Biplane fracture of distal femoral epiphysis: a case report.

We report a case of an unusual pattern of fracture in an 11 year-old girl who sustained a distal femoral epiphyseal injury of her right knee after a motorcycle accident. It is a combination of type I and type III epiphyseal injury. This is the so-called biplane fracture of distal femoral epiphysis. The pathomechanism of the injury is a combined adduction and hyperextension type. Open reduction and internal fixation are used in order to achieve anatomic reduction. Three months after operation the fracture was healed with good functional result.

Contemporary Distal Femoral Replacements for Supracondylar Femoral Fractures Around Primary and Revision Total Knee Arthroplasties.

BACKGROUND: There is a paucity of data on the outcomes of distal femoral replacements (DFRs) in patients with total knee arthroplasty (TKA) periprosthetic fractures. We sought to characterize these patients' survivorship free from rerevision. METHODS: We retrospectively identified 49 patients, including 34 after primary TKA (primary cohort), 9 after revision TKA, and 6 conversions for failed open reduction and internal fixation (revision cohort) that underwent DFR for a periprosthetic femur fracture. The mean age was 76 years, and 40 patients (82%) were female. The mean follow-up was 4 years.

Reverse distal femoral locking compression plate a salvage option in nonunion of proximal femoral fractures.

BACKGROUND: When primary fixation of proximal femoral fractures with implants fails, revision osteosynthesis may be challenging. Tracts of previous implants and remaining insufficient bone stock in the proximal femur pose unique problems for the treatment. Intramedullary implants like proximal femoral nail (PFN) or surface implants like Dynamic Condylar Screw (DCS) are few of the described implants for revision surgery. There is no evidence in the literature to choose one implant over the other. We used the reverse distal femur locking compression plate (LCP) of the contralateral side in such

Femoral nerve block for diaphyseal and distal femoral fractures in the emergency department. Surgical technique.

BACKGROUND: Diaphyseal and distal femoral fractures are painful injuries that are frequently seen in patients requiring a trauma work-up in the hospital emergency department prior to definitive management. The purpose of this study was to determine whether a femoral nerve block administered in the emergency department could provide better pain relief for patients with femoral fractures than currently used pain management practices. METHODS: Patients who presented with an acute diaphyseal or distal femoral fracture were identified as potential candidates for this study. Eligible patients were

[Femoral shaft fracture and secondary diagnosis of ipsilateral femoral neck fracture. Typical constellation or a complication of the distal femoral nail (DFN)?].

We present a case of two patients with a distal femur shaft fracture due to a high velocity trauma. Both were treated with a distal femur nail (DFN). After mobilisation a fracture of the neck of the femur was diagnosed which was not seen in the x-rays on admission. In this paper we discuss whether this is a typical constellation of ipsilateral fractures of the neck of the femur in femur shaft fractures or a complication of implantation of a DFN. Immediately after operative treatment of a femur shaft fracture specifically after high velocity trauma or in polytraumatized patients an x-ray of the

Optimizing Dual-Plate Fixation of Distal Femoral Periprosthetic Fractures Using a Distal Tibial Metaphyseal Plate.

INTRODUCTION: Distal femoral periprosthetic fractures following total knee arthroplasty (TKA) are complex and technically demanding injuries in orthopedic trauma surgery. With the increasing prevalence of TKA in an aging population, the incidence of these fractures has risen steadily. This study aims to share our experience of implant selection and surgical strategy, which provides an alternative and out-of-the-box thinking process for fracture fixation. METHODS: Between 2022 and 2024, 10 consecutive patients with distal femoral periprosthetic fractures involving a well-fixed femoral componen

[Is distal femoral replacement an adequate therapeutic option after complex fractures of the distal femur?].

