest in the ligamentum teres has been renewed as evidence has emerged that it not only plays a role as a pain generator in the hip but also acts as a secondary stabilizer to supplement the work of the capsular ligaments. Furthermore, an association has been proposed between tearing of the ligamentum teres and the development of degenerative hip disease. However, the question remains, is it a cause-and-effect relationship?

Hip Osteoarthritis: Etiopathogenesis and Implications for Management.

Highly prevalent among the elderly, hip osteoarthritis (OA) carries a heavy burden of disease. Guidelines for the management of hip OA are often extrapolated from knee OA research, despite clear differences in the etiopathogenesis and response to treatments of OA at these sites. We propose that hip OA requires specific attention separate from other OA phenotypes. Our understanding of the etiopathogenesis of hip OA has seen significant advance over the last 15 years, since Ganz and colleagues proposed femoroacetabular impingement (FAI) as an important etiological factor. This narrative review s

Editorial Commentary: Let's Continue the Debate of Hip Capsule Management: Periportal Capsulotomy Without Closure Can Lead to High Rates of Achieving Meaningful Clinical Outcomes.

Adequate visualization for femoroacetabular impingement syndrome treated by hip arthroscopy is critical to avoid leaving residual structural disease. Proponents of access to the hip via interportal and T-capsulotomy argue that performing a larger capsulotomy allows for appropriate management of intra-articular pathology, with capsular closure required to prevent iatrogenic instability. However, advocates of smaller capsular incisions have recently published literature that suggests management of intra-articular hip pathology with periportal capsulotomy incision confers to high rates of patient

Editorial Commentary: Lumbar Pathology Likely Negatively Affects Surgical Outcomes in Patients With Femoroacetabular Impingement Syndrome, Though the Strength of the Relationship is Unclear.

A complex interplay exists between the hip joint and the lumbosacralpelvis. As a patient moves from a standing to seated position, the lower back, pelvis, and hips must move in concert to properly function. Conceptually, any deviation from the norm risks early hip impingement. Current evidence is conflicted regarding whether concomitant lumbar pathology (LP) negatively affects outcomes following hip arthroscopy with labral repair for patients with femoroacetabular impingement syndrome (FAIS), though the preponderance of evidence suggests some degree of negative relationship. Fortunately, studi

Ischiofemoral Impingement Syndrome in 2024: Updated Concepts and Imaging Methods.

Ischiofemoral impingement syndrome (IFI) is a source of extra-articular pain caused by narrowing between ischium and femur in native hips. Secondary compression of quadratus femoris muscle leads to edema, tears, and deep gluteal pain. IFI is more frequent in females, with evidence pointing to a combination of hip, spine, and pelvic biomechanics and morphology leading to abnormal osseous relationships. This article provides updated concepts regarding the diagnosis, biomechanics, imaging, and treatment strategies for IFI. Moreover, IFI is emphasized as a multifactorial native hip syndrome, in co

Editorial Commentary: Acetabular Cartilage Delamination in Patients Having Femoroacetabular Impingement Syndrome Surgery Requires a Single-Stage Solution.

Acetabular cartilage delamination (ACD) can result in focal chondral defects, increasing contact stresses and contributing to osteoarthritis. This is commonly associated with femoroacetabular impingement, particularly with cam deformities due to shearing of the cam on the acetabulum. Additionally, ACD associated with labral tears or chondrolabral separation, when untreated, can compromise labral repair outcomes. ACD is often missed on preoperative magnetic resonance imaging and magnetic resonance arthrography, which show a sensitivity of <80%. Thus, given the ubiquity of ACD in patients underg

[Femoroacetabular impingement in adolescents].

Femoroacetabular impingement syndrome (FAIS) is caused by a repetitive mechanical conflict between the acetabulum and the proximal femur, occurring in flexion and internal rotation. In cam impingement, bony prominences of the femoral head-neck junction induce chondrolabral damage. The acetabular type of FAIS, termed pincer FAIS, may be either due to focal or global retroversion and/or acetabular overcoverage. Combinations of cam and pincer morphology are common. Pathological femoral torsion may aggravate or decrease the mechanical conflict in FAI but can also occur in isolation. Of note, a hig

Acetabular retroversion does not affect outcome in primary hip arthroscopy for femoroacetabular impingement.

PURPOSE: The surgical treatment of femoral-acetabular impingement syndrome (FAIS) in patients with acetabular retroversion (AR) is arthroscopical or by a reverse periacetabular osteotomy (PAO). The purpose of the present study was to investigate the results after arthroscopic treatment of FAIS in patients with and without radiographic signs of AR in a large, prospective cohort from the Danish Hip Arthroscopy Registry (DHAR). The hypothesis was there is no difference in clinical outcome between the two groups. METHODS: Data on 4914 hip arthroscopies performed during 2012-2019 were obtained fro

Editorial Commentary: The Pelvis is the Lowest Vertebral Level: Diagnostic Approach to Hip-Spine Syndrome.

The human pelvis represents a wonderful example of apparent idealistic simplicity overwhelmed by realistic complexity. Traditionally, the pelvis has been termed a "ring" linking the lower extremity to the spine via the sacroiliac joint. In essence, the pelvis is the lowest vertebral level-"the hip bone's connected to the spine bone." Thus, the law of parsimony seemingly applies in the diagnosis and management of both arthritic and nonarthritic hip and spine disorders in isolation or combination. However, an inverse Occam's razor is much more likely. The layered theory of hip disorders illustra

Editorial Commentary: Lumbosacral Anatomy and Mechanics Influence Femoroacetabular Impingement Syndrome and Surgical Outcomes: The Hip Bone Is Connected to the Back Bone.

The hip joint in general and femoroacetabular impingement (FAI) in particular do not exist in a vacuum. Impingement kinematics are very closely tied to the relationship between spinopelvic motion and posture, and that of the hip joint itself. While the relationship of lumbar degenerative disease, fusion, and sagittal balance to hip arthroplasty has been well studied, there is a paucity of data on the analogous relationship of the stiff spine with hip arthroscopy and FAI. While further studies are critical in advancing our understanding of this relationship in this unique population, surgeons s
