Long proximal femoral nail in ipsilateral fractures proximal femur and shaft of femur.

BACKGROUND: Ipsilateral fractures of the proximal femur and femoral shaft are extremely uncommon injuries which occur in young adults who sustain a high energy trauma. A variety of management modalities have been tried to treat this complex fracture pattern ranging from conservative approach to recently introduced reconstruction nails. All these approaches have their own difficulties. We studied the outcome of long proximal femoral nail (LPFN) in the management of concomitant ipsilateral fracture of the proximal femur and femoral shaft. MATERIALS AND METHODS: We analysed the prospective data

The femur too short? 1373 fetuses with short femur during second-trimester screening.

PURPOSE: A short fetal femur in prenatal diagnosis might be an indicator for intrauterine growth retardation (IUGR), a genetically determined small child (SGA) with or without associated fetal malformations and/or an adverse fetal outcome. METHODS: 1373 singleton pregnancies with a femoral length < 5th percentile detected between 1999 and 2015 during second-trimester screening in a tertiary prenatal diagnostic center were subjected to a descriptive retrospective analysis with regard to fetal characteristics as well as pregnancy outcome. RESULTS: 685 (49.9%) fetuses presented an isolated shor

Reliability of measuring lateral bowing angle of the femur in patients with atypical femur fractures.

BACKGROUND: Lateral femoral bowing causes a higher tensile mechanical load on the lateral side of the femur, which plays a critical role in the pathogenesis of atypical femoral fractures (AFFs). There are many ways to measure lateral femoral bowing on plain radiographs, and there are difficulties in finding a correspondence point between measurements among multiple measurers. The purpose of this study is to prove the best method of correspondence between the investigators by analyzing the reliability of various methods for measuring lateral femoral bowing. MATERIALS AND METHODS: We retrospect

[Necrosis of the head of the femur as a complication of the medial fracture of the neck of the femur (author's transl)].

In spite of the progress achieved in accident surgery, medial fractures of the neck of the femur still present a lot of problems. The rate of success achieved in most other cases of bone fracture is not obtained here. The necrosis of the head is the most frequent and most serious complication of the medial fracture of the neck of the femur. Its formation is influenced by different factors, such as fracture line, dislocation of the fragments, time and method of reposition, and method of osteosynthesis. A prognosis whether a necrosis of the head of the femur is to be expected or not can be made

Differences in femur geometry and bone markers in atypical femur fractures and the general population.

This study aimed to identify differences in femur geometry between patients with subtrochanteric/shaft atypical femur fractures (AFFs) and the general population, and to evaluate the biomechanical factors related to femoral bowing in AFFs. We retrospectively reviewed 46 patients. Data on age, and history and duration of bisphosphonate use were evaluated. Femur computed tomography images were reconstructed into a 3D model, which was analyzed with a geometry analysis program to obtain the femur length, femur width and length, and femoral bowing. Patients were divided into two groups according to

Functional Outcome of Distal Femur Nail Multilocking (DFN-ML) in Distal Third Femur Fractures.

Background Distal femur fractures (DFFs) are relatively common injuries, particularly in younger individuals due to high-energy trauma and in older individuals due to lower-force events and poor bone quality. Distal femur nail multilocking (DFN-ML) is a less commonly reported method of fixation with potential biomechanical advantages. This method involves the use of an intramedullary nail that has multiple directional locking screws, which provide better stability and fixation compared to conventional plating and nailing methods. Objective The present study aimed to evaluate the functional out

Civilian Ballistic Proximal Femur Fractures and Blunt Proximal Femur Fractures: Comparing Outcomes and Complications.

INTRODUCTION: To assess ballistic proximal femur fracture outcomes in comparison with proximal femur fractures sustained by blunt mechanisms. We hypothesized that ballistic proximal femur fractures would have higher rates of infection, nonunion, and compartment syndrome than nonballistic fractures. METHODS: A retrospective cohort was collected from the electronic medical record of a single, Level I, trauma center over a 10-year period (2013 to 2022) using Current Procedural Terminology codes. All consecutive adult patients with ballistic proximal third femur fractures (femoral neck, intertroc

Checklists in Femur Fractures: High Adherence After Implementation of Computer-based Pediatric Femur Guidelines.

INTRODUCTION: The American Academy of Orthopaedic Surgeons (AAOS) created an evidence-based clinical practice guideline for the care of pediatric diaphyseal femur fractures in 2010. Our institution implemented checklists based off these guidelines embedded in a standardized EMR order. The purpose of this study was to describe compliance with checklist completion and to assess safety improvement in a large urban pediatric hospital. METHODS: Retrospective and prospective data were collected from 2 years before and 5 years after checklist implementation. This included the patient safety checklis

Total femur fixation using the "nail-plate docking technique" for ipsilateral femur shaft fracture.