BACKGROUND: Modular distal femur replacements originally were developed for reconstructing a full weight-bearing and functional extremity after resection of primary bone tumours with large bony defects located in proximity to joints. The aim of this study was to examine the use of the modular distal femur replacements for complex fractures and severe post-traumatic sequelae of the distal femur in context to comparable studies. PATIENTS AND METHODS: Fourteen patients with complex fractures or post-traumatic sequelae and extensive bone defects requiring distal femur replacement were analysed re

Operative treatment of distal femoral fractures proximal to total knee replacements.

Twenty fractures of the distal part of the femur proximal to a total knee replacement were treated operatively by members of the New England Trauma Study Group. Notching of the anterior aspect of the femoral cortex was associated with only two of these fractures, and none of the knee prostheses was loose at the time of the fracture. All twenty fractures were treated with open reduction and stable internal fixation, and the operation on fifteen fractures was supplemented with bone grafts. Every fracture healed, and eighteen healed after a mean of sixteen weeks (range, six to forty weeks). Union

Dual Plating Through a Single Anterior Approach for Distal Femoral Fractures.

PURPOSE OF THE STUDY: Comminuted distal femoral fractures and very low periprosthetic fractures present significant treatment challenges, which are commonly associated with nonunion and varus collapse. Dual plating has emerged as an effective solution to address these issues. The aim of this study was to evaluate the clinical outcomes of osteosynthesis with double plates through a single anterior incision. MATERIAL AND METHODS: Forty-nine patients (38 females, 11 males, mean age 65.7 ± 16.2 years, range 23-90 years) were included. The study comprised 18 acute distal femoral fractures (AO/OTA

Blade-plate fixation for distal femoral fractures: a case-control study.

BACKGROUND: The blade-plate is the earliest of the contemporary internal fixation devices introduced for distal femoral fractures. The recent development of dedicated, fixation devices has considerably limited its use. The objective of this study was to evaluate outcomes after blade-plate fixation and after fixation using other devices. HYPOTHESIS: Outcomes after blade-plate fixation are similar to those after condylar screw-plate, distal femoral nail, or locking condylar plate fixation. MATERIAL AND METHODS: We reviewed outcomes after 62 patients managed with blade-plate fixation and includ

Distal locking stem for revision femoral loosening and peri-prosthetic fractures.

Revision total hip arthroplasty in the setting of a large proximal femoral deficiency or a peri-prosthetic fracture remains a challenging problem. We describe the development, surgical technique and the use of cementless revision stems with distal inter-locking screws to provide immediate stability of the femoral implant. Results were assessed in a large multicentre French study conducted with the french hip and knee surgery society (SFHG). We retrospectively reviewed 725 revisions using interlocking stems from 14 French orthopaedic departments. Seven different stems were used in this series.

Simultaneous Osteosynthesis and Distal Femoral Osteotomy in a Patient With Distal Femoral Fracture Combined With Valgus Knee Deformity: A Case Report.

Distal femoral fractures are commonly treated with osteosynthesis. However, in older patients with osteoarthritis of the knee, acute primary total knee arthroplasty (TKA) may be performed to treat these fractures. Notably, no studies have documented the use of osteosynthesis in combination with distal femoral osteotomy (DFO) for treating distal femoral fractures in patients with knee osteoarthritis. This report presents the case of a 66-year-old woman with lateral compartment osteoarthritis accompanied by severe valgus knee deformity who underwent osteosynthesis for a distal femoral fracture c

Distal femoral replacement or internal fixation for management of periprosthetic distal femur fractures: A systematic review.

BACKGROUND: The number of periprosthetic fractures above a total knee arthroplasty continues to increase. These fractures are associated with a high risk of morbidity and mortality. Techniques for addressing these fractures include open reduction internal fixation (ORIF) and revision arthroplasty, including distal femoral replacement (DFR). The primary aim of this review is to compare mortality and reoperation rates between ORIF and DFR when used to treat periprosthetic distal femur fractures. METHODS: A systematic review including MEDLINE, Embase and Cochrane Library databases was completed
