Obturator nerve transfer as an option for femoral nerve repair: case report.

OBJECTIVE: Nerve transfers have proved to be an important addition to the armamentarium in the repair of brachial plexus lesions, but have been used sparingly for lower extremity nerve repair. Here, we present what is believed to be the first description of a successful transfer of the obturator nerve to the femoral nerve. CLINICAL PRESENTATION: A 45-year-old woman presented with a complete femoral nerve lesion after removal of a large (15-cm) schwannoma of the retroperitoneum involving the lumbar plexus. INTERVENTION: The obturator nerve was transferred to the distal stump of the femoral ne

Surgical Neuropelveology: Lateral Femoral Cutaneous Nerve Endometriosis. Laparoscopic Resection and Nerve Transplantation.

STUDY OBJECTIVE: To demonstrate the application of surgical neuroanatomic principles for the diagnosis and treatment of deep infiltrating endometriosis involving the lateral femoral cutaneous nerve. DESIGN: Video demonstration of laparoscopic lateral femoral cutaneous endometriosis resection with nerve transplant. SETTING: Endometriosis infiltrating somatic nerves is a poorly known condition, which can cause severe neuropathic symptoms [1] and is often unrecognized with a subsequent treatment delay [1]. Intimate knowledge of pelvic neuroanatomy and expertise in minimally invasive surgery are

Nerve transfers for femoral nerve palsy: an updated approach and surgical technique.

OBJECTIVE: Femoral nerve palsy results in significant impairment of lower extremity function due to the loss of quadriceps muscle function. The authors have previously described their techniques utilizing the anterior obturator and sartorius nerves for transfer in cases of femoral nerve palsy presenting within 1 year of injury. In the current study, the authors discuss their updated techniques, results, and approach to partial and complete femoral nerve palsies using femoral nerve decompression and nerve transfers. METHODS: They conducted a retrospective review of patients with femoral nerve

Obturator nerve transfer for femoral nerve reconstruction: anatomic study and clinical application.

BACKGROUND: Femoral nerve lesion causes significant disability. In many cases, the availability of the proximal stump is in question and further complicates surgical management by severely limiting reconstructive options and precluding nerve graft reconstruction. The purpose of this report is to describe the successful restoration of quadriceps function by distal nerve transfer at the level of the thigh without functional donor morbidity and the findings of cadaveric dissections of the obturator and femoral nerve branches. METHODS: Eight fresh frozen cadaveric lower limbs were dissected at th

Transfer of obturator nerve for femoral nerve injury: an experiment study in rats.

BACKGROUND: Quadriceps palsy is mainly caused by proximal lesions in the femoral nerve. The obturator nerve has been previously used to repair the femoral nerve, although only a few reports have described the procedure, and the outcomes have varied. In the present study, we aimed to confirm the feasibility and effectiveness of this treatment in a rodent model using the randomized control method. METHODS: Sixty Sprague-Dawley rats were randomized into two groups: the experimental group, wherein rats underwent femoral neurectomy and obturator nerve transfer to the femoral nerve motor branch; an

Saphenous nerve evoked potentials and the assessment of intraabdominal lesions of the femoral nerve.

A technique for recording somatosensory evoked potentials after stimulation of the patellar branch of the saphenous nerve is described and the results from 11 normal subjects and from two patients with intraabdominal femoral nerve injuries are presented. One patients developed a nerve lesion after an abdominal surgical procedure and the second suffered a gunshot wound causing femoral nerve paralysis. Both patients recovered spontaneously. Evoked potentials from saphenous nerves showed a significant discrepancy between faster motor recovery and delay in reinstating sensory axonal conduction.

Regressing intraneural nodular fasciitis of the femoral nerve.

Intraneural nodular fasciitis is an extremely rare phenomenon with thirteen prior published reports, only one of which involved the femoral nerve. Our case involves intraneural nodular fasciitis of the femoral nerve discovered on imaging after a patient reported months of debilitating symptoms initially attributed to labral pathology from femoroacetabular impingement. MRI of the right hip demonstrated an avidly enhancing intraneural mass with partially infiltrative, spiculated margins. Given that these features have only been described in one prior case of intraneural nodular fasciitis with th

Femoral to sciatic nerve transfer: A cadaver study.