In an aging society, the number of femoral fractures is increasing, as well as the incidence of periprosthetic fractures. These secondary fractures are often difficult to fixate stably because of the osteoporotic bone and the existence of the former implant. Herein, we present two cases of secondary femoral shaft fractures after osteosyntheses for distal femur fractures with polyaxial locking plates (Non-Contact-Bridging Distal Femur, NCB-DF®, ZimmerBIOMET, Winterthur, Switzerland). Antegrade intramedullary nails (Natural Nail®-GT Femoral, ZimmerBIOMET, Winterthur, Switzerland) were utilized w

Stability of Intertrochanteric Femur Fractures.

The stability of intertrochanteric fractures depends upon multiple factors including the fracture displacement, location and pattern, the type of fixation used for stabilization, and the loading that the fracture undergoes postfixation. Traditional classification systems are of limited utility because they typically stratify intertrochanteric fractures as stable or unstable based purely on the fracture pattern without consideration of the stability after fixation. Biomechanical studies evaluating the stability of various fixation constructs should include physiologic loading, including rotatio

Churchill's fractured neck of femur.

In June 1962 at the age of 87 years, Sir Winston Churchill (1874-1965) fell over in his hotel room at the Hotel de Paris in Monte Carlo and sustained a fracture to the neck of his left femur. He was flown back to London and the fracture operated on at The Middlesex Hospital by two eminent orthopaedic surgeons, Mr Phillip Newman (1911-1994), Consultant to the The Middlesex Hospital and The Royal National Orthopaedic Hospital, Stanmore, and The Institute of Orthopaedics, London, and Professor Herbert Seddon (1903-1977), Consultant to the The Royal National Orthopaedic Hospital, Stanmore, and Dir

Detection of Atypical Femur Fractures.

The 2019 International Society for Clinical Densitometry (ISCD) Position Development Conference Task Force for monitoring with dual-energy X-ray absorptiometry (DXA) identified detection of atypical femur fractures (AFFs) as an important topic and established this working group to answer key questions in this area. The authors conducted a systematic review of the literature and deliberated on proposed ISCD positions, which were then reviewed by an external expert panel and vetted at the 2019 ISCD Position Development Conference in Kuala Lumpur on March 23, 2019. This paper summarizes the final

Management of Interprosthetic Femur Fractures.

Femoral fractures between a total hip arthroplasty prosthesis and total knee arthroplasty prosthesis, also called interprosthetic fractures, are challenging clinical problems. The number of patients who have undergone ipsilateral primary or revision joint arthroplasty procedures in both the hip and the knee continues to rise, and the number of interprosthetic fractures is increasing, as well. The growing body of biomechanical and clinical literature on interprosthetic fractures reflects the increased frequency of and interest in these injuries. Similar to the management of periprosthetic fract

Computer-assisted Femur Fracture Reduction.

INTRODUCTION: When using intramedullary nailing of femur fractures false axis alignment or malrotation is the greatest deterrent to successful reduction often causing extended x-ray exposure and the need to revert to an open procedure. The aim of this study was to demonstrate the advantages of using a trauma navigation system during intramedullary nailing of femur fractures. MATERIALS AND METHODS: A fluoroscopic navigation unit was used and successful closed fracture reduction, x-ray exposure, and operation time were measured. Between 2004 and 2007 19 patients receiving intramedullary nailing

Knee in congenital short femur.

The abnormalities of the knee associated with congenital short femur (CSF) and proximal femoral focal deficiency (PFFD), have been recognized for many years. The emergence of new and more effective lengthening techniques that can correct major congenital leg length discrepancies has made the abnormalities of the knee much more relevant. Some investigators have reported a high incidence of knee subluxation and dislocation during lengthening procedures in patients with CSF, attributing the incidence to knee anomalies. We assessed knee function in a group of 24 patients with CSF. Detailed evaluat

Femur fractures in alpine skiers.

From the years 1982 to 1989, 377 patients with alpine skiing-related injuries were admitted to St. Anthony's Hospital (a level one regional trauma center). Fifty femur fractures were recorded in 48 patients: 29 femoral shaft fractures (2 open), 15 hip fractures, and 6 supracondylar fractures (2 open). In the femoral shaft fracture group, the predominant fracture pattern was a short, transverse, comminuted fracture, illustrating the high kinetic energy of the accident. In this group, 11 patients returned to skiing, 7 of whom had hardware in place. Of the 10 Garden femoral neck fractures, 4 prog