BACKGROUND: Proximal sciatic nerve injuries are a challenge to treat due to the limited options for donor nerves and the long distance needed for regeneration. METHODS: In our cadaveric study using five human cadavers, we aimed to evaluate the feasibility of transferring the tibial and common peroneal components of the sciatic nerve to the femoral nerve motor branches of the vastus medialis (VM) and vastus lateralis without the need for interposition nerve graft. The femoral nerve branches of the VM and lateralis were exposed anteriorly. The sciatic nerve was exposed posteriorly and passed th

Femoral nerve injury following a lumbar plexus blockade.

BACKGROUND: Lumbar plexus blockade (LPB) combined with sciatic nerve block (SNB) is frequently used for lower extremity surgery. Perioperative nerve injury is a rarely encountered complication of peripheral nerve blocks (PNB). CASE REPORT: Here we report a 44-year-old male patient who developed a partial femoral nerve injury (FNI) following a LPB which was performed before the surgery of a patellar fracture. The clinical and electroneuromyographic findings of the patient were recovered almost completely within the following six months. CONCLUSION: The presented case demonstrated a FNI despit

Femoral nerve neuropathy after the psoas hitch procedure.

Femoral nerve neuropathy as a complication from abdominopelvic surgery was firstly described in 1896, by Gumpertz, in a case report of femoral nerve injury following hysterectomy. The authors report two cases of femoral nerve neuropathy following psoas hitch vesicopexy in ureteral reimplantation, and to discuss the etiology and clinical manifestations of this complication. Femoral nerve neuropathy secondary to psoas hitch is a rare complication, although it should be considered during the surgical procedure, as well as in postoperative care.

Distal lesion of the lateral femoral cutaneous nerve.

We report three patients with a typical clinical picture of unilateral meralgia paresthetica in whom routine nerve conduction studies were normal. However, cortical somatosensory evoked potentials were absent after lateral femoral cutaneous nerve (LFCN) stimulation on the affected side. After stimulation of the LFCN in the anterosuperior iliac spine (ASIS) region and recording the responses distal to conventional sites (20 cm from the ASIS), sensory nerve action potentials (SNAPs) were absent in the symptomatic leg, but present in the normal leg. We suggest that thigh paresthesias may be cause

Femoral nerve injury complicating continuous psoas compartment block.

OBJECTIVES: The objective of this case report is to describe a femoral nerve injury after a psoas compartment block (PCB) and to discuss the probable mechanisms of injury and neuron regeneration. To date, this is the first report of severe femoral nerve injury after PCB. CASE REPORT: A 60-year-old, American Society of Anesthesiologists II woman underwent right total knee replacement under general anesthesia and continuous PCB for postoperative analgesia. Postoperatively, she showed signs of severe femoral nerve injury. A physical therapy program and muscle electrical stimulation were institut

Prevention of femoral nerve injuries in gynecologic surgery.

Abdominopelvic operation is the most frequent cause of iatrogenic femoral nerve injury, particularly when self-retaining retractors are used. Transverse abdominal incisions and thin body habitus are significant risk factors. Patients typically have mild sensory and/or motor disturbances with diminished knee jerk reflexes. Most cases are self-limited, and symptoms resolve within several weeks. Most femoral nerve injuries that are associated with pelvic surgery can be prevented with a thorough understanding of the pelvic anatomy and proper placement of self-retaining retractors.

Femoral Nerve Block after Anterior Cruciate Ligament Reconstruction.

Femoral nerve block (FNB) has been proposed for pain control following anterior cruciate ligament (ACL) reconstruction. Although numerous studies have assessed the efficacy of FNBs, there has been little to no research into the effect of such blocks on postoperative strength and patient-reported outcomes. We hypothesized that performance of an FNB would result in decreased quadriceps strength and poorer patient-reported outcome scores within the first 6 months following ACL reconstruction. A total of 30 patients scheduled to undergo hamstring autograft ACL reconstruction following an acute ACL
